Abnormalities of various serum enzyme activities in patients with congenital adrenal hyperplasia.

Recently, attentions are being aroused as to the enzymatic network abnormalities lying behind congenital enzyme deficiency syndromes. We investigated abnormalities in activities of various hydrolytic enzymes in serum of patients with congenital adrenal hyperplasia (CAH, 21-hydroxylase deficiency). Several enzyme activities including trypsin-like enzyme, cathepsin C and esterase were significantly decreased in patients' serum. Especially the esterase activity in patients' serum was reduced to one third of controls and this may have some relations to the abnormal steroid metabolism of these patients. A multivariate analysis showed unexpectedly extensive abnormalities in enzyme interrelationships. These results suggest that wide variety of abnormal metabolism may be related to an apparent enzyme deficiency.

[Choriocarcinoma: cerebral metastasis from choriocarcinoma--a successfully surgical treated case].

A surgical case of multiple cerebral metastases from choriocarcinoma was reported, who was a 26 year-old female. She had received treatment of hydatid mole on April 1979, and admitted to our hospital for the consciousness disturbance and right hemiparesis. CT scan showed two high density tumors with marked brain edema. Emergency craniotomy was performed and five tumors were completely removed. Following the operation, symptoms of increased intracranial pressure and disturbance of consciousness were markedly improved in about a week. She received radiation therapy. After radiochemotherapy, a metastatic lung tumor was removed. Now her chorionic gonadotropin titers remain normal and she is able to perform all her household responsibilities despite a mild right hemiparesis. Recently, surgical treatment has been getting done for cerebral metastasis of choriocarcinoma. Because most of the metastatic cerebral lesions occur in relatively easy position for the operation. If symptoms of cerebral metastasis appear, we should performed surgical treatment as soon as possible.

[Intra-arterial infusion through 2 routes in liver (primary and metastatic cancer].

Patients with unresectable liver cancer (primary and metastatic) were treated with a newly-devised arterial infusion chemotherapy using Cis-DDP and its antidote Sodium thiosulfate (STS). The patients consisted of 4 with primary hepatoma and 1 with metastatic liver cancer originated from intestinal cancer. For the purpose of reducing unfavourable side effects without changing anticancer effect, Cis-DDP was infused into hepatic artery through the catheter while STS being administered systemically. According to Koyama and Saito's criteria, 2 of 5 patients showed partial response (PR) and the others showed no change (NC). The median survival time after onset of therapy was 6.6 months ranging from 2 to 11 months in all the patients. The myelosuppression and renal toxicity, which were considered to be the most serious side effects of Cis-DDP, were not found and liver function was not affected in all the patients. However, all the patients complained of nausea and vomiting. Thus, our experience has indicated that this newly devised infusion chemotherapy is a promising method for treating unresectable liver cancer, although further efforts are necessary for reducing the gastrointestinal toxicity.

Hepatic artery aneurysm. Report of a case and review of the literature.

One hundred and nineteen cases, including the one reported here, of hepatic artery aneurysm were reviewed. The clinical features and surgical management in four etiologic groups, consisting of arteriosclerosis, cholecystolithiasis, trauma and, specific diseases, were investigated. Interruption of the hepatic artery, entailing ligation of the hepatic artery, excision of the aneurysm without vascular reconstruction, and obliterative aneurysmorrhaphy, was done in 50 patients. Its high mortality rate (26%) was principally ascribed to delayed diagnosis or inappropriate site of interruption. If conditions such as shock, anoxia and portal thrombosis are avoided, interruption of the hepatic artery seems to be a safe and useful operative method for hepatic artery aneurysms, especially accompanied by severe local infection.

[Effects of angiotensin-induced hypertension on cancer chemotherapy. Application to intra-arterial infusion chemotherapy for advanced breast cancer].

In an attempt to enhance effects of cancer chemotherapy, Angiotensin II (AT), a vasoconstricting catecholamine, was applied to intra-arterial infusion of the drug for advanced breast cancer. Quantitative measurement of tumor blood flow was conducted with scintigraphic techniques (81mKr). Angiotensin II given continuously through the internal thoracic artery induced prompt responses in local blood flow with a decrease in normal breast tissue and a concomitant increase in tumor. Thus, the tumor/control ratio of blood flow with AT rose to 3 times as much as that without AT. In 11 patients receiving adriamycin (ADR) alone intra-arterially, 73% responded clinically with CR or PR in their lesions, while in 11 patients receiving ADR + AT, 91% responded. Histologically, as high as 45% of patients treated with ADR + AT underwent marked regressive change with no viable cancer cells remaining in their lesions, compared to 18% in patients treated with ADR alone. Angiotensin II combined with chemotherapy, therefore proved to enhance the anticancer effect of chemotherapy.

[Evaluation of aspiration cytology in the diagnosis of mammary tumors].

Aspiration cytology was performed in 1603 patients with breast lesions. Of 298 histologically confirmed malignant tumors, 234 (78.5%) had a concordant cytologic diagnosis. In 75% of 24 patients infiltrating cancer of 1.0 cm or less in greatest dimension, a correct diagnosis was obtained. The ratio of true-positive cases was higher and that of suspicious and false-positive cases lower, in aspiration cytology compared to mammography or echography. Cytologic diagnosis showed 4.7% false-positive results in this series. Atypical cells from intraductal proliferation and low malignant cancer cells were sometimes difficult to distinguish. In patients with such atypical cells, surgical biopsy was required. No difference in the 5-year survival was found between breast cancer patients who had aspiration cytology and their matched controls. Therefore, this method is very safe and is considered to be the most valuable in the correct diagnosis of breast cancer.

Effect of operation on cell-mediated immune response to autologous tumor extract in cancer patients.

Utilizing lymphocyte proliferation assay, effects of operation on cell-mediated immune response to autologous tumor extract were studied in 33 patients with gastric or colon carcinoma. To evaluate the lymphocyte proliferation response to autologous tissue extract preoperatively, the lymphocytes were left in culture for 24-30 hours before addition of the extract while 3 M KCl extract of autologous tissue was being processed. A significant level (P less than 0.001) of correlation was observed between stimulation index values to autologous tumor extract added at the initiation of culture and after a 24-30-hour period of incubation. Using this type of assay system, lymphocyte proliferation response to autologous tumor extract was examined immediately before operation and 1, 2, and 4 weeks after operation serially. Data comparing the responses of 20 patients who had curative resection and 13 patients who underwent noncurative resection appeared to fall into three categories in each group. The clinical courses of these patients in each category might clarify the significance of changes in the response following operation.

Clinical application of the Nd:YAG laser endoscopy.

We report here our clinical experiences with Nd:YAG laser therapy, and evaluate the results of this treatment. From July 1980 to December 1981, we carried out endoscopic laser treatment for 31 patients with 33 lesions. Bleeding gastric ulcers except stomal ulcers were treated successfully. For mucosal lesions of the stomach, Nd:YAG laser irradiation was effective in extirpating them. In advanced gastric cancers, symptoms of cardiac stenosis could be relieved in 80% of the cases. Endoscopic laser therapy was also effective in 80% of postoperative stenosis in the gastrointestinal tract. It is concluded that endoscopic irradiation with the Nd:YAG laser is useful for hemostasis and also for the treatment of malignant tumors and stenosis of the gastrointestinal tract.

Extra echo spaces: ultrasonography and computerised tomography correlations.

Among the echocardiograms of 844 patients of the International Goodwill Hospital from January 1980 to April 1981, 700 showed clinically inexplicable extra echo spaces. Fifty of the 700 had computerised tomography of their hearts which showed the extra echo spaces to be caused either by anterior or posterior subepicardial fat. Six of the 50 cases had both fat and pericardial effusions, which are difficult to differentiate echocardiographically unless follow-up clinical observations are performed. Subepicardial fat deposits are reasonable explanations for the extra echo spaces frequently observed by echocardiography: they correlate well with clinical findings. Subepicardial fat should be recognised as the cause of such extra echo spaces.

Effect of cimetidine on prolactin secretion in normal controls and hyperthyroid patients.

Nine healthy female controls and 10 hyperthyroid female patients were studied. The intravenous administration of 200 mg cimetidine, an H2-receptor antagonist, was followed by a significant and marked rise in serum prolactin levels in all control subjects. There was no significant difference in serum PRL response to cimetidine injection between the euthyroid controls and hyperthyroid patients. But max delta PRL, the change from basal to peak values, is significantly lower in the hyperthyroid patients than in the controls. There was a significant negative correlation between max delta PRL and serum T4 or T3 levels in hyperthyroid patients before and after treatment with MMI or PTU. It appears from our data that cimetidine induced PRL release was blunted in hyperthyroid patients.
