Lymphography in the diagnosis of non-seminoma tumours of the testis.

In a series of 86 lymphograms correlated to histological findings after retroperitoneal lymph node dissection or to laparotomy findings, reliability proved to be 78%. The reliability of lymphograms evaluated as positive was 88%, while that of negative lymphograms was 74%. Retroperitoneal lymph node metastases were detected with the aid of lymphography in 58% of the patients. It was impossible to establish a node size above which metastases could be demonstrated with certainty by lymphography.

Tumors of the testis in two brothers: a case report.

This paper describes two nontwin brothers with a nonseminoma tumor of a testicle: an embryonal carcinoma in one and an embryonal carcinoma with seminomatous components in the other. Testicular tumors of the same type have the same frequency in twin brothers as in nontwin brothers. For the present, there is no insight into the genetic influence in the pathogenesis of testicular tumors.

The treatment of infantile embryonal carcinoma (Yolk-Sac Tumour) of the testis in children.

The case histories of five patients with an infantile embryonal carcinoma are presented. Personal observations and data from the literature indicate that treatment of this tumour is dependent on the patient's age. Non-operative staging is possible under age 1 (alpha 1-foetoprotein and CT-scan). Staging should include a laparotomy in patients aged between 1 and 2 years. Under age 2, retroperitoneal lymph node dissection is indicated only if metastases are demonstrable. From age 2 on, retroperitoneal lymph node dissection should be performed if possible: unilaterally in the absence, and bilaterally in the presence of retroperitoneal metastases. Chemotherapy is required only in cases with metastases. Radiotherapy is not indicated in the treatment of infantile embryonal carcinoma.

The relation between testicular tumours, undescended testes, and inguinal hernias.

A group of 230 patients with a malignant testicular tumour was found to include 12 patients with a history of an undescended testis (5.2%). Eight patients had a history of inguinal hernia (3.4%). The risk of malignant degeneration in an undescended testis proved to be 17 times greater than in the normal population. We found no increased risk for patients with a history of inguinal hernia. Nine patients with a history of orchiopexy developed a malignant testicular tumour; inguinal lymph node metastases were found in three of them. We advise the removal of all undescended testicles after puberty. In cases of malignant degeneration of a testis, which have been fixed in the scrotum, or in cases with a history of an operation in the inguinal region, the inguinal lymph nodes have to be treated. For seminoma, irradiation of these nodes is necessary; for non-seminoma tumours, hemiscrotectomy and inguinal lymph node dissection have to be performed.

Embryonal rhabdomyosarcoma of spermatic cord.

This article discusses 2 patients with embryonal rhabdomyosarcoma of the spermatic cord and presents a survey of the literature and therapeutic methods. In recent years the prognosis of this malignant tumor has improved significantly by virtue of a combination of radical inguinal orchiectomy and retroperitoneal lymph node dissection with supplementary chemotherapy and/or radiotherapy.

Extragonadal germ-cell tumours.

The case histories of two patients with an extragonadal germ-cell tumour are discussed. One had a retroperitoneal embryonal carcinoma, and the other had a mediastinal choriocarcinoma-seminoma. The pathogenesis, symptomatology, diagnosis and treatment of these rare tumours are discussed in some detail.?

Alveolar soft part sarcoma.

This paper presents the case history of a 22-year-old woman with an alveolar soft part sarcoma of the scapular region. Treatment consisted of resection. Cytostatic medication with adriamycin and dacarbazine (DTIC) failed to prevent rapid general metastatic growth.

Isolated fracture of the pisiform bone.

The case is reported of a patient with a fracture of the pisiform bone. The causation, diagnosis, treatment and complications of this lesion are discussed.

Rhabdomyosarcoma of the esophagus.

Case report of a woman aged 63 years suffering from a rare lesion: rhabdomyosarcoma of the esophagus. Surgical treatment was successful: over a year postoperatively the patient was in good condition. A review is presented of the ten cases reported in world literature. Etiology, morbid anatomy and treatment are discussed.
