Carcinosarcoma and spindle cell carcinoma of the lung. Clinicopathologic and immunohistochemical studies.

We examined pulmonary carcinomas with prominent sarcoma-like lesions both clinicopathologically and immunohistochemically. Grossly, two tumors had predominantly endobronchial growths, four bulky parenchymal growths, and two endobronchial, parenchymally mixed growths. In these eight patients, six tumors were completely resected, one patient was given irradiation only, and one patient died in the early postoperative period. On the basis of specific differentiation of the sarcoma-like lesions, the tumors were separated into three groups: two with "true" sarcoma differentiated into soft tissues such as striated muscle or osteoid tissue; three with a fibromatous sarcoma resembling atypical pseudosarcomatous stroma; and three with spindle cell carcinoma with evidence of epithelial differentiation. The prognosis was poor, and tumors with specific differentiation into rhabdomyosarcoma, chondrosarcoma, or spindle cell carcinoma progressed more rapidly than did those with a fibromatous sarcoma. Because the fibromatous sarcoma-like lesions were found to relate to a longer survival time for the patients, we wish to emphasize that a distinction of sarcomatous components should be made with regard to assessing the prognosis of pulmonary carcinoma with sarcoma-like lesions.

[Influence of peritoneal fluid with endometriosis on the development of mouse embryos].

To clarify the mechanism of infertility associated with endometriosis (EM), the effect of peritoneal fluid (PF) on early embryogenesis was examined. The addition of PF (5%) had no effect on the development of mouse 2-cell embryos. Since the PF is supposed to enter the oviductal cavity, PF may influence reproductive processes by modulating the intraoviductal microenvironment. As expected, PF with EM inhibited the development of 2-cell embryos co-cultured with oviducts whereas PF without EM had no effect. An increase in interleukin 1 (IL-1) has been identified in PF with EM. As with PF with EM, IL-1 inhibited the development of 2-cell embryos inasmuch as the oviducts were co-cultured. Indomethacin, an inhibitor of prostaglandin (PG) synthesis, effectively abolished the inhibitory action of both PF with EM and IL-1 on embryonic development. Moreover, PGE2 directly inhibited embryonic development. PGE2 was shown to inhibit the synthesis of protein by early embryos. These results demonstrate that PF with EM inhibits the development of early mouse embryos by acting on the oviducts. From these results it seems that an increase in IL-1 may be a causative factor in the embryo-toxic properties of PF with EM. We further suggest that PGE2 secreted from the oviducts by stimulation with PF with EM, may be an ultimate contributing factor in embryo-toxicity by inhibiting protein synthesis by early embryos.

[Four cases of simultaneous discovery of familial tuberculous infection].

Index case is a 17-year-old boy who was admitted to our hospital with pleurisy and a minimal pulmonary lesion, and tubercle bacilli were recovered from pleural fluid. A diagnosis of primary tuberculosis was made based on the onset by pleurisy and the existence of hilar and mediastinal lymph node swelling. On the same day, a 76-year-old man, grandfather of the index case was admitted for precise examination of suspected extensive pneumonia. Tubercle bacilli were also isolated from the pus of infected bulla obtained by puncture. Neither of these two cases, however, seemed to be the source of the familial tuberculous infection because of such sudden onset of the disease as pleurisy and pneumonia. Two months later, a 46-year-old man, father of the index case was examined at our hospital. He was considered to be the source of the familial infection because he was diagnosed as tuberculosis with positive smear and a thick wall cavity (3.2 cm in diameter) on the left apex, and abnormal shadow was detected on his chest X-ray already two years ago. The fourth case was a mother of the index case, and wife of the third case, whose chest radiography revealed an infiltrative shadow on the right apex by a family contacts examination. Though tubercle bacilli were not isolated from her sputum, pulmonary lesions considered to be tuberculosis due to their typical location and nature, a positive PPD skin test, and the response to antituberculous drugs.

[A case of subacute necrotizing lymphadenitis with localized interstitial pneumonia].

A 16-year-old male was admitted with a history of cervical lymph node swelling, high fever, cough and hemosputum. On admission, bilateral cervical lymph nodes swelling and fine crackles in the right lower lung field were noted. A chest X-ray film showed an infiltrative shadow in the right lower lung field and right hilar enlargement. Cervical lymph node biopsy specimens revealed wide areas of necrosis with nuclear debris. Transbronchial lung biopsy showed infiltration of lymphocytes in the interstitium and bronchoalveolar lavage fluid showed increased T-lymphocytes and a decreased T4/T8 ratio. The patient was diagnosed to have subacute necrotizing lymphadenitis with T-lymphocyte alveolitis. Additionally, antinuclear antibodies were positive, and anti HTLV-I antibody was false positive. These findings suggested an immunological abnormality in this case. His cervical lymph node swelling and infiltrative shadow on chest X-ray film improved with steroid therapy. Our case may be the first case of subacute necrotizing lymphadenitis with T-lymphocyte alveolitis.

[Veno-arterial ECLA (extracorporeal lung assist) for severe respiratory failure due to meconium aspiration].

A full term newborn female, 3262g, aspirated meconium at birth and began to suffer from severe hypoxia and acidosis due to progressing pneumonitis, pneumothorax and pneumomediastinum. She also had severe hypotension and anuria. Venoarterial ECLA with a Kolobow membrane lung via the right internal jugular vein and the right common carotid artery was initiated. Blood gas parameters and blood pressure improved, and urine output increased to normal. ECLA permitted a reduction in FIO2 and airway pressure of mechanical ventilation, as well as frequent lavage of the lung. As the physical condition improved, the bypass flow was gradually decreased from 200 ml.kg-1.min-1 at the start to 130 ml.kg-1.min-1 for maintenance, then to 25 ml.kg-1.min-1 at the end. Bleeding throughout the ECLA for 69 hours could be minimized by a meticulous control of the activated coagulation time with a minimum dose of heparin and the transfusion of fresh frozen and platelet rich plasma. After ECLA, the carotid artery was simply ligated, and mechanical ventilatory support was carried out for 5 days. Her condition improved and she left the hospital without any neurological sequelae. ECLA will become an effective means of life support for a baby with severe MAS irresponsive to conventional ventilatory support.

[Two cases of spontaneous pneumomediastinum due to Xiao-lin Temple boxing vocal exercise].

Two cases of spontaneous pneumomediastinum and subcutaneous emphysema were reported. The first cases was a 21-year-old man who had a past history of recurrent spontaneous pneumothorax. The second was a healthy 20-year-old man. They were admitted to our hospital because of dyspnea and chest pain, which occurred at or after vocal exercise for Xiao-lin Temple boxing. Physically, a mediastinal crunch on auscultation heard over the cardiac apex and the left sternal border (Hamman's sign) was recognized in the first case, and subcutaneous emphysema was palpated in both cases. The chest X-ray films revealed intramediastinal air and subcutaneous emphysema in both cases. We diagnosed these patients as spontaneous pneumomediastinum because of no underlying disease. This is a report of rare cases of spontaneous pneumomediastinum occurring at the same time, and one of the mechanism causing them was considered to be a sudden increase in intrathoracic pressure due to the vocal exercise of chinese boxing.

Effect of desipramine treatment on pulmonary phospholipid metabolism in rats.

Changes in pulmonary phospholipid (PL) composition, fatty acid composition in phosphatidylcholine (PC) and the factors contributing to regulate fatty acid composition in PC were studied in rats treated with desipramine (DP). DP level and PL content in post lavage, surfactant and alveolar macrophages (AMs) showed significant increases by DP treatment, and the accumulation of DP and PL into post lavage was the largest of three pulmonary fractions. The overall increase of PL content in post lavage was mostly sustained by PC. Only palmitic acid involved in PC from post lavage was significantly increased by DP administration, and other fatty acids showed a decrease tendency. Concerning the factors affecting fatty acid composition in PC, the ratio of palmitic acid in free fatty acid pool and the uptake of palmitoyl-CoA into PC via deacylation-reacylation system were increased by DP treatment.

[Difference of clinical features between bacilli positive tuberculosis patients diagnosed at the outpatients dept. (OPD) and diagnosed after admission].

In order to determine the clinical features of patients whose tubercle bacilli were found after admission, the characteristics of 23 cases (17 males, 6 females) diagnosed after admission (Group I) were compared with 16 other cases (12 males, 4 females) diagnosed before admission (Group II). The patients in Group I were more elderly and had severer underlying diseases than those in Group II. In addition, the majority of the patients of Group I showed manifestations of respiratory infections similar to bacterial pneumonia. Therefore, 14 cases (61%) in Group I were diagnosed as bacterial pneumonia on admission. Radiological findings showed that less than half of Group I showed typical X-ray findings of tuberculosis such as location of lesions in the apex and upper lung field, cavity formation, and mixture of or new and old infiltrative lesions. Also on the PPD skin test, 7 cases (30%) of Group I were negative. Due to the above results, in many cases it took more than 10 days from admission to make a final diagnosis as tuberculosis. There were several differences in findings between two groups, but there was no definite signs to diagnose all patients at the OPD. It is therefore considered to be important to keep in mind the following to prevent admission of open tuberculosis patients into a general hospital: (1) Admission of patients suspected to have open tuberculosis at the OPD should be postponed until their smears are proven negative. (2) Sputum examinations for AFB always should be carried out in patients with respiratory symptoms.

Thiobacillus ferrooxidans cytochrome c-552: purification and some of its molecular features.

Soluble cytochrome c-552 was purified from Thiobacillus ferrooxidans to an electrophoretically homogeneous state. The cytochrome showed absorption peaks at 276, 411 and 523 nm in the oxidized form and peaks at 315, 417, 523 and 552 nm in the reduced form. The molecular weight of the cytochrome was estimated to be 13,800 on the basis of the amino acid composition and heme content, and 14,000 from SDS-polyacrylamide gel electrophoresis analysis. Its midpoint redox potential at pH 7.0 was determined to be +0.36 V. The N-terminal amino acid sequence of the cytochrome was determined as follows: A-G-G-A-G-G-P-A-P-Y-R-I-S-?-D-?-M-V-?-S-G-M-P-G-. Ferrocytochrome c-552 was oxidized by the membrane fraction of T. ferrooxidans, and the oxidation rate was more rapid at pH 3.0 than at pH 6.5. Ferricytochrome c-552 was reduced by Fe(II)-cytochrome c oxidoreductase with Fe2+ at pH 3.5, while horse ferricytochrome c was not reduced by the enzyme under the same reaction conditions.

Cytochrome oxidase of an acidophilic iron-oxidizing bacterium, Thiobacillus ferrooxidans, functions at pH 3.5.

Cytochrome oxidase of Thiobacillus ferrooxidans was partially purified. The oxidase preparation had haems a and c, and oxidized ferrocytochrome c-552 of the bacterium. The optimal pH of the reaction was 3.5. The enzyme also oxidized the reduced form of rusticyanin, a copper protein of the bacterium. Our results indicate that the reduction of molecular oxygen by this enzyme may occur in the periplasm.

Properties of recombinant interleukin 2-cultured tumor-infiltrating lymphocytes in human lung cancer.

It is thought that TIL can be activated in vitro by rIL-2 and acquire specific anti-tumor activity. In this study, we investigated this possibility, using lymphocytes isolated from primary lung cancer tissues. In a first series of experiments, TILs and autologous PBLs from 16 patients were cultured in rIL-2 from 7 to 14 days under identical conditions, and were compared for proliferation (16 cases), cytolytic activity (11 cases), gamma interferon (IFN-gamma) production (8 cases), and phenotypes (10 cases). TILs grew in response to rIL-2 as well as PBLs. However, the induced cytolytic activity of TIL was significantly lower than that of PBL against autologous tumor cells and 2 human tumor cell lines. IL-2-mediated IFN-gamma production by TILs was also significantly lower than that of PBLs. TILs were phenotypically characterized by their high CD4/CD8 ratio and lack of Leu11-positive cells. Further investigations with 7 other cases showed that exogenous addition of IFN-gamma to rIL-2 cultures of TILs enhanced cytolytic activity in 4 cases. Our results indicate that IL-2 alone is sufficient for TILs to proliferate but not to acquire new functions (cytotoxicity and production of IFN-gamma).

Genetic changes in human adrenocortical carcinomas.

Recent studies have suggested that loss of heterozygosity at loci on the short arm of human chromosome 11 (11p) may be important in the pathogenesis of benign and malignant adrenal cortical tumors. To test this concept, adrenocortical carcinomas from nine patients and benign adrenal cortical lesions from eight patients were tested for loss of alleles at loci on human chromosomes 11, 13, and 17. All patients with adrenocortical carcinoma whose normal somatic tissues were heterozygous for a locus on chromosome 17p had lost alleles in the tumor. Four of six patients with adrenocortical carcinoma who were heterozygous for one or more alleles on chromosome 11p in normal tissues had lost 11p alleles in the tumor. Three of six patients with adrenocortical carcinoma showed loss of 13q alleles in the tumor. Loss of alleles on chromosomes 11p, 13q, and 17p was observed in primary tumors and metastases but not in adrenocortical adenomas or hyperplastic lesions of the adrenal cortex. One patient with adrenocortical carcinoma had a somatic mutation in the HRAS1 gene in the normal adrenal gland. The consistency of the genetic changes on chromosomes 11p, 13q, and 17p suggests that they are important in the pathogenesis of adrenocortical carcinoma.

Serodiagnosis of cancer, using porcine antigens recognized by human monoclonal antibody, HB4C5.

Antigens, recognized by human monoclonal antibody (HB4C5) generated from a lung cancer patient, were found to occur in porcine pancreas. The antigens-I and -II were purified from crude trypsin of porcine pancreas, only by Mono Q column chromatography, and were eluted at 260 and 300 mM NaCl in 10 mM Tris-HCl buffer, pH 7.4, respectively. These antigens differed from trypsin in molecular weight, elution pattern from the Mono Q column, and their reactivity with HB4C5. The molecular weights of the two antigens were almost the same at around 35000. These were used for serodiagnosis with an assay system based on 96-well immunoplates. The reactivities of antigens-I and -II with various sera were similar. When the reactivity of IgG in serum with antigen-II was measured, absorbance at 415 nm in the case of normal and lung cancer patients was 0.178 +/- 0.056 and 0.492 +/- 0.136 (p less than 0.005). The rates of positive reaction in ovary, larynx, uterus, lung and liver cancers were more than 50%, but the rates in stomach and breast cancers were less than 30%. Positive reaction was hardly detected in pancreas cancer and normal controls.

Longer survival after resection of non-small cell lung cancer in Japanese women.

From 1974 to 1988, 492 patients (361 male and 131 female) with non-small cell lung cancer underwent pulmonary resection at our department. A retrospective study was done with special reference to the sex of the patients. There was no sex-related association with age distribution, operation, pathological T or N status, or staging. On the other hand, 77.9% of women had an adenocarcinoma and only 44.6% of men had this type of lesion. This was considered at least partly a result of sex difference in smoking habits. Women survived for a significantly longer time (p = 0.0036), and this difference could not be explained by differences in histological type. Presumably, hormonal factor(s) may contribute to the prognosis.

Phenotypic characterization of lymphokine-activated killer cells from human lymph node lymphocytes.

We previously reported that lymphokine-activated killer (LAK) activity can be generated in human lymph node lymphocytes (LNL) at the same level as that in peripheral blood lymphocytes (PBL), despite the absence of active natural killer (NK) cells. In the present study, we investigated the surface phenotype of LNL-LAK cells by fractionation of lymphocytes, using a panning method. LNL isolated from lung cancer patients were cultured in the presence of recombinant interleukin 2 for 8 days and separated into T cells and non-T cells according to the expression of CD3 antigen. LAK effectors were enriched in the CD3- non-T cells. However, the CD3+ cells also mediated a low but substantial level of LAK activity, which was attributed to a CD8+ T-cell subset. Further investigation of the CD3- cells revealed that most of the CD3- effector cells expressed neither B-cell (CD20) nor NK-cell (CD16) markers. Precursors of this CD3-CD20-CD16- (null) population appeared to be also CD3-, CD20-, and CD16-. From these results, we would stress the significant contribution of CD3-CD20-CD16- null cells to the LAK phenomenon, which has not been focused on in PBL.
