[Diagnosis of internal endometriosis of the uterus].

The authors analyze the informative value of instrumental methods of examination (hysteroscopy, hysterosalpingography, ultrasonic scanning) and measurements of blood phosphatidyl inosites in 50 patients with internal endometriosis and come to a conclusion that it is in proportion with the disease severity, hysteroscopy being the most informative. A reliable reduction (p < 0.001) of blood phosphatidyl inosites were seen in endometriosis patients as against normal subjects. Combined examinations making use of instrumental methods and blood phosphatidylinosite measurements essentially improve the accuracy of internal endometriosis diagnosis and help develop a rational treatment strategy.

[The transplacental passage and distribution of ceftazidime in the fetal and maternal organs in a 2nd-trimester abortion].

Transplacental passage of ceftazidime was studied in 17 women with second-trimester pregnancy which had to be discontinued for medical causes. A single intramuscular injection of the antibiotic resulted in creation of therapeutically active concentrations in fetal blood and organs, amniotic fluid, and placenta. Transplacental passage of ceftazidime from the mother to the fetus was 19.12%, diffusions to the amniotic fluid 14.46%, diffusion to the placenta 48.46%. Ceftazidime may be effective for the prevention and therapy of fetal intrauterine infection and chorioamnionitis.

[Changes in the phosphoinositide metabolism in the blood and tissues of benign and malignant uterine tumors].

Measurements of phosphoinositide levels in the blood, immunocompetent cells, and tumors of 105 patients with uterine myomas, 24 patients with cancer of the corpus uteri, and 17 ones with uterine sarcoma showed that the parameters of phosphoinositide metabolism in the blood of patients with tumors of the uterus reliably differed from those in healthy women. The content of phosphatidylinosites and other phosphoinositide fractions in patients with uterine myomas reliably differed from those in patients with malignant tumors of the uterus, this permitting the use of such measurements in the differential diagnosis. Phosphoinositide mechanism of development of tumors of the uterus is discussed, which is related to the "new" phosphoinositides and secondary messengers directly participating in transfer of cell growth signals.

[Reconstructive plastic operations on the cervix uteri using preserved tissues].

The authors analyze the results of surgical treatment of 57 patients with third-degree ruptures of the uterus and high cervicovaginal fistulae operated on with the use of preserved lyophilized human dura mater. Control group consisted of 56 patients with second and third-degree ruptures of the uterus and cervicovaginal fistulae operated on by routine methods. Use of dura mater as a biologic transplant for plastic repair of the cervix uteri in the above-mentioned patient population improved the efficacy of treatment of this condition: good results were attained in 52 patients, satisfactory in 4, and were poor in only 1 patient.

[An alternative approach to the surgical correction on malpositions of the uterus].

Endosurgical correction of the uterus was carried out in 57 patients. Indications for surgery were improper positions of the uterus-retroversio, retrodeviatio uteri, excessive mobility of the uterus, and complaints of heaviness at the bottom of the abdomen, leukorrhea, pain of different type in the lower portions of the abdomen, profuse and long menses, dyspareunic pain upon deep penetration, reduced libido, a sensation of a foreign body in the vaginal cleft, urine incontinence upon strain. The mean age of the patients was 35 +/- 3 years. The disease duration varied from 1 to 8-9 years. In 9 women endosurgical correction of the uterus was combined with other interventions on the pelvic organs: colpoperine-orrhaphy with levator plasty was carried out in 5, laparoscopic myomectomy in 2, and dissection of intraperitoneal adhesions in 2 patients. In addition, surgical sterilization was carried out in 54 patients. After surgery the patients were administered only nonnarcotic analgetics. All the patients were discharged on days 2-3, those subjected to combined treatment on days 8-9. The patients were followed up for up to 2 years. The general and psychoemotional status of the majority of patients improved, and the incidence of such symptoms as leukorrhea, pain at the bottom of the abdomen, profuse and long menses was 4, 6, and 5 times reduced, respectively. Endosurgical correction of the uterus in women of a reproductive age should be a method of choice; it can be carried out separately or in combination with plasty of the vaginal walls.

[Diagnostic value of cardiac monitoring in women with threatened fetal distress].

Use of cardiotocography as the fetal status index in complex with other criteria and medical algorithms in pregnant women during the IIIrd trimester helped better select the term and method of delivery for each patient with threatened fetal distress. The choice of the optimal individual method of delivery for each pregnant woman decreased the risk of the process of labor and reduced by 2.2 times the number of children born in a state of complicated hypoxia.

[Clinical-physiological characteristics and hormone therapy of patients with typical form of climacteric syndrome].

Clinical and physiological examinations of 76 women in the postmenopause revealed in 64 of them the typical form of the climacteric syndrome, characterized by not only typical symptoms ("flushes", increased exudation, headaches, etc.), but also by various emotional vegetative disturbances detected at profound clinical neurological examination of the autonomic nervous system and the emotional sphere. Individual hormone therapy was associated with an appreciable alleviation of all psychovegetative disorders: "flushes", exudation, irritability, arterial hypertension, severity of vegetative dystonia, hysterical stigmata.
