The bladder function of forty five women was studied once before and three times following a Wertheim Operation for Carcinoma of the Cervix. Three quarters of the patients had a diminished bladder capacity two weeks after surgery with an increased bladder filling pressure and a decreased urethral occlusion pressure. After eight and twenty weeks the pre-operative state was almost reached. However there was no complete restoration of the function at that time. Almost all women had a disturbance of the bladder sensitivity. Modern investigations of bladder physiology show that the lax bladder is an expected post-operative neurogenic problem of the vesical plexus. At first this is masked by a hypertonic bladder due to muscle trauma. The postoperative decrease of the urethral occlusion pressure appears to be caused by a partial injury of sympathic nerve fibers. The problem of functional infravesical obstruction is discussed. The treatment is at first accomplished by general perioperative measures. Later on receptor effective drugs are used as necessary following a repeat urodynamic investigation.

Subsequent to the injection of horseradish peroxidase into the parietal eye of adult Lacerta sicula, the course of the parietal nerve and its projections were determined. The parietal nerve enters the left habenular ganglion where it branches into a medial and a lateral route. Some nerve fibers decussate within the habenular commissure. Whereas this pathway exhibits a striking asymmetry at the level of the habenular ganglia, its projections to the dorsolateral nucleus of the thalamus, the periventricular hypothalamic area, the preoptic hypothalamic and telencephalic regions, and the pretectal area are arranged in a strictly symmetric manner. A possible innervation of tegmental areas could not be proven due to the presence of endogenous peroxidase within these regions. No parietal nerve fibers were observed in the optic tectum. In a few animals investigated, scattering labeled perikarya were located in the periventricular hypothalamic gray indicating a parietopetal innervation in Lacerta sicula. The injection of horseradish peroxidase into one of the lateral eye revealed terminal areas of the optic nerve within the preoptic region, and the thalamic and pretectal nuclei, displaying partial overlapping with the projection of the parietal nerve to these areas. From the present investigation further evidence is obtained that the pineal complex of lower vertebrates is a component of the photoneuroendocrine system. Particular emphasis is placed upon the nervous connections between the parietal eye and the hypothalamus, described for the first time in the present study.
At defined times prior and following a Wertheim operation for cancer of the uterine cervix the patients were investigated for the functional parameters of the lower urinary tract. The residual urine was determined. Stress incontinence and urgency incontinence were measured with urethrocystometry. The sensitivity and the micturition were evaluated from subjective data. The results of 237 investigations in 192 operated women are described and compared to previous papers on the subject. Stress incontinence increased to double the pre-operative incidence following the Wertheim operation. An urgency incontinence was quite rare. A lack of feeling of fullness to the bladder as a sign of disturbed bladder sensitivity was present 5 months post-operatively in 6 of 45 patients (13%). This was present in only 1 of 119 patients (1%) after five years. 1/3 of the women had difficulty with spontaneous micturition five months following the operation. 1/10 of the women had difficulty with spontaneous micturition 5 years after the operation. There was no residual urine five months after the operation in 9/10 of the patients and five years following the operation almost 100% of the patients had no residual urine. It was noted that difficulty with spontaneous micturition with residual urine and urgency incontinence did not necessarily result in concomitant bacteriuria. Suggestions are presented how future investigations with similar questions may lead to comparable results.

Eighty-eight patients with the stage 1 hypertension according to the WHO classification exercised on a bicycle ergometer for 30 min daily at the heart rate 130-140 beats/min for 28 days. In 42% of the patients the systolic pressure at rest, during exercise, and in the recovery period decreased significantly; in 58% of them the systolic pressure did not sink. The diastolic pressure during exercise sank in those patients whose systolic pressure was reduced by the effect of the exercise, whereas in those patients who did not respond to exercise by a reduction of the systolic pressure, the diastolic pressure did not change, either. The following indicators differentiated the responders and non-responders to exercise: height of systolic pressure during exercise and recovery period, height of diastolic pressure at rest, disturbed glycide metabolism, presence of hypertension in the family history, sports in anamnesis and cardiac index.
Following the injection of horseradish peroxidase into the pineal organ of the guinea pig, approximately 30 nerve fibers were demonstrated in the pineal stalk due to the retrograde transport of the tracer enzyme in these elements. Finely branched extensions of these nerve fibers are directed toward the distal portion of the pineal organ. This projection of central nervous elements enters the pineal organ via the habenular or posterior commissures. Neuronal perikarya projecting into the pineal organ are found in the region of the paraventricular nucleus near the border of the third ventricle.

Severely keratinizing squamous cell carcinomata are very infrequently observed oral cavity tumors with a relatively nonmalignant character. Problems of clinical diagnosis and the significance of such carcinomata are discussed with reference to the case history of a 38-year-old male patient.
The Kasaach-Merritt syndrome involves the very infrequent symptom complex of "giant hemangioma--thrombocytopenia--coagulation disorder". Hemorrhagic diathesis is here based on consumption coagulopathy. This paper reports upon secondary bleeding after extraction of a tooth in a 25-year-old male affected with this syndrome. Continuation or initiation of anticoagulant therapy is recommended for surgical stomatological treatment on an inpatient basis, and measures to be taken in connection with wound treatment are described.
The epidemiology of hepatitis B in hospital personnel was studied by testing of sera from 3,770 employees of the Medical School of Hannover (Hannover, West Germany) for hepatitis B surface antigen (HBsAg) and its corresponding antibody (anti-HBs) by solid-phase radioimmunoassay. An average prevalence of 2.2% for HBsAg and 11.7% for anti-HBs was found. Physicians (18.2%), nurses (20.1%), and members of the cleaning service (26.3%) showed the highest frequencies of HBsAg or anti-HBs carriage. In a study of age- and sex-matched personnel, nurses showed a significantly (P less than 0.01) higher rate of infection than a control group with less exposure to infectious materials. The frequency of HBsAg or anti-HBs was highest in persons associated with dialysis (31.3%), anesthesiology (31.0%), ophthalmology (29.4%, neurosurgery (28.0%), and surgery (24.4%). The rate of infection was significantly higher in surgical departments (24.4%) than in nonsurgical ones (13.3%). Persons who had been nursing patients with hepatitis were significantly (P less than 0.05) more frequently carriers of HBsAg or anti-HBs than a comparable control group.
From 1962 to 1975 severe stress incontinence or recurrent stress incontinence was operated according to Marshall-Marchetti In 64 cases. Sixty patients were followed up. More than 2/3 of the patients were satisified with the result. Objective follow-up with urethrocystotonometry showed that only slightly more than 50% of the patients were cured. It was possible to divide the cases in 4 groups to elucidate the discrepancy between subjective and objective cure. The operative and postoperative complications were minor. The recurrence rate was the same with follow-up from 3 to 14 years. The results can probably be improved by precise pre-operative diagnosis regarding intrinsic urinary tract disease and by an extension of the indication for this operation or risk cases. The patients must be warned that the result also depends on their cooperation.
