[Bladder dysfunctions in encephalomyelitis disseminata--drug and interventional therapeutic options].

Patients with disseminated encephalomyelitis have various urological presentations, ranging from pollakisuria to urge incontinence. After detailed evaluation (neuro-urological examination, urodynamic investigation) drug therapy and various interventional methods must be adapted to the individual manifestations. Patients with detrusor hyperreflexia are treated with oral anticholinergic agents (oxybutynin, trospium chloride, propiverine). Patients with urinary retention are recommended to be managed with clean intermittent (self)-catheterisation. The various interventional therapeutic options (bladder denervation, electrostimulation, local treatment with botulinum toxin) and the surgical therapy (sacral deafferentation and anterior root stimulation, bladder neck closure and cystostomy, sphincterotomy or augmentation cystoplasty) must be reserved for special cases.

DNA aneuploidy in prostatic adenocarcinoma: a frequent event as shown by fluorescence in situ DNA hybridization.

Fluorescence in situ hybridization (FISH) using specific DNA probes for chromosomes 1, 7, 10, and Y was performed on 53 prostatic tissue samples obtained from 33 radical prostatectomy specimens and two benign control specimens. The 53 samples from carcinomatous prostates included 33 cancerous and 20 noncancerous samples. Additionally, four metastatic lymph node specimens were examined. Clonal chromosome abnormalities were observed in 78% of the tumors studied. They were detected in a higher proportion in stage pT2 and pT3 tumors (86% and 88%, respectively) compared with stage pT1 tumors (25%). No stage pT4 tumor was analyzed. There was evidence of remarkable focal intratumoral heterogeneity documented by the study of two samples from the same tumor in three of six cases. Comparing FISH determined ploidy patterns with DNA flow cytometry (FCM) in 22 samples, FISH showed aneuploidy whereas FCM showed none.

Simultaneous characterization of in vitro growing cells by fluorescence in situ hybridization (FISH) and Ki-67 labeling.

Because cytogenetic analysis and fluorescence in situ hybridization (FISH) techniques fail to provide information about the immunological phenotype with respect to differentiation and malignancy of in vitro growing cells, we developed a system which combines immunophenotyping and FISH. Using immunological detection of the proliferation fraction in short term human cell cultures from different origins by Ki-67 labeling and genetic characterization by FISH, both signals were detectable simultaneously on the same monolayer. Because determination of the proliferation behavior of tumor cells by Ki-67 labeling has now become a widespread laboratory tool, this particular technique may be very useful in tumor cell characterization. This approach allows genetic changes to be assigned to a particular immunological phenotype of individual cells.

Genotypic characterization of prostatic carcinomas: a combined cytogenetic, flow cytometry, and in situ DNA hybridization study.

Cytogenetic studies were performed on 36 biopsies obtained from 26 primary prostatic adenocarcinomas. Following histopathological characterization of control sections, the biopsies were investigated using metaphase cytogenetics, DNA flow cytometry, and fluorescence in situ DNA hybridization. In 12 specimens, no carcinoma was found in control sections by histopathological means. In 24 carcinoma biopsies clonal aberrations were detected in 15 specimens. Tetraploidy as sole aberration was detected in five specimens. Loss of the Y chromosome was seen in eight samples. Only one tumor revealed structural abnormalities. Eight samples were found to be normal (46,XY). Remarkably, nonclonal chromosome aberrations, particularly marked chromosome loss, were frequently detected in prostatic carcinomas and premalignant lesions (prostatic intraepithelial neoplasia). In the series of biopsies investigated by means of cytogenetics and flow cytometry, biopsies with aneuploid DNA content were found to be cytogenetically normal. Conversely, the cytogenetically aberrant clones were found to be of diploid DNA content. Evidence of focal intratumoral heterogeneity was revealed by cytogenetics, flow cytometry, and in situ hybridization.

Bilateral multicentric papillary renal tumors with heteroclonal origin based on tissue-specific karyotype instability.

BACKGROUND: Papillary chromophilic renal tumors were cytogenetically characterized by combined trisomies 7 and 17 along with other numeric chromosome aberrations. They occurred as bilateral multifocal lesions. METHODS: A patient was presented who simultaneously developed bilateral multicentric papillary renal tumors. Tissue specimens from six tumors and tumor-free kidneys were cytogenetically characterized. RESULTS: The tumors showed trisomy of chromosomes 7, 16, and 17 along with various other numeric abnormalities. In normal tissue from both kidneys, a wide variety of clonal and nonclonal numeric and structural chromosome aberrations were found. CONCLUSIONS: The cytogenetic heteroclonality of the tumors strongly favors the assumption that they arose as independent primaries. A tissue-specific karyotype instability may be the basic event of multiple tumorigenesis.

Cytogenetic evidence of intratumoral focal heterogeneity in prostatic carcinomas.

Although prostatic cancer is the most common malignant disease in men in western countries, only limited data on chromosomal changes are available. We report on our cytogenetic findings in cell cultures from four primary prostatic carcinomas indicating the existence of intratumoral focal heterogeneity concerning chromosomal anomalies in prostatic cancer.

Eicosanoid synthesis in the isolated human renal pelvis, ureter and bladder.

Prostaglandin synthetase inhibitors are used for the treatment of ureteric colic. However, there is controversy regarding the mechanism of action of these drugs. In this study, differential prostaglandin synthesis in the human renal pelvis, ureter and bladder was measured using specific radioimmunoassays and gas chromatography/mass spectrometry. There was a significant quantitative predominance of the smooth muscle constrictor eicosanoids, PGF2 alpha and TXA2 over the dilatory PGE2 in tissue from all sites--renal pelvis, ureter and bladder. The results indicate that prostaglandins play a direct role in smooth muscle activity of the upper urinary tract and the inhibition of this activity with indomethacin indicates a further mode of its action in pain relief in ureteric colic.

Effects of prostaglandins and prostaglandin synthetase inhibitors on acutely obstructed kidneys in the dog.

An intact canine model was developed to study the effects of prostaglandins (PG) and prostaglandin synthetase inhibitors on acutely obstructed kidneys. Totally implanted nephrostomy tubes were placed to measure renal pelvic pressure. Complete ureteral obstruction was obtained with a Fogarty balloon catheter inflated in the distal ureter; by this method renal pelvic pressure reached 40-50 mm Hg. Renal pelvic pressure was reduced after intravenous indomethacin and dipyrone administration, whereas blood pressure showed no major changes. Exogenous prostaglandins had both immediate and contrary effects: PGE2 caused a significant decrease, whereas PGF2 alpha caused a significant increase in renal pelvic and blood pressure. The reduced rise in renal pelvic pressure appears to be the main reason for the analgesic effects of prostaglandin synthetase inhibitors. The efficiency of these drugs in the treatment of renal colic is supported by this study, that of prostaglandins cannot be proved.

[Electronic data processing-assisted text processing at the clinic and in general practice].

Word processing is currently the most frequent application for personal computers, and a wide variety of standard software is available. The capabilities of modern word-processing software includes the convenient typing and correction of all routine correspondence, as well as the professional layout of scientific manuscripts. The decision to purchase a certain word-processing programm should be made according to local needs and prerequisities. Three to six months may be necessary to fully adapt the organization of a clinic or private practice to the new technology.

[Computed tomographic studies of the scrotal contents, particularly the testis].

CT examination of the testes was carried out in 49 patients for the investigation of testicular tumours. Hypodensity and inhomogeneity were typical of teratomas and hyperdensity and relative homogeneity of seminomas. Granulomatous orchitis and lymphomas showed the same characteristics as seminomas. Three testes in the abdomen could be localised and classified. One non-palpable primary tumour was found as well as an old partially calcified tumour, three old torsions and one lipo-sarcoma. In about 10% it was not possible to distinguish between tumour and inflammatory lesions. Compared with sonography, CT has advantages, particularly in the diagnosis of old torsion.
