Raja pleural biopsy needle. A comparison with the Abrams needle in experimental pleural effusion.

The Raja pleural biopsy needle, 3 mm in external diameter and fitted with a titanium alloy biopsy flap, was compared with the Abrams needle in six dogs with experimentally induced pleural effusion. One hundred twenty-four pleural biopsies were performed using either the Raja needle or the Abrams needle (62 with each needle). No complication occurred with either needle. The Raja pleural biopsy needle yielded a statistically significant larger pleural tissue specimen with a statistically significant lower frequency of crush artifacts. The size of the pleura obtained by the Abrams needle averaged 4,401 +/- 559 microns2 (mean +/- SE) compared with 8,652 +/- 1,099 microns2 with the Raja needle (p less than 0.01; Student two-tailed t test). Biopsies by the Abrams needle showed crush artifacts in 25.8% of the specimens, whereas 8.06% of biopsies with the Raja needle showed crush artifacts (p less than 0.02; Fisher's two-tailed exact test). In conclusion, the Raja pleural biopsy needle is safe to use and, despite its smaller diameter (3mm versus 4mm external diameter of the Abrams needle), yields significantly larger and less frequently distorted pleural specimens.

A study on composition and hypolipidemic effect of dietary fibre from some plant foods.

Dietary fibre content of foods namely, khejri beans (Prsopsis cinceria), peepalbanti (Ficus religiosa), barbanti (Ficus bengalensis), gullar (Ficus glomerata) and teent (Capparis decidua) varied from 38.5% to 55.7%. Cellulose and lignin were predominating constituents in peepalbanti, barbanti and gullar; hemicellulose in teent and pectin in khejri beans. Fibre from all these plant foods, fed at the 10% dietary level to rats, induced a greater resistance to hyperlipidemia than cellulose. Teent had the most pronounced hypocholesterolemic effect which appeared to operate through increased fecal excretion of cholesterol as well as bile acids. Dietary hemicellulose showed a significant negative correlation with serum and liver cholesterol and a significant positive correlation with fecal bile acids. The dietary fibre influenced total lipids, cholesterol, triglycerides and phospholipids of the liver to varying extents.

Non-Hodgkin's lymphoma in northern India. A retrospective analysis of 238 cases.

A retrospective analysis was performed of 238 patients, aged 12 years and older, with non-Hodgkin's lymphoma presenting to the All India Institute of Medical Sciences, New Delhi, India, between September 1975 and December 1982. Pathologic material was reviewed and classified according to the modified Rappaport classification. The most common histologic type encountered was diffuse histiocytic lymphoma (39%), followed by diffuse poorly differentiated lymphocytic lymphoma (29%), and diffuse mixed histiocytic and lymphocytic lymphoma (9%). Nodular lymphomas constituted 9% of all non-Hodgkin's lymphomas. A lower frequency of nodular lymphomas, a lower median age of onset (45 years), and a higher male to female ratio (4.5:1) as compared with Western countries was observed. Survival information on 90 patients revealed no effect of age, sex, stage of disease, and "B" symptoms on survival, whereas histologic diagnosis had a significant influence on survival (P less than 0.05). A median survival of 24 months in 58 patients receiving chemotherapy is comparable to that reported by other investigators.

A randomized controlled trial of complete bed rest versus ambulation in the management of proteinuric hypertension during pregnancy.

Forty patients participated in a randomized controlled trial of complete bed rest versus ambulation as desired in the management of proteinuric hypertension during pregnancy. Daily increases in serum human placental lactogen and oestriol concentrations were greater in the rested group. An especially 'at risk' group of 10 patients with both hyperuricaemia and severe fetal growth retardation was identified. Strict confinement to bed in these cases seemed to encourage the development of the premonitory symptoms of eclampsia, but was associated with a better prognosis for the fetus.

The effect of rest and ambulation on plasma urea and urate levels in pregnant women with proteinuric hypertension.

Plasma urea and urate concentrations were determined daily for up to seven days in 40 pregnant women who had been admitted to hospital because of proteinuric hypertension and who were then allocated at random to either complete rest in bed or to being allowed to move freely in the ward. Neither management was superior to the other in improving renal function. The prognostic significance of plasma urea and urate concentrations to maternal and fetal outcome was confirmed.

A double-blind trial of single-dose chemoprophylaxis with co-trimoxazole during vaginal hysterectomy and repair.

Twenty-five patients received 10 ml co-trimoxazole (800 mg sulphamethoxazole and 160 mg trimethoprim) and 25 a placebo by intravenous infusion during one hour from the commencement of surgery for vaginal hysterectomy with anterior colporrhaphy and posterior colpo-perineorrhaphy. Single-dose chemoprophylaxis with co-trimoxazole was shown to be effective in reducing the incidence of postoperative febrile morbidity and urinary tract infection, especially that caused by Proteus species.

A randomized controlled trial of a short course of cephaloridine in the prevention of infection after abdominal hysterectomy.

One hundred patients took part in a randomized controlled trial to test the efficacy of three 1 g doses of cephaloridine in preventing infection after abdominal hysterectomy. The first dose was given as an intravenous bolus at the commencement of surgery and the second and third as intramuscular injections 6 and 12 hours later. Given in this way, cephaloridine was found to be very effective in preventing febrile morbidity, abdominal wound infection and urinary tract infection. It was less effective in preventing pelvic wound infection and postoperative colonization of the vagina with Escherichia coli.

Childhood autism in tuberous sclerosis.

The article reports two cases of childhood autism in tuberous sclerosis (TS). Certain atypical features are highlighted. The probands did not show the common seizure types associated with either TS or autism. No ventricular dilatation, cerebral atrophy or temporal lobe involvement was evident. The high prevalence of childhood autism in TS probands with moderate to severe mental retardation has been emphasized.
