Percutaneous lung biopsy in cattle.

OBJECTIVE: To determine safety of lung biopsy in healthy cattle and compare 3 biopsy instruments for suitability of use. DESIGN: Prospective study. ANIMALS: 60 healthy cows. PROCEDURE: Bilateral lung biopsy through the ninth intercostal space was attempted in all cows. The procedure was performed with cows standing and restrained by an assistant. To study the short-term effects of lung biopsy, 50 cows were slaughtered 1 day after the procedure and examined. To study the long-term effects of lung biopsy, the remaining 10 cows were examined daily for 10 days and then slaughtered. Three biopsy instruments--the Autovac biopsy cannula, the Tru-Cut needle, and the Bard Biopty-System--were evaluated. RESULTS: 2 cows collapsed momentarily during the procedure but recovered quickly. The remaining cows did not develop adverse effects, and general attitude and condition were not affected. Postmortem examination of the 50 cows slaughtered 1 day after the procedure revealed lesions < or = 5.0 mm long and with a surface area < or = 4.0 mm2 at 108 of 154 (71%) biopsy sites involving the pulmonary pleura. Biopsy specimens were obtained during 48 of 50 (96%) procedures in which the Tru-Cut needle was used, during 38 of 39 (97%) procedures in which the Bard Biopty-System was used, and during only 1 of 11 biopsy procedures in which the Autovac cannula was used. At postmortem examination of the 10 cows slaughtered 10 days after the procedure, lesions were seen as small scars; adhesions were not observed. CONCLUSIONS AND CLINICAL RELEVANCE: Percutaneous lung biopsy is a safe procedure in healthy cows.

Decreased sedation by xylazine and high blood pressure in cows with BSE.

Fifteen cows with bovine spongiform encephalopathy (BSE) and 90 healthy cows were given xylazine intramuscularly at a dosage of 0.15 mg/kg bodyweight. The onset of sedation and of drooling was recorded, and the heart and respiratory rates and the systolic and diastolic blood pressure were measured every five minutes for 40 minutes. All the healthy cows but only five of the 15 cows with BSE became sedated, and the period between the administration of xylazine and the onset of sedation was twice as long in the cows with BSE than in the healthy cows (15.0 [7.5] and 7.6 [2.6] minutes). Throughout the observation period, the blood pressure of the cows with BSE was significantly higher than that of the healthy cows, and the blood pressure of the healthy cows, but not of the cows with BSE, decreased significantly towards the end of the observation period.

Ultrasonographic findings in three cows with chronic ragwort (Senecio alpinus) poisoning.

Three cows with regwort (Senecio alpinus) poisoning were examined clinically, haematologically and ultrasonographically, and biopsy specimens of the liver were examined histologically. At the end of the study, the cows were euthanased and examined postmortem. The major clinical signs included severely disturbed general demeanour and behaviour, and severe diarrhoea. One cow was photosensitive. The activities of liver enzymes and the concentration of bilirubin were high in all of the cows. In two of the cows, ultrasonographic examination revealed a heavy accumulation of abdominal fluid, which was diagnosed as non-inflammatory ascites. In all the cows, the liver parenchyma was heterogeneous, and cows 1 and 2 had multiple echogenic foci 5 to 10 mm in diameter. In cow 3, the facies diaphragmatica of the liver appeared irregular in outline owing to the presence of nodules which were approximately 5 cm in diameter. The liver was also greatly enlarged and extended almost to the linea alba ventrally and beyond the reticulum cranially. All the cows had portal hypertension and the portal vein was dilated, resulting in oedema of the walls of the gall bladder, the small intestines and the omentum. The diameter of the caudal vena cava was reduced as a result of the impaired hepatic circulation. Histological examination of liver biopsy specimens revealed severe hepatic fibrosis in all the cows.

[Bilateral hydrothorax with hydromediastinum after accidental catheter dislocation].

We report a complication during the insertion time of a central venous catheter in a patient with intracerebral bleeding. This complication was caused by an inadvertent dislocation of a subclavian catheter. Hydromediastinum and bilateral hydrothorax developed. There was a clear diagnosis followed by removal of the central venous catheter after radiological investigations could explain the cause of the complication and the clinical symptoms. In the course of events mediastinitis was diagnosed. The clinical condition improved under antibiotic therapy. The patient could be transferred to another clinical unit for endovascular treatment of an arterio-venous cerebral malformation.

Effects on vocal function and incidence of laryngeal disorder when using a laryngeal mask airway in comparison with an endotracheal tube.

A study of 56 patients was undertaken to determine whether there is a difference in the effect of the laryngeal mask airway and the endotracheal tube on the vocal tract after short-duration anaesthesia. All the patients were interviewed pre- and post-operatively. In 43 patients, it was possible to assess the larynx using videoendoscopy and videostrobolaryngoscopy both pre- and post-operatively. Selected acoustic characteristics were investigated in 51 patients pre- and 18-24 h post-operatively. Thirty-four patients that did not have anaesthesia were used as an age-matched control. The interview showed, that patients in the LMA group complained less frequently of having post-operative laryngeal discomfort. The videostrobolaryngoscopy demonstrated minor lesions of the vocal tract in six patients in the ETT group and in one patient in the LMA group. Of the 12 voice variables evaluated, there was no significant difference in any parameter between the two groups. Both groups had a higher fundamental frequency post-operatively.

Intrauterine infection with bovine virus diarrhoea virus on alpine communal pastures in Switzerland.

This study involved 13 calves, one to 50 days of age, born to first calf heifers that had been pastured on one of seven alpine communal pastures in the canton of St. Gallen during the summer of 1995. Of a total of 993 cattle pastured, 61 were pregnant heifers that were negative for bovine virus diarrhoea (BVD) antigen and for BVD antibodies at the start of pasturing. Seroconversion occurred in 26 of these pregnant heifers during the pasture period. Blood samples and skin biopsy specimens of calves born to 13 of these were examined for BVD antigen by antigen-ELISA and by an immunohistochemical technique, respectively. Blood samples were positive for BVD antigen in four calves, questionable in one calf, negative in seven and missing in one prematurely born calf. The four calves that were positive for BVD antigen in the blood were also positive in skin biopsies. Of the seven calves with a negative or missing blood test, six had positive and two had negative skin samples. Based on the combined results of blood and skin testing, 11 of 13 calves were positive for BVD antigen. Of the 11 infected calves, six were normal at birth, four were smaller than normal and one was premature and weak and was euthanized on humane grounds. Of the four small calves, two developed diarrhoea and died within the first month of life. The two calves that were negative for BVD antigen were clinically normal. The results of this study not only demonstrate the occurrence of in-utero infection with BVD virus, but also stress the importance of alpine communal pasturing in the spread of BVD virus. Because the prevention of infection with BVD virus on communal pastures does not seem feasible, it is recommended that all calves born to cows from such pastures be tested for BVD antigen.

Serological evidence of infection with Ehrlichia spp. in red foxes (Vulpes vulpes) in Switzerland.

Serum samples from 1,550 red foxes in Switzerland were tested for antibodies to the agents of canine granulocytic and monocytic ehrlichiosis by an indirect immunofluorescent technique. Forty-four (2.8%) of the samples were positive for Ehrlichia phagocytophila, which is an antigen marker for granulocytic ehrlichiosis. In contrast, none of the samples had antibodies specific to Ehrlichia canis, the agent of monocytic ehrlichiosis.

Quantitative real-time PCR for detection of members of the Ehrlichia phagocytophila genogroup in host animals and Ixodes ricinus ticks.

A TaqMan PCR was established for identification and quantitation of members of the Ehrlichia phagocytophila group in experimentally infected cows and in Ixodes ricinus ticks. The TaqMan PCR identified a 106-bp section of the 16S rRNA gene by use of a specific fluorogenic probe and two primers. This technique was specific for members of the E. phagocytophila group, which include E. phagocytophila, Ehrlichia equi, and the agent of human granulocytic ehrlichiosis. The TaqMan system identified 10 copies of a cloned section of the 16S rRNA gene of E. phagocytophila. The sensitivity and specificity of the TaqMan PCR were similar to those of conventional nested PCR. The numbers of ehrlichiae in leukocytes of the two cows experimentally infected with E. phagocytophila were measured daily by TaqMan PCR and had a course similar to that of the percentages of infected leukocytes determined daily by light microscopy. The prevalence of infected free-living ticks, which were collected from areas where bovine ehrlichiosis is endemic and from regions with sporadic occurrences of granulocytic ehrlichiosis in dogs and horses, was identical as determined by nested PCR and TaqMan PCR.

Evidence of the human granulocytic ehrlichiosis agent in Ixodes ricinus ticks in Switzerland.

A total of 1,667 Ixodes ricinus ticks were collected from five regions in Switzerland where there have been sporadic occurrences of granulocytic ehrlichiosis in dogs and horses. The ticks were examined for rickettsiae of the Ehrlichia phagocytophila group via nested PCR. Twenty-one ticks (1.3%) were positive; 3 (0.5%) were nymphs, 6 (1.3%) were adult males, and 12 (1.9%) were adult females. The number of positive ticks varied with the stage of development and with the geographical origin. Nucleotide sequencing of the isolated PCR products identified these products as part of the 16S rRNA gene of Ehrlichia. In addition, these products had 100% homology with the agent of human granulocytic ehrlichiosis. The occurrence of this agent in I. ricinus in Switzerland presents a potential danger of transmission of granulocytic ehrlichiosis to dogs, horses, and humans.

[Investigations on the treatment of cerebrospinal nematode infections in goats].

In 17 goats the therapeutical outcome of cerebrospinal nematodiasis, a parasitic infection in the central nervous system with the nematode Elaphostrongylus cervi, is described. The diagnosis was made on the basis of physical findings and cerebrospinal fluid eosinophilia. The therapy was based on the administration of a non-steroidal antiinflammatory drug (flunixin meglumine) and two anthelmintics (fenbendazol, ivermectin) for five days. The response to therapy was documented immediately and 30 days after therapy and by owners report up to 30 months. Eleven goats (64.7%) showed an improvement of the neurological signs directly after therapy. Three had a complete recovery. Signs of a light and middle ataxie were still present in 6 and 2 goats, respectively. Six recumbent goats (35.3%) had to be euthanasied because of a non-therapeutical response. This study has shown a clinical impression of successful use of flunixin meglumine, fenbendazole and ivermectin in goats with light to middle neurological signs. The goats could accommodate the remaining ataxia without showing a reduced production. Goats with high neurological deficits could not be successful treated based on the high degree of the neurological damage.

[Epidemiology and risk factors of cecal dilatation/dislocation and abomasal displacement in dairy cows].

The aims of the study were to determine the prevalence of cecal dilatation/dislocation (CDD) and abomasal displacement (DA) in Switzerland, to identify risk factors for both diseases, and to compare directly their epidemiologic situation. The epidemiologic study included 158 cases of CDD and 149 cases of DA from the cases referred to both University Clinics of Berne and Zurich. The results showed that DA was associated with nutrition-related risk factors: use of minerals and sodium chloride, inadequate concentrate feeding and beginning of the feeding. Furthermore, breed was significantly associated with DA, but not with CDD. Finally, milk yield and pasture were also significantly included in the models. For CDD, nutrition-related risk factors were also found: pasture in summer, use of corn pellets and corn silage in winter. In the final model, protein concentrate was also included. Although both diseases were found at comparable frequencies, the results of this study indicate marked differences between the epidemiology of occurrence of CDD and DA. Therefore, the hypothesis of a common etiopathogenesis appears unlikely.

Experimental infection of four horses with Ehrlichia phagocytophila.

Four clinically healthy horses which were negative for antibodies to Ehrlichia phagocytophila, the agent of bovine ehrlichiosis, were infected experimentally with E phagocytophila-containing bovine leucocytes, administered intravenously. The horses were examined daily for four weeks, and blood samples were collected daily for cytological, haematological and biochemical examination and for a nested polymerase chain reaction (PCR). An indirect immunofluorescence test was used to determine when the horses seroconverted and the duration of positive titres. There were no abnormal clinical, haematological or biochemical findings in any of the four horses and all the PCRs yielded negative results. However, all the horses seroconverted with reciprocal titres of up to 1280, and the positive titres persisted for up to five months.

Experimental oral transmission of Ehrlichia phagocytophila to calves.

Three groups of four calves were used to determine whether Ehrlichia phagocytophila could be transmitted orally to calves via infected milk. Groups 1 and 2 consisted of four-week-old calves and group 3 of newborn calves. The calves in group 1 were fed for several days with milk from cows infected experimentally with E phagocytophila. The calves in groups 2 and 3 were fed 200 ml of whole blood containing E phagocytophila organisms; for group 2 the blood was added to milk before being fed, and for group 3 the blood was added to colostrum before being fed within three hours after birth. Blood samples for haematological, serological and cytological examination, and for polymerase chain reaction (PCR) were collected from all the calves, starting on the first or only day of administration and then every four days for four weeks. The calves of groups 1 and 2 showed no clinical, haematological or serological changes, and there was no direct or indirect evidence of the agent. In contrast, all the calves in group 3 had mild pyrexia and seroconverted on day 8, and in one of them E phagocytophila organisms were visible in leucocytes, and the PCR on the buffy coat was positive on day 8.

Diagnostic reliability of clinical signs in cows with suspected bovine spongiform encephalopathy.

The clinical findings in 50 cows with suspected and subsequently confirmed bovine spongiform encephalopathy (BSE) (group A) were compared with the clinical signs in 22 cows with suspected BSE, but with no histological evidence of the disease (group B). The chi-square test for association was used to compare the frequencies with which diagnostic signs or combinations of signs, were positive in the cows of groups A and B. When the frequency of a sign differed significantly, its sensitivity, specificity, efficiency and positive and negative predictive values were calculated. With respect to changes in behaviour the cows in group A more frequently showed increased excitability, nervous ear and eye movements, increased salivation and increased licking of the muzzle than the cows of group B. With respect to changes in sensitivity the cows in group A were more frequently hypersensitive to touch, noise and light than the cows of group B. With respect to changes in locomotion the cows in group A were more frequently ataxic than the cows in group B.

Ultrasonographic findings in five cows before and after treatment of reticular abscesses.

Five cows with reticular abscesses were examined clinically, haematologically, radiographically and ultrasonographically. They all had clinical signs typical of traumatic reticuloperitonitis, including chronic indigestion, pyrexia, an absence of or reduced ruminal motility, weight loss and a positive reaction to foreign body test. A haematological examination revealed anaemia, increased concentrations of plasma protein and fibrinogen and a decreased clotting time in the glutaraldehyde test. On the basis of the radiographic examination, a tentative diagnosis of reticular abscess was made in four of the cows, because the reticulum was displaced from the peritoneum or because there was an extensive gas-fluid interface in the reticular region. By ultrasonography, a large reticular abscess with a well developed capsule was visible in each of the cows. The abscess was located between the reticulum and ventral peritoneum in two of them, between the reticulumn and right thoracic wall in two and between the reticulum and spleen in the other cow. A foreign body penetrating the abscess could be visualised ultrasonographically in one cow. In two cows, the abscesses were drained through an ultrasound-guided transcutaneous incision. In the other three cows, the abscess was incised and drained from within the reticulum during a rumenotomy. Ultrasonographic examination revealed that the abscess had been completely evacuated in four cows, but only by about two-thirds in the remaining cow. All the cows were clinically healthy when they were discharged.
