[Undulating course of precocious puberty].

The normal course of precocious puberty is progredient resulting eventually in full sexual maturation. In few cases a transient form of the disease has been recorded, characterized by a temporary appearance of signs of sexual maturation. We report on a female patient with an undulating course of the disorder: after precocious initiation of puberty, a temporary interruption and subsequent resumption of sexual maturation occurred. Presently, the patient is successfully treated with an LHRH-analogon. Within the framework of this case problems in diagnosis and treatment of precocious puberty are being discussed.

[Effects of various alfentanil doses on blood pressure, heart rate and plasma catecholamine level in endotracheal intubation].

METHOD: In 56 patients undergoing arthroscopy of the knee blood pressure (BP), heart rate (HR) and plasma catecholamines were measured during induction of anaesthesia. To a standard treatment including etomidate (K), either 1 (A1), 2 (A2) or 3 (A3) mg alfentanil were added. The control-group included 8 patients, the other groups consisted of 16 patients. Blood pressure and heart rate were measured the day before anaesthesia (T1), at the arrival in the operation theatre (T2), 1 minute after the induction (T3) and 1 minute after intubation (T4). Catecholamines were analysed at T2 and T4. - RESULTS: At T1 and T2 no significant differences were measured. The control-group had a significant rise of BP, HR and adrenalin at T4. In group A1 (1 mg alfentanil) BP and catocholamines remained at the same levels, HR rose significantly. In group A2 all parameters did not change. The group A3, BP dropped significantly, whereas the other parameters remained stable.- DISCUSSION: 2 mg Alfentanil given 1 minute before endotracheal intubation depress the sympathoadrenergic reactions but do not impair the circulation.

Basilar artery disease--clinical outcome and Doppler sonographical follow-up.

In the past 5 years we have investigated 29 patients with symptomatic basilar artery stenoses (14 cases) and occlusions (14) and a patent primitive trigeminal artery with thin-calibered basilar and vertebral arteries (1) using directional continuous-wave Doppler sonography of the vertebral arteries. A total of 19 patients survived, and 17 of them were clinically and sonographically reexamined after 40.4 +/- 15.8 months (mean +/- SD). Among the 8 patients with basilar stenoses, 6--with no further transient ischemic attacks (TIAs) in the interval--exhibited an increase in the summed modified Pourcelot indices (relative end-diastolic flow velocities) of the vertebrals by 0.18 +/- 0.16; the other 2 showed a decrease by 0.26 each, in 1 case temporally related to a TIA, in the 2nd case without further clinical deterioration. In the 8 survivors with basilar occlusions, 5 remained--by sonographic criteria--unchanged with summed modified Pourcelot indices of the vertebrals of 0.00, while 3 patients exhibited a slight increase in the summed modified Pourcelot indices of 0.13 +/- 0.03. While the difference between the outcome of subsets of patients treated with regimens of 30,000-40,000 units heparin/day or phenprocoumon and less radical drugs were statistically not significant, the former regimen appeared clinically more efficacious in preventing further deterioration in approximately two-thirds of the patients affected. Due to the potential recurrence of neurological symptoms, a treatment period with phenprocoumon of 6 months after discharge from hospital appears justified. Due to these therapeutic efforts, approximately half of the patients initially affected survived with no or only a mild neurological deficit.(ABSTRACT TRUNCATED AT 250 WORDS)

[A monoclonal antibody against peripheral human blood monocytes (RoMo-1)].

The monoclonal IgG2a-antibody RoMo-1 binds to peripheral human blood monocytes but does not react with other blood cells, tissue macrophages, HL-60, K 562 or U 937. It seems possible, that the antibody defines an antigen expressed on the premonocytic and monocyte stages of cells from the mononuclear phagocytic system only. The antibody is cytotoxic.

The effect of amrinone on LDH release and perfusion pressure in isolated ischemic rabbit hearts.

Isolated rabbit hearts were perfused normoxically or ischemically using the technique of Langendorff. The hearts were perfused with a modified Krebs-Henseleit solution, perturbated with 95% O2/5% CO2 or in case of ischemia with N2 which replaced the O2. The perfusion rate was 25 ml/min under normoxic and 2.5 ml/min under ischemic conditions. The oxygen pressure was about 65 kPa in the normoxic and about 6 kPa in the ischemic medium. Reperfusion of ischemic hearts was realized in some cases normoxically. During the ischemic perfusion and reperfusion LDH was released time depended into the perfusion medium and the aortic inflow pressure increased. The LDH release and the increase of the inflow pressure are strong correlated. Both parameters direct to ischemic myocardial lesions. Amrinone applied as bolus in the start period of ischemia or as an infusion during the ischemic and reperfusion time limited the release of LDH and the increase of the inflow pressure. The results were discussed as a direct protecting effect of amrinone against a myocardial ischemic lesion, which is in correspondence with the improving effects of amrinone on the ischemia caused heart failure of canines in vivo, shown by Campbell et al.

Complications of local beta radiation of uveal melanomas.

The results of local beta ray radiation of 295 eyes containing a uveal melanoma (including 74 melanomas of the anterior choroid and ciliary body) are presented with special respect to the complications of treatment. The most frequent complications were radiation retinopathy and optic neuropathy. Complications known to occur from other radiation methods, such as dry eye syndrome, loss of cilia, and scleral necrosis, did not occur in our series. The incidence of radiation cataract, as compared to the treatment with 60Co plaques, was lower in our series. An average of 700 Gy delivered to the scleral base of the tumor was needed for uveal melanomas which could be destroyed. This radiation dose resulted in a 10% incidence of radiation retinopathy.

[The action of Cordemcura on the energy balance of the isolated rabbit heart].

The effect of Cordemcura, a new cardiotonic substance, was tested on retrograd perfused isolated rabbit hearts in comparison with equieffective doses of dopamine. The substances were applied continously via the perfusion medium in the following dosages: Cordemcura 3.34 X 10(-4) mol/l, 2.67 X 10(-3) mol/l; dopamine 3.9 X 10(-6) mol/l, 1.6 X 10(-5) mol/l). The effects of the substances were studied during normoxic and hypoxic perfusion. The heart rate was increased under normoxic perfusion of Cordemcura about 37.1% (3.34 X 10(-4) mol/l) and 17.9 (2.67 X 10(-3) mol/l). The oxygen consumption in relation to the performance of the hearts (contractility. heart rate) was increased by Cordemcura on 1.45 and 1.93 (3.34 X 10(-4) mol/l and 2.67 X 10(-3) mol/l) in the same way like by dopamine (1.30 and 2.66; 3.9 X 10(-6) mol/l and 1.6 X 10(-5) mol/l). The content of adenine nucleotides decreased in the hypoxic perfused hearts on nearly 20% of the control values. There was no difference between control, Cordemcura and dopamine treated hearts. Cordemcura increased the level of cAMP in the myocardium after 90 min of hypoxic perfusion in comparison with control hearts (control hearts 2.8 +/- 0.7 nmol/g protein; Cordemcura 4.7 +/- 0.8 nmol/g protein). In comparison to control hearts and dopamine treated hearts the hypoxic perfusion with Cordemcura resulted in a significant lower release of lactate dehydrogenase from the heart.

[Indications for and results of postoperative radiotherapy of vulvar cancer].

Between 1968 and 1979, 132 patients with squamous cell carcinomas of the vulva were submitted to a more or less radical operation. Then they were irradiated at the Radiologic Center of the GHS Essen. All patients received irradiations of the vulvar region with high energy electrons combined with photons (n = 78) as well as photons or electrons alone (n = 15 and 39, respectively). The dose was generally 40 to 60 Gy. 81 patients (61.4%) were additionally irradiated in the inguinal region. The overall three-year and five-year survival rate is 48.5 and 34.1%, respectively. The median survival time is 31.5 months. Contrary to expectation there were no significantly different recurrence rates after radical vulvectomy and large excision combined with postoperative irradiation. This is attributed to the favorable effect of radiotherapy. A graduated conception has been elaborated based on the results of our retrospective analysis and the communications of literature. Radical vulvectomy and inguinal lymphadenectomy is the therapy of choice. Radiotherapy in the vulvar region should be applied if radical surgery seems problematic or is not possible. Irradiation of lymph node regions is indicated if they are involved or if a resection is not possible.

[Radiotherapy of kidney cancer].

The benefit of low-dose preoperative radiotherapy in hypernephroma has not been proven in phase-II studies. Medium-dose preoperative radiotherapy many transform inoperable to operable tumors in numerous cases. In a randomized study, no prolongation of survival has been found, however. Many retrospective studies, in part with questionable design, have found no value of routinely used postoperative irradiation. Presently, it is indicated in advanced local tumor stages (pT3 and pT4) and residual tumors. Postoperative radiotherapy in high-risk patients using defined techniques and preoperative radiotherapy applying modern diagnostic equipment for treatment planning remains to be evaluated. For palliation, radiotherapy is useful treating brain-, lung- and bone metastases and inoperable primary tumors.

Influence of hypoosmotic and ammonium chloride-mediated haemolysis on the integrity of human mononuclear blood cells.

Side effects of both hypoosmotic and ammonium chloride-mediated hemolysis were compared looking for cellular integrity of human peripheral blood mononuclear cells. Recovery and viability of mononuclear cells significantly declined only following water treatment. Cell loss by lysis of non-erythrocyte cells (monitored by 51Cr release) preferentially occurred in the lymphocyte population resulting in a relative enrichment of monocytes (identified by peroxidase and esterase staining as well as phagocytosis of fluorescent latex particles). Consequences of this shifted monocyte/lymphocyte ratio for immunological tests are obvious. Necessity of red cell lysis and disadvantages referred especially to NH4Cl-induced white cell functional losses are outlined.

5q- chromosome in acute leukemia with lymphoid morphology and expression of myeloid membrane determinants.

We present three patients, two children and one adult, with an unusual type of acute leukemia. Whereas the blast cells showed lymphoid morphology with correlating cytochemical staining, immunological phenotyping exhibited a pure myeloid in one patient and a biphenotypic membrane marker profile in the other two patients. Cytogenetic studies revealed a 5q- chromosome as a common marker and additional individual changes. Two of the patients who were treated according to acute lymphoblastic leukemia (ALL) therapy protocols died without remission five and four weeks after diagnosis, respectively. Despite relapsing several times, another patient survived for over eight years. These three patients seem to represent one new subgroup of leukemias that can only be distinguished from typical ALL by both determination of cell surface markers and cytogenetic analysis.

Qualitative and quantitative Con A binding to mouse lymphocytes, T cell enriched spleen cells and allogeneically stimulated lymphocytes.

The qualitative and quantitative Con A binding behaviour of mouse spleen cells and allogeneically stimulated lymphocytes was investigated by means of fluorescence-optical and cell-electrophoretical methods. Contrary to B lymphocytes and unstimulated cultured T lymphocytes, normal as well as allogeneically stimulated T cells show a decreased electrophoretic mobility after incubation with Con A. This decrease correlates with a higher affinity and specificity to Con A on the one side and with a functional signal of Con A on the other side. Incubation with Con A is a stimulating signal for normal T lymphocytes and a suppressive one for allogeneically stimulated lymphocytes. The 3H-thymidine incorporation of B lymphocytes and unstimulated T cells is not influenced by Con A in spite of Con A binding. The amount of bound molecules is not of decisive importance for the mitogenic cultivated properties of Con A.

[Combined chemotherapy and radiotherapy: cyclophosphamide].

Compared to the other chemotherapeutic preparations discussed today, cyclophosphamide in combination with radiotherapy is a relatively unproblematic substance. Despite the great number of experimental and clinical data, the evaluation does not prove an advantage of the combined therapy with cyclophosphamide and irradiation as against radiotherapy alone in local tumor treatment. This should give occasion to further experimental investigations in adequate models. On the other hand, the practice of combined treatment with irradiation and preparations containing cyclophosphamide should be reconsidered for those cases where local control of a tumor is the principal therapeutic aim.

Renal abscess: report of a case with sonographic, urographic and CT evaluation.

The appearance of a renal abscess on realtime-ultrasound, excretion urography and computed tomography is presented of an operatively and histologically proven case. An unusually hyperdense rim of the lesion is observed on native CT-scan. Knowledge of clinical symptoms is most important for correct interpretation of diagnostic imaging techniques.
