[Conclusions from the Viagra case? Definition of disease in social legislation as exemplified by erectile dysfunction].

The article describes the controversy about the question whether statutory social health insurances are obliged to reimburse the costs for the treatment of erectile dysfunction. Next to the question whether the 'Bundesausschuss der Arzte und Krankenkassen' was entitled to decide it was highly controversial whether erectile dysfunction is a disease according to the laws of social insurance. This enforces more general considerations regarding the possibility to define disease and the relevancy of a concept of disease for the justification and limitation of socially financed services in medicine.

[Insurance and genetics - options to prevent undesirable consequences].

The article analyses the different options to prevent or alleviate the undesirable consequences of molecular genetic information in the sphere of insurance. It distinguishes between macro-, meso- and micro-options. The options are examined in respect of their probability to be realised.

Euroscreen 2: towards community policy on insurance, commercialization and public awareness.

The project Euroscreen 2 has examined genetic screening and testing with particular reference to implications for insurance, commercialization through marketing of genetic tests direct to the public, and issues surrounding raising public awareness of these and other developments in genetics, including the practical experiment of a Gene Shop. This paper provides a snapshot of the three year project. The study group's work included monitoring developments in different European countries and exploring possibilities for regulation in insurance and commercialization together with public attitudes to regulation. The success or failure of different strategies is not independent of public awareness. Exploration of policy, however, also requires examination of fundamental concepts such as solidarity and geneticization.

Why should medicine consider a theory of practice? Introduction to the issue.

In reference to historical developments, this article introduces the topic of this special issue of Theoretical Medicine and Bioethics, that is, the relationship(s) between theory and practice. The authors emphasize the need for scientific research in this neglected area for the sake of both clinical practice and medical education.

Genetic screening and ethics: European perspectives.

Analysis and comparison of genetic screening programs shows that the extent of development of programs varies widely across Europe. Regional variations are due not only to genetic disease patterns but also reflect the novelty of genetic services. In most countries, the focus for genetic screening programs has been pregnant women and newborn children. Newborn children are screened only for disorders which are treatable. Prenatal screening when provided is for conditions for which termination may be offered. The only population screening programs for adults are those for thalassaemia carrier status in Cyprus, Greece and Italy. Social responses to genetic screening range from acceptance to hostility. There is a fundamental tension between individual and community in the debates in various European countries about implementation of screening programs. Opposition to genetic screening is frequently expressed in terms of arguments about "eugenics" with insufficient regard to the meaning of the term and its implications. Only a few countries have introduced explicit legislation on genetic screening. Legislation to address discrimination may provide more safeguards than legislation protecting genetic information itself.

[Loneliness of the physician and the "vital drive for history". On the historical self concept of medicine by Richard Koch].

This article tries to identify the theory of medical history in the work of Richard Koch (1882-1949). Rejecting the wide-spread notion of medicine as a science he vigorously argued for a self-image of medicine as an art. From that point of view he built up his theory of medical history and histoography. He wanted medical histoography to be part of practical medicine.

The article tries to clarify the relation between history of medicine and medical ethics. Therefore it distinguishes between the double meaning of the German word "Geschichte", which means both: "development of events" and "story". For the first option no systematic relation between history and ethics can be reconstructed. In the second sense historical knowledge can be a readable and worth reading text for the ethical debate. Therefore the methodological professionality of the historian is required.

Style and responsibility: medicine in postmodernity.

To what extent can postmodern developments be observed in modern medicine and which theories of postmodern philosophy can we draw on with regard to medicine's theoretical problem? This article explores these questions with special emphasis on the epistemological status of medicine, the concept of disease, and the anthropological model. It is examined whether medicine's inherent duty to act can be questioned in the light of the plurality that characterizes postmodernity. It is concluded that, according to postmodern philosophy, medicine should be characterized by a justified variety of paradigms. Cooperation within the variety must continue to be guaranteed, always in favor of the constitutive moral duty of medicine.

[Responsibility of the physician in reproductive medicine].

The article explores the responsibility of the physician in reproductive medicine as a therapist of the couple and the child. It is based on the understanding of responsibility as a term with six relations. The article emphasizes that responsibility can only be ascribed to the physician with regard to the structure of the physicians acting.

[The history of the Bamberg glass factory. An example for medicine and environmental protection in the early 19th century].

The theme is the story of the building and setting in motion of a glass factory in Bamberg between the years 1802-1811. It is an historical example of the problem how to handle industrialization with reference to the ecological consequences. Two german physicians, Andreas Röschlaub and Anton Dorn, were against this project; they realised the possible results to man's health. Röschlaub, through his "Erregungstheorie" especially showed a subtitle understanding of the relationship between organism and its environment. Both presented arguments for moderate industrialization with consideration of influences on the environment.
