Aldosteronoma in childhood: a review of clinical features and management.

Primary hyperaldosteronism is rare in children and usually results from bilateral adrenocortical hyperplasia. Adrenal adenoma causing primary hyperaldosteronism (aldosteronoma) in children is even rarer, with only eight reported cases in children under 18 years of age. Aldosteronoma is a very rare, correctable cause of childhood hypertension. A review of all the reported cases, including that of the authors, is presented. Also discussed are the available biochemical and radiological investigations, the preoperative medical management, the surgical treatment, and the postoperative management.

Conservative surgery for splenic hydatid cyst.

Splenectomy has been the treatment of choice for hydatid cyst of the spleen. By successful enucleation of a deep-seated splenic hydatid cyst and preservation of the spleen, the authors show the technical feasibility of such a procedure and recommend it as the treatment of choice.

pH of melanosomes of B 16 murine melanoma is acidic: its physiological importance in the regulation of melanin biosynthesis.

It was postulated by us that melanin biosynthesis cannot take place unless the melanosomal pH is acidic. That melanosomal pH may be acidic was suggested by other researchers as well. However, the actual pH was never determined. The current study was undertaken to estimate the intramelanosomal pH. Lightly melanized and heavily melanized melanosomes from B16 murine melanoma tissue were prepared by sucrose density gradient centrifugation and their internal pH was estimated. It was shown that quenching of fluorescence of 9-aminoacridine by melanosomes was to a large extent dependent on the pH difference between melanosomes and the medium in which they were suspended and it was used to determine the pH of lightly and heavily melanized melanosomes from B16 melanoma. The pH of lightly melanized melanosomes and heavily melanized melanosomes was found to be in the range of 4.2 to 4.6 and 3.0, respectively. These organelles were shown to hydrolyze fluorescein diacetate to fluorescein. The excitation spectrum of fluorescein generated in lightly melanized melanosomes and heavily melanized melanosomes also indicated that the pH in lightly melanized melanosomes was 4.6 and lower than 4.6 in heavily melanized melanosomes. The enzymes are active at such low pH. These organelles are also shown to contain proton-translocating ATPase, which may be involved in the acidification of lightly melanized melanosomes, possibly in addition to melanin biosynthesis. These results confirm our original postulate that melanin biosynthesis that occurs in melanosomes takes place under acidic conditions.

Studies on antibacterial properties of gentian violet impregnated silastic.

The antibacterial activity of gentian violet (GV) impregnated silastic discs was studied by measuring the zone of inhibition (ZOI), (in mm) produced by the discs planted on Staphylococcus epidermidis seeded agar plates. The effect of varying concentration of GV, the duration of antibacterial activity and effect of autoclaving were studied. The mean ZOI for 1 per cent GV was 19.28 +/- 0.89 mm and for 2 per cent GV it was 22.55 +/- 0.71 mm (P < 0.001). There was no significant difference between GV concentrations of 2-5 per cent. Over a period of 1-5 wk, no significant difference was found in the antibacterial activity of GV for any of the concentrations. In each of the concentrations there was a significant reduction in the ZOI after multiple autoclavings. The study thus indicates that it is possible to impart antibacterial properties to silastic implants so that their colonization with Staph. epidermidis can be prevented and this finding has obvious clinical implications.

Primary tubercular abscess of the spleen.

A 12-year-old boy with localized tubercular abscess of the spleen is presented. The diagnosis was established on histopathologic examination. Treatment consisted of splenectomy and postoperative antitubercular therapy.

Hydrometrocolpos: diagnosis and follow-up by ultrasound--a case report.

Hydrometrocolpos is accumulation of secretions in the vagina and uterus, caused by excessive intrauterine stimulation of the infant's cervical mucous glands by maternal oestrogen in the presence of an intact hymen (Wilson et al 1978). Hydrocolpos is dilatation of the vagina proximal to a congenital obstruction. If the uterus is also dilated, the condition is called hydrometrocolpos. Most cases of hydrocolpos are associated with an imperforate hymen that forms a thin bulging membrane between the labia. Though many cases of hydrometrocolpos have been reported in literature, there is no case report which shows classical ultrasound findings and follow up ultrasound scans to show the involuted uterus. We are presenting a case report whose diagnosis was established on ultrasound. This case highlights the value of ultrasound in diagnosing this condition and excluding other associated renal anomalies.

Pulmonary blastoma in a neonate.

A rare case of pulmonary blastoma in a neonate is reported. Only one other neonate has been documented in the available English literature. The treatment consisted of surgical excision only.

Pharmacokinetics of intravenous metronidazole in neonates.

Metronidazole is being extensively used in neonates for the prevention and treatment of anaerobic infections. However, detailed pharmacokinetic studies are not available. Thirty neonates receiving intravenous metronidazole 7.5 mg/kg at eight-hour intervals were studied with serial blood levels. The levels increased up to the third day of treatment and then reached a plateau. Neonates less than seven days of age showed higher levels than older infants. Septicemic neonates, however, showed continually rising levels. The half-life of metronidazole was 22.3 to 22.5 hours, approximately twice that in adults. This study indicates that neonates handle the drug differently than adults and may require it in lesser doses or at greater time intervals.

Hemangiolymphangioma of the urinary bladder in a child.

A rare case of hemangiolymphangioma of the urinary bladder with associated cutaneous hemangiomas of the shaft and glans penis and the scrotum is presented. Unusual features of clinical presentation, noninvasive diagnosis, and the surgical management are discussed.
