[Evaluation on the effect of Benazepril for hypertension through postmarketing surveillance].

OBJECTIVE: To evaluate the efficacy and safety of Benazepril used among the essential hypertensives. METHODS: 1 831 essential hypertensive patients aged 35 to 75 were randomly selected from a community and followed for 18 months. The level of blood pressure, status of taking Benazepril and side effects were sequencially collected. RESULTS: 1/3 of the patients had taken antihypertensive drug before the study and the rate of compliance was over 96%. The effective rate of Benazepril was 73.6% at three months and increased to 84.7% at 18 months. Comparing with the baseline data, SBP and DBP declined 10.8 mmHg and 6.7 mmHg respectively. The rate of side effect was 22.7%. Cough was most commonly seen among side effects. The peak of first recording on side effect occurred at three months including 60% of them mild. CONCLUSION: Results showed that Benazepril had good efficacy and safety for the essential hypertension patients in a long-term observation.

[An genetic epidemiology study of MTHFR gene and blood level (sibpair linkage analysis)].

OBJECTIVE: To explore the relationship between MTHFR gene and blood pressure. METHODS: A total of 142 sibpairs were collected from rural communities of CCPACH. MTHFR gene genotypes were determined by an assay based on the polymerase chain reaction. Phenotype of this study was the level of blood pressure. Covariates were measured by questionnaire, physical examination and clinical indices. Sibpair linkage analysis was used to analyse data by SAGE software. RESULTS: In this study, prevalence of TT genotype of MTHFR gene was 9.3%, while CT was 41%. There was no evidence for linkage of the MTHFR gene with either systolic or diastolic blood pressure identified. CONCLUSION: In our study population, MTHFR gene did not seem to be not related to blood pressure.

[Geographic information systems spatial analysis on transmission of schistosomiasis in China].

OBJECTIVE: To understand the epidemiologic status and geographical distribution of schistosomiasis in China. METHODS: Relevant detabases were set up after collection of data from two National Sampling Surveys on Schistosomiasis, in 1989 and 1995. Spatial analysis was undertaken after the database linked to the GIS software which was supported by Arc View 3.0a. Correlation analysis was performed to understand the relationship between rates of human infection and cattle infection. RESULTS: The epidemic areas of schistosomiasis with high risk are mainly distributed in the marshland along the Yangtze River and can be identified as five spatial distribution regions based on the results of spatial analysis. Both epidemic areas and positive rates of stool examination in cattle and water buffalo are much wider and higher than that in humans. The positive correlation was seen between infection rate in human and in cattle from data of both sampling surveys. CONCLUSION: The relevant strategy for schistosomiasis control in different spatial regions should be performed accordingly and the measures of control for cattle and water buffalo should be strengthened in the endemic areas.

[Pathological features of liver biopsy from 66 cases of long term blood donors with chronic hepatitis C].

OBJECTIVE: To study the pathological features of liver biopsy from long-term blood donors with chronic hepatitis C. METHODS: From 250 long-term blood donors with positive serum anti-HCV antibody for more than five years, 66 donors with abnormal ALT level were selected and enrolled in this study. Liver biopsy was performed on these cases and the specimens stained with HE, and reticulin. Modified Schemer's classification for grading and staging was used during pathological observation. Twenty biopsies with serum HCV-RNA levels higher than 5.0 Meq/ml (bDNA) were studied immunohistochemically (SABC) for NS5. RESULTS: All cases displayed mild pathological changes. Grade 1 (G(1)) inflammation was 4.8% (3/63), G(2) 63.5% (40/63), G(3) 28.6%(18/63), G(4) two case. Stage 1 (S(1)) fibrosis was 60.3% (38/63), S(2) 34.9% (22/63), S(3) 3.2% (2/63), S(4) only one case. The main lesion was periportal inflammation, such as bile duct damage (100.0%), dense lymphoid aggregates (82.5%) and piecemeal necrosis (89.5%). Intralobular injury of hepatocytes was rather mild. Bridging necrosis and steatosis were not common and observed in only 12.3% (8/66) and 9.2% (6/66) respectively. As for fibrosis, proliferation and deposition of collagen were mainly distributed in the portal and perisinusoidal areas. Among 20 specimens with serum HCV-RNA levels higher than 5.0 Meq/ml (bDNA), all bile duct epithelial cells were positive for HCV-NS5 antigen, while hepatocytes in 4 cases were negative. CONCLUSION: (1) The pathological changes in this series show overall similarity with that of patients with posttransfusion hepatitis C. The main damages are portal and periportal inflammation, such as lymphoid aggregates, bile duct damage, piecemeal necrosis, and extensive periportal and perisinusoidal fibrosis. Intralobular damage was mild with occasional steatosis, which may be related to malnutrition, low level of serum iron due to long term donation. (2) Epithelial cells of the bile duct may be of another site for virus replication, thus it is related with bile duct damage and periportal inflammation.

[The clinical significance of the dynamic change in serum cardiac troponin T in burned patients].

OBJECTIVE: To evaluate the diagnostic value of serum cardiac troponin T (cTnT) in myocardial injury in burned patients. METHODS: Sixty-two burned patients were cardiac divided into 3 groups, i.e. mild and moderate (M, n = 25), severe (S, n = 22) and extra (E, n = 15) groups on the basis of patients' severity. Radio immunoassay was adopted to detect serum cTnT and CK-MB in patients of all groups on 1, 3, 7 and 14 postburn day. Ultrasound cardiogram and ECG (electrocardiogram) were employed to monitor cardiac function. Twenty healthy volunteers were enrolled in the control group (C). RESULTS: (1). The serum cTnT levels at all time points in patients in S and E groups were evidently higher than those in C group (P < 0.01) and paralleled very well with the severity of the patients' injuries. (2). CK-MB exhibited similar change as that of cTnT. (3). There exhibited abnormal findings with ultrasound cardiogram and ECG in five cases in E group. CONCLUSION: The dynamic changes in serum cTnT and CK -- MB could well reflect the degree of the myocardial injury in burned patients, and cTnT was much more reliable as an index of myocardial injury than the serum enzymes due to its higher specificity and sensitivity.

[Experimental study of mechanism and measures of prevention and treatment of hypothermal vasoconstriction].

OBJECTIVE: To investigate the mechanism and to explore the measures of prevention and treatment of hypothermal vasoconstriction. METHODS: By the techniques of endothelial cell culture and scanning electron microscopy, and vasomotor functional test of isolated vascular vessels, the relation of hypothermal vasoconstriction and the release of endothelium-derived contractile and vasodilative factors were observed. RESULTS: Hypothermia obviously induced vasoconstriction of isolated vascular vessels, whether endothelium was intact or removed, the lower the temperature, the higher the vascular tension. Removal of endothelium could decrease the effect of vasoconstriction by hypothermia. The conditioned medium of bovine aortic endothelial cell could induce significantly vasoconstriction of isolated rat common neck arterial ring in hypothermia. It indicated that the bovine aortic endothelial cells secreted contractile factors into the medium. Reheating to 37 degrees C or vasodilator or reheating plus vasodilator did not obviously influence the hypothermia-induced vasoconstriction within 2 hours. When reheating to 50 degrees C, vascular tension was decreased, but only changed in range of 28% to 42%. CONCLUSION: Hypothermia vasoconstriction is relative to vasoconstrictor factors secreted by endothelium. Reheating to 37 degrees C or vasodilator does not antagonize the constriction of vascular vessels. Reheating to 50 degrees C only partially eliminates the constrict effect of blood vessels, so the prevention of hypothermia vasoconstriction should be emphasized.

Effects of acute mild and moderate hypoxia on human mood state.

OBJECTIVE: To explore the effects of acute mild and moderate hypoxia on human mood state. METHOD: The mood states of 18 healthy male volunteers were evaluated by self-assessment questionnaires, profile of mood state (POMS) and state anxiety inventory (S-AI) after random exposure to simulated altitude of 300 m (control), 2800 m, 3600 m and 4400 m for 1 h in a hypobaric chamber. RESULT: The data at 300 m level were taken as the baseline control. The negative mood state factor points (tension, fatigue etc.) increased gradually as the altitude level increased while V (vigor-activity) points had a tendency to decrease (P<0.05 or P<0.01). No significant difference was found in the points of (D, A) even under exposure to 4400 m (P>0.05). At the early period of 2800 m exposure the tension points of POMS and S-AI scores were higher than those of control level (P<0.05) then dropped to baseline level when exposure to this altitude for 1 h. CONCLUSION: Exposure to acute mild hypobaric hypoxia at 2800 m for 1 h has adverse effect on mood state of healthy person and the negative effect was further aggravated with the increment of altitude level.

[Preliminary isolation and identification of neurotrophic factors sensory neurons in spinal dorsal horn of embryonic chicken].

Our previous studies have demonstrated that the extract from dorsal horn of embryonic chicken can promote apparently the neurite growth of spinal ganglion during development. In this study, we investigated progressively the neurotrophic factors for primary sensory neurons in spinal dorsal horn. The tissues of spinal dorsal horn from five hundred Hamburger Stage-40 embryonic chickens were taken. After homogenization and centrifugation, the supernatant was subjected to SDS-polyacrylamide gel electrophoresis (SDS-PAGE) for analyzing the protein in dorsal horn. The simple protein band fishing by cell (SPBFC) technique was employed in finding the protein bands with neurons attached, and then such protein bands, were taken and transferred to nitric cellulose paper for Western hybridization analysis by antibody for NGF, BDNF, NT-3 and GDNF respectively. The results of SPBFC showed that some neurons were attached to gel within the scope of mobility relative front (Rf) 0.10-0.19, 0.30-0.35, 0.50-0.55, 0.60-0.67 and 0.90-0.94 respectively, indicating there were some neurotrophic factors for maintaining neuron survival in the above Rf scope. Moreover, the result of Western hybridization showed that the positive hybridization bands in each scope were that of NGF, BDNF, GDNF; NGF, BDNF, NT-3; NGF, BDNF, NT-3; NGF, NT-3 and BDNF, NT-3 respectively; indicating that the neurotrophic effect of spinal dorsal horn tissue on primary sensory neurons involves NGF, BDNF, NT-3 and GDNF. These also suggest that the neuronal survival effect in each Rf scope involves at least two or more neurotrophic factors.

[Morphological observation on dorsal root ganglion neurons of embryonic chicken in vitro].

The aim of this study was to explore the developmental character of neurons from dorsal root ganglion (DRG) of embryonic chicken in vitro. The DRG neurons of Hamburger stage 35 were cultured and prepared at 0, 1, 2, 3, 4, 5, 7, 14 and 21 days respectively. The number of neuronal survival was counted at each cultured stage (observed area: 2.5 x 10(5) microns 2) and the morphological change of neurons in vitro was observed and compared with that of neurons in vivo. The results showed the counts of neuronal survival were 94 +/- 4, 83 +/- 6, 67 +/- 4, 65 +/- 7, 68 +/- 5, 71 +/- 9, 73 +/- 6, 22 +/- 4, and 0 respectively; the cultured neuronal shape during 48 h incubation was mainly oval or sphere, similar to that of neurons in vivo. However, the shape of some neurons changed into cone or shuttle after three days incubation, while the neuronal shape in vivo did not change. These findings indicate that only some of neurons can survive and grow and the lifetime is limited in vitro, on the other hand, the developmental character of neurons in vitro may be different from that of neurons in vivo.

[A study on the shape of nose of the Jingpo people in China].

The objective of this survey was to provide data for clinical plastics of nose, medical aesthetics and cosmetology, anthropology, nationality study and medical jurisprudence. We carried out a somatological survey on the nose of 611 Jingpo people living in Yunnan. The survey covered 277 males (aged 15 to 60 years) and 334 females (aged 13 to 55 years). The findings of the features of nose are as follows: the Nasal Root Height is high (average 55.83%); the Nasal Tip Direction is forward (average 81.17%); the Nasal Base is horizontal (average 79.17%); the Alae Nasi is slightly projecting (average 91.83%); the Nasal Profile is straight (average 74.83%). The type of the Nasal Index is middle (70.0-84.9). The average of the Nasal Height (NH) of both male and female is 49.09 mm, the average of the Nasal Length (NL) is 41.99 mm, the average of the Nasal Breadth (NB) is 36.44 mm, the average of the Nasal Index is 74.93, but there is significant difference between male and female. The average of the Nasal Depth (ND) is 14.59 mm, the average of the Nasal Deep Index is 40.19; there is no significant difference between male and female. There is no difference between adults and youths, but there is marked difference between adults and teenagers, suggesting that the development of the nose of the Jingpo people tends to end up in youth.

[The expression of trkC in spared dorsal root ganglion following unilateral dorsal root rhizotomy].

This study sought to explore the expression of trkC in spared dorsal root ganglion(DRG) following unilateral dorsal root rhizotomy. 15 cats were divided into three groups: the intact group, the 3-day and 10-day groups after operation (unilaterral L1-L5 DRG and L7-S2 DRG were sectioned, but L6 DRG was spared). The animals were sacrificed on the 3rd day and 10th day after operation, respectively. The L6 DRG from the three groups were taken and cut into frozen sections (20 microns). The sections were stained using specific antiserum for trkC by the immunohistochemical ABC method. The numbers of large sized neurons (> 57 microns) and small sized neurons (< 42 microns) for trkC-IR were counted respectively. The results showed that the number of positive large sized neurons for trkC of the 10-day group after operation decreased significantly than that of the intact group and the 3-day group after operation, while the number of positive small sized neurons for trkC of the 10-day group increased apparently than that of the intact group and the 3-day group after operation (P < 0.05). This experiment demonstrated that unilateral partial dorsal root rhizotomy could change the expression of trkC might in large neurons and small sized neurons, and the results suggested that trkC might involve in the spinal cord plasticity following partial dorsal root rhizotomy.

[Study on the callus and cell culture of Hypericum perforatum].

Callus were induced from the leaves and stems of Hypericum perforatum L. The cell suspension culture and plate culture were also investigated in this paper. The results showed that the medium of MS which contained 2,4-D 1 mg/L and NAA 1 mg/L was the best one for inducing callus. The plant growth regulator of 2,4-D was assential for callus and suspension culture. Factors such as the time of cell suspension culture, the ways of plating and cell density, which affects the plating efficency were studied. The results showed that plating at density of 5 x 10(3) cells/ml and with monocells seperated from the cells cultured 14-21 days, high plating efficiency could be obtained either by conditional plate culture or by nursing plate culture.

[Upper airway occlusion sites of obstructive sleep apnea].

OBJECTIVE: To locate the obstruction sites in patients with obstructive sleep apnea syndrome (OSAS) during sleep by MRI imaging and dynamic fiberoptic pharyngoscopy. METHODS: Fifteen OSAS patients confirmed by polysomnography (PSG) underwent ultrafast MR imaging during sleep. Their sequential midline sagittal and axial images were obtained and displayed in the cine mode. Then we examined the upper airway of the retropalate region (RP), retroglottal region (RG) and epiglottal region (EPG). We also did dynamic fiberoptic pharyngoscopy examination during sleep in 8 patients in another night. RESULTS: Among the 15 patients, 3 had only RP region obstruction, 2 had only RG region obstruction, and the other 10 had both RP & RG airway obstruction. In the 8 cases monitored with both MRI and fiberoptic pharyngoscopy, the results obtained from the 2 methods were identical. CONCLUSION: MRI and fiberoptic pharyngoscopy are useful in locating the pharyngeal airway obstruction sites of OSAS patients and would be helpful in selecting treatment measures.

[The upper airway MRI of obstructive sleep apnea patients].

OBJECTIVE: To investigate the mechanisms of obstructive sleep apnea syndrome(OSAS) through observing the upper airway caliber and its corresponding pharyngeal wall of OSAS patients by MRI. METHODS: The upper airway(UA) of 18 OSAS patients were examined by MRI. The cross-sectional area of the UA and lateral parapharyngeal fat pad (LPFP) were calculated. The thickness of lateral and posterior pharyngeal wall (LW & PW) and other related indices were also measured. Nineteen nonsnoring age-matched normal subjects were selected as the control group. RESULTS: 1. The retropalatal (RP) and retroglossal (RG) region UA cross-sectional areas of OSAS patients were smaller than that of the control (P < 0.05). The epiglottal (EPC) region UA cross-sectional area was not significantly different between the 2 groups (P = 0.204). 2. The anterior-posterior/Lateral ratio(A-P/L) of UA in RP, RG, EPG regions was bigger in patients group than that of the control (P < 0.05). 3. The cross-sectional area of LPFP in patients group was larger than that of the control group (P = 1.76E-7). 4. The thickness of the PW in the RP, RG and EPG region was thicker in patients group than that of the control (P < 0.05). Although the thickness of the LW in the RP region was not statistically different between the 2 groups (P = 0.94), this index in RG and EPG region was larger in patients group than that of the control (P = < 0.05). 5. Sagittal MRI image showed that the length, thickness and cross-sectional area of the palate in patient group was larger than that of the control (P < 0.01). CONCLUSIONS: The study suggests that the pathogenesis of OSAS is related to the following factors: UA caliber and its AP/L ratio, the thickness of LW and PW, the size of LPFP in RP region, and the size and length of the palate.

[Jugulotympanic glomus tumors: a report of 11 cases].

OBJECTIVE: To evaluate the surgical technique in removal of jugulotympanic glomus tumor. METHODS: Retrospective analysis of surgical techniques and effects of the surgical treated 11 cases (1982-1998) of jugulotympanic glomus tumors (tympanic type I, 1, II, 1, IV, 3; jugular type I, 1, III, 5). RESULTS: The tumors of all 11 patients were removed completely and the wounds healed smoothly. Only one case had postoperative laryngeal nerve paralysis. There was no recurrence after 1-8 years follow-up. CONCLUSION: Suitable surgical techniques are available for removal of jugulotympanic glomus tumors and are chosen according to the size and location of the tumor. Postauricular incision is suitable for Glomous tympanicum Type II, III, IV. Since postauricular incision with superior and inferior extension provides a good exposure of the neck and temporal bone, it is suitable for surgical removal of Glomus jugulare type I. In case of Glomus jugulare type III, postauricular large "C" incision may be chosen for resection of the mid- and infra-temporal fosse, neck and skull base tumor.

[The clinical study of tongue flaps repairing after resecting pharyngeal neoplasm and laryngeal neoplasm].

OBJECTIVE: To study the repair methods of the defect after resecting pharyngeal neoplasm and laryngeal neoplasm. METHODS: Four kinds of tongue flap, such as 1/3 tongue flap, 1/2 tongue base flap, tongue base flap and transverse tongue flap were devised and applied in 15 patients with pharyngeal neoplasm and laryngeal neoplasm. RESULTS: These tongue flaps were alive in all patients. The wound of 13 patients healed in I stage. Two patients dehiscenced because of infection and healed after treatment. All the patients deglutited well. One-year, 3-year, disease-free survive rate were 92.9% (13/14), 72.7% (8/11) respectively. CONCLUSION: Tongue flaps are obtained easily, with enough blood flow, adapted to pharyngeal environment, easily alive and low complication in repairing the pharyngeal defect. We think that these tongue flaps should be applied in clinic.

[Re-evaluation of an embolic material for aneurysm cellulose acetate polymer].

OBJECTIVE: To re-evaluate the safety, effect, and pathological reactions of cellulose acetate polymer (CAP). METHODS: 30 right carotid arteries of rats were embolized and got two embolized vessels for histological study in each of the seven stages within four months. Fifteen canine aneurysmal models were created by transplantation of venous pouches. Twelve of them were occluded with CAP. Digital Subtraction Angiography (DSA) follow-ups of occluded aneurysmal models were carried out 1, 2 and 8 weeks after occlusion. One animal was sacrificed randomly at each stage for pathological study. RESULTS: Only five of the 12 aneurysmal models were completely or partially occluded within the patency of parent arteries within two months of observation after embolization with CAP. Stenosis of one branch of aneurysmal parent artery was occurred. The residual cavity of one partially occluded model artery was enlarged. Two models ruptured on four and five days after embolization. Three embolized models and their parent arteries were all occluded. PATHOLOGICAL FINDINGS: acute pathological reaction occurred within vessels embolized with CAP. Endothelial cell and basal membrane were damaged and vanished. Elastic lamina was exposed to arterial lumen and attached by thrombus layer. Fibrous cells and smooth muscle cells showed obvious degeneration. CAP and thrombus were gradually organized in two months after thrombosis. The orifices of aneurysmal models were covered with newly developed fibrous tissue and endothelial cells in two months after thrombosis. CONCLUSIONS: We think that acute chemical erosive effect of CAP was evident. A strong effect could lead to rupture of some of the CAP thrombosed aneurysmal models. Besides, stenosis or occlusion of the parent arteries could be induced in some cases after embolization with CAP. For the above shortcomings and limitations, it should be improved carefully before it is applied in clinic.

Synthesis and structure-activity relationships of new second-generation taxoids.

A series of second-generation taxoids bearing a substituent on the C-2-benzoyl group and modifications at C-3'/C-10 positions was synthesized. These taxoids exhibited 2-3 orders of magnitude higher potency than that of paclitaxel against drug-resistant human breast cancer cell lines. It is also noteworthy that three taxoids showed almost no difference in activity against drug-resistant and drug-sensitive cell lines, which are categorized as "advanced second generation taxoids".
