[Prognostic value and possibilities of the treatment of arterio-portal fistula in tumors of the liver].

Angiograms demonstrated arterio-portal shunts in tumors of the liver in 15 patients. Five patients received symptomatic therapy, all of them died from hemorrhage from varicose esophageal veins in the immediate 2 months. Resection of the liver was performed in 4 patients, 3 of them are alive and in a state of remission. Esophageal hemorrhage developed only in 2 of 6 patients who underwent embolization of the hepatic artery. It is concluded that the prognosis is extremely unfavourable in untreated patients with marked arterio-portal shunting in the hepatic tumor. Such patients must be treated surgically or, if inoperable, by radiologically-guided intervention, which makes it possible to normalize portal hemodynamics in most cases and avoid fatal hemorrhage.

[Embolization of the hepatic artery in hemangiomas of the liver].

Embolization of the proximal part of the hepatic artery was carried out in three patients with hemangioma of the liver who did not meet the criteria for radical surgery. Metal spirals were used for this purpose. Response was observed in all the cases. Patients were followed for 9-36 months. The data obtained suggest that embolization of the hepatic artery is effective and may be used for the treatment of inoperable vascular tumors of the liver.

[Choice of the approach in catheterization and embolization of the hepatic artery in malignant tumors of the liver].

The results of catheterization (transfemoral and transaxial) and embolization of the hepatic artery in 67 patients with malignant hepatic tumors are analysed. In patients with hepatomegaly, the unfavourable topographic-anatomical interrelations of the intraorganic hepatic vessels were established. Distribution of the vessels more accurately is defined by means of ultrasonography. In detection of the unfavourable anatomical factors, the more wide use of primary transaxial catheterization of the hepatic arteries without an attempt of transfemoral catheterization is recommended.

[X-ray endovascular embolization of the hepatic artery in the palliative treatment of hepatocellular cancer].

X-ray endovascular embolization of hepatic artery (HAE) was carried out in 10 cases of liver cell carcinoma (hepatic cirrhosis included). Immediate beneficial effect was recorded in 8 cases. Remission for 12 months and more was registered in three cirrhosis-free patients; mean survival in five patients was 15 months. In cases of cancer aggravated with cirrhosis, HAE effectiveness proved much lower and survival dropped to average 5.5 months.

[Re-artelialization of the liver after intravascular occlusion and the possibilities of re-embolization of the arterial bed of the liver].

Angiographic examinations were performed in 52 patients with portal cirrhosis and 6 patients with tumours of the liver in remote terms (1.5-38 months) after embolization of the hepatic artery. In 20 patients (34.5%) a considerable degree of reestablishment of the hepatic blood flow was noted which was considered to be an indication to a repeated endovascular intervention. Unlike the surgical operation, the intravascular occlusion can be performed repeatedly in the process of observation of the patient.

[Embolization of the hepatic artery in the treatment of malignant tumors of the liver complicated by jaundice].

Embolization of the hepatic artery (EHA) was performed in 7 patients with malignant tumors of the liver. Before EHA all the patients had an increased level of bilirubin and transaminase of blood serum. After performing EHA their levels decreased within 5-12 days in 6 patients, the average survival of the patients was 14 months. One patient died 5 days after EHA from hepatic insufficiency resulting from necrosis of the massive tumor. The conditions for safe performing of EHA under conditions of hyperbilirubinemia are discussed.

[Angiographic evaluation of the results of roentgeno-endovascular occlusion of the hepatic artery].

Angiographic examinations of 42 patients after percutaneous embolization of the hepatic artery system were performed for dearterialization of the tumour or reduction of the arterial bloodflow in cirrhosis of the liver complicated by portal hypertension. High effectiveness of metallic spiral emboli was revealed for the creation of stable permanent action on different portions of the hepatic artery. The results obtained show high efficiency of proximal embolization of the hepatic artery in treatment of tumors and cirrhosis of the liver and its advantages over the surgical ligation.

[Characteristics of extrahepatic collateral blood flow in patients with cirrhosis of the liver].

Percutaneous transhepatic portography was performed in 120 patients with cirrhosis of the liver with the intrahepatic form of pleural hypertension. It was shown that the degree of portal hypertension was associated with the presence of collateral circulation rather than with its distribution. The preferable way of collateral shunt is thought to be the gastroesophageal anastomosis which is not however hemodynamically effective. The results obtained show that natural collateral circulation is not able to provide decompression of the portal system.

Collateral Circulation↗

[Embolization of the splenic artery in liver cirrhosis].

An experience with 70 embolizations of the splenic artery in 62 patients points to the importance of differential approach in choosing this method of treatment. Of primary importance in solving this problem is thought to be the degree of the disease which must determine the variant of reduction of the splenic bloodflow.

[Roentgenoendovascular occlusion in patients with cancer of the kidneys].

Roentgenendovascular occlusion of the renal artery was used in 116 patients with renal carcinoma, in 70 of them the embolization was performed as a preoperative measure 3-12 days (7 days at an average) before nephrectomy. Embolization was also performed in 46 patients with spreaded renal carcinoma with metastases or with somatic diseases preventing surgical interventions. The roentgenendovascular occlusion reduced the time of operation and blood loss in nephrectomy, especially in patients with great hypervascularized tumors. In patients with inoperable renal carcinoma embolization was followed by arrest of hematuria, relief of pain and prevented progress of the disease.

[Spiral coils for arterial occlusion].

Roentgen endovascular occlusion with spiral emboli was used in 240 patients with different diseases. The introduction of spiral emboli into the arteries of abdominal and retroperitoneal organs was fulfilled through usual angiographic catheters. Duration of the arterial occlusion was checked up in the following angiographic examinations and showed the devices described to be appropriate for a continuous arterial blockade. No complications resulting from the introduction of the spiral embolus were noted.
