Genital mycoplasma colonization in neonatal girls.

Specimens obtained from the genital tract of 90 pairs of mothers and their female infants were cultured for Ureaplasma urealyticum (Ureaplasma) and Mycoplasma hominis (M.hominis). Ureaplasma and M.hominis were isolated from 80% and 17% of pregnant women, and 41% and 4.4% of female infants, respectively. Fifty nine per cent of infants were positive for Ureaplasma when they were borne per vaginam by Ureaplasma-positive mothers. However, Ureaplasma was not isolated from any of 11 infants delivered by cesarean section before rupture of the fetal membranes, in spite of positive colonization by Ureaplasma in the mothers. Quantitation of Ureaplasma revealed that neonatal colonization was closely associated with the colonization number of this microorganism in the cervical canal of mothers. Prolonged interval between rupture of fetal membranes and delivery caused a slightly increased frequency of isolation of this organism, but it was less significant. A study of mean birth weight revealed that there was no correlation between maternal or neonatal colonization by Ureaplasma and low birth weight.

[Estimation of 99mTc-dimercaptosuccinic acid renal uptake using single photon emission computed tomography].

Renal function was assessed by measurement of 99mTc-dimercaptosuccinic acid (DMSA) uptake by the kidney based on the transsectional tomographic image obtained by single photon emission computed tomography (SPECT). The renal uptake was expressed as percentage of the total radioactivity detected in the kidney, the volume of which was measured by convolution method, against the amount dosed. Absorption was corrected by GE-STAR method using cut off level of 42%. In order to determine normal range, measurement was made for 40 kidneys of each of 10 male and female volunteers confirmed of having normal kidneys both morphologically and functionally. The average volume of the kidney was 220.4 ml for the right and 239.3 ml for the left for males, and 205.9 ml and 236.5 ml, respectively for females. The renal uptake of radioactivity (at 2 h after injection), was 26.8% for the right and 27.6% for the left for males, with corresponding figures for females being 26.4% and 27.9%, respectively. Distribution range of renal volume and renal uptake was obtained by bivariate analysis with 90% and 95% probability. From these results, our method of renal function determination based on renal uptake of 99mTc-DMSA obtained from renal transactional tomogram by SPECT is considered to be accurate and potentially useful for clinical purpose.

[A case of massive fatal hemorrhage in a recurrent medulloblastoma during radiotherapy].

A medulloblastoma in a 7-year-old girl showed complete remission after the initial radiotherapy of 5,000 rad. But she had three episodes of recurrence in the cerebellum and in the spinal cord, and was treated by additional radiotherapy for the lesions 3,000 to 5,000 rad. When the total cerebellar tumor dose became 13,300 rad, a fatal intratumoral hemorrhage occurred. Autopsy revealed a massive intratumoral hematoma and thin-walled tumor vessels which might be designated as "sinusoid" or "telangiectasia" microscopically. The authors proposed a possibility that the thin-walled vessels and the high dose of radiotherapy caused the hemorrhage.

[A case of adult respiratory distress syndrome (ARDS) induced by radio-chemotherapy (RAFP therapy) of brain metastasis of stomach cancer].

A 77-year-old male was admitted to the hospital because of left hemiparesis secondary to multifocal cerebral metastases from adenocarcinoma of the stomach. He was treated with combination of radiotherapy and chemotherapy consisting of ACNU, Tegafur and PSK. He was in good condition, but abruptly developed severe dyspnea 40 days after administration of Tegafur and 28 days after that of ACNU. Chest X-ray at that time revealed diffuse opacity involving entire lung fields associated with marked hypoxia. The patient expired 9 days after this episode. The autopsy revealed acute interstitial pneumonitis associated with hyaline membrane formation consistent with adult respiratory distress syndrome involving entire lobes of both lungs without metastases. As to the etiology of the ARDS in this case, we concluded that the administration of Tegafur was the most likely as to the cause, although the possibility of betamethasone was not ruled out. The remaining factors were not likely as to the cause of the ARDS in this case.

The dissociation between the plasma levels of aminopeptidases A and B in spontaneously hypertensive rats.

We performed a longitudinal study for 20 weeks on spontaneously hypertensive rats (SHR) and Wistar-Kyoto rats (WKR) to determine the relationship between peptide metabolism and the age-dependent increase in blood pressure. In both SHR and WKR, the plasma level of aminopeptidase A (AP-A) clearly showed an age-dependent decrease. The plasma level of aminopeptidase B paralleled that of AP-A in WKR, but such an age-dependency was not observed in SHR, thus showing a dissociation between the two aminopeptidases. With age in both strains, the level of angiotensin-converting enzyme tended to decrease, while that of kallikrein activity tended to increase. In addition to these findings, a multivariate study testing the relationship of blood pressure to these enzyme activities, as well as to plasma levels of angiotensin I and renin activity, suggested abnormalities in the networks of proteolytic enzymes and in the peptide metabolism surrounding the renin-angiotensin system in SHR. These abnormalities may play some important roles in pathophysiological mechanisms of hypertension in SHR.

[Treatment of carotid-cavernous sinus fistula by the balloon technique combined with a by-pass procedure using a vein graft-technical note note].

A case with spontaneous carotid-cavernous fistula treated by balloon catheter technique combined with a extra-intracranial by-pass using a saphenous vein graft between the common carotid artery and the middle cerebral artery is reported. A 72-year-old female was admitted with complaints of recurrent symptoms of carotid-cavernous fistula 4 months after spontaneous remission. Angiograms showed the presence left internal carotid-cavernous sinus fistula. Poor cross-filling from the right internal carotid artery to the territory of the left internal carotid artery via the hypoplastic right A1 and left posterior communicating artery was also observed. Symptoms such as chemosis, exophthalmos and visual disturbance gradually progressed during the hospitalization. A common carotid-middle cerebral artery anastomosis was performed with a saphenous vein graft and the carotid cavernous fistula was occluded directly with a balloon catheter. The blood flow via the graft, cortical blood flow, cortical tissue O2 and CO2 were continuously measured during the operation. After the internal carotid artery occlusion, sufficient blood flow via the graft and no significant changes of the cortical blood flow, cortical tissue O2 and CO2 were observed. The postoperative angiogram showed that the by-pass was patent and all branches of the left internal carotid artery were filled via the by-pass. The postoperative course was uneventful and the progressive symptoms improved except for visual disturbance on the left. She is in good condition one year after the operation. The usefulness of this surgical procedure with monitoring of cerebral blood flow and metabolism during operation is also discussed.

Hormonal status and mycoplasma colonization in the female genital tract.

Three hundred seventy-four cervical or vaginal specimens from patients with various hormonal states were cultured for Ureaplasma urealyticum and Mycoplasma hominis. Significantly, low recovery rates of U urealyticum were obtained in the prepuberty (5%), puerperium (24%), and postmenopause (25%) groups, whereas pregnant women showed the highest incidence of U urealyticum (82%). The recovery rate of U urealyticum from neonates was 42%. The incidence of U urealyticum in sexually inactive females was significantly lower (40%) than that in sexually active nonpregnant women (67%). Significantly, a higher incidence (36%) was observed in postmenopausal women with a uterine cervix than in those whose cervix had been removed by surgery (10%). It is suggested that there is a close relationship between hormonal status and the occurrence of genital mycoplasmas. Possible modes of transmission of genital mycoplasmas other than sexual contact are also discussed.

[Efficacy of extrapleural internal mammary node dissection in breast cancer].

The results of extended radical mastectomy for breast cancer in 180 randomized patients treated at this department between 1965 and 1984 are reported. About 18% of the women with breast cancer had internal mammary lymph node metastasis. The incidence of metastasis in internal mammary lymph nodes increased from 9.5% when the tumor was locate in the outer half to 24.7% when it was in the center or inner half. The increase in the number of axillary lymph node metastasis was correlated with the increase in the incidence of internal mammary lymph node metastasis. The five-year survival rate of patients with no internal mammary lymph node involvement was better than that of involvement (81% versus 47%). But when the internal mammary nodes alone were involved without axillary lymph node metastasis, 83% of the patients survived for five years.
