The goal of the present study was to further knowledge on gender and role (i.e. patient versus partner) differences in psychological distress and quality of life as a consequence of dealing with cancer. There is some evidence that being the patient or the caregiver makes more difference for men than for women. In total, 173 couples facing various forms of cancer (two samples) and a control group of 80 couples completed the CES-D and Cantril's Ladder. Analyses of variance revealed that both female patients and female partners of patients perceived more psychological distress and a lower quality of life than women in healthy couples. In contrast, role did have an effect on men. Specifically, male patients scored as high on psychological distress and as low on quality of life as female patients and female partners, but psychological distress and quality of life did not differ between male partners of patients and their healthy controls. However, this effect was found in only one patient sample. The finding that female partners perceived more psychological distress and a lower quality of life than male partners could not be accounted for by differences in the physical condition of the patient or the partner.
Quantitative structure-activity relationships (QSAR's) are described for the rate of conjugation of a series of fluoronitrobenzenes with cytosolic as well as with two major alpha and mu class enzymes of rat and human liver, viz., glutathione S-transferases (GST) 1-1, 3-3, A1-1, and M1a-1a. For all purified enzymes studied, the natural logarithm of the rate of conversion of the fluoronitrobenzenes correlates with both the calculated reactivity of the fluoronitrobenzenes for an electrophilic attack (i.e., E(LUMO)) and the calculated relative heat of formation for formation of the respective Meisenheimer complex intermediate (delta delta HF). In addition, the regioselectivity of the reaction was determined and compared. The results obtained strongly support the conclusion that chemical reactivity of the fluoronitrobenzenes is the main factor determining the outcomes of their conversion by all glutathione S-transferase enzymes. The regioselectivities vary only a few percent from one enzyme to another, whereas QSAR lines for all purified enzymes are in the same region and run parallel. This indicates that in the overall reaction the nucleophilic attack of the thiolate anion on the fluoronitrobenzenes, leading to formation of the Meisenheimer complex, is the rate-limiting step in the overall catalysis. The fact that chemical reactivity of the fluoronitrobenzenes is the main factor in setting the outcomes of the overall conversion by the different glutathione S-transferase enzymes implies that extrapolation from rat to results of other species including man, and also from one individual to another, must be feasible. That this is actually the case is clearly demonstrated by the results of the present study.
BACKGROUND: It has been demonstrated that the Doppler Perfusion Index (DPI) is increased in patients who are at risk of developing liver metastases from colorectal cancer. It has been postulated that a circulating hormonal factor is involved in the relative vasoconstriction throughout the splanchnic bed. Endothelin-1 (ET-1), a potent vasoconstrictor which has been associated with tumor growth and is produced by colorectal tumors, may play an important role in this phenomenon. In this paper the prognostic value of serum ET-1 in colorectal cancer is discussed. METHODS: Preoperative serum levels of ET-1 were assessed in three groups of patients: group A underwent resection of the colorectal tumor and remained free of recurrence (n=20); group B developed metachronous liver metastases at least six months after colorectal resection (n=14); and group C presented with colorectal cancer and synchronous liver metastases (n=22). RESULTS: The mean (SD) serum ET-1 levels in groups A, B and C were 1.59 (0.41) pmol/L, 1.70 (0.32) pmol/L and 1.85 (0.47) pmol/L, respectively. These values were significantly different from those of healthy controls (1.22 (0.31), p<0.05). Kaplan-Meier analyses revealed no prognostic value of preoperative serum ET-1 levels. CONCLUSIONS: These preliminary results demonstrate that serum ET-1 levels are raised in patients with colorectal cancer. Serum ET-1 levels do not seem to be of prognostic value for survival.
BACKGROUND: In vivo labelling with synthetic thymidine analogues, such as Iododeoxyuridine (IdU) and Bromodeoxyuridine (BrdU), has frequently been used to estimate tumour proliferation. However, this method requires intravenous administration of IdU or BrdU, thymidine analogues that are potential mutagens. Recently, the monoclonal antibody MIB-1 has been developed, recognizing the Ki-67 nuclear antigen, which is associated with cell cycle proliferation and is found throughout the cell cycle (G1, S, G2 and M phases), but not in resting (G0) cells. We studied the correlation between the MIB-1 labelling index (LI) and the IdU labelling index in normal and malignant gastric tissue. PATIENTS AND METHODS: Twenty patients with gastric cancer received an intravenous injection of IdU (200 mg/m2) before surgery. Specimens were obtained from gastric carcinoma and adjacent normal gastric tissue. The samples were fixed in formalin and immunohistochemical analyses of IdU LI and MIB-1 LI were performed. The LI was defined as the percentage of labelled nuclei of 5000 nuclei counted. RESULTS: The IdU LI ranged from 3.3% to 18.2% in gastric carcinoma and from 0.5% to 5.6% in adjacent normal gastric mucosa, whereas the MIB-1 LI ranged from 4.2% to 46.0% in gastric cancer and from 1.3% to 25.1% in adjacent normal gastric mucosa. Comparison of IdU LI with MIB-1 LI, using the Spearman rank correlation coefficient test showed a significant correlation between IdU LI and MIB-1 LI in normal gastric tissue (r = 0.63, p < 0.05). However, in gastric carcinoma no significant correlation was found between either proliferation marker (r = 0.07, N.S.). CONCLUSION: MIB-1 accurately reflects the in vivo IdU LI in normal gastric tissue, whilst in gastric carcinoma the MIB-1 LI does not seem to be a substitute for the in vivo IdU LI.
Liver metastases occur in more than 40% of the patients with colorectal cancer. The best prospect of cure is achieved by resection of the metastases. Only 10-15% of the patients with colorectal metastases are candidates for resection. When resection is impossible, other treatment options are limited. Response rates to chemotherapy are around 40%, but survival benefit is generally limited to a few months. Recently, cryosurgery has provided a new therapeutic approach for unresectable colorectal liver metastases. The metastases are localized by ultrasonography and are subsequently frozen with the aid of cryosurgical equipment. The treatment results in necrosis of the tumor and is characterized by low mortality and acceptable morbidity. Experience with cryosurgery is limited, but the results are promising. Various studies report a 1-year survival of more than 70% and a 2-year survival of > 50%. Disease-free survival after 2 years varies between 20-29%. Recurrences in the liver are usually other lesions than those treated with cryosurgery. Cryosurgery in combination with resection or as sole treatment, can result in radical treatment for patients formerly considered unresectable. Resection of liver metastases, however, remains the gold standard in the treatment of liver metastases. The main indication for cryosurgery may be as a complement to hepatic resection in those patients in whom liver resection can not achieve complete tumor clearance.
Computed tomography of the spleen was correlated with histologic examination or clinical data in 53 selected patients with non-Hodgkin lymphoma. Calculation of the splenic index with CT by multiplying spleen length, width and thickness is a simple and rapid procedure for defining splenic size in vivo. A good correlation was found between size of the spleen, as estimated by splenic index calculation, and the proven or probable histologic state of the spleen.
In a retrospective study 101 patients who underwent surgery for a parotid mass were evaluated. The different diagnostic modalities (sialography, ultrasound, needle biopsy and frozen section) are described and discussed. Eighty-one tumors proved to be benign, of which 56 patients had a pleiomorphic adenoma, ten of them had a recurrent pleiomorphic adenoma, all after an enucleation, which again proved to be an inadequate operation for a parotid mass. The 20 malignant parotid tumors had a poor clinical outcome. Of the 18 patients with a malignant primary parotid tumor three showed a local recurrence and five metastatic disease within five years. The morbidity of parotid surgery is mainly due to post-operative facial nerve weakness (30%), which is mostly reversible, and the Frey syndrome (10%).
The results of treatment in 74 patients treated in 1978-1990 for adenocarcinoma of the esophagus were retrospectively analyzed. The overall five-year survival was 7.8%, the median survival 10.0 months. Eighteen patients underwent a resection, in 13 patients with curative and in 5 patients with palliative intent. The median survival for these groups was 13.5 and 5.8 months, respectively. Thirty-one patients were irradiated. The response rate according to the WHO passage score was 79%. The median duration of response and survival was 6.0 and 9.5 months, respectively. Twenty patients were given palliative treatment, in the form of an endoprosthesis (10 patients), an endoprosthesis with chemotherapy (4 patients) or only chemotherapy (6 patients). The median duration of survival of patients in the palliative group was 6.5 months. The data are discussed in relation to results of treatment described in the literature.

Glutathione S-transferases (GSTs) are a family of isoenzymes that play an important role in protecting cells against cytotoxic and carcinogenic agents. The distribution and levels of GST Alpha and Pi in normal and malignant gastric tissue of 34 patients with gastric cancer were examined immunohistochemically. Expression of GST Alpha and Pi was observed in 47 and 100 percent of the tumors, respectively. In normal mucosa both enzyme classes were present in 100 percent of the specimens. Mucous cells showed staining for GST Alpha and Pi in 88 and 97 percent, parietal cells in 93 and 67 percent, and chief cells in 82 and 30 percent, respectively. No correlation was observed between the amount or pattern of GST Alpha or Pi in carcinomas and the clinical and pathological characteristics of the patients. So it can be concluded that both GST Alpha and Pi cannot be considered as prognostic factors for gastric cancer.
Glutathione S-transferases (GSTs) are enzymes involved in the detoxification of xenobiotics and are divided into four subclasses. Alpha, Mu, Pi and Theta. Most human gastrointestinal tumors contain increased amounts of GST Pi. In order to compare data on the expression of GSTs obtained by biochemical as well as immunohistochemical methods, we characterized the presence of GST Alpha and Pi by Western blot analysis and immunohistochemistry in 22 samples of human gastric carcinoma and adjacent non-neoplastic mucosa. Biochemical analyses revealed the presence of GST Alpha and Pi in 95% and 91% of normal tissues and in 82% and 100% of tumor specimens, respectively. Immunohistochemically all cases of normal gastric tissue stained for both GST Alpha and Pi, whereas immunostaining for GST Alpha and Pi was seen in 36% and 100% of the gastric tumor specimens, respectively. No statistically significant correlation however was observed between biochemical and immunohistochemical determination of GST Alpha and Pi both in normal as well as in malignant tissue. The absence of a statistically significant correlation between biochemical and immunohistochemical determination of GST Alpha and Pi implies that a high degree of caution must be taken in interpretating data derived solely from biochemical or immunohistochemical assays.
