[Delayed respiratory arrest following spinal anesthesia].

A 71 year-old woman was admitted for transurethral resection of bladder carcinoma. The procedure was performed under spinal anaesthesia with mepivacaine 4% hyperbaric. The procedure was carried out without any technical problems. Despite appropriate positioning the patients spinal anaesthesia spread slowly cephalad. 60 minutes after injection of the local anaesthetic aphonia and respiratory insufficiency occurred. The patient was intubated and controlled ventilation was started. 4 hours later the tube could be removed again. 6.5 hours after performing the block no residual block could be detected. The authors discuss the aetiology of this rare complication. It is recommended to keep all patients under continuous anaesthesiologic supervision where the spread of spinal anaesthesia is not definitely declining.

[Concentrations and levels of activity of important serum protein components in preserved serum].

The content of transport proteins, immune globulins, components of the complement system and protease inhibitors were estimated in two different plasma preparations, using bioassays by chromogenic substrates and laser nephelometry. The content of proteins under consideration was found about 50% less than in untreated human plasma. Some proteins such as antithrombin III and factor XIII are missing. The results are discussed in respect to clinical used of plasma preparations.

Influence of parenteral fat administration on the pulmonary vascular system in man.

In 16 patients with multiple injuries the effect of parenteral administration of a fat emulsion on the pulmonary vascular system has been studied. Using the Hewlett Packard 5600 A Intensive Care System, which allows vital parameters to be sampled at a rate of two values per min, pulmonary artery pressure (PAP) was monitored before, during, and after the administration of Lipofundin S 20% (0.24 g/kg/h). In addition right ventricular stroke work index (RVSWI), alveolar-arterial oxygen difference (A-aDO2) and pulmonary shunt (Qs/Qt) were calculated. The results indicate that there are no changes of the PAP, the RVSWI, and the A-aDO2 during the period of observation.

[The Mancao cannula--a new system for the facilitation of peridural puncture].

A cannula system for epidural puncture is presented, that has been developed in order to prevent the risk of dural puncture and nerve damage and allows the epidural catheter to be withdrawn without the risk of tearing or cutting off the end. The cannula system ("Mancao-cannula") consists of a sharp outer and a blunt inner needle with a lateral aperture. First experiences in 400 epidural anaesthesias using the Mancao cannula are reported.

[Experiences with caudal anaesthesias in paediatric surgery (author's transl)].

The results of more than hundred caudal anaesthesias in surgical paediatric urology in children aged one to twelve years are reported. Modifying the method of Schulte-Steinberg, we used Bupivacain - CO2 0.5%, and attained sufficient analgesia in all cases for whatever surgery applied. There were no complications except in one case where an overdose had been applied. We feel that the postoperative period is easier to bear because of the longer lasting local analgesia. Earlier onset of oral food intake is possible.

[Parenteral nutrition in polytrauma - comparison between a fat-free and a fat-containing nutritional regime].

Two groups of patients with multiple injuries were treated with high-caloric parenteral nutrition over a period of 10 days. One group received amino acids (1 g/kg BW/day) and dextrose (2.0 x resting metabolic rate calories). In the other group dextrose was partially substituted by an isocaloric amount of lipids (2 g/kg BW/day). As for blood and serum glucose, pyruvate, lactate, triglycerides, cholesterol, phosphatides, free fatty acids, glycerol, acetoacetate and beta-hydroxybutyrate there were no significant differences between the two groups. Nevertheless, in the lipid group more balanced blood-sugar levels were seen with smaller amounts of insulin to be needed. This suggests a beneficial effect on the carbohydrate metabolism.

Isobutene formation during halothane anaesthesia in man.

The concentrations of volatile hydrocarbons were studied in breath samples from patients before and after anaesthesia. In patients to whom halothane was administered, there was a significant increase in the concentration of isobutene (from 0.43 +/- 0.09 to 4.66 +/- 2.22 nmol litre-1). There was no change in the concentration of isobutene in six patients anaesthetized with ketamine and diazepam. The concentrations of ethane and pentane were not altered significantly in either group.

[Laryngoscope with suction tube (author's transl)].

A laryngoscope blade with an integrated suction tube is described. Suction and laryngoscope can be handled simultaneously by one hand. The suction guarantees a free sight during intubation in cases of bleeding in the nasopharyngeal region and may also prevent the aspiration of acid gastric contents.

[Treatment of acute renal failure by hemofiltration (author's transl)].

Hemofiltration as an alternative to haemodialysis in the treatment of acute renal failure is described. The "Hämoprocessor" (Sartorius Membranfilter GmbH, Göttingen) as an computerized apparatus for haemofiltration is presented. First experiences with the clinical application of haemofiltration are reported, and the advantages and disadvantages are discussed.

[On the abortive and teratogenous action of volatile and gaseous anaesthetis agents (author's transl)].

The abortive and teratogenic effect of inhalational anaesthetics is well established. This cannot be explained by metabolic activation of the halogenated hydrocarbons but could be due to a noxious effect on the mitotoc spindle. The inhibition of mitosis is correlated with the lipid solubility and the anaesthetic effect. Numerous experiments give evidence that the effect of anaesthetics can be explained by their reversible binding to nonpolar sites of protein by Van der Waals' bonds affecting protein conformation. The tubular protein of mitotic spindle seems to be highly sensitive to this change of conformation. To prevent abortion and miscarriage in female operating room personnel it is essential not only to remove the halogenated compunds from room atmosphere but also all other anaesthetic gases and vapors such as nitrous oxide and diethyl ether.
