Detection of viable myocardium in segments with fixed defects on thallium-201 scintigraphy: usefulness of magnetic resonance imaging early after acute myocardial infarction.

To determine if magnetic resonance imaging (MRI) can be used to detect tissue viability in segments with persistent 201T1 defects early following acute myocardial infarction, 24 patients underwent MRI and adenosine 201T1 single photon emission computed tomography (SPECT) imaging at approximately 6 days. Infarction was demonstrated on MRI using a velocity-compensated, T2-weighted spin-echo pulse sequence. Wall thickening was assessed using a gradient-echo pulse sequence obtained in the same anatomic position. Viable myocardium was defined by MRI as a segment with increased signal intensity and preserved wall thickening. A fixed defect on the 201T1 SPECT images was defined as the absence of any redistribution 4 hours after the 201T1 infusion. Of 11 patients with redistribution on the 201T1 images in the infarction region, 10 (91%) had preserved wall thickening by MRI. Of 13 patients with fixed defects on the 201T1 images in the infarction region, 6 (46%) had preserved wall thickening by MRI. Of 7 patients with absent thickening, all had one or more segments with absent perfusion on redistribution imaging. Wall thickening tended to occur in patients who received thrombolytic therapy or who underwent revascularization procedures prior to imaging. The results of the present study suggest that spin-echo MRI with motion compensation can be used to identify viable myocardium in patients with fixed defects on 201T1 SPECT following acute myocardial infarction.

Complete heart block complicating retrograde left-heart catheterization of patients with cardiac allografts.

The development of complete heart block during left-sided cardiac catheterization is an uncommon event. We describe three cases of complete heart block complicating left-sided cardiac catheterization of patients with cardiac allografts. Review of the cases performed at our institution suggests that complete heart block during left-sided catheterization may be more common in patients with cardiac allografts.

Role of "buttoned" double-disc device in the management of atrial septal defects.

Sixteen patients seen over a 9-month period ending in August 1990 were offered transcatheter closure of their ASD with a custom-made "buttoned" double-disc device. The study was approved by the Institutional Review Board and informed consent was obtained in each case. The device consists of an occluder, a counteroccluder, and a loading wire and is delivered to the ASD site via an 8F sheath. Parents of two children elected surgical closure. In five children the stretched diameter of the ASD was too large (greater than 20 mm) and transcatheter closure was not attempted. These seven children underwent elective surgical closure without incident. In one child the defect measured 5 mm and the Qp:Qs was 1.4:1 and therefore ASD closure was not recommended. In the remaining eight children transcatheter closure was attempted. In two of the children the occluder pulled through the ASD and was successfully retrieved and the children later underwent uneventful elective surgical closure. The device was implanted across the ASD in six children. In one child the device dislodged from the ASD site within minutes after implantation and the child was sent to emergency surgery, where the device was removed and the ASD was closed. In the remaining five patients, aged 7 months to 45 years (weight 3.6 to 50 kg), with a Qp:Qs range of 1.3 to 2.3 and a stretched diameter of 10 to 19 mm, the ASD closure was successful with 25 to 40 mm size devices. Repeat echo-Doppler studies 2 weeks and 3 months after the procedure in all patients and 6 months later in two children did not reveal any residual shunt. It is concluded that (1) the custom-made "buttoned" double-disc device can be implanted across the ASD safely and effectively via an 8F sheath, thus making transcatheter ASD closure feasible even in very young infants; (2) measurement of stretched diameter of the ASD in the catheterization laboratory is a useful guide for selection of an appropriate-sized device; and (3) additional clinical trials are warranted to confirm the efficacy and safety of the device.

Diagnostic evaluation of ascitic adenosine deaminase activity in tubercular peritonitis.

Activity of adenosine deaminase (ADA) in serum and peritoneal fluid was studied prospectively in 24 aetiologically proved cases of ascites and 10 age-matched controls. Patients were divided into 3 groups according to causes of ascites, viz. malignant ascites (11), tubercular peritonitis (7) and cirrhosis of liver (6). Serum ADA values and peritoneal: serum ADA ratio did not show any consistent pattern in any group. But in patients with tubercular peritonitis ADA activity in ascitic fluid was significantly higher (P < .001) than in the other groups. An ascitic ADA level of 30 units/L had a sensitivity of 100% and specificity of 94.1% for tubercular peritonitis. These findings suggest that the ascitic fluid ADA activity is useful for the diagnosis of tubercular peritonitis; this method is simple and least invasive.

Accordion effect in tortuous right coronary arteries during percutaneous transluminal coronary angioplasty.

Coronary spasm and intimal dissection are well-known complications of coronary angioplasty with potentially serious consequences. Their treatments are different and need to be instituted quickly to prevent vessel closure. We report two cases of mechanical deformation caused by the angioplasty hardware masquerading as dissection or spasm during coronary angioplasty of tortuous native right coronary arteries.

Left ventricular dual-energy digital subtraction angiography: a motion immune digital subtraction technique.

Digital subtraction angiography (DSA) allows quantitative analysis of ventricular function via densitometric and parametric imaging techniques. However, DSA is limited by the artifacts in temporal subtraction images that result from patient and cardiac motion. Dual-energy subtraction imaging is insensitive to motion. This study evaluated the initial application of dual-energy subtraction in cardiac patients. The image quality of dual-energy subtraction left ventriculograms obtained from a pulmonary artery injection of contrast was assessed in 13 patients, ranging in weight from 54 to 100 kg. The dual-energy images were compared with left ventricular images obtained using standard left ventricular injection cine angiography. End-systolic and end-diastolic ventricular volumes calculated from the cine (C) and dual-energy (DE) images using the Area-Length method were compared. The resulting regression line was DE = 0.98 C+ 7.0 ml, and the r value was 0.987. Dual-energy subtraction provided good left ventricular visualization, free from misregistration artifacts, even during patient motion.

An efficient method for production of purified inactivated Japanese encephalitis vaccine from mouse brains.

The efficiency of production of purified Japanese encephalitis (JE) vaccine from mouse brains was increased by reprocessing the brain material after recovery of the virus and by pooling of a few extra fractions after zonal ultracentrifugation. By the routine production method, one mouse yielded approximately 2.5 doses of the vaccine while the improved method gave about five doses from each mouse. All the batches of purified JE vaccine made by improved technology passed all the quality control tests as specified by the Minimum Requirements for biological products of the Japanese Government and World Health Organization.

Spectrophotometric method for the determination of paraquat in water, grain and plant materials.

A sensitive spectrophotometric method for the determination of paraquat using ascorbic acid (an easily available reducing agent) is described. Paraquat is reduced with ascorbic acid in alkaline solution to give a blue radical ion with an absorbance maximum at 600 nm. Beer's law is obeyed in the range 12-96 micrograms of paraquat in 10 ml of the final solution (1.2-9.6 ppm). The important analytical parameters and the optimum reaction conditions were evaluated. The method was applied successfully to the determination of paraquat in water, grain and plant materials.

An outbreak of aflatoxicosis in Angora rabbits.

An outbreak of aflatoxicosis in Angora rabbits involving a large number of rabbitries was investigated. Mortality was more in weaners than in adults. Affected animals showed anorexia, dullness and weight loss followed by jaundice in terminal stages. Death occurred within 3-4 d of the appearance of clinical signs. Livers were moderately to severely congested, icteric and were hard to cut. Gall bladders were distended and had inspissated bile. Liver sections showed degenerative changes of hepatic cells along with dilatation and engorgement of sinusoids. Bile ducts had mild to severe periportal fibrosis. Focal areas of pseudolobulation and regenerative foci were also predominant. The level of aflatoxin B1 in feed samples from various farms submitted at the time of the investigation varied from 90 to 540 ug aflatoxin B1/kg of feed. Withdrawal of feed and supplementary therapy resulted in gradual disappearance of signs and mortality.

Spectrophotometric determination of acetone using vanillin.

A sensitive method is described for the spectrophotometric determination of acetone in water. Acetone reacts with vanillin in alkaline medium forming a yellow-orange dye with an absorption maximum at 430 nm. Beer's law is obeyed in the range from 0.5 to 5 p.p.m. of acetone in water. Molar absorptivity and Sandell's sensitivity were found to be 11.0 x 10(3) +/- 100 l mol-1 cm-1 and 0.005 micrograms cm-2, respectively. The important parameters for complete colour development were optimised and the method was used for the determination of acetone in biological samples. The method was also applied successfully to the determination of acetone in air after absorption in sodium hydrogen sulphite solution.

Embolization of large saccular aneurysms with Gianturco coils.

In five patients embolization of large saccular aneurysms involving the popliteal artery, common carotid artery, arch of the aorta, and abdominal aorta was performed with custom-designed spring coils. Surgery was attempted before embolization for the aneurysm involving the popliteal artery. The other aneurysms were considered nonresectable due to their critical location. Although maximal packing of the aneurysmal sac was attempted, delayed follow-up angiography revealed displacement of the coils away from the parent vessel as well as further expansion of the residual aneurysmal sac. In these cases complete embolization of the aneurysmal sac was not possible without obstructing the parent artery.

Indirect haemagglutination test for the assay of antibodies against Japanese encephalitis viral antigen in human & animal sera.

The indirect haemagglutination (IHA) test was standardized for the assay of antibodies against Japanese encephalitis (JE) virus. Glutaraldehyde fixed sheep erythrocytes were sensitized with purified and concentrated JE vaccine (200-300% brain concentration). The JE vaccine made from Nakayama-NIH strain of JE virus was purified by protamine sulphate treatment and by ultracentrifugation in a sucrose gradient. The sensitized cells were quite stable in liquid as well as in lyophilized state both at -70 degrees C and 4-8 degrees C. These cells could be used for two years without much loss (4-8 times loss) in titre. The IHA test was as sensitive as the neutralization (N) test performed by plaque reduction method in chick embryo fibroblasts. The sensitivity of the test was influenced by the source of erythrocytes i.e., from the different sheep from which these were drawn. After standardization of the test, 16 human sera and 18 sera of immunized mice were assayed for antibodies against JE virus by N and IHA tests. There were no significant differences between titres of both human and mice sera determined by N and IHA tests (P greater than 0.05). The correlation coefficient between N and IHA titres for human sera was 0.60 (P less than 0.05) and for mice sera 0.82 (P less than 0.01). The IHA test has been found to be very simple, inexpensive, sensitive and reproducible.
