[Vaginal swab in pediatric age].

Vaginal secretion sampling in young patients. The aim of the study was to evaluate whether different instrumental tools (the classical cotton swab, the urethral cotton swab, the balloon shape cotton swab and cyto-brush) yield different sampling success in young patients presenting with suspected vulvovaginitis. The study was conducted in a randomised prospective way on 100 children with symptoms of vulvovaginal infection (age range: 4 months -8 years ) seen at the Clinic of Paediatric Gynaecology of the Department of Obstetric and Gynaecology, "V.Buzzi" Children's Hospital, Milano (Italy). Children's compliance and the number of samples which had to be discontinued, because of children's intolerance have allowed the evaluation of each tools. Each of these tools was tested on 25 young patients. The vaginal secretion sampling was not possible on 8 patients out of 25 with cyto-brush (32%), on 7 out of 25 with the balloon shape cotton swab (28%), on 4 out of 25 with the classical cotton swab (16%) and only 1 out of 25 with the urethral cotton swab (4%). In conclusion all the instruments tested allow sampling of vaginal secretion in young children, but the reaction and compliance of the patients has been best with the otologic cotton swab.

[Prenatal diagnosis of a case of Hallopeau-Siemens recessive dystrophic epidermolysis bullosa].

Fetal dystrophic recessive epidermolysis bullosa of the Hallopeau-Siemens type (EBdr-HS) was prenatally diagnosed in a couple at risk whose previous child was severely affected of the same disorder. Prenatal diagnosis was made at 18 weeks of gestation by ultrastructural examination of fetal skin samples obtained under continuous ultrasonic guidance. Four of the five specimens obtained showed dermal epidermal separation under the lamina densa with athophic and rare anchoring fibrils and superficial collagenolysis. The parents decided to terminate the pregnancy and ultrastructural skin examination of the aborted fetus, performed in multiple sites including the scalp, trunk, abdomen and limbs, confirmed the diagnosis showing no regional variation in the expression of the ultrastructural marker.

[Prenatal diagnosis of hemoglobinopathies in the 1st and 2nd pregnancy trimesters].

Fetal diagnosis of haemoglobinopathies, mainly thalassaemia, is widely used for the prevention of these diseases. Since 1975 fetal diagnosis has been carried out in the second trimester of pregnancy (18th - 22th week) on fetal blood samples obtained by fetoscopy or placentocentesis. The biochemical technique most commonly employed has been represented by carboxymethylcellulose chromatography and more recently in our center by isoelectric focusing of haemoglobins which is a faster and cheaper technique. First-trimester fetal diagnosis is now feasible by DNA analysis on chorionic villi. The Milan center experience in second trimester fetal diagnosis of haemoglobinopathies on more than 300 cases and the preliminary data on first trimester diagnosis are reported.

Serial amniocenteses in the management of twin-twin transfusion syndrome: when is it valuable?

Serial decompressive amniocenteses were performed at 18-23 weeks of gestation in 23 pregnancies referred due to echographic signs typical of twin-twin transfusion syndrome. The procedures were repeated until delivery or permanent normalization of the amniotic fluid volume both in the donor and the recipient twin sac. The overall survival rate was 57%, but in only 39% of the pregnancies did both twins survive without handicaps. The absence of end diastolic flow in the umbilical artery of the donor twin was associated with poor chances of survival for both fetuses; in contrast, the presence of hydrops or ascites in the recipient twin did not worsen the prognosis. A policy of aggressive amniotic fluid decompression may achieve permanent resolution of the fluid in over 50% of the pregnancies complicated by twin-twin transfusion syndrome.
