[Diagnostic value of R amplitude behavior at a recording site below the conventional thoracic wall recording in the diagnosis of chronic ischemic heart disease].

Sensitivity and specificity of the behaviour of the R-amplitudes under exercise are investigated at an additional lead position on the anterior chest wall, which in previous multi-electrode-examinations showed a higher sensitivity than the conventional Wilson-leads V 4 and V 5. For the criterion "increase of R-amplitudes" could certainly be confirmed a higher sensitivity at the additional lead position, the bad specificity both at the conventional and at the additional lead position allows, however, only the inclusion of the decrease of the physiological R-amplitude into the exclusion diagnostics of the chronic ischaemic heart disease.

[Primary biliary liver cirrhosis and granulomatous pulmonary infiltrates].

Primary biliary cirrhosis was diagnosed in a 48-year-old woman in the light of clinical and laboratory data, antimitochondrial antibodies and histologic features in liver biopsy. Four years later the patient developed pulmonary infiltration of changing localization. Histologic investigation of transbronchial biopsy revealed granulomas in the bronchial mucosa and peribronchial tissue. There were no further signs of sarcoidosis. The question is discussed whether granulomatous pulmonary infiltrations are an extrahepatic manifestation of primary biliary cirrhosis.

[Acromegaly due to a pancreatic tumor producing growth hormone releasing hormone: clinical picture and morphology].

Acromegaly due to ectopic production of growth hormone releasing hormone (GH-RH) by a tumor is rare, but several cases have been described after isolation and identification of GH-RH from a pancreatic tumor. The authors observed a patient with acromegaly caused by a pancreatic tumor in whom GH-RH production was demonstrated by measurements of GH-RH in peripheral plasma, tumor veins, tumor extracts and immunohistochemical identification. Endocrinological, histological and ultrastructural results are discussed. The problem of potential malignancy is investigated in the light of DNA measurements. This observation shows that GH-RH measurements can be a useful diagnostic procedure before surgery for acromegaly.

Epoxidation of 1,3-butadiene in liver and lung tissue of mouse, rat, monkey and man.

When 1,3-butadiene is incubated with liver postmitochondrial fractions from mouse, rat, monkey or man and a NADPH-regenerating system the formation rate of butadiene monoxide is different in the four species. With the exception of rhesus monkey the amount of epoxide is proportional to the monooxygenase activity. The sequence of epoxide formation is B6C3F1-mouse, Sprague Dawley rat, man, rhesus monkey. The relation between mouse and monkey was about 7:1. When 1,3-butadiene is incubated with homogenates from lung tissue, only tissues from mouse and rat produces measurable butadiene monoxide concentrations. The monooxygenase activity in lung tissue of the mouse was only 1/30 that in mouse liver. By contrast, lung tissue formed the epoxide concentrations comparable to those formed by liver tissue, whereas monkey and human lung tissue did not produce any measurable levels of butadiene monoxide. The data might suggest that the results of recent rodent inhalation studies with 1,3-butadiene could not automatically be extrapolated to man.

Reactions of olfactory bulb neurons to different stimulus intensities in laboratory mice.

The activity of 219 cells in the olfactory bulb of waking, freely breathing mice was analysed, and it was found that their spontaneous discharge rate varied between 0.3 and 33 AP/s. Both butyric acid and geraniol elicited responses of four types: 38% of the 98 responses were of type 1 (excitation), 43% of type 2 (inhibition), 10% of type 3 (complex structure), and 8% were characterized by a change in the temporal pattern of the activity. Response duration varied from less than 500 ms to more than 1 min. 52 secondary neurons were stimulated with four different concentrations of the odor substances. All of the responsive cells showed a clear ability to discriminate concentration. That is, response magnitude varied with intensity, producing non-monotonic curves. Most of the neurons responded only in a region of a more or less limited concentration, and in no case was a saturation curve observed. Approx. 40% of the neurons responded to the lowest concentration tested (10(-8) vol.% of butyric acid or geraniol). The strongest stimuli, 10(-2) vol.%, were relatively ineffective.

Serial xenotransplantation of a human embryonal carcinoma in experimental urology.

The rare incidence of testicular germ cell tumours makes it necessary for clinicians to look for a valid experimental model to investigate basic tumour properties and different therapeutic modalities preclinically in order to improve clinical cure rates. A human embryonal carcinoma with hCG-production was successfully established in nude mice for more than 40 passages. We were able to show striking maintenance of histological and ultrastructural features, tumour markers (Beta-hCG) and DNA indices of the original tumour during subsequent xenograft passages as well as stability of tumour growth. Dedifferentiation of the tumour with changing growth fractions or loss of hCG production was not evident.

[Premedication with H1- and H2-receptor antagonists for intravenous urography using contrast media].

Intravenous urography using Telebrix was performed on 500 patients. Two hundred patients (group I) received no pre-medication; 300 patients (group II) were premedicated with H1 and H2 receptor antagonists. All patients were examined for signs of contrast reactions, blood pressure, pulse rate and plasma histamine levels. These were measured before and three minutes after the administration of the antihistamine substances and/or the contrast medium, and also at the end of the examination. Following the administration of H1 and H2 receptor antagonists, a slight, transient and insignificant rise in plasma histamine could be demonstrated. Both groups showed a significant rise in plasma histamine levels after the injection of the contrast medium. Although the pre-medicated group contained a higher percentage of high risk patients, there were significantly fewer patients with contrast reactions. Pre-medication with H1 and H2 receptor antagonists in high risk patients is therefore advisable.

Aspects of driving after hypnotic therapy with particular reference to temazepam.

In a double-blind study, 32 outpatients with sleep disorders received oral doses of temazepam 20 mg (n = 16) or flunitrazepam 2 mg (n = 16) once a night for 7 days. On the morning after the first and seventh doses, patients completed psychomotor tests and a real driving test on the road over a 25 km course. The road test included a 1 km straight stretch driven at constant speed. In the car, various parameters were automatically recorded every second for approximately 60 minutes. These included angular velocity of steering, lateral acceleration and velocity. An optimization quotient, a measure of the efficiency of information processing in the driver-vehicle-road interaction, was derived from these parameters. An observer recorded driving performance by scoring standardized tasks. After a single dose of temazepam, the improvement in driver performance as shown by a decreased optimization quotient was significantly different to the deterioration seen after flunitrazepam (p less than 0.05). After the seventh dose, this trend was still apparent but was not statistically significant. After both one and seven doses, the angular velocity of steering was significantly decreased in the temazepam group compared with an increase after flunitrazepam (p less than 0.001). Over the straight, this deterioration in steering ability in the flunitrazepam group was apparent after one dose and reached statistical significance compared with temazepam after seven doses (p less than 0.01).(ABSTRACT TRUNCATED AT 250 WORDS)
