[Adverse medical aspects of hypnotic therapy with temazepam].

In a double blind comparison the driving ability of 32 outpatients with disomnia was investigated in psychometrical tests and standardized trial runs in road traffic every morning after a single dose and after a 7-day therapy with 20 mg temazepam compared with 2 mg flunitrazepam. As well as standardized driving performance observation a test car was fitted with a drive-recorder which synchronically recorded and digitally registered at 1 s intervals during a 60 min run the speed, the steering wheel velocity, fore-hand-aft and lateral acceleration amongst other data. The individual optimal quotient, as a measure of the degree of efficiency of the information processed in the driver-vehicle-road system, deteriorated significantly after the single dose of flunitrazepam as opposed to temazepam. In a trial run on a well defined test course under strictly standardized conditions, the steering wheel velocity, signifying the quality of steering ability, was highly significantly increased after the single dose as well as after the 7-day treatment with flunitrazepam therefore showing a distinct deterioration of steering ability compared to the temazepam group. The number of errors in technical handling was significantly highly with flunitrazepam at both control time checks than with temazepam.

[Diagnosis of lymphatic metastases in intraoperative thoracic cytology].

The diagnosis of metastases to lymph nodes of the thoracic or neck region reveals great importance for the thoracic surgery. In the last two years we investigated materials taken from 730 patients undergoing thoracotomies, mediastinoscopies and by exstirpations of lymph nodes. Altogether there were imprints from 1400 lymph nodes cytologically screened. Different tumor types exhibited different numbers of metastases. Usually we took smears from 2 till 12 lymph nodes from one patient. We got the imprints from several planes of the lymph node. All imprints were stained by Giemsa fast staining. The results of this cytological method are not inferior to the histological section technique.

[Histomoniasis in pullet stock and its effect on later laying performance].

In a 17 1/2 weeks old pullet stock typhlohepatitis was diagnosed on the occasion of a stock examination by the poultry health service. Liver and gut of randomly selected pullets showed gross and histologic alterations as known in cases of typhlohepatitis in turkeys. Histomonads could be shown using both the PAS-reaction according to McManus and the silverstaining according to Grocott. In the following controls of the herd macroscopic visible alterations of the liver could not be seen, but some of the animals still exhibited gross lesions in the ceca. The course of the development of the herd from point-of-lay to the 9th month of laying period is described.

[CT in senile dementia and depression].

This article reviews a series of investigations, which evaluated CT in senile dementia and depression in late life. Although both ventricular and sulcal dilatation were strongly associated with the clinical diagnosis of SDAT, such radiological changes were also found in healthy controls, with an overlap of about 15-20%. Cognitive decline correlated equally well with the diagnosis of SDAT, but within the demented group CT changes correlated poorly, if at all, with intellectual impairment. It is argued that CT is useful in the assessment of dementia only in conjunction with other clinical and psychometric data. In old age depression ventricular enlargement predicted an increased mortality at two years, and a reduced brain density, which may have reflected an ageing process within the CNS. With improvements in the technology and analysis of CT further clinical and research progress can be expected.

[Implantation technics for long-term recording of the compound action potentials in the guinea pig, Cavia porcellus. A comparison of methods].

Long-term recordings of the compound action potential of the acoustic nerve of young, pigmented guinea pigs were carried out by two different techniques of implantation. The electrodes were inserted either by way of the mastoid process into the bony recess above the round window or they were introduced into the facial canal via the stylomastoid foramen. Before and after each implantation, special tests (Preyer-reflex, otoscopy, impedance audiometry) were performed for preliminary selection of the animals and to discover postoperative induced disturbances of sound conduction in the middle ear. These tests showed that the proportion of operatively induced middle ear inflammations was reduced from 24 to 10%, if electrodes were implanted into the facial canal.

[The effect pf antacids on the tissue histamine and prostaglandin (PGE2) content of the gastric mucosa].

Stimulation tests (pentagastrin) induced significant changes in tissue histamine levels in the gastric mucosa within 30 minutes. Acid secretion and a decrease of histamine after application of antacids containing calcium could not be observed in our study; neither was application of antacids over a period of eight days followed by a change in gastric mucosal tissue histamine concentrations. Patients under non-steroidal antiphlogistic medication showed diminished histamine levels. Administration of antacids did not produce changes in histamine levels, but induced a rise of PGE2 levels in the fundal and corpus area of the stomach. This may be due to a cytoprotective effect of antacids containing calcium on the gastric mucosa.

[Ultrastructural findings in Arndt-Gottron's scleromyxedema].

The ultrastructure of a skin biopsy taken from a 64-year-old patient with clinically typical scleromyxedema has been studied. Histologically, the disease is characterized by dermal depositions of acid mucopolysaccharides, as well as an increased number of fibroblasts and tissue mast cells. Electron microscopically, the collagen microfibrils exhibited irregularities of their shapes and widths. In addition, the elastic fibres were unevenly contrasted with small defective areas of the matrix, and there was loss of fibrils. We also encountered a substantial number of highly active "dendritic" fibroblasts that frequently contained phagolysomal cytoplasmic inclusions. Probably these cells are responsible for the increased deposition of newly formed collagen fibres and elastic material. Furthermore, peculiar variants of fibroblasts are described, the cytoplasms of which were replete with heterolysosomal and autolysosomal inclusions. Finally, the tissue mast cells showed unusually large specific granules, so that these cells may also take part in the pathological process.

[Idiopathic hemochromatosis--diagnosis and therapy].

The excessive storage of iron in idiopathic haemochromatosis leads to severe organic lesion up to life-threatening conditions (cardiac insufficiency, portal decompensation). The symptoms melanodermia , diabetes mellitus and other endocrine failures, liver cirrhosis, cardiac insufficiency and arthropathy appear together or in various combinations. The diagnosis is ascertained by the proof of iron storage, the multiple organic affection and by familial accumulation of the various laboratory diagnostic possibilities are particularly to be emphasized the serum iron value together with the percetal transferrin saturation (as search test), serum ferritin, the desferrioxamine test, simple ferrokinetic investigations and the quantitative determination of iron in the liver in the bioptate . For family examinations, apart from the search test, a HLA typisation is reasonable, in order to estimate the risk of the disease (particularly of brothers and sisters). The therapy of choice are blood- lettings (0.5 l once to twice a week) up to obtaining a permanent easy iron deficiency anaemia. The maintenance therapy should be performed with monthly to quarterly blood- lettings . Only in cases exception a desferal treatment is indicated. Endocrine failures and cardiac disturbances need a particular therapy.

[Method of quantitative iron determination in the serum, urine and tissue].

In this paper the methodology of the quantitative determination of the iron in serum, urine and tissue is described. For the quantitative recognition of the iron in the serum the photometric determination with bathophenanthroline as well as the atomic absorption spectrophotometry may be used. The atomic absorption spectrophotometry may be used. The atomic absorption spectrophotometry is particularly suited for the measurement of the iron content in the urine. Tissue is disintegrated by wet ash formation and also determined by atomic absorption spectrophotometry.
