Morpho-functional differentiation in lobular carcinoma of the breast.

Lobular carcinoma of the breast has been studied using histochemical methods for mucosubstances; immunocytochemical methods for casein and actin; the ruthenium red electronycytochemical method for acid glycoproteins and an immunoelectroncytochemical method for casein. Mucosubstances and casein showed a similar cytoplasmic localization, but casein production was much more intense and also showed a more diffuse cytoplasmic localization. Occasionally casein assumed the form of target-like 'inclusions' as seen characteristically with the mucosubstances. The neoplastic cells were not stained by antisera against actin. Ultrastructurally, some cells showed an intracytoplasmic lumen with microvilli and/or an irregular outline at one extremity which was covered by microvilli. An electron-dense 'fuzz' and casein coated the microvilli of cells exposed respectively to ruthenium red and an anticasein serum followed by peroxidase--anti-peroxidase complexes. It is concluded that lobular carcinoma shows evidence of epithelial rather than myoepithelial differentiation with the emphasis on epithelial secretory cells engaged in intensive milk protein production. All 10 tumours tested for oestrogen receptors were positive in contradistinction to ductal carcinoma with a lower incidence of positivity. It appears that, in addition to distinctive histological and histochemical features, lobular carcinoma has an almost constant endocrine pattern in respect of its oestrogen receptor content.

Vascular infiltration in benign breast disease.

Infiltration of the walls of blood vessels by mammary epithelium was found in two cases of sclerosing adenosis. Survey of a series of cases of sclerosing adenosis and severe epitheliosis revealed a 10 per cent incidence of vascular infiltration. Usually only a single vein was involved but in the propositus an artery and two veins were infiltrated. The findings highlight the infiltrative propensities of sclerosing adenosis, already recognised as infiltrating nerve sheaths. The vascular lesions appear to be benign structurally and this is borne out by the clinical behaviour.

Multiple pigmented naevi in an albino.

A case of pigmented naevi arising in an albino is described. The histological appearance of a naevus is described in the nature of the pigment discussed.

Oat-cell carcinoma of the oesophagus: a recently recognized entity.

A primary oat-cell carcinoma of the oesophagus is described with an adjacent squamous carcinoma, both parts having characteristic ultrastructure. The origin of this mixed tumour is discussed together with the literature on oesophageal carcinomas having an oat-cell pattern.

Rhabdomyoma of the vagina.

A case of rhabdomyoma of the vagina in a 47-year-old woman is reported. This newly recognised entity is distinct from sarcoma botryoides and the socalled "benign vaginal polyp".

High-metastatic clones selected in vitro from a recent spontaneous BALB/c mammary adenocarcinoma cell line.

The metastatic TS/A line has been recently derived from a spontaneous BALB/c mammary tumor. When TS/A cells were cultured in 0.33 per cent agar, two morphologically distinct types of colonies were observed from which two sets of clones were obtained. E clones were derived from small, transparent colonies, whereas F clones were from large, thick, actively growing colonies. All the clones were tumorigenic in syngeneic BALB/c females. However, E clones showed higher ability than F clones to metastasize spontaneously to the lung. Comparison between E and F clones shows that the high level of spontaneous metastasization to the lung is associated with epithelial-like in vitro growth pattern, spontaneous dome formation and growth pattern in 0.33 per cent agar cultures. The ability to give rise to lung colonies following intravenous inoculation is not a predictive parameter for the spontaneous metastatic potential.

Myoepithelial cells in lobular carcinoma in situ of the breast: a parallel immunocytochemical and ultrastructural study.

The distribution and morphology of myoepithelial cells in one case of normal breast and in 4 cases of lobular carcinoma in situ (LCIS) were studed by immunoperoxidase and immunofluorescence with anti-actin antisera and by electron microscopy. The actin-rich cells appeared to lay flattened on the basement membrane, or perpendicular to it, or appeared dislodged toward the center of the neoplastic ductules in the different cases. It is proposed that the presence of actin-rich cells inside the neoplastic aggregates represents a process of colonization of lobular carcinoma by myoepithelial cells dislodged from the basement membrane; a differentiation of some neoplastic cells toward myoepithelial elements might also be taken into consideration. The features here described could identify a preinvasive stage of LCIS.

Ultrastructural features of neuroendocrine differentiated carcinomas of the breast.

The ultrastructural patterns of neuroendocrine (NE) differentiated breast carcinomas are analyzed and discussed. Reports in the literature describe wide variations in the size of observed dense-core membrane-bound granules and discrepancies in their interpretation. In the present study 24 cases of breast carcinoma with recognized morphologic, histochemical, and immunocytochemical features of NE tumors were investigated. Five different types of dense-core granules of neurosecretory (NS) type (confirmed by the ultrastructural localization of chromogranin A) and five different cell types were recognized. Some amphicrine cells were found to contain both mucin and NS granules. Another notable ultrastructural feature of breast NE carcinomas was the presence of clear vesicles of presynaptic type, which correlated with expression of synaptophysin.

Diagnostic value of silver-stained interphasic nucleolar organizer regions in breast tumors.

Seventy-six specimens of normal breast tissue and benign and malignant breast lesions were studied to assess the mean area occupied by silver-stained proteins of the nucleolar organizer regions (MNORA) of the nucleolus. The assessment was performed with a computer-assisted image analyzer. The results indicate that only 30% of malignant lesions have a MNORA value greater than that of normal breast tissue or benign lesions. On the other hand, MNORA values of ductal carcinoma in situ were significantly greater than those of epitheliosis (papillomatosis). MNORA values were also significantly different in grade I and grade III invasive ductal carcinomas, the latter exhibiting the highest MNORA values of all the cases observed. Evaluation of MNORA values may therefore help in differentiating benign epithelial proliferations from ductal carcinomas in situ. Furthermore, because there is evidence that MNORA values are indicative of the cell duplication rate, MNORA values may ultimately be considered an objective prognostic parameter in addition to grading for invasive ductal carcinomas.
