[Functional treatment of acute Achilles tendon rupture. A histological, immunohistological and ultrasonographic analysis of the animal model].

In 105 rabbits the course of healing was examined at 1, 2, 4, 8 and 12 weeks (21 rabbits per group) after an experimental Achilles tendon rupture. The following treatment modalities were compared: a. operative functional treatment (resorbable suture, Kleinert technique) b. operative functional treatment with fibrin glue c. primary functional treatment. For the functional (after)-treatment a special orthosis was applied. A 7.5 MHz Ultrasound probe was used for the ultrasonographic evaluation. The histological specimens were stained after Masson-Goldner with Azan. Collagen Type III was depicted immunohistologically with polyclonal antibodies. A semiquantitative fibrocytes count was performed. The histological results showed a smooth healing in the prim. functional treatment group reaching parallel orientation of collagen fibers at 12 weeks. In the fibrin glue-group the fibrin was resorbed after 4 weeks without essential influence to the course of healing. In the suture-group a secondary gapping of the tendon stumps was detectable. At 12 weeks the histological evaluation in all groups showed approximately normal tendon pattern. Immunohistochemically all groups showed cell-associated positive reactions for type III-collagen after 1 week with a maximum after 2 weeks. The semiquantitative fibrocyte count in the primary funct. group showed a maximal number after 1 week, in the fibrin glue- and suture-groups the maximal number could be found after 2 weeks. Sonographically an increase in tendon thickness was detectable up to the 4th weeks in all groups. The secondary gapping of the tendon stumps in the suture group could be detected sonographically. The echogenicity of the tendon during the course of healing showed increasing homogeneity and parallelism in all groups. At 12 weeks the echogenicity was comparable in all groups. The experiment could prove the equivalence of the primary functional treatment to operative therapy in Achilles tendon rupture.

[Augmented tenoplasty in a case of tibialis anterior tendon rupture].

A tendon transfer or interposition tendon graft is usually recommended for treatment of late diagnosed ruptures of the tibial anterior tendon. A new operative technique using augmented tenoplasty is described in one patient. After 3 years the patient was asymptomatic and showed normalized function. The operative method has the advantage of no additional loss of function in comparison with other methods.

Medium-term results of the operative treatment of recurrent patellar dislocation by Insall proximal realignment.

Between 1984 and 1991, 36 patients with the diagnosis of recurrent patellar dislocation were treated operatively using the proximal realignment procedure. Thirty patients were available for follow-up. The average follow-up period was 6.3 years (range 2-9.6 years). The average age at injury was 21.3 years with a predominance of female patients. At follow-up all knees were physically examined. The results were evaluated using the score of Larsen and Lauridsen as well as the Tegner score and subjective assessment. Radiographs from 19 patients (63%) were available for review. At follow-up one patient suffered from a recurrence of patellar dislocation. All patients had stable knee joints and a full range of motion. There was no statistically significant difference between pre- and postoperative sports activity level. Seven patients (23.3%) had excellent results, 12 patients (39.9%) good results and 1 a poor result using the Larsen and Lauridsen score. Subjective assessment revealed the operative result as very good, good or satisfactory in 90%. Patellofemoral osteoarthritis was seen in 7 of 19 patients (36.8%). With a proximal realignment procedure good clinical results can be achieved for recurrent patellar dislocation. Subjective satisfaction with this procedure is rated as good. It is successful in preventing redislocation.

Immunohistochemical localization of collagen VI in arthrofibrosis.

Arthrofibrosis is a disabling complication after knee trauma and surgery. Clinically, it is characterized by pain and joint stiffness due to massive connective tissue proliferation. In similar pathological conditions with fibrotic transformation such as lung fibrosis or superficial fibromatoses, an increased expression of collagen type VI has been reported. Collagen VI, which forms a filamentous network, is thought to serve as an anchoring element between collagen I/III fibrils and basement membranes and as a cell binding structure. Collagen VI may also play a contributing role in the pathogenesis of arthrofibrosis. The aim of the present study was therefore to demonstrate the localization and distribution of type VI collagen in arthrofibrotic tissue. Tissue samples from the infrapatellar fat pad and intercondylar synovia of 13 patients suffering from arthrofibrosis were taken at surgery. The expression of type VI collagen was studied immunohistochemically using an immunoperoxidase method for light microscopic visualization. Histologic analysis showed a synovial hyperplasia with inflammatory cell infiltration and vascular proliferation. Compared with normal synovial tissue, type VI collagen was widely distributed as a network subsynovially and around the capillary walls. The results of the present study suggest that dysregulation of collagen VI synthesis could be an important contributing factor in the complex mechanisms of disordered matrix protein deposition leading to arthrofibrosis.

[Interim results of surgical therapy of patellar dislocation by Insall proximal reconstruction].

Between 1984 and 1991, 57 patients with diagnosis of an acute or recurrent patellar dislocation were treated operatively using proximal realignment with vastus medialis transfer and lateral release (Insall operation). A total of 45 patients (15 with acute and 30 with recurrent patellar dislocation) were reviewed at an average follow-up of 6.5 years (2-9.6 years). The average age at injury was 21.5 years, with a predominance of female patients. Follow-up examination included routine knee examination, clinical review using the modified knee scoring scale of Larsen and Lauridsen, sports activity level and subjective satisfaction. Radiographics from 27 patients (60%) were evaluated. One patient from each group suffered recurrence of patellar dislocation. All patients had stable knee joints and a full range of motion. There was no statistical difference in the pre- and postoperative sports activity level in both groups. Three patients (19.9%) with acute patellar dislocation and seven patients (23.3%) with recurrent patella dislocation had excellent results using the Larsen and Lauridsen score scale. Ten patients (66.6%) with acute and 12 (39.9%) with recurrent dislocation had good results. One patient with recurrent patellar dislocation had a fair result. Subjective evaluation revealed the operative result in 93% of cases as very good, good or satisfactory. Patellofemoral osteoarthritis was seen in 11 (40.7%) of 27 patients. Our results show good clinical results for the treatment of acute patellar dislocation in young, active patients with the proximal realignment procedure. The recurrence rate of patellar dislocation can be reduced for acute and recurrent patellar dislocation. Subjective satisfaction with this procedure is rated very good.

[Endoscopic versus limited open technique for replacement of the anterior cruciate ligament. 4 years outcome of a prospective study].

ACL reconstruction with patellar tendon graft has become a standard procedure. The graft can be inserted either using two tunnels and a lateral femoral incision or with a femoral half tunnel drilled from the joint, thus avoiding the lateral incision. Advantages of the single-incision technique in the early rehabilitation period have been claimed. 40 patients with ACL deficiency were included in a prospective randomized trial comparing single and two-incision technique with a follow-up period of 4 years. Preoperative data did not show any significant difference between the two groups. At follow-up no difference were observed with respect to complications or he progress of rehabilitation. Evaluation after 1 and 4 years according to the IKDC form revealed good to excellent results in 70% of all patients. The Tegner-score increased significantly, however most patients did not regain their former activity level. ACL-reconstruction reduced anterior translation of the knee significantly at 6 months follow-up. However, we observed a slight increase of anterior translation after 1 and 4 years in both groups; stability was comparable in both groups at all time periods. We conclude that an arthroscopic single-incision technique has no advantage compared to a mini-open two-incision technique for ACL reconstruction with patellar tendon graft in terms of subjective or objective parameters.

[Can knee joint proprioception by reconstruction of the anterior cruciate ligament be restored? A prospective longitudinal study].

Knee proprioception was assessed in 20 healthy volunteers, 10 patients with acute anterior instability and in 20 patients with chronic anterior instability preoperatively as well as after 3 and 6 months postoperatively. To determine proprioception, an angle reproduction test was performed. There were no differences between the right and the left knee joint or between men and women. Best proprioception was measured near full extension. The acute trauma causes massive damage of the proprioception, which could be improved significantly by rehabilitation. However, rehabilitation could not restore proprioception. 3 months postoperatively, there was a slight decrease of proprioception as compared to the preoperative values, but 6 months after reconstruction, restoration of proprioception was documented near full extension and full flexion. In the mid range position, the proprioception could not be restored. There was no difference between open and arthroscopic techniques. The highest correlation was found between proprioception and patient's satisfaction.

Level of physical activity in elderly patients after hemiarthroplasty for three- and four-part fractures of the proximal humerus.

We report on 13 physically active patients who performed sports activities at a weekend or recreational level when they suffered a severe proximal humerus fracture or fracture dislocation. All patients were treated using the NEER II hemiarthroplasty, 9 patients within 4 weeks after trauma and 4 patients 2, 5, 10 and 15 months after trauma. At an average follow-up of 50 (range 6-98) months, all patients were evaluated, both by clinical review and radiographs. Patients obtained 69.0 points using the HSS score (Hospital for Special Surgery) for evaluation. Subjectively, 85% of the patients evaluated the result as 'good' or 'excellent' on a visual analogue scale for satisfaction. There was no radiographic evidence of implant loosening. At an average period of 33 (range 16-52) weeks 10 patients started with their physical activities again without a change in their participation level.

[Intrinsic properties of arthrofibrosis tissue: histologic and cell biology analysis].

A chronic inflammatory reaction and an increased expression of Ki-67 positive cells in arthrofibrotic tissue and the increased proliferation of fibroblasts from arthrofibrotic tissue could be due to an immunopathological reaction. Compared to systemic and organ fibrotic tissue arthrofibrotic tissue is characterized by an increased expression of collagen type VI.

[Bone lengthening osteotomy in malunited clavicular fracture].

Most fractures of the clavicle heal well with nonoperative treatment. On rare occasions, malunited clavicular fractures are associated with a pattern of disability that includes not only pain but also functional or neurovascular impairment. In these cases, clavicular osteotomy and correction of the deformity should be considered. In four patients with a malunited fracture of the clavicle following nonoperative treatment, we performed a clavicular osteotomy for deformity. In all four patients the clavicular fracture healed with shortening associated with pain and ipsilateral glenohumeral dysfunction. The deformity was not associated with neurovascular dysfunction. In preoperative X-ray studies, the shortening of the malunited clavicle was between 0.9 and 2.2 cm in comparison to the contralateral clavicle. All patients underwent an extension osteotomy of the clavicle with interposition of an autogenous iliac crest bone graft. The follow-up was 1-4 years postoperatively. The functional outcome was evaluated using the Constant-Murley and the UCLA score. All patients had immediate pain relief following osteotomy and correction of the deformity. The shoulder function rapidly improved, and the functional outcome was good in three out of the four patients. If there is associated functional impairment in malunited clavicular fractures, extension osteotomy combined with autogenous bone grafting may produce predictably good results.

[Proximal radioulnar synostosis within the scope of fetal alcohol syndrome].

The extremely rare finding of a proximal radio-ulnar synostosis in a 5 years-old girl with a fetal alcohol syndrome is presented. Despite the elbow's being fixed at 30 degrees pronation, the patient exhibits good ipsilateral arm function. The difficulties in differential diagnosis and treatment modalities are discussed.

Arthroscopic repair of the posterior cruciate ligament in a 3-year-old child.

We report a case of symptomatic acute posterior cruciate ligament (PCL) rupture in a 3-year-old boy. At arthroscopy, the PCL was avulsed from the femoral attachment with intact ligament substance. Arthroscopic repair of the PCL was performed by transosseous sutures, which were placed using a suture-punch and an anterior cruciate ligament drill guide. Two years later there was a perfect objective and functional result.

Screening for Leber's hereditary optic neuropathy associated mitochondrial DNA mutations in patients with prominent optic neuritis.

Previous case reports demonstrated the presence of Leber's hereditary optic neuropathy (LHON) associated mitochondrial (mt) DNA mutations in patients presenting with prominent optic neuritis (PON). By screening the mtDNA, we have excluded the presence of these mutations in 22 patients with PON, indicating that the frequency of these mutations is less than 4.5% in our selected patient population. Reviewing the clinical data of these patients revealed that severe optic nerve atrophy developed in association with both the benign and the severely disabling form of Multiple Sclerosis (MS). This observation suggests that the prominent feature of ON in MS may be related to local factors or to a selective vulnerability of the optic nerve in some patients. However, it also may be consequence of a deleterious process associated with inflammatory demyelination in the central nervous system (CNS) of another, genetically probably distinct subgroup of severely disabled patients.

Effect of anastomosis and geometry of vessel curvature on blood flow velocity and patency in microvessels.

The effect of the geometry of the vessel and the number of anastomoses on the blood flow was studied. Four different shapes of the vessel were constructed by using a 6-cm-long double vein graft model with three anastomoses: (1) an alpha loop, (2) an omega loop, (3) a sigmoid curve, and (4) straight. Blood flow was measured by an ultrasound Doppler flowmeter. The result showed no alternation in blood flow across different geometry and through three patent microanastomoses. However, six out of seven vein grafts were thrombosed at 24 hr postoperative due to vascular kinks. This model demonstrates potential sites of kinking at the dissection end of the femoral artery, the microanastomoses, the side branches of the vein graft, and the adventitial adhesions. This model is recommended to microvascular trainees for the study of kinking and the management of redundant pedicles and vein grafting.

The morphological effects of synthetic augmentation in posterior cruciate ligament reconstruction: an experimental study in a sheep model.

The treatment of posterior cruciate ligament (PCL) injuries remains controversial. Due to various problems, PCL reconstruction has not consistently produced the knee stability desired. Biological graft tissue undergoes a remarkable healing process comprising different phases. The strength of autogenous graft material decreases soon after operation. During this early healing phase synthetic augmentation could protect the graft tissue from overloading or overstretching, supporting the tissue revitalization and remodeling process. In order to evaluate the morphological effects of the ligament augmentation device (LAD) on a free patellar tendon autograft in PCL reconstruction, a comparative study in sheep was conducted. In 24 mature sheep, the PCL was replaced with either a patellar tendon autograft alone or a patellar tendon autograft augmented by the LAD. The LAD was fixed at both ends. After the operation the animals were not immobilized. Tibial fixation was released 8 weeks postoperation. The autografts of both groups were histologically evaluated after 2, 6, 16, 26, 52, and 104 weeks. In addition to necrotic and degenerative alterations, a remarkable inflammatory reaction could be seen in the LAD-augmented autografts early postoperation. Compared with the nonaugmented autografts, tissue formation and remodeling were delayed in the augmented group. After 1 and 2 years, the morphology of the autograft tissue was similar in both the augmented and nonaugmented group and differed from that of a normal PCL. The LAD was surrounded by a chronic inflammatory reaction, and collagen fiber ingrowth into the LAD was not observed. Using transmission electron microscopy, small diameter collagen fibrils were predominant in the graft tissue of both groups. Thus, a better remodeling of the autograft tissue in the presence of the LAD could not be demonstrated in this particular study. The value of synthetic augmentation of biological grafts in PCL reconstruction seems to be questionable at present.

Role of posterior capsulotomy for the treatment of extension deficits of the knee.

Chronic flexion contracture of the knee is difficult to treat, especially in cases with long-standing extension deficits and with generalised arthrofibrosis. We present a technique combining arthroscopic or open anterior debridement with a posterior capsulotomy. This capsulotomy is performed via a posteromedial incision and a posteromedial arthrotomy. All scar tissue is resected, and the entire posterior capsule is detached from its femoral attachment. Of 24 patients treated with arthroscopic arthrolysis and posterior capsulotomy from 1989 to 1993, 21 were reviewed with a mean follow-up of 18 months (range 6-38 months). The mean extension deficit preoperatively was 17 degrees (range 10-30 degrees), and symptoms and persisted from 6 months to 7 years. Extension improved to a mean value of 2 degrees; no patient had more than 5 degrees of extension deficit at follow-up. The knee function improved significantly (Lysholm Score preoperative 62, postoperative 88, Tegner Scale preoperative 2.2, postoperative 4.0). No neurovascular complications were observed, and we conclude that posterior capsulotomy is a safe and efficient adjunct procedure to anterior arthrolysis and is indicated in cases with chronic flexion contracture.

[Hemi-arthroplasty--primary or secondary measure for 3- and 4-fragment fractures of the proximal humerus in the elderly?].

Successful treatment of three- and four-part proximal humerus fractures is a therapeutic challenge to the surgeon, particularly in the case of elderly patients. Open reduction and internal fixation have been advocated, but have not consistently produced acceptable results. The results of humeral head replacement as a salvage procedure after non-union or failed open reduction and internal fixation are less predictable. The outcome of hemiarthroplasty (Neer II) performed for three- and four-part proximal humerus fractures in elderly patients was studied. The average patient follow-up was 42.9 months (range 5-98). Eighteen women and eight men with an average age of 64.5 years were evaluated according to the UCLA. Constant-Murley and HSS Score. A Visual Score (0-100 points) was also used. Hemiarthroplasty was performed in 11 patients within 4 weeks of trauma and in 15 patients after 4 weeks. Fair, good, or excellent results were achieved in 80% (UCLA and Visual), 73% (HSS) and 46% (Constant-Murley) of the patients, respectively. Ninety-six percent of the patients reported only slight or no pain. The range of motion was limited in almost all cases. The outcome was not significantly influenced by age, sex and follow-up time. However, there was a significant correlation between the outcome and the length of time between injury and humeral head replacement (r = -0.5). The outcome after early hemiarthroplasty was better than after late humeral head replacement (UCLA: 27.1 +/- 4.6 vs 22.5 +/- 5.6 P = 0.04; Constant-Murley: 65.6 +/- 18.5 vs 47.5 +/- 18.6, P = 0.02; HSS: 74.0 +/- 14.4 vs 63.5 +/- 17.6, P = 0.17). Self-assessment did not differ between these two groups. After early hemiarthroplasty, active forward flexion was significantly better. This study indicates that early humeral head replacement for three- and four-part proximal humerus fractures in elderly patients achieved better functional outcome than delayed humeral head replacement. The decision to perform prosthetic humeral head replacement in these cases should be made as early as possible after trauma.
