[Pathological study of the therapeutic effect of compound qinggan granula on chronic hepatitis C].

OBJECTIVE: To assess histopathologically the therapeutic effect of Compound Qinggan Granula (QTG) on chronic hepatitis C. METHODS: Among 48 cases of chronic hepatitis C, 36 were treated by QTG (30g for one time and three times a day) for 6 months, and 12 were not treated with any drugs as control. Liver biopsy was done in all patients before and after the observation except 3 patients who refused to take biopsy again. Specimen were stained with HE and James method, and their grading of inflammation (G), staging of fibrosis (S) and scoring for both were then evaluated. RESULTS: In the treatment group, 14 and 26 cases showed a decreasing grading (42. 5%) and inflammation scoring (78.8%), respectively. Score for G was reduced from 8.42 to 5.58 (P<0.05). As for the fibrosis, the stage didn't change significantly (P>0.05), but the score was reduced in 19 cases (57.6%) from 3.83 to 3.13 (P<0.05). In the control group, there were not evident changes for grading and staging. The scoring of G increased from 7 to 8.6 and S from 4.15 to 6.6 (P<0.05). CONCLUSION: QTG is effective in treating chronic hepatitis C by attenuating inflammation and stopping or reversing the fibrosis.

[3-D direct matching interpolation for rotary scanning echo-cardial images].

In the present paper, a method of three dimension direct matching interpolation for a series of 2-D echo-cardial images with intersecting each other and arranging at an angle in space has been developed. It is different from the conventional interpolation. During the interpolation, sectional images are acquired and the 3-D spatial position of "empty voxel" in sectional images is located; secondly, the cubic volume is constructed, and the motion orbit of images between two windows where the nearest original image intersects to the volume is found up, passing through the "empty voxel"; In the end, based on the matching pixels, the "empty voxel" is interpolated. This method simplifies the two interopolating procedures of the conventional methods into one procedure, and overcomes the shortcoming of the overlap of image data. Experiments show that the accuracy and reasonability of interpolation can be improved by the proposed method.

Potentiation of thioacetamide liver injury in diabetic rats is due to induced CYP2E1.

Thioacetamide (TA)-induced hepatotoxicity is potentiated in streptozotocin (STZ)-induced diabetic rats. The relative roles of CYP2E1 and FMO1 in the mechanism of TA-associated liver injury were investigated. In the STZ-induced diabetic rat, hepatic CYP2E1 protein concentration and p-nitrophenol hydroxylation were induced 8- and 5.6-fold, respectively. Pretreatment with the CYP2E1 inducer, isoniazid (INH, 250 mg/kg, i.p.) before TA (300 mg/kg, i.p.) administration significantly increased TA-associated liver injury as assessed by plasma alanine aminotransferase (ALT). Hepatic CYP2E1 expression and p-nitrophenol hydroxylation were induced 2.2- and 2. 5-fold in the INH-pretreated rat, respectively. Inhibition of CYP2E1 by diallyl sulfide (DAS, 200 mg/kg, p.o., two doses) before TA administration, decreased plasma ALT activity by 60% in the nondiabetic rat and by 75% in the diabetic rat. Abolition of microsomal p-nitrophenol hydroxylation and CCl(4)-induced liver injury confirmed that hepatic CYP2E1 was highly inhibited by DAS. Hepatic flavin-containing monooxygenase (FMO) form 1 expression and methimazole-dependent oxidation of thiocholine were induced 2.5- and 1.8-fold in the diabetic rat, respectively. Dietary administration of 0.25% indole-3-carbinol (I3C) for 10 days inhibited FMO1 expression and enzyme activity in both nondiabetic and diabetic rats. Paradoxically, TA-induced liver injury was increased in these I3C-pretreated rats. These findings indicate that hepatic CYP2E1 appears to be primarily involved in bioactivation of TA. In the STZ-induced diabetic rat, diabetes-induced CYP2E1 appears to be responsible for the potentiated liver injury; Even though hepatic FMO1 is induced in the diabetic rat, it is unlikely to mediate the potentiated TA hepatotoxicity.

Remnant-like particle cholesterol and triglyceride levels of hypertriglyceridemic patients in the fed and fasted state.

Potentially atherogenic triglyceride-rich lipoprotein (TRL) remnants can be isolated and quantitated as remnant-like particles (RLP), using an immunoaffinity gel containing specific anti-human apolipoprotein A-I (apoA-I) and apoB-100 monoclonal antibodies. The aim of the present study was to determine the relationship between postprandial changes in RLP levels and changes in total serum triglyceride (TG) in patients with different forms of hypertriglyceridemia (HTG). Three groups of patients were selected, having similarly elevated serum TG levels: a) HTG with TRL remnant accumulation (i.e., type III patients, n = 15, TG: 3.8 +/- 0.2 mm), b) HTG with increased LDL (i.e., type IIb patients, n = 15, TG: 3.7 +/- 0.2 mm), and c) HTG without evidence of remnant or LDL accumulation (i.e., type IV patients, n = 15, TG: 3.9 +/- 0.3 mm). Ingestion of a 45-g fat meal caused a significant increase in serum TG (30;-50%) in all patients. Mean serum TG levels of the three groups were not significantly different at 4 or 6 h after the meal. RLP cholesterol (C) and TG levels increased after the meal in all patients, but these postprandial increases were also not significantly different among groups. Type III patients had significantly higher (P < 0.01) levels of RLP-C and RLP-apoE in the fasted and fed state, and also had significantly higher RLP-C-to-serum TG ratios (P < 0.001) compared with the other groups. These results indicate that 1) RLP-C and RLP-TG levels are significantly increased in the fed versus fasted state in patients with elevated fasting TG levels; 2) patients with different forms of HTG, but similar TG levels, have similar postprandial increases in RLP-C and RLP-TG; and 3) type III patients have significantly elevated levels of RLP-C and RLP-apoE in both the fed and fasted state.

Phospholipids in dialysate and the peritoneal surface layer.

Dialysate concentration of phospholipids has been used to monitor peritoneal membrane status. However, we recently found that the peritoneum has a surface layer in which phospholipids may be the main constituent. Therefore, in this study, we compared the phospholipids composition of peritoneal dialysate and of the peritoneal surface in rats. Eight male Sprague-Dawley rats were used in the study. Five rats received an intraperitoneal injection of 25 mL 4.25% glucose dialysis solution. After four hours, the rats were killed, and the dialysate was drained completely. Then 20 mL of Folch solution was infused into the peritoneal cavity for 30 seconds and drained completely. The other three rats received the Folch solution without dialysis. The effluent and Folch solution were then processed for phospholipids analysis using high performance thin-layer chromatography (HPTLC). The total phospholipids content was ten times higher in the surface layer than in the dialysate effluent. In the effluent, four clearly different components were seen: lysophosphatidylcholine (LPC), sphingomyelin (SM, 29%), phosphatidylcholine (PC, 66%), and phosphatidylinositol (PI, 4.5%). However, in the surface layer, as well as LPC, SM (20.6%), PHC (47%), and PI (6.3%), two additional components were seen, phosphatidylserine (PS, 17.1%) and phosphatidylethanolamine (PE, 8.9%). The quantity of phospholipids in the peritoneal surface of non dialyzed rats was similar to the total quantity of phospholipids (in effluent and in the peritoneal surface) of dialyzed rats. Our results suggest that: (1) a surface layer is present on the peritoneum; this layer could well be extracted by Folch solution; and, with appropriate incubation time, one can separate the surface layer without damaging the mesothelial cells; (2) the composition of phospholipids in the effluent is different from that in the peritoneal surface layer, which contains membrane phospholipids (PS and PE); (3) shielding from the peritoneal surface may be the main reason for the presence of phospholipids in the dialysate.

Comparison of three chronic dialysis models.

The chronic peritoneal dialysis model is important for understanding the pathophysiology of peritoneal transport and for studying biocompatibility of peritoneal dialysis solutions. In this study, we compared three different chronic peritoneal dialysis models. A peritoneal catheter was placed in 23 male Sprague-Dawley rats, 12 of which had an intact omentum (model 1) and 11 of which received an omentectomy (model 2). Seven other rats, without a catheter, received a daily intraperitoneal injection (model 3). Each rat received a daily infusion of 25 mL of 3.86% glucose dialysis solution either through the catheter (models 1 and 2) or through injection (model 3) for 4 weeks. Then, a 4-hour dwell study using 3.86% glucose solution with an intraperitoneal volume marker and frequent dialysate and blood sampling was performed in each rat. The intraperitoneal volume was significantly lower in all the dialysis groups as compared to a control group (n = 6) in which the rats had no chronic dialysate exposure. The peritoneal fluid absorption rate, as well as the direct lymphatic absorption rate, was significantly higher in the three dialysis groups as compared to the control group. In general, no significant differences were seen in any of the parameters among the three dialysis models. Owing to catheter obstruction, three rats in model 1 and four rats in model 2 were lost during dialysis. Histological examination showed no significant differences among the three dialysis groups. Our results suggest that omentectomy may not be necessary in the chronic peritoneal dialysis model when using dialysate infusion and no drainage. Based on the present study, we think that perhaps model 1 may be the method of choice to test new peritoneal dialysis solutions. However, owing to its simplicity, model 3 could also be used if great care is taken to avoid puncturing the intestine or injecting into the abdominal wall.

CD40 ligand expression on macrophages during peritonitis in continuous ambulatory peritoneal dialysis patients.

CD40-CD40 ligand (CD40L) interaction plays an important role in macrophage/monocyte-mediated inflammatory processes by up-regulating cytokine production by macrophages/monocytes and by preventing macrophage apoptosis at the inflammation sites. The present study investigated the possible regulation of CD40L expression in peritonitis during continuous ambulatory peritoneal dialysis (CAPD). We used fluorescence-activated cell sorter (FACS) analysis to detect CD40L expression on macrophages obtained from peritoneal dialysate. Our results showed that CD40L expression on macrophages was significantly increased in a peritonitis group (4.62 +/- 6.54) as compared to a control group (0.76 +/- 0.30, p < 0.01). The CD40L-positive cells were also significantly increased during peritonitis (97.86% +/- 1.67% in the peritonitis group as compared to 73.10% +/- 26.94% in the control group, p < 0.05). After successful treatment, the expression of CD40L was significantly reduced (3.66 +/- 1.12 vs 1.05 +/- 0.02, p < 0.05). We conclude that functionally expressed CD40L on macrophages may take part in acute inflammatory response during peritonitis in CAPD and may play an important role in the local defense against infection in the peritoneal cavity.

Chloride channel expression in cultured human fetal RPE cells: response to oxidative stress.

PURPOSE: The human fetal cell line RPE 28 SV4 has been useful for studies of oxidative stress and apoptosis in retinal pigmented epithelium. This cell model is now assessed in functional investigations of chloride channel activity. The study aims to determine the presence of specific chloride channels, including CFTR and ClC channels, to identify the properties of membrane chloride currents and to assess their modulation by hydrogen peroxide, cAMP, and other agents. METHODS: Channel expression was determined using RT-PCR and cDNA cloning and biochemical and immunocytochemical methods. Membrane currents were analyzed using whole-cell, patch-clamp techniques. RESULTS: RT-PCR results confirmed the presence of ClC-5 mRNA, and a full-length clone encoding ClC-3 was isolated from a cDNA library for RPE 28 SV4 cells. Specific staining for CFTR and several ClC channels was detected by immunocytochemistry. Whole-cell chloride currents (under conditions of symmetrical chloride concentrations) averaged 16.9 +/- 3.4 pA/pF (at +100 mV; n = 8), showed outward rectification, and had an anion permeability sequence of Cl(-) > I(-) > cyclamate. Currents were stimulated by cAMP cocktail (250 microM cAMP, 100 microM IBMX, and 25 microM forskolin) and were inhibited by 1 mM DIDS. The oxidative agent hydrogen peroxide (100 microM) decreased the current by 34% +/- 10% (n = 4). CONCLUSIONS. These findings suggest that RPE 28 SV4 cells possess regulated chloride channels including CFTR and members of the ClC chloride channel family. The inhibition of chloride currents by H(2)O(2) suggests that this cell line may be advantageous for studies of chloride channel modulation by oxidative stress.

[Three-dimensional reconstruction of experimental fibrotic liver tissue and effect of herbal compound on it].

OBJECTIVE: To observe the three-dimensional structure of fibrotic liver tissue and the effect of herbal compound 861 (Cpd861) on it. METHODS: Experimental rat liver fibrosis was induced by human albumin. Liver biopsy specimens were stained with Masson trichrome staining and liver fibrosis was divided into 6 stages. Three-dimensional structure of fibrotic and Cpd861 treated liver tissue were observed by computer aided three-dimensional reconstruction with serial slices. RESULTS: The fibrous septa of fibrotic liver were continuous and wide, the sinusoids were sparsely, irregularly distributed and dilated. After treatment with Cpd861, the fibrous septa were much less than that of without treatment. The sinusoids were more regular and arranged in radius to central veins. CONCLUSION: Observed three-dimensionally, experimental rat liver fibrosis can be inhibited and reversed by herbal compound.

Effect of selective IL-6 inhibitor Am-80 on experimental autoimmune encephalomyelitis in DA rats.

AIM: To observe the role of interleukin (IL)-6 in the development of experimental autoimmune encephalomyelitis (EAE). METHODS: DA rats were immunized by injecting bovine myelin basic protein (MBP). mRNA of cytokines, such as IL-6, IL-10, TNF-alpha, TGF-beta 1, IFN-gamma, and iNOS, were detected by RT-PCR. MBP was injected into ear to induce delayed type cutaneous hypersensitivity response (DTH). Histological studies were performed on the spinal cord with HE staining. Nitric oxide (NO) production from cultured murine macrophage clones was stimulated with LPS plus IFN-gamma. RESULTS: DA rats developed EAE disease with a peak of severity on d 13 and d 14. Am-80 (1.0, 3.0 mg/kg), a selective IL-6 inhibitor, inhibited the symptoms in terms of deterioration as observed by the clinical score, body weight and histological findings, in a dose-related manner. A high dose of Am-80 (3.0 mg/kg for 12 d) did not completely inhibit the disease, but delayed the symptoms and enhanced the delayed response. By prolonging the duration of treatment (18 d), Am-80 inhibited the onset of EAE during administration, but the symptoms of EAE appeared after the administration was stopped. Am-80 administerd for 12 d inhibited the DTH response on d 11 but not on d 22. RT-PCR studies demonstrated a strong expression of IFN-gamma, IL-6, IL-10, TGF-beta 1, TNF-alpha, and iNOS mRNA in spinal cord 13 d after immunization. However IFN-gamma, IL-10, TNF-alpha, and iNOS mRNA expression (on d 13) was suppressed by Am-80, except in the case of IL-6, hence the effect of Am-80 on the expression of IL-6 mRNA was examined in additional experiments. After Am-80 was administered for 12 d or 18 d, the expression of IL-6 mRNA was inhibited on d 12 or d 18, but increased on d 13 or d 19, respectively. CONCLUSION: These findings suggest that inhibition of EAE by Am-80 is initiated by inhibition of IL-6 production.

[Caloric values and ash contents of some mangrove woods].

The caloric values and ash contents of some mangrove woods in Dongzhai Harbor Mangrove Forest Reserve of Hainan Province were determined in Summer, and the changes of the caloric value of Kandelia candel woods at different latitudes were studied. The ash content of mangrove woods was low, ranging from 2.43% to 5.17%; and their gross and ash-free caloric values ranged from 17.23 kJ.g-1 to 19.21 kJ.g-1 and from 17.70 kJ.g-1 to 20.10 kJ.g-1, respectively. Sonneratia apetala wood had the lowest gross and ash-free caloric values. The caloric value of Kandelia candel wood decreased with increasing latitude in Summer.

Development of cytotoxic cerebral edema in rats following intracaudatum injection of tACPD, an agonist of metabotropic glutamate receptors.

OBJECTIVE: To explore the involvement of metabotropic glutamate receptors (mGluRs) in the formation of cerebral edema. METHODS: Male Wistar rats weighing from 250 g to 300 g were used. Trans-1-aminocyclopentane-1, 3-diacarboxylic acid (tACPD), an agonist of mGluRs, was microinjected into the right caudatum. Brain water content was determined by a wet weight/dry weight technique and Na+, K+ and Ca2+ contents were measured by inductive Couple Plasma-9000 at 6 h, 24 h and 48 h post-injection. Extravasation of Evan's blue (EB) into the brain was determined as an indicator of disturbance in the blood-brain barrier (BBB) and endothelial cells. Histologic studies were performed under a Leitz microscope and a Philips EM208s electron microscope. RESULTS: Dose-dependent and time-related increase of brain water was induced after tACPD (10, 50, 500 and 1000 nmol) injection. A significant increase in Na+ and K+ content but not in Ca2+ content was observed. EB extravasation showed no blue stain, indicating no increase in BBB permeability induced by tACPD-injection. Electron microscope study confirmed this finding and revealed remarkable swelling of astrocytes especially endfoot processes of astrocytes around capillaries at 6 h after tACPD-injection. In addition, all changes mentioned above occurred in both caudatum. CONCLUSION: These results indicate that activation of mGluRs by tACPD injected into the caudatum induced cytotoxic brain edema and interfered with astrocyte K+ buffering. This may provide new clues for therapeutic intervention.

Carcinoma of the lung and multiple primary tumors.

OBJECTIVE: To assess the incidence of multiple primary malignancies combined with lung and other organs. METHODS: All patients who were consecutively treated for lung cancer from December 1964 to December 1992 were retrospectively analyzed regarding the occurrence of simultaneous primary tumor elsewhere in the body. RESULTS: Of 1019 patients reviewed, 21 (2.1%) had associated carcinoma of other organs. In these 21 patients, the incidence of associated aerodigestive tract cancer was 71% and cancer of other organs was 29%. Five patients had simultaneous cancer with the lung cancer and 16 metachronous lung cancer associated with cancer of other organs. CONCLUSIONS: Multiple primary tumors in patients with carcinoma of the lung is not a rare phenomenon. Surveillance programs for detection of second primary tumors should focus on this group of patients. Surgeons should be aware of the possibility of double cancer, and the patient's condition and stage of the carcinoma must be taken into consideration in the determination of operative indications and method.

[Effects of xiaopi yishen oral liquid on hypothalamus-pituitary-adrenal gland-thymus axis in stressed rats].

OBJECTIVE: To investigate the effect of Xiaopi Yishen (XPYS) oral liquid on the rats under repeated forced cold water swimming (RFCWS) stress. METHODS: The changes of serum monoamine, ascorbic acid levels in adrenal gland, monoamine levels in hypothalamus and hypophysis, and thymus index were determined. RESULTS: XPYS lowered the increasing of serum DA, 5-HT, improved the reducing of ascorbic acid in adrenal gland and NE, HVA, 5-HT, 5-HITT in hypothalamus and hypophysis of RFCWS-stressed rats. CONCLUSION: The mechanism of XPYS is in connection with the regulation of hypothalamus-pituitary-adrenal gland-thymus axis.

[Study of prevention and treatment on acute radioactive injury with huoxue yiqi yangyin recipe].

OBJECTIVE: To reduce the acute toxic and side-effect of radiotherapy with Huoxue Yiqi Yangyin recipe (HYYR). METHODS: Seventy-two nasopharyngeal carcinoma patients that using radiotherapy were divided into two groups: HYYR group (A) and the control group (B) using Wisk lotion for gargling or apply at the skin lesion. RESULTS: The injury on mucous membrane of mouth cavity and skin were 47.2%, 91.7%, 13.9%, 33.3% in group A and B respectively, the degree of injury was different (P < 0.01). The acute reaction of the mucous membrane of mouth cavity did not occur until the cumulative radiation were 41.4 +/- 9.4, 30.9 +/- 8.9 in group A and B respectively, there was significant difference between the two groups (P < 0.01). CONCLUSION: HYYR can reduce the acute radioactive reaction of the skin and mucous membrane of mouth cavity and inhibition of bone marrow.

[Cytokine-induced release of plasminogen activator inhibitor-1 by human pleural mesothelial cells].

OBJECTIVE: To investigate the effects of inflammatory cytokines on the production of plasminogen activator inhibitor-1 (PAI-1) by human pleural mesothelial cells (HPMC). METHODS: Six primary cultures of HPMC isolated from patients with exudative pleural effusion were incubated in DMEM/F12 medium to examine the effect of interleukin-1beta (IL-1beta), tumor necrosis factor-alpha (TNFalpha) or transforming growth factor-beta1 (TGF-beta1) on the fibrinolytic properties of HPMC in vitro. The purity of the cell population was assessed by morphologic, ultrastructural and immunohistochemical characteristics. PAI-1 activity in the conditioned media of third passage HPMC was detected using chromogenic substrate method. RESULTS: As compared with the controls, PAI-1 activity in the conditioned media of HPMC increased significantly by stimulation with IL-1beta, TNFalpha or TGF-beta1 for 24 hours at the level of 0.20 microg/L (IL-1beta: q = 3.38, P < 0.05; TNFalpha: q = 4.41, P < 0.05; TGF-beta1: q = 22.03, P < 0.01), 2.00 microg/L (IL-1beta: q = 12.28, P < 0.01; TNFalpha: q = 16.60, P < 0.01; TGF-beta1: q = 19.23, P < 0.01) and 20.00 microg/L(IL-1beta: q = 24.30, P < 0.01; TNFalpha: q = 25.00, P < 0.01; TGF-beta1: q = 49.30, P < 0.01). Furthermore, HPMC responses to TGF-beta1 were significantly greater than those to IL-1beta or TNFalpha. CONCLUSION: These results suggest that inflammatory cytokines might mediate the release of PAI-1 by HPMC in vivo, affect the deposition of fibrin within the pleural cavity and favor fibrogenesis.

[Ultrasound biomicroscopic examination of secondary pupillary block glaucoma].

OBJECTIVE: To study the imaging characteristics of ultrasound biomicroscopy of secondary pupillary block glaucoma. METHODS: 11 cases with pupillary block glaucoma diagnosed by ultrasound biomicroscopy were studied which had been suspected as malignant glaucoma at the beginning. RESULTS: There were no obviously typical characteristics of pupillary block glaucoma shown in the images of ultrasound biomicroscopy in these cases. However, the existence of posterior chamber in each case strongly supported the diagnosis of pupillary block glaucoma. The imaging characteristics of ultrasound biomicroscopy were different in terms of the different primary disorders which resulted in the pupillary block glaucoma including iridocyclitis, lens subluxation resulted from trauma and glaucoma filtration surgery. In the cases of lens subluxation, the main characteristic was the increase of the distance between the lens and the ciliary process at the site of subluxation and the lens forward movement. In the cases of iridocyclitis, anterior and posterior synechiae were the main characteristics. And in the cases of glaucoma filtration surgery there were two main characteristics. One was that the distance between the ciliary process and the equator of lens existed obviously, and the other was the occlusion of the inner ostium of glaucoma filtration surgery or peripheral iridectomy. CONCLUSION: Ultrasound biomicroscopy (UBM) has important value in the diagnosis of secondary pupillary block glaucoma and the differentiation between it and malignant glaucoma. Whether the posterior chamber exists or not is the differential point between the pupillary block glaucoma and the malignant glaucoma.

[The epidemiological characteristics of multiple metabolic disorders in hypertensive patients in the communities].

OBJECTIVES: To describe the epidemiological characteristics of multiple metabolic disorders on hypertensive patients living in the communities. METHODS: The characteristics of metabolic disorders in the hypertensives from one community in Shanghai urban area were analyzed. Metabolic disorders would include dyslipidemia (high cholesterol and/or high triglycerides and/or low HDL cholesterol), diabetes and obesity. RESULTS: 59.2% of the hypertensives at least had one metabolic disorder. Proportionally, 83.9%, 15.5%, 0.6% of the patients would combine one, two, or all of the disorders. The percentages of patients combined with dyslipidemia, diabetes and obesity were 53.0%, 9.7%, 6.3%, respectively. 74.0% of patients with metabolic disorders were combined with dyslipidemia while 9.1% were combined with both dyslipidemia and diabetes. The standardized prevalence rate of male patients with metabolic disorders was 58.1%, higher than the rate of women (54.5%). The prevalence rate of postmenopausal female patients with metabolic disorders was 67.6%, significantly higher than that of the premenopausal women (44.4%) (chi(2) = 37.49, P < 0.01). CONCLUSION: Nearly 60% of the total patients combined with metabolic disorders. Dyslipidemia occurred in more than half of the patients in total. The proportion of senile patients with metabolic disorders was higher than the younger patients. Results suggested that attempts to maintain the coronary heart disease at low levels in the population should include treatment of dyslipidemia to keep lipid and diabetes prevalence at low levels other than regular antihypertensive therapy.
