[Medical rehabilitation of patients after amputation of the leg in sarcoma].

An analysis of data of 348 patients with malignant tumors of soft tissues has shown that 128 of them had been subjected to the amputation of extremities at different levels. The use of microsurgical techniques favours the improvement of the method of amputation of lower extremities, in particular the transplantation of a calcaneoplantar flap onto the stump end. In order to make the treatment of phantom limb pains more effective acupuncture should be included into the complex of therapeutic measures in addition to traditional methods. Close contacts with prostheses-makers can facilitate earlier prosthesis which can give positive effects on the following rehabilitation of such patients.

[Liposarcoma of the retroperitoneal space].

Date of an examination of 60 patients with liposarcomas of the retroperitoneal space aged from 40 to 60 are presented, women comprised 62%, men 38%. Operative treatment was undertaken in 48 patients (96%), radical operation--in 82%, palliative operations--in 14%. Recurrent tumors were revealed in 33 patients (66%), metastases--in 8%. Most frequent recidivations were noted in myxoid liposarcomas. A complete removal of the recurrent tumor was made in 72% of the cases. Out of 50 patients with liposarcomas 54.2% of the patients survived 5 or more years after the operation, in the myxoid variant--72.3%, in polymorphocellular form--23.1%.

[Treatment of soft tissue sarcomas of the leg].

An analysis of data of 548 patients with different diseases of soft tissues of lower extremities after using a complex of diagnostic measures (clinical, radionuclide data, thermography, puncture, biopsy) has shown that the number of erroneous conclusions may be minimized to 2%. Modern advances of plastic surgery can widen indications for preserving operations in malignant tumors of soft tissues of lower extremities without prejudice to the main principle of oncology concerning its fascial character. Combined treatment can also improve the results.

[Reoperations in retroperitoneal tumors of non-organic origin].

Great, solid and fixed tumors should not always be considered as a reason for refusal of an operative treatment of patients with retroperitoneal tumors of nonorganic origin. Resection of such tumors especially those of benign genesis results in a complete recovery of the patients and their rehabilitation. The appearance of a recurrent retroperitoneal tumor of nonorganic origin should not be regarded as an inoperable condition. The timely diagnosis and operative treatment of patients with recurrences give three times better remote results of the treatment.

[Treatment of patients with soft tissue neoplasms].

The methods of treatment of 1,410 cases of soft tissue sarcoma were compared. Tumor was less than 5 cm only in 19.4% of patients admitted to the hospital by the beginning of treatment. Radical surgery was performed in 65.3% of cases, it still remaining the major procedure of treatment. Application of combined (surgery + radiation) treatment is increasingly used, every fourth patient (25.1%) being given this therapy. The percentage of radically-treated cases of soft tissue sarcoma may be raised by 15% unless patients refuse to be operated on.

[Rehabilitation of patients with cancer of the proximal section of the stomach].

A clinical and socio-psychological study showed that more than 50% of patients with cancer of the proximal part of the stomach may be rehabilitated following radical treatment (surgery or radiation plus surgery). Large contingents of cancer patients may return to work if a number of requirements e. g. provision of regular meals at the place of work, sparing diet, reduced working hours, healthy psychological climate, etc. are met.

Combined Modality Therapy↗

[Comparative evaluation of surgical and combined treatment of cancer of the proximal region of the stomach].

The paper deals with an analysis of the results of treatment of 351 cases of radical surgery for cancer of the proximal part of the stomach (surgery-303, surgery + preoperative large-fractionated radiation-48). It was found that radiation was not followed by increased blood loss during operation nor did it interfere with application of any surgical procedure. Concentrated irradiation did not involve an increment in postoperative lethality and complication rates. Five-year survival rates were 37.4 +/- 8.0 in cases of combined treatment and 20.4 +/- 2.7% in those of surgery. The improvement in survival rates recorded in cases of combined treatment was largely due to a decreased incidence of local recurrences.

[Non-organ tumors of the lesser pelvis].

An analysis of the author's findings showed a correlation between clinical manifestations of tumor, on the one hand, and its growth rate, size and, to a lesser degree, morphological pattern, on the other. Large size of tumor should not necessarily be interpreted as an indication of inoperability. A better 5-year survival following surgery in benign tumor group (82.1%) as compared with malignant tumors (24.5%) was confirmed.

[Interiliac-abdominal amputation in sarcoma of the bones and soft tissues of the pelvic girdle].

The interilio-abdominal amputation was fulfilled in 62 patients for the period from 1960 to 1979. The postoperative mortality was 6,4%. General five-year survival was 24,3% +/- 5,6. The survival terms were dependent first of all on the tumor type. The advances of anesthesiology, reanimatology and operative oncology have considerably reduced the danger of interilio-abdominal amputation. So, indications for the operation can become wider even in the cases when it has a palliative character.

[Combined (radiation and surgical) treatment of esophageal cancer].

The article gives an analysis of results of the combined (radiation and surgery) treatment of carcinoma of the oesophagus. The results are estimated on the basis of an analysis of postoperative complications and survival of the patients during 3 and 5 years. The authors consider that the combined treatment does not only not complicate the operative intervention and postoperative management of the patients but also improves the remote results.

[Primary and recurrent retroperitoneal neoplasms of non-organic origin].

Having analyzed results of the treatment of 216 patients the authors came to a conclusion that recurrencies of tumors, repeat ones included, can not be considered as contraindications for operations. The most favourable results were obtained in the operative treatment of recurrent lymphosarcomas.
