[Malignant catarrhal fever in Switzerland. 1.Epidemiology].

Malignant catarrhal fever (MCF) is a usually fatal infectious disease of cattle with global distribution. Based on the recent introduction of a diagnostic PCR assay and a competitive inhibition ELISA (ciELISA) epidemiological data were collected on field cases in Switzerland. Throughout a three-year period, an MCF incidence of 0.6@1000 was observed, with a gradient of cases from Eastern to Western Switzerland. While the cantons Wallis, Vaud and Geneva reported no and the remaining western cantons only reported a few cases, the highest incidence was observed in the cantons Appenzell Innerrhoden, Lucern, Glarus, Grison, St. Gallen, Schwyz, and Thurgau. MCF occurred seasonally and an age-related clustering was also observed. About 50% of all cases and all outbreaks with more than one animal in a single herd occurred between April and June. Animals between six months and two years were strongly over represented. Observations on four surviving cattle showed that the outcome of the disease is not invariably fatal and that these persistently infected cows can produce healthy negative calves. Investigations on the aetiology indicate that the main reservoir for OvHV-2 is in sheep and possibly goats, while cattle do not normally harbor the virus. An OvHV-2 negative sheep herd was raised from lambs, which were reared colostrum-free and in isolation from their mothers. The success rate clearly indicated that vertical intrauterine infection is not the main mode of transmission among sheep. Therefore, horizontal, seasonally occurring transmission of OvHV-2 among sheep has to be assumed.

[Clinical findings and diagnostic procedure in a dairy sheep with visna].

The goal of this report was to describe the clinical signs and diagnosis of Visna in a seven-year-old East Friesian milk sheep. A striking feature was that the ewe's behaviour changed frequently. At one time, the ewe was somnolent. A few minutes later, the sheep was alert and eating hay. The ewe was thin. It had a slight head tilt and a severe generalised ataxia. Based on the neurological symptoms and chronic weight loss, a tentative diagnosis of visna was made. Serological testing for maedi-visna was positive, and the ewe was euthanised. A postmortem examination was performed, and lung and brain samples were collected aseptically. Cell cultures from these organs were positive for viral enzymatic reverse transcriptase and for maedi-visna RNA.

[Malignant catarrhal fever in Switzerland: 2. Evaluation of the diagnosis].

Malignant catarrhal fever (MCF) is a mostly fatal lymphoproliferative disease of cattle. In 1995 a PCR based method was introduced for the detection of the ovine herpesvirus 2 (OvHV-2), which is regarded as the causative agent of the sheep-associated form of the disease. This PCR can be regarded as a gold standard for the in vivo diagnosis of sheep-associated MCF in cattle (Müller-Doblies et al., 1998). This semi-nested PCR was now used as a reference test for the reassessment of diagnostic criteria in the clinical and post mortem diagnosis that could previously not be quantitated. Based on 83 suspected cases with a complete clinical record the clinical signs were weighted and grouped according to their sensitivity and specificity into lead signs indicative of MCF and frequently accompanying signs supportive for the diagnosis of MCF and general clinical signs that were less reliable for the diagnosis. Differential diagnoses are discussed, which are of particular significance due to their status as OIE list A diseases e.g. foot-and-mouth disease or rinderpest. 38 PCR confirmed cattle with MCF served for the quantitative analysis of organ lesions. For the post mortem diagnosis an essential set of organ samples is defined to permit a reliable histological diagnosis, as the gross pathology often did not give any indication for the diagnosis. These criteria should help to improve the diagnostic efficiency and to select the appropriate laboratory diagnostic procedures for MCF-suspected cattle.

Ultrasound-guided percutaneous portocentesis in 21 cows.

An ultrasound-guided portocentesis was performed in 21 clinically healthy cows, and blood was collected for haematological, biochemical and blood-gas analyses. At the same time blood samples were collected from the left jugular vein of each cow for the same analyses. Immediately after the centesis, portal blood pressure was measured. The cows were examined daily thereafter for eight days and then slaughtered for postmortem examination of the internal organs. The mean (sd) portal blood pressure was 37.1 (5.47) mmHg, and ranged from 24 to 49 mmHg. There were small but significant differences between many of the haematological and biochemical parameters in jugular and portal blood, and the concentration of bile acids was on average 2.7 times higher, and the concentration of ammonia was 19.3 times higher in portal blood. The demeanour and behaviour of the cows, and their appetite and rectal temperature remained normal during the eight-day observation period. The haematocrit, total and differential leucocyte counts, and the concentrations of total solids and fibrinogen were not affected by portocentesis. On postmortem examination, no lesions were apparent in the peritoneum, liver and portal vein.

Different sensitivity to receptor editing of B cells from mice hemizygous or homozygous for targeted Ig transgenes.

Ig knock-in mice have been used to study the relative contribution of receptor selection versus clonal selection in the control of autoreactive B cells. The anti-MHC class I 3-83Ig knock-in (3-83Igi) mice manifest extensive receptor editing in the presence of H-2(b). However, receptor editing is also observed on the H-2(d) background, although reactivity toward this antigen is below detection and its presence does not affect the generation of 3-83Ig(+) mature B cells in classical 3-83Ig transgenic mice. In this study we have analyzed the contribution of genetic background, B cell receptor signaling, and transgene copy number on the initiation and extent of receptor editing in the 3-83Igi;H-2(d) mice. Crossing the 3-83Ig insertion into either CD45-deficient H-2(d) mice or onto the BALB/c background reduces the extent of receptor editing and increases the fraction of 3-83Ig-expressing B cells, indicating that in the original line editing depends on B cell receptor signaling induced by cross-reacting antigen(s). However, receptor editing is still detectable in hemizygous 3-83Igi mice even on the BALB/c background, on which the 3-83 antibody was originally raised, whereas it is abrogated in homozygous 3-83Igi;H-2(d) animals. This latter observation indicates that immature B cells expressing immunoglobulin from single heavy and light chain loci, as they do physiologically, utilize receptor editing for an exquisite quality control of their antigen receptor that may only partly be based on self-reactivity.

Ultrasound-guided percutaneous lung biopsy in sheep.

Lung biopsies were taken through the ninth intercostal space on the two sides of the thorax of 10 clinically healthy sheep, using a Tru-Cut needle on one side and the Bard Biopty-System on the other. Nine of the 10 sheep remained clinically healthy, but one coughed transiently and had a mild bloody discharge from the right nostril immediately after the biopsy. The sheep were slaughtered 10 days later and the lungs and pleura were examined macroscopically; there were either no lesions or only small scars visible at the sites of the biopsies. However, well-developed subpleural nodules due to parasites were observed in some of the lungs. There were no adhesions between the costal and pulmonary pleurae. Of the 20 biopsy specimens, 18 were ideal for histological examination, and none of them was histologically normal. Mild interstitial pneumonia was diagnosed in 15 specimens, chronic bronchiolitis in nine specimens, and hyperplasia of smooth muscle or connective tissue in 17.

Diagnosis of Lyme disease in two cows by the detection of Borrelia burgdorferi DNA.

Two cows from different herds in a district of Switzerland known to harbour ixodid ticks had erythematous lesions on the hairless skin of the udder, were in poor general condition with a poor appetite and decreased milk production, and had a stiff gait and swollen joints. Borrelia burgdorferi sensu strictu DNA was detected in samples of synovial fluid and milk from one of the cows and Borrelia afzelii DNA was detected in synovial fluid from the other by means of a real-time PCR.

Urethral duplication and chromosomal translocation in a Swiss braunvieh heifer.

As it was urinating, a six-month-old Swiss braunvieh heifer produced a second stream of urine from a fistula that opened on the ventrolateral margin of the left vulval lip. A catheter was introduced into this opening and passed easily into the bladder. Urethrography showed that the fistula joined the urethra in the mid-pelvic region and that a single canal originated from the bladder. Endoscopy confirmed this finding and also revealed a duplication of the vaginal portion of the cervix, a division of the cranial vagina by a septum and a fibrous band in the region of the hymenal ring. Cytogenetic examination revealed reciprocal translocation between chromosomes 20q23 and 22q23. A diagnosis of urethra duplex, duplication of the vaginal portion of the cervix and reciprocal autosomal translocation between chromosomes 20 and 22 was made on the basis of these findings.

Developmental changes in regulation of mitochondrial respiration by ADP and creatine in rat heart in vivo.

In saponin-skinned muscle fibers from adult rat heart and m. soleus the apparent affinity of the mitochondrial oxidative phosphorylation system for ADP (Km = 200-400 microM) is much lower than in isolated mitochondria (Km = 10-20 microM). This suggests a limited permeability of the outer mitochondrial membrane (OMM) to adenine nucleotides in slow-twitch muscle cells. We have studied the postnatal changes in the affinity of mitochondrial respiration for ADP, in relation to morphological alterations and expression of mitochondrial creatine kinase (mi-CK) in rat heart in vivo. Analysis of respiration of skinned fibers revealed a gradual decrease in the apparent affinity of mitochondria to ADP throughout 6 weeks post partum that indicates the development of mechanism which increasingly limits the access of ADP to mitochondria. The expression of mi-CK started between the 1st and 2nd weeks and reached the adult levels after 6 weeks. This process was associated with increases in creatine-activated respiration and affinity of oxidative phosphorylation to ADP thus reflecting the progressive coupling of mi-CK to adenine nucleotide translocase. Laser confocal microscopy revealed significant changes in rearrangement of mitochondria in cardiac cells: while the mitochondria of variable shape and size appeared to be random-clustered in the cardiomyocytes of 1 day old rat, they formed a fine network between the myofibrils by the age of 3 weeks. These results allow to conclude that in early period of development, i.e. within 2-3 weeks, the diffusion of ADP to mitochondria becomes progressively restricted, that appears to be related to significant structural rearrangements such as formation of the mitochondrial network. Later (after 3 weeks) the control shifts to mi-CK, which by coupling to adenine nucleotide translocase, allows to maximally activate the processes of oxidative phosphorylation despite limited access of ADP through the OMM.

[Accuracy of intraoperative urinary bladder temperature monitoring during intra-abdominal operations].

OBJECTIVE: This study investigates whether the site of abdominal surgery or the urine flow rate affects the accuracy of urinary bladder temperature monitoring. METHODS: After approval by the local ethics committee we studied 7 patients during upper abdominal and 10 patients during lower abdominal surgery. Temperatures were recorded with a Hi-Lo Temp Esophageal-Stethoscope (Mallinckrodt Medical) and a Foley Catheter Temperature Sensor FC400-18 (Respiratory Support Products, Mallinckrodt Medical). Each probe and its recording unit were calibrated over a range of 30-40 degrees C against a reference quartz thermometer (Hewlett Packard Model 2801 A) in a water bath before the investigation. Urine flow rate was measured using a urometer. Temperatures and urine flow rate were recorded every 30 min. Agreement between the methods of measurement was assessed as described by Bland and Altman. RESULTS: 124 measuring points could be analyzed. Bladder temperature had a bias (B) of -0.06 degree C compared to oesophageal temperature. Limits of agreement (LOA; +/- 2 s) were +/- 0.68 degree C. In upper abdominal surgery (B: 0.02 degree C; LOA: +/- 0.42 degree C) a higher precision of oesophageal temperature estimation could be demonstrated compared to lower abdominal surgery (B: -0.14 degree C; LOA: +/- 0.82 degree C). Lower urine flow rates generally increased the limits of agreement. Regarding lower abdominal surgery the bias additionally increased to -0.22 degree C. CONCLUSION: Urinary bladder temperature recording is a clinically acceptable method to measure core temperature during abdominal surgery. The accuracy during lower abdominal surgery is decreased compared to upper abdominal surgery, especially in case of a urine flow rate below 250 ml/h.

[Value of reflecting disposable insulation (Thermodrape) in preventing perioperative hypothermia].

OBJECTIVE: The aim of the study was to evaluate the value of reflecting disposable insulation for the prevention of perioperative hypothermia. METHODS: After approval by the local ethics committee 36 patients undergoing long lasting urological intraabdominal surgery were studied. Anaesthesia was performed using etomidate, fentanyl, midazolam, pancuronium and succinylcholine. Patients were randomly assigned to 4 groups. These groups were treated as follows: Gr. 1: Infusion warmer (Hotline HL-90 with System L-70, Level 1 Technologies Inc., Marshfield, USA) and standard O.R. draping with two layers of cotton drapes. Gr. 2: Infusion warmer and reflecting disposable insulation (Thermadrape, O.R. Concepts Inc., Roanoke, USA) covering the legs, upper body, arms and head. Gr. 3: Infusion warmer and convective air warming with upper body blanket (WarmTouch, Mallinckrodt Medical, Hennef/Sieg, Germany). Gr. 4: Infusion warmer, convective air warming and reflecting disposable insulation. RESULTS: After 2 hours of surgery patients of groups 1 and 2 became hypothermic with core temperatures of 35.1 and 35.6 degrees C respectively. No relevant difference could be found between the two groups. The combination of an infusion warmer and convective air warming was an effective method to prevent hypothermia in groups 3 and 4. After 2 hours of surgery these patients had core temperatures of 36.6 and 36.4 degrees C respectively. Reflecting disposable insulation did not improve the effect of convective air warming. CONCLUSION: Reflecting disposable insulation was insufficient in the investigated operative setting.

Carbon dioxide absorption during extraperitoneal and transperitoneal endoscopic hernioplasty.

UNLABELLED: Transabdominal preperitoneal (TAPP) or total extraperitoneal (TEP) hernioplasty are probably associated with differing degrees of CO(2) absorption which can influence anesthetic management and perioperative morbidity. We studied 20 patients with either TAPP or TEP for perioperative CO(2) absorption (calculated from CO(2) elimination and metabolic CO(2) production) and ventilatory changes required to maintain normocapnia (blood gas analyses). CO(2) absorption reached plateau values in the TAPP group, but increased over time in the TEP group. Median CO(2) absorption during insufflation was 61 mL/min (range 43-78) for TAPP and 114 mL/min (range 75-178) for TEP, with a maximum of 114 mL/min (range 75-178) for TAPP and 258 mL/min (range 112-585) for TEP. Median minute ventilation (V(E)) required for maintaining normocapnia was 9. 5 L/min (range 7.7-11.5) for TAPP and 12.9 L/min (range 9.0-22.6) for TEP (P: < 0.01). Seven patients in the TEP group required over 18 L/min V(E), although no patient in the TAPP group required more than 14 L/min V(E). All patients in the TEP group had significant subcutaneous emphysema resulting in one case of delayed tracheal extubation. We conclude that CO(2) absorption is consistently less with TAPP. IMPLICATIONS: The greater magnitude of carbon dioxide absorption during total extraperitoneal hernioplasty puts an additional load on the lungs and could pose a risk for patients with chronic lung disease who might be unable to eliminate excess carbon dioxide.

Effect of different anaesthetic regimes on the oculocardiac reflex during paediatric strabismus surgery.

The oculocardiac reflex (OCR) is induced by mechanical stimulation and therefore is frequently encountered during strabismus surgery. This study was designed to determine how various anaesthetic regimes modulate the haemodynamic effects of the OCR during paediatric strabismus surgery. Thirty-nine patients (4-14 years, ASA I) were randomized to one of four anaesthetic regimes: group P: propofol (12 mg.kg(-1).h(-1)) and alfentanil (0.04 mg.kg(-1).h(-1)); group S: sevoflurane 1-1.2 MAC in 30% O(2)/70% N(2)O; group K: ketamine racemate (10-12 mg. kg(-1).h(-1)) and midazolam (0.3-0.6 mg.kg(-1).h(-1); group H: halothane 1-1. 2 MAC in 30% O(2)/70% N(2)O. Electrocardiogram (ECG), beat-to-beat heart rate (HR) and blood pressure (BP) changes were measured during and after a standardized traction was applied to an external eye muscle (4-6 Newton, 90 s). OCR was defined as a 10% change in HR induced by traction. OCR occurred in 77% of patients. Whereas virtually all patients in the P, H and S groups developed OCR, only 22% developed it in group K. Median HR change in group P (-37 bpm) was significantly greater (P < 0.05) than in group H (-17 bpm) or group K (-7 bpm). Median BP change in group K (+10 mmHg) was significantly different (P < 0.05) from group H (-5 mmHg), group S (-3 mmHg) and group P (-8 mmHg). Atrioventricular rhythm disorders were significantly more frequent in group P compared with group K (P < 0.02). Respiration-induced sinus dysrhythmia was significantly less frequent (P < 0.001) in group K (0%), compared with group P (100%), group H (56%) and group S (55%). Of the anaesthetic techniques studied, ketamine anaesthesia is associated with the least haemodynamic changes induced by OCR during strabismus surgery in paediatric patients.

[Enzootic calcinosis in 16 cows from 6 dairy farms in Unterengadin].

Enzootic calcinosis was diagnosed in 16 cows, which originated from 6 dairy farms located in the Unterengadin valley in Switzerland between 1,402 and 1,650 metres above sea level. During the winter, the cows were fed haylage, which contained up to 50 per cent golden oat-grass (Trisetum flavescens), and hay and grain. The first symptoms of enzootic calcinosis were noted in March 1998, when some of the cows developed locomotor abnormalities. Additional cases were diagnosed during the following three months. The most important clinical findings were decreased milk production, weight loss, frequent recumbency, difficulty in rising, kneeling while rising and feeding, stilted gait, arched back, shifting weight from one foot to another and reluctance to remain standing after being roused. Of the 16 cows, 5 had elevated concentrations of serum calcium and 10 had decreased concentrations of serum magnesium. All cows had normal or decreased concentrations of serum phosphorus, because the feeding of haylage containing golden oat-grass had been discontinued two months previously. In all of the cows, the most important postmortem findings were severe calcification of the aorta, iliac artery, brachiocephalic trunk and pulmonary artery.

[Ehrlichiosis in Switzerland--significance for veterinary medicine].

Ehrlichiosis is a rickettsial disease of animals and humans caused by various species of Ehrlichia. These obligate intracellular microorganisms infect granulocytes or monocytes of mammalian hosts and are transmitted mostly by ticks. The febrile disease is often undiagnosed and characterized by leukopenia, anemia and thrombocytopenia. The diagnosis of ehrlichiosis is based on clinical findings, tick infestation, direct or indirect detection of the agent, and serology. The role of native and imported ehrlichiae of animals in Switzerland is discussed.

[Demodicosis in a Toggenburg goat].

This report describes the findings in a four-year-old Toggenburg goat with demodicosis. The skin had multifocal nodules, which were approximately 5 mm in diameter and contained thick yellow exsudate. Microscopic examination of the exsudate revealed numerous Demodex caprae. The goat was clipped and treated topically every five to seven days for a total of 12 treatments with a 1:100 dilution of amitraz (Ectodex, Hoechst Roussel Vet). The treatment resulted in a marked decrease in the number of skin nodules. However, new nodules appeared after treatment was discontinued and complete clinical cure was not achieved.

Experimental cross-infections with Ehrlichia phagocytophila and human granulocytic ehrlichia-like agent in cows and horses.

Four cows and four horses were infected experimentally with Ehrlichia phagocytophila, the cause of tickborne fever in ruminants, and with human granulocytic ehrlichia-like agent, a recently discovered species that infects people, horses and dogs in the USA and Europe. They were infected in either order, 30 days apart, to investigate serological cross-reactivity within the Ephagocytophila genogroup. The course of infection was assessed by routine clinical, haematological, serological and polymerase chain reaction (PCR) examinations. Two of the cows infected with Ephagocytophila and two of the horses infected with granulocytic ehrlichia-like agent, developed characteristic signs of ehrlichiosis. When the same animals were infected with their heterologous ehrlichial isolate 30 days later, they did not develop clinical signs of disease. The infection of the other two cows with human granulocytic ehrlichia-like agent and the other two horses with Ephagocytophila, resulted in asymptomatic seroconversion. When the same animals were infected with their homologous ehrlichial isolate 30 days later, they remained asymptomatic and had normal haematological results and negative PCRS until the end of the monitoring period, 60 days after the first infection. In these animals, there was an increase in antibody titre after the second infection which was interpreted as a specific immune response, and as a reactivation of the immune response to the first infection.
