Naevocytic naevi with vascular-like spaces.

Fourteen cases of naevocytic naevi with anastomising lacunae suggestive of vascular spaces are reported. The cells lining these lacunae were consistent with naevus cells, being positive for vimentin and S100 protein and negative for factor VIII-related antigen and Ulex europaeus I. The cells were not surrounded by laminin or type IV collagen. We suggest the formation of these vascular-like spaces may be due to defective production or increased degradation of components of the basement membrane with a consequent lack of cohesion.

Primary rhabdomyosarcoma of the ileum in an adult.

We report a case of primary rhabdomyosarcoma of the ileum occurring in a 45-year-old man. The rhabdomyoblastic nature was immunohistochemically confirmed by positivity with anti-myoglobin and anti-striated actin antisera. We discuss the differential diagnosis with similar tumors. To our knowledge, ileal rhabdomyosarcomas have not been previously reported in adults.

Calcitonin free oat-cell carcinoma of the thyroid gland.

Two cases of primary oat-cell carcinoma of thyroid, in a 63-year-old woman and a 73-year-old man, are described. Case 1 was a compound tumour with the oat-cell component merging with a papillary component. Both tumours, in addition to histological features consistent with oat-cell carcinoma, showed immunohistochemical positivity with anti-chromagranin A and anti-synaptophysin antisera. Negative results were obtained when anti-calcitonin and anti-thyroglobulin antisera were employed. Using in situ hybridization, chromogranin A and B messenger RNAs were localized with biotinylated oligonucleotide probes. In contrast, with in situ hybridization, no localization for calcitonin messenger RNA was seen using radioactive and biotinylated probes. It is concluded that these calcitonin-free, small-cell carcinomas should be considered separately from medullary thyroid carcinomas and be regarded as a distinct entity, probably the thyroid equivalent of oat-cell carcinomas of the lung.

Diabetic fibrous mastopathy. Report of two cases.

Two cases of a characteristic form of fibrous mastopathy associated with type I diabetes mellitus are described. Well-circumscribed nests of mature lymphocytes in a hyaline stroma are the hallmark of this condition. In one case mastectomy was performed, as the lesion had been interpreted as malignant on frozen section. It appears that this form of "idiopathic mastopathy" deserves wider recognition.

Squamous cell carcinoma with prominent myxoid stroma.

Three cases of a squamous cell carcinoma with a prominent myxoid stroma are reported. One case in a 70-year-old woman had presented as a lump in the breast, the other two presented as polypoid lesions of the larynx and cervix uteri in a 65-year-old man and 61-year-old woman, respectively. The carcinomatous component was immersed in abundant extracellular mucosubstances. In addition to occasional squamous pearls, it displayed immunocytochemical evidence of high-weight keratin present in the neoplastic cells and, in one case, desmosomal attachment under electron microscopy. The extracellular mucosubstances proved to be similar to those seen in connective tissue. The differential diagnosis with histologically similar lesions has been taken into consideration, and it has been suggested that this newly described entity showing abundant myxoid stroma has to be distinguished from numerous benign and malignant myxoid soft tissue tumors.

Keratin-positive epithelioid angiosarcoma of thyroid. A report of four cases.

We present four cases of a malignant thyroid tumor showing morphologic, immunocytochemical, and ultrastructural features of endothelial cell differentiation. The tumor cells had epithelioid features and displayed strong immunoreactivity for keratin. There was no evidence of follicular or C-cell differentiation in any instance. We interpreted these cases as keratin-positive epithelioid angiosarcomas. The findings presented here support the existence of primary malignant vascular tumors in the thyroid even in the presence of keratin positivity, a marker traditionally regarded as indicative of epithelial differentiation.

Primary malignant lymphoma of sciatic nerve. Report of a case.

A 72-year-old man with a 2-year history of motor-sensory neuropathy of the right foot was found to have a lymphoma involving a 50-cm length of the sciatic nerve. This occurred in the absence of any other evidence of disease by detailed clinical staging. The lymphoma was of large follicular center-cell type. The cells strongly expressed a B-cell marker detected by the 4KB5 monoclonal antibody and selectively infiltrated the nerve bundles, dissociating preexisting myelin-producing Schwann cells and axons. This is the second report of similar localization. Primary selective involvement of a nerve is a rare mechanism of peripheral neuropathy in lymphoproliferative disorders, to be added to systemic dissemination of lymphomas and leukemias, direct spread of an adjacent tumor, and immunologically mediated disease.

Nucleolar organizer regions in normal skeletal muscle and benign and malignant rhabdomyoblastic tumors.

The silver staining of interphase nucleolar organizer regions (NORs) has been shown to have an important application in diagnostic histopathology for distinguishing some benign from malignant conditions. In this study, normal fetal and adult skeletal muscles and tissue from fetal and adult rhabdomyomas as well as rhabdomyosarcomas were stained with the silver method for NORs. The morphologic distribution of NORs in rhabdomyosarcomas was found to be very different from that in normal skeletal muscles. In addition, cases of rhabdomyoma were easily differentiated from rhabdomyosarcomas. Statistical analysis of data, from all cases, regarding the diameter of NORs and number per nucleus confirmed these observations.

Primary sarcomatoid carcinoma of the adrenal gland. First case report.

The first case of a sarcomatoid carcinoma of the adrenal gland is reported. The patient, a 68-year old woman, developed a recurrence two months after presentation and died of the disease 7 months later with multiple metastases. The differential diagnosis of this entity is discussed and its aggressive behaviour is emphasized.

Alveolar soft part sarcoma of the cervix uteri. A case report.

A case of alveolar soft part sarcoma (ASPS) of the cervix uteri of a 32-year old woman is described. Its differential diagnosis from histologically similar tumours is discussed. ASPS has been reported in the cervix uteri in one instance only in the literature. In spite of their unusual localization, these cases have to be recognized in order to avoid improper treatment.

Pleomorphic adenoma with lymph-node metastases report of two cases.

Two cases of pleomorphic adenoma showing morphologically benign metastases to lymph nodes are reported. In the first case, a 26-year old woman, the lymph node metastases appeared nine years after the primary tumor. In the second case, an 8-year old boy, six years elapsed between the primary and metastatic lesion. A review of the literature of similar cases has been performed.

Neuroendocrine differentiation in carcinomas of the breast: a study of 51 cases.

A group of human breast carcinomas shows morphologic and histochemical evidence of neuroendocrine (NE) differentiation. This study presents a structural, immunologic, and electron microscopic analysis of 51 cases in order to establish positive criteria for identification of these tumors, their incidence, variants, and biological behavior. Argyrophilia (by the Grimelius procedure), presence of chromogranin A and/or B, and of synaptophysin are the most reliable histochemical features, correlating with the ultrastructural demonstration of dense-core secretory granules and of clear vesicles of the synaptic type. Structural features alone may be suggestive, but do not prove NE differentiation, which has to be established by additional techniques. Seven histologic types were identified, but those herein described as types A, B, and C, which show cohesive, mucoid, and mixed patterns, respectively, comprise the vast majority of the tumors. Rare NE carcinomas of the breast show structural similarities to Merkel cell and oat cell carcinomas, and behave as highly aggressive tumors. Type B (mucinous) tumors proved to be relatively indolent, while the most frequent types of endocrine tumors (types A and C) have an intermediate grade of malignancy. The analysis of a consecutive series of 100 cases of breast cancer indicates that about 8% of breast carcinomas display some degree of NE differentiation.

Long-term follow-up of in situ carcinoma of the breast with special emphasis on clinging carcinoma.

Forty-two cases of in situ duct (28 cases) and lobular (14 cases) carcinoma were identified after a review of 4,397 "benign" breast biopsies obtained from the files of departments of pathology of a small area of Northern Italy, between 1965 and 1971. None of the patients with in situ lobular carcinoma developed an invasive carcinoma. On the other hand, three patients with in situ duct carcinoma (one comedo and two clinging type) developed subsequent invasive carcinoma. Therefore, it appears that clinging carcinoma has the same biological behavior as other types of small in situ ductal carcinoma; the likelihood of our patients developing invasive breast carcinoma is four times greater than that of the general population.

Omental autoimplantation of splenic tissue and intravenous pneumococcal challenge. A comparative study in rats.

Splenectomized subjects show a higher incidence of myocardial ischemia and of Overwhelming Post-Splenectomy Infection (OPSI). It is doubtful that implanted splenic tissue guarantees an adequate protection from OPSI. The histological characteristics and the capacity of protection from OPSI of two models of omental autoimplantation of splenic tissue in rats were examined. The implanted splenic tissue offers a significant protection from pneumococcal sepsis, even though there is no relation between implant architecture and survival rate.

Carcinoma in situ in sclerosing adenosis of the breast: an immunocytochemical study.

In this report, seven cases of sclerosing adenosis of the breast are reported in which a lobular (5) or ductal (2) in situ carcinoma was also present. In two cases, mastectomy was performed because invasive carcinoma was diagnosed on frozen section. In the other cases, the referring pathologists initially considered the diagnosis of invasive carcinoma. A well-defined PAS-positive basal lamina (BL) was always present around the proliferating glandular structures. In addition, residual myoepithelial cells lying on the BL were immunologically demonstrated, a feature usually seen in in situ carcinomas. Therefore, it appears that the study of BL and myoepithelial cells should be added to pattern recognition in the diagnosis of these lesions.

Hypophosphatemic oncogenic osteomalacia: report of three new cases.

Three cases of connective tissue tumors causing hypophosphatemic osteomalacia are reported and the literature is reviewed. In two of our patients the tumors were completely excised with total disappearance of the symptoms. In one case a total excision was not possible and the symptoms of this patient have not completely disappeared. The substance responsible for the syndrome has not been identified yet, but probably interferes with vitamin D renal hydroxylation, thus causing osteomalacia. As more than 30 per cent of cases of this condition have been reported in the last 5 years, it is suggested that these tumors are more frequent than previously believed.

Alveolar soft part sarcoma: immunological evidence of rhabdomyoblastic differentiation.

Two cases of alveolar soft part sarcoma have been studied immunocytochemically using antisera against epithelial membrane antigen, lysozyme, keratins, S-100 protein, desmin, vimentin, fetal myosin, slow myosin, alpha-skeletal muscle actin, alpha-smooth muscle actin and myoglobin. The neoplastic cells were negative with all antisera employed with the exception of the alpha-skeletal muscle actin antiserum which stained the cytoplasm of numerous neoplastic elements, including the crystalloid rods, typical cytoplasmic inclusions of these tumours. It is suggested that the presence of this protein indicates rhabdomyoblastic differentiation of these tumours.
