[Prevention of hemorrhage in women with retroperitoneal pelvic tumors by embolization of the internal iliac arteries].

Presents data on the clinical course and surgical treatment of 26 patients with retroperitoneal tumors and hemangiomas of the pelvis. The diagnosis was confirmed by x-ray, echographic, computer tomographic, and angiographic data on pelvic vessels. To prevent bleedings, embolization of the internal iliac arteries and their branches as far as the 3-4th order was carried out in 9 patients with angiodysplasias and 4 with retroperitoneal pelvic tumors. Endovascular occlusion of the main vessels not only permitted surgery with the minimal blood loss, but was conducive to better visualization of the tissues and made the operation less traumatic.

[The prospects for using new suture materials in emergency gynecology].

The results of using new bactericidal suturing material, Etikon (Germany), in 534 urgent surgical gynecological interventions are analyzed. Employment of new dissolving surgical threads helped reduce threefold the incidence of postoperative complications and almost 1.5-times the mean hospital stay after surgery. A favorable course of the postoperative period is explained by a more swift healing of the operation wound and reduction of the rate of postoperative complications.

[Ways of reducing perinatal morbidity and mortality due to hereditary pathology].

The paper presents different ways of preventing the perinatal morbidity and mortality due to genetically predisposed abnormalities, such as environmental protection; definition of the factors having a mutagenic influence on the female; family planning, medico-genetic advisory. It also outlines the methods of perinatal diagnosis, which is now the most effective tool for birth prophylaxis of a baby with a hereditary abnormality.

[Characteristics of surgical treatment of pathologic conditions of the cervical stump].

The paper gives the results of surgical management in 65 patients with cervical stump diseases. According to the pattern of the disease, 4 groups of patients were identified: 1) those with inflammatory diseases, 2) those with endometriosis, 3) those with myoma, and 4) those with prolapse of the cervical stump. Low-quality and inadequate suture material was found to be a cause of inflammatory diseases of the cervical stump. It is concluded that it is not expedient to use capron, lavsan, and silk to suture the cervical stump during supravaginal amputation of the uterus. Catgut, vicryl, debone were shown to be an optimal suture material. The practice of surgical management of patients with inflammatory diseases, endometriosis, cervical stump myoma and prolapse indicates that it is advisable to extend indications for uterine extirpation.

[Functional evaluation of the effectiveness of experimental reconstructive and plastic operations on the uterine horns].

In the given experimental model, tubo-plastic surgery was done with the use of locally produced fibrin glue (Fk-1), and commercial product "Beriplast" (FRG). In the control group microsurgical technique was adopted in the uterine horn of rats. It was proved that Fibrin glue reduces the frequency of post operative adhesion formation in the area of anastomosis and the chance of pregnancy is increased in comparison to microsurgical techniques.

[Use of fibrin glue in gynecologic operations].

Anastomosing of the uterine horns in 98 experimental rats has employed a fibrin glue FK-1 (USSR) and Beriplast (FRG) or microsurgical techniques in a control group. Intactness and sealing of the anastomoses and the incidence of pelvic adhesions were assessed at 5 minutes, 1 hour and 1, 2, 3, 4, 5, and 6 days following the operation. The fibrin glue Beriplast was used clinically in 6 patients during reconstructive-plastic operations. The results with seamless anastomosing of uterine horns in rats using the fibrin glue and preliminary clinical evidence suggests a high effectiveness of the glue in gynecologic reconstructive plastic surgery.

[Experience with the use of the CO2 laser in reconstructive and plastic gynecologic surgery in patients with uterine myoma and endometriosis].

CO2 laser has been used during reconstructive plastic surgery for uterine myoma, endometriosis of various locations, and associated gynecologic conditions in 71 patients. Comparison in patients with uterine myoma who underwent conservative myomectomy with and without the use of CO2 laser showed that treatment with CO2 laser diminished blood loss and duration of the operation, and improved postoperative progress.

[Combined treatment of retrocervical, cervical and vaginal endometriosis using CO2 laser, cryosurgery and electrocoagulation].

Clinical data and results of combined treatment have been analyzed in 163 patients with common types of endometriosis: retrocervical in 116, cervical in 43 and vaginal in 5 patients. Growth of retrocervical endometriosis into the vaginal wall with cyst formation was seen in 21 patients. Conventional surgical procedures were adjuncted by physical treatments of endometriosis foci or beds in 116 of 163 patients: electrocoagulation (n = 44), cryodestruction (n = 43), focal vaporization with a CO2 laser (n = 29). Only surgical methods with subsequent hormonal therapy were used in 47 patients who made a control group. The results showed that the additional cryo-, electro- and laser surgery considerably reduced the incidence of complaints and relapses. CO2 laser treatment had a significantly higher efficacy.

[Status of the problem of spontaneous abortion].

Findings of research into the problem of recurrent spontaneous abortion that has been conducted in major obstetric-gynecological clinics of this country in the recent five-year period are analysed. Risk factors of spontaneous abortion and preterm labour are specified, and the influences of environmental effects, pathogenesis, diagnosis, management, and prognosis are evaluated. Prospects of research in this field are outlined.

[Abortion as a high risk factor in perinatal pathology].

Analysis of the pregnancy course and neonatal outcomes in 896 women with a history of recurrent spontaneous abortion showed a high rate of perinatal disease and neonatal problems. This is underlain by hormonal, immunological, and some other disorders, and also by a high infection level both during pregnancy and without it. To reduce the perinatal morbidity rate, a system of the reproductive function check-up and rehabilitation for the non-pregnant and a stage-by-stage control over the pregnancy course and the status of the fetoplacental unit was suggested.

[Reflex labor induction in premature labor complicated by premature rupture of fetal membranes].

Labor induction in impending premature delivery complicated by premature rupture of the membranes is controversial. Problems arise both with choice of a mode of delivery and induction manoeuvres to be used in biologically premature delivery. The paper addresses the results with a reflex method of labor induction based on transcutaneous electroneurostimulation (TENS). An appropriate TENS regimen was found to be a highly efficient method of labor induction allowing one to reduce the incidence of perinatal complications.

[The prostaglandin E content of the blood serum in mother and fetus during the reflex regulation of premature labor].

Prostaglandin (PG) biosynthesis is promoted by enzyme activation induced by neurohormonal regulation, an important determinant of labor. However, contribution of PG to premature labor is largely uncertain. Maternal and fetal blood PGE levels have been determined during the reflex regulation of premature labor in 39 women and in 20 patients with spontaneous labor. The reflex regulation was performed as transcutaneous suprapubic and sacral electric stimulation. Comparison of PGE levels in the women and their newborns led to the conclusion that the reflex regulation of premature labour has modulating effects on PGE biosynthesis both in the mother and fetus, thereby modifying progress of labor.
