Invasion of renal tumor into retroperitoneal major vessels with thrombosis should be characterized as local spread of renal carcinoma and a serious complication. Extensive interventions were conducted in 30 subjects out of 196 nephrectomy cases. Nephrectomy was attended by colectomy (3 cases), pancreatic resection and adrenalectomy (3 cases), resection of the liver (2 cases), one-stage lobectomy (2 cases), adrenalectomy (9 cases), resection of the uterine appendages (1 case), resection of the colon, splenectomy, opening of an intraorganic abscess. 12 patients underwent thrombectomy from the major vein via the thoracophrenoabdominal approach. Cavathrombectomy was carried out in 7 (3.6%) patients, in 3 of which vena cava inferior was resected. Removal of the thrombus from the renal vein with resection of the opening and suturing of the vena cava inferior was performed in 5 patients. The thrombus originated from the right kidney in 9, while from the left one in 3 patients treated surgically. The thrombi occupied 4-10 cm along the renal vein from its opening. The removed kidney weighted from 400 to 3200 g. One death occurred due to pulmonary embolism during the operation, one on day 5 due to cardiopulmonary insufficiency. Histological examinations of the thrombi showed them to consist of fibrin, blood elements and tumor cells within the thrombus. The thrombi grow slowly, undergo organization and vascularization. Tumor cells multiply in the thrombus. Fibrin coating restricts cancer cell free dissemination via the venous system. Cavathrombectomy is considered the only way to prolong survival for the above patients.
The drug erik was used in 32 patients with chronic bronchitis and acute bacterial pneumonia. Out of 132 strains assessed for sensitivity, 94 (68.6%) were sensitive to the above antibiotic, the rest demonstrated moderate sensitivity or drug resistance. The dose 0.5 g persisted in the blood for up to 12 hours allowing its use three times in 24 hours. The response was achieved in 32 patients (72.18%).

The clinical pharmacokinetics and the effect of bonnecor on parameters of the pharmacodynamics were studied in 53 patients with cardiac rhythm disorders, including 23 patients in the acute period of myocardial infarction. At intravenous administration of bonnecor in a dose of 0.4-0.6 mg/kg there was noted a pronounced antiarrhythmic effect with respect to both ventricular and supraventricular cardiac rhythm disorders. Bonnecor exerted no significant effect on the hemodynamic parameters. The pharmacokinetic parameters of bonnecor possess a great variability, the mean values of the parameters are close to the corresponding ones for ethacizine. Along with the unchanged drug one can detect in the blood mono-N-demethylated metabolite. The ranges of effective concentrations and those inducing side effects of the drug are given.
A comparative evaluation of immunological and epidemiological effectiveness of commercial live and killed vaccine preparations used simultaneously or separately was first carried out in simultaneous field trials of various methods of influenza vaccine prevention in a human population of 10,449 subjects. The advantage of simultaneous immunization was confirmed by immunological parameters and protective effect in the period of influenza A epidemic of 1983. The correlation of immunological and epidemiological effectiveness of influenza vaccines in joint or separate administration is discussed.

The serum content of immunoreactive insulin and growth hormone was determined during the intravenous glucose tolerance test in 104 patients suffering from chronic pancreatitis at the stage of exacerbation and of clinical remission. Reduction of glucose tolerance was more pronounced during the exacerbation of the disease than at the period of clinical remission. In comparison with the control group, insulin hypersecretion (more pronounced during the exacerbation) was revealed in these groups of patients as a whole. A reduction of the insulin/glucose ratio in the patients indicated a relative character of insulin deficiency. The maximal reduction of the insulin/glucose ratio was noted in the patients with a diabetic character of glucose tolerance. Elevation of the growth hormone basal secretion in comparison with control was revealed in the patients at the period of exacerbation.
A comparative study of the immunizing activity of strain variants derived from one epidemic influenza virus A/USSR/77 (H1N1) by using different attenuation methods was carried out. Serial passages in chick embryos preserved the biological activity of the virus better than genetic recombination which was manifested by a significant superiority of the passaged virus over the reassortant in the capacity of replicating in the upper respiratory tracts of the vaccinees, of inducing immune responses and creating the protection of the portals of entry of infection. The problem of optimal preservation of biological potentials of influenza virus in the process of attenuation is discussed.
Simultaneous immunization with inactivated and live influenza vaccines is characterized by a more rapid and intensive formation of local protection and humoral immunity persisting for 5 months at a higher level than after separate administration of inactivated and live vaccines. The live vaccine was found to be the major factor enhancing the immune response and local protection in combined immunization. Administration as components of a live bivaccine of influenza serotype A viruses with a different degree of attenuation was not accompanied by higher reactogenicity but led to a sharp decrease of immunogenic activity that indicated incompatibility of such viruses in a bivaccine.

A significant increase in immunological effectiveness of simultaneous immunization with a killed and live influenza vaccine as compared with separate use of these preparations has been demonstrated. The simultaneous immunization better protected the upper respiratory tracts of the vaccinated volunteers, inhibiting reproduction of the challenge influenza virus with residual virulence. A live vaccine from the "old" influenza A/6/50 (H1N1) was found to produce a protective effect on the current influenza A/1977 (H1N1) virus with residual virulence.
