Inhibitory effect of (-)-epigallocatechin gallate on carcinogenesis with N-ethyl-N'-nitro-N-nitrosoguanidine in mouse duodenum.

(-)-Epigallocatechin gallate (EGCG) is the main polyphenolic constituent of green tea infusion and inhibits tumor promotion by teleocidin in two-stage carcinogenesis on mouse skin. In this work, EGCG was found to inhibit tumor promotion in the gastrointestinal tract in a model system of mouse duodenal carcinogenesis with N-ethyl-N'-nitro-N-nitrosoguanidine. The duodenal tumors that developed were studied stereomicroscopically and histologically.

Effect of extracts of Zingiberaceae herbs on gastric secretion in rabbits.

Some of the Zingiberaceae herbs are known to be useful as stomachics. Water extracts and methanol extracts of eight such herbs were examined in intact unanesthetized rabbits for their effect on gastric secretion. Oral administration of either water extracts or methanol extracts caused a significant decrease in gastric secretion. A significant effect of these extracts appeared at 3 h after administration. The effect of water extracts on gastric secretion was very similar to that of cimetidine, with a significant decrease in acid output. The effect of the methanol extracts was primarily observed as decreased pepsin output.

Microvascular organization of human palatine tonsils.

We describe the three-dimensional organization of the microvasculature of human palatine tonsils as revealed by the vascular corrosion casting/scanning electron microscope method and light microscopy of sections. The tonsillar arteries travel in the connective tissue septa and give off many branches. They further branch into arterioles which in turn enter the follicle and the interfollicular region. These arterioles, giving off capillaries en route, reach the subepithelial region where they break up into sinusoidal capillaries. The subepithelial capillary network overlying the follicle protrudes hemispherically towards the crypt, while that overlying the interfollicular region has many switch-back loops of capillaries projected towards the crypt. The subepithelial sinusoids gather into the high endothelial venules (HEVs) which, collecting capillaries in the follicle and the interfollicular region en route, course down into the interfollicular region alongside the follicle. The HEVs surround the lateral and basal surfaces of the follicle and ultimately lead into the ordinary veins in the septa. The subepithelial sinusoids seem to be involved in taking up immunoglobulins secreted by plasma cells and any other substances released by lymphocytes and/or macrophages as well as supplying the tissues with necessary oxygen and nutrients. That the HEVs are downstream to the subepithelial sinusoids suggests that some substances which are taken up into the sinusoids and transported to the postcapillary venules induce differentiation of HEVs and maintain them.

[Metastatic malignant lymphoma of the skull with extradural and extracranial extension. Case report].

A 72-year-old male was hospitalized because of a steadily growing mass (4 x 4 x 2 cm) in the frontal region, noticed following head injury. Six months earlier, he had declined treatment for malignant lymphoma (small non-cleaved cell type), discovered upon evaluation of a subcutaneous mass in his forearm. Neurological examination on admission was negative. Plain skull films revealed a large, osteolytic lesion with an irregular margin. Bone scintigraphy disclosed no uptake other than in the frontal bone. Computed tomography (CT) showed a large, homogeneously enhanced mass with both extradural and extracranial extension, which had destroyed the left frontal bone. The skull tumor was totally resected, and the diagnosis was malignant lymphoma, large cell, immunoblastic and plasmacytoid type. Postoperative combined chemotherapy with vincristine, cyclophosphamide, prednisolone, and adriamycin resulted in the complete disappearance of liver metastases. The patient was readmitted 2 months later, however, because of headache and gradually progressive impairment of consciousness. A CT scan disclosed brain metastasis in the paraventricular region. Radiation therapy (20 Gy) was administered but no further treatment was given. Skull metastasis is not uncommon in cases of malignant lymphoma, but a large mass with both extradural and extracranial extension is rare. Although aggressive chemotherapy was effective, prophylactic irradiation of the brain should have been considered in this case.

One-day intravenous therapy with high-dose gamma-globulin for idiopathic thrombocytopenic purpura.

The effects of one-day intravenous therapy with gamma-globulin in a 25-year-old man with idiopathic thrombocytopenic purpura are reported. The patient showed resistance to prednisolone and had bleeding tendency. The thrombocytopenia improved after the intravenous administration of gamma-globulin at a dose of 1 g/kg. Splenectomy was carried out. The results suggested that the usual dose of gamma-globulin is not always necessary. The half-dose increased the platelet count.

[Two color flow cytometry of lymphocytes from patients with adult T-cell leukemia].

Lymphocytes from eight patients with adult T-cell leukemia were analyzed by two color flow cytometry. Monoclonal antibodies (Leu 3 a, Leu 8, Leu 2 a and Leu 15) labelled with fluorescein isothiocyanate or phycoerythrin were used. The purpose was to identify the subsets of the lymphocytes as helper, suppressor/inducer, suppressor or cytotoxic by the surface marker of the cells. All eight patients had antibodies for ATLA. Proviral DNA in the lymphocytes was found in six patients. Summarising the results, OKT4-positive ATL cells were all of the helper T-cell subset, not the inducer subset. OKT8-positive ATL cells were also positive for OKT4 and were all of the cytotoxic T cell subset, not the suppressor subset. In two patients, some ATL cells had both OKT4 and OKT8 on the same cells, especially in the lymph nodes. In our study, ATL cells from eight cases of ATL had all of the helper T subset. These results suggest that the target cells of the human T cell leukemia/lymphoma virus type will be helper T cells.

[Adult T-cell leukemia/lymphoma with OKT4 lack of OKT8 in peripheral blood and with OKT4 and OKT8 in lymph node. Study of two color flow cytometry].

Adult T-cell leukemia (ATL) cells usually express the helper/inducer associated antigen OKT 4 with lack of OKT8. However, there are a few case reports indicating that there are atypical cell phenotypes in ATL including OKT4+/OKT8+. The analysis of surface phenotype of peripheral lymphocytes and abnormal cells in lymph node was done with monoclonal antibodies. ATL cells of peripheral blood and small lymphocytes of lymph node had the usual phenotype, OKT4+/OKT8-, but neoplastic cells of large lymphocytes of lymph node had the unusual phenotype OKT4+/OKT8+. The neoplastic cells were immunophenotyped by two-color flow cytometry analysis. OKT4+/OKT8- cells had a helper cell phenotype. OKT4+/OKT8+ cells had a helper cell and a cytotoxic cell. These findings suggest that in this case ATL cells arise from common thymocyte and one of them mature in peripheral blood other remain in lymph node.

[Virus-associated hemophagocytic syndrome due to EB virus].

A 30-year-old man was admitted to our hospital because of fever and cervical lymphadenopathy. Hematological examination revealed leukocytosis with atypical lymphocytes in peripheral blood. Mature histiocytes with erythrophagia were detected in the bone marrow. GOT and GPT were elevated. Anti-EB virus antibody titer was high. The titers of VCAIgM and VCAIgG on admission were 1:320 and 1:160, respectively, and those at convalescence stage were 1: less than 10 and 1:640. The diagnosis of virus associated hemophagocytic syndrome (VAHS) due to EB virus was made. Immunosuppressive and cytotoxic therapy are thought to be contraindicated in the treatment of VAHS. In this case only a febrifuge was administrated and the condition of the patient was improved.

[Treatment of acute leukemia with "MB-triple V" therapy--a comparative study of "MB-triple V" therapy and B-triple V therapy].

Eleven patients with acute leukemia were treated with MB-triple V therapy consisting of mitoxantrone, behenoyl-arabinoside, etoposide, vincristine and vindesine. In this regimen, the target points were set for WBC count in the peripheral blood (less than 1,500/microliters) and TNC count in the bone marrow (less than 15,000/microliters). In this study, we compared the WBC count in nadir state and the duration until WBC count returned to above 1,000/microliters between 11 patients with MB-triple V therapy and 15 patients with B-triple V therapy in which target points were not set. In particular, the standard distribution of WBC count in nadir state of MB-triple V group was significantly smaller than that of B-triple V group. The number of early deaths in the course of MB-triple V (0/11, 0%) decreased compared with B-triple V group (3/15, 20%). Seven patients of the eleven patients with this regimen achieved CR state (63.6%). Hereafter, further cases are necessary before this combination chemotherapy as a part of the induction therapy is evaluated.

[A comparative study of the treatment of vulvar carcinomas].

The results of treatment in our department of 24 patients with various vulvar carcinomas between 1969 and 1987 are described. There were 8 patients in stage I, 3 patients in stage II, 10 patients in stage III, 2 patients in stage IV and one patient in recurrence. Eighteen patients could be followed for more than 5 years. Particular attention was given to stage and treatments. The 5-year survival rate was 87% for stage I patients, 28% for stage III patients, 0% for stage IV patient and 0% for a recurrent patient. The 5-year survival rate was 80% (4/5) for operation group, 60% (3/5) for radiation group and 20% (1/5) for radiation and chemotherapy group.

Ovarian involvement as an initial manifestation of malignant lymphoma.

An immunohistochemical study of a rare initial manifestation of non-Hodgkin's lymphoma (NHL) in the ovaries is presented. There have been very few reports to date on immunohistochemical studies of lymphoma involving the ovaries. A 53-year-old woman suffering from lower abdominal pain and abnormal genital bleeding was diagnosed as having a tumor in her left ovary by ultrasonic echograms and CT scanning. The patient underwent a simple total hysterectomy and bilateral salpingo-oophorectomies. The tumor, measuring 14 x 10 x 10 cm, was located in the left ovary and extended to the major omentum, mesocolon and left ureter. The histology of the tumor was that of NHL showing diffuse proliferation of small cleaved cells. Immunohistochemical studies of the ovarian tumor showed that the tumor cells were of a B-cell lymphoma nature with LCA+, MB-1+, lambda+, keratin-, IgG-, IgM-, IgA-, kappa-, and MT-1-. Although the main lesion involved the ovary, the case could not be identified definitely as primary lymphoma of the ovary.

Leukemia cutis in acute monocytic leukemia.

We describe the case of a patient with acute monocytic leukemia who demonstrated striking leukemic infiltration of the abdomen. This cutaneous involvement occurred in association with systemic relapse. Histologic examination of a biopsy specimen of a cutaneous lesion showed a diffuse infiltrate of leukemic cells in the dermis. However, blood vessels and skin adnexa were well-preserved, despite the destructive nature of monocytic leukemia.

Clinical study of malignant lymphoma of head and neck in the nasal cavity and Waldyer's ring.

Clinical outcome of fifty-one cases of non-Hodgkin's lymphoma limited to the head and neck were studied for differences with involvement of one of two sites: Waldyer's ring, and the nasal cavity and paranasal sinuses taken together. The median survival for patients with involvement of Waldyer's ring was 58.6 months, and for the other group, 10.3 months. The patients with involvement of the nasal cavity and paranasal sinuses responded poorly to treatment and the lymphoma rapidly progressed. In these patients, the lymphomas were not of the diffuse small cell type, which has a relatively good prognosis: but T cell type. The poor outcome with involvement of the nasal cavity and paranasal sinuses may be due to histological and immunological factors, and to inadequate therapy. Early combination chemotherapy might give better results for this group.

[Leukemic Burkitt's lymphoma with an initial symptom of ocular motor disturbance].

A 58-year-old male was admitted to Osaka City University Hospital because of diplopia in May, 1987. Hematological examinations revealed abnormal blastoid cells in his peripheral blood and bone marrow. We found many mitochondria and polysomes in the cytoplasm of the blastoid cells by the electron microscopy. Moreover, left axillary lymph node biopsy specimen disclosed the typical "starry sky" pattern. The blastoid cells in the bone marrow had chromosomal abnormality; 47XY, 8q-, 14q+, 22q-, +mar. Therefore, the diagnosis of Burkitt's lymphoma (leukemic change) was performed. He was treated with chemotherapy, but the blastoid cells resisted many anticancer agents and he died in September 30. On admission left ocular motor disturbance was revealed. Generally, it was reported that meningitis with CNS involvement of lymphoma is accompanied by cranial nerves palsy. However in this case lymphoma cells was not found in the cerebrospinal fluid. The cause of ocular motor disturbance was the adhesion to the medial rectus muscle and compression to the ocular motor nerves of the lymphoma mass.
