[Cytogenetic response as a marker of efficacy of chronic myeloid leukemia therapy with a BCR-ABL thyrosine kinase inhibitor glivek].

AIM: Clinical practice with the drug glivek (imatinibe mesilate, ST1571) blocking activity of oncoprotein p210 shows that a cytogenetic response can be reached in 50-60% of patients with chronic myeloid leukemia (CML), in a late chronic phase (CP) in resistance to or intolerance of interferon alpha (IF-alpha) and in 24-43% of patients in the acceleration phase (AP). This study aimed at assessment of the rate and stability of a cytogenetic response (CR) and long-term results of survival in CML patients on glivek. MATERIAL AND METHODS: Glivek was given to 195 CML patients (median of the treatment duration was 42 months, 1-156 months, of the patients' age--46 years). 79 patients were in CP, 116--in AP. The doses were 400 mg/day and 116 mg/day, respectively. Karyotype was studied before the treatment and later after each 6 months. RESULTS: A considerable CR was achieved in 57% patients in CP and 44%--in AP. Of them complete CR was obtained in 48 and 35%, respectively. Marked CR is a favourable prognostic factor. Survival of patients with marked CR in CP (97% 0 and AP (89%) was significantly higher than without CR (58 and 47%, respectively, p < 0.05). Marked CR persisted in 95% cases in both phases of CML. In complete CR, a repeated study of karyotype revealed residual number of Ph+ cells both in CP and AP in 86% patients. This demonstrates necessity to take glivek continuously in achievement of a complete CR by karyotypic test. Glivek inhibits the disease progression, lowers annual lethality. 42-month (median of glivek treatment duration) overall survival reached 91 and 59% in CP and AP, respectively. CONCLUSION: CR is an integral index prognosticating CML course. Survival rose significantly in patients with marked CR both in CP and AP of CML. Marked CR is persistent in continuous glivek therapy. The rate of a CR depends much on the disease stage.

[Relationship of different risk factors of ischemic heart disease].

The spread of ischemic heart disease (IHD) risk factors, such as obesity, smoking, inactivity is discussed. Their relationship with the values of the serum lipid spectrum was examined. A wide spread of IHD risk factors and a low awareness of their implication were found in the population. The former can be reduced by questionnaire survey, interpretation of the presence of individual risk factors, and evaluation of the total risk of IHD. Sanitary and educational work on the pathogenesis of IHD is required for its successful prevention.

[Effects of nutrition characteristics on blood lipids in patients with dyslipoproteinemia].

Prevalence of risk of ischemic heart disease, such as nutrition and dyslipoproteinaemias are discussed. The interrelation of these risk factors with blood lipids was examined. It was found that risk factors are widespread, and population has low knowledge about their significance. Food, contained surplus of cholesterol, saturated fats, carbohydrates and deficiency of polyunsaturated fat acids increased atherogenic blood lipids and decreased antiaterogenic lipoproteins.

Transcription factors NF-kappaB and AP-1/c-fos in cell response to nocodazole.

The effects of nocodazole (an agent causing degradation of microtubules in vitro) on the content of transcription factors NF-kappaB and AP-1/c-fos in the nuclei of cultured cells CHO-K1 and HeLa, as well as on the activity of these factors in the nuclei of T-cell hybridoma 2B4 were studied. Immunoblotting, immunofluorescence, and gel retardation techniques revealed that in all cell lines studied nocodazole induced rapid and considerable activation (an increased content in the nuclei) of transcription factor AP-1/c-fos. In 2B4 cells, nocodazole had no effect on the activity of NF-kappaB, but in CHO-K1 and HeLa cells it caused a transient decrease, sometimes followed by an increase in the content of subunit p50. Besides, nocodazole inhibited proteolytic degradation of NF-kappaB in CHO-K1 and HeLa cells and caused a short-term decrease in the content of subunit p65 in CHO-K1 cells. The results suggest that nocodazole may interfere with the genome expression, and these alterations should be taken into consideration, whatever experimental studies this substance is used in.

[Polarization microscopy of bile for diagnosis of microcholedocholithiasis].

Comparative analysis of the results of microscopic examination of ductal bile was carried out in 2 groups of patients with clinical manifestations of the disease of extrahepatic bile ducts (choledocholithiasis, mechanical jaundice, cholangitis). In group 1 (17 patients) choledocholithiasis was diagnosed on the basis of ultrasound examination (USE) and direct contrast examination of the bile ducts at retrograde pancreatocholangiography (RPCG). In group 2 (17 patients) USE and RPCG proved to be uninformative and only polarizable microscopy of ductal bile taken by the method of direct endoscopical catheterization of the major duodenal papillae was able to reveal microcholedocholithiasis. It served an indication for endoscopical papillosphincterotomy performance. Thus polarizable microscopy of ductal bile is a reliable method of diagnosis of microcholedocholithiasis, which makes it possible to establish indications for correction of the major duodenal papilla.

Rigidity of human alpha-fetoprotein tertiary structure is under ligand control.

Comparative study of the natural ligand effect on structural properties and conformational stability of human alpha-fetoprotein (AFP) and its homologue, human serum albumin (HSA), was performed using several approaches, including circular dichroism, fluorescence spectroscopy, and scanning microcalorimetry. Here we show that denaturation of AFP, induced by the increase of temperature or urea concentration, is irreversible. We have established the fact that this irreversibility is caused by ligand release from the AFP molecule. Interestingly, the ligand-free form of AFP has no rigid tertiary structure but exhibits substantial secondary structure and high compactness. This means that the rigid tertiary structure of AFP is controlled by interaction with ligands, while their release results in transition of a protein molecule into a molten globule-like intermediate. In contrast, processes of HSA denaturation and unfolding are completely reversible. Release of ligands from HSA results only in a small decrease in stability but not transformation into the molten globule state.

Ligand-free form of human alpha-fetoprotein: evidence for the molten globule state.

By means of circular dichroism and fluorescence spectroscopy, viscometry and scanning microcalorimetry we have shown that the release of ligands from human alpha-fetoprotein (AFP) results in a considerable rearrangement of the protein molecule. Ligand-free form is practically as compact as the native molecule and has native-like content of secondary structure but no rigid tertiary structure. This means that the release of ligands transforms the AFP molecule into a molten globule state. Stripping the ligands from AFP is the irreversible process, i.e., native protein molecule cannot be reconstituted from the ligand-free form of AFP by adding back ligands. A possible functional role of such a structural transformation is discussed.

Solid-phase extraction on Styrosorb cartridges as a sample pretreatment method in the stereoselective analysis of propranolol in human serum.

Sample pretreatment using solid-phase extraction (SPE) on cartridges filled with small-particle Styrosorb porous polystyrene-based sorbent has been used in the analysis of propranolol enantiomers in human serum by high-performance liquid chromatography (HPLC) with fluorescent detection. SPE on Sep-Pak C18 cartridges was used as a reference pretreatment method. The propranolol content of the samples was determined by achiral normal-phase HPLC and the enantiomeric ratio of propranolol (S/R) was then determined by chiral HPLC on a column with silica-bonded cellulose-tris(3,5-dimethylphenyl carbamate). Recoveries of propranolol from serum using SPE on Styrosorb and C18 phases were 97 +/- 5% and 96 +/- 5%, respectively. Detection and quantification limits for propranolol enantiomers were 4 and 7 ng/ml, respectively.

[Failures, errors and complications of laparoscopic cholecystectomy].

Failures and complications are analysed from experience in 325 operations of laparoscopic cholecystectomy. Change to laparotomy was undertaken in 5 (1.5%) patients. The total amount of complications was 45.2%, among them 36.5% were intraoperative and 8.7% postoperative, but only 3.8% were hazardous for the patients. Mortality was 0.3% (one patient dies from cardiovascular failure). A classification of complications and the technical principles of the operation are discussed.

[Fat formula for human milk substitutes].

Chemical composition and technology of preparation of fat part of breast milk substitutes closed in lipid composition of prototype (in PUFA, phospholipid and cholesterol contents) are presented. It was shown that chosen combination of fats with antioxidant vitamins provides effective defense of food against spoiling during storage.

[Biologic activity and tolerance of a preparation of polyunsaturated fatty acids, obtained by a microbiological route ["biopolyene"]].

Biopolyene is a mixture of ethyl ethers of polyunsaturated fatty acids isolated from biomass of Entomophthora virulenta, a mycelial fungus. Its acute and chronic toxicity was studied on rats and guinea pigs. After oral administration of the preparation in single doses exceeding 50 g/kg there were no disorders in the general state of the rats. In chronic experiments with oral biopolyene in doses of 100 and 500 mg/kg and its local application to the intact skin of the animals in a dose of 1 g/kg there were no significant changes in the functional state of the liver and kidneys as well as the peripheral blood count. Insignificant changes in the serum levels of liver enzymes and coagulation were transient. The preparation showed no allergenic or immunomodulating effects. It had neither embryotoxic, teratogenic nor mutagenic action.

[Therapeutic tactics in pulmonary hemorrhage].

An experience with treatment of 57 patients with pulmonary bleeding (12 of them being operated upon) has shown advantages of complex therapy with controlled hypotony and filling the bronchi. Resection of the lung is performed after arrest or reduction of bleeding. Such tactics has advantages over emergency operations fulfilled "at the height of bleeding".

[Experimental vaccinal prophylaxis of Pseudomonas aeruginosa burn sepsis].

After the injection of P. aeruginosa live culture under the burned skin of mice sepsis develops within the first 24 hours, finally leading to the death of the animals. The microorganisms can be isolated from the blood, liver, kidneys and mesenterial lymph nodes till day 3 and from the spleen till day 5. After the intraperitoneal injection of P. aeruginosa live culture into mice, sepsis also develops within 24 hours, and the culture can be isolated from the blood and parenchymatous organs till day 3. The LD50 of the culture is equal to 5.1 X 10(6) microbial cells when introduced intraperitoneally and to 30 microbial cells in experimental burn sepsis. Experimental burn sepsis clearly demonstrates the effectiveness of Pseudomonas acellular protein vaccine: its index of effectiveness exceeds 3,000.

[Characteristics of Pseudomonas aeruginosa pili and an elucidation of their relation to the virulence of the bacteria].

The electron microscopic study of 175 P. aeruginosa museum and newly isolated strains has been made and their virulence has been determined. In 32% of these strains the presence of pili has been established. However, among the museum strains those with pili have been found to constitute only 19.5%, while among newly isolated strains pili have been detected in 39.7% of cases (the difference is statistically significant at p less than 0.01). No correlation between the formation of pili and the virulence of P. aeruginosa has been established.

[Development of a data bank for control of antibiotic resistant causative agents of purulent-inflammatory diseases and complications].

To provide constant control of drug resistance in causative agents of surgical infections an automatized data bank based on computer SM-4 was filed at the All-Union Research Institute of Antibiotics. The bank includes information on 1500 objects described by 60 indices referring to the isolated pathogens. For every particular strain there are indicated data on the patients, characteristics of the pathogen biochemical profiles and sensitivity to various antibiotics (28 drugs). The data were obtained during identification of the cultures with automatized microbiological systems. The major functions of the data bank and standard information requests performed on its basis during solving particular epidemiological and clinical problems are described.
