A case of hypophosphataemic vitamin D-resistant rickets treated with initial massive doses of 1 alpha-hydroxy-vitamin D3 alone.

A 1 5/12-year-old girl with hypophosphataemic vitamin D-resistant rickets was treated with initial massive doses of 1 alpha-hydroxy-vitamin D3 (1 alpha-OH-D3) alone. A dramatic improvement of bone lesions and growth rate and normalization of the characteristic biochemical defects were noted within four months after the start of massive 1 alpha-OH-D3 therapy. The results of this study suggest that early therapy with massive doses of 1 alpha-OH-D3 alone improves the radiological findings and restores to normal the biochemical defects and the growth rate as early as possible. This regimen may improve motor strength and endurance and obviate the need for corrective orthopaedic procedures.

A case of immunodeficiency with decreased serum IgA levels and impaired polyclonal B cell differentiation in vitro.

We report a patient with low serum IgA levels and persistent pulmonary infection. In spite of the normal contents of serum IgG and IgM, the patient had a deficiency for plasma cell differentiation of all major classes of immunoglobulins (IgG, IgA and IgM). Cross culture systems between normal T or B cells and the patient's T or B cells showed a defect of both T and B cell function.

Modulation of fetal mouse liver cells cultured on a pigskin substrate.

Mouse fetal liver cells cultured on a pigskin epidermal substrate grew for 7 weeks. Different enzymes and proteins, i.e. gamma-glutamyl transferase (GGT), nonspecific esterase (NE), lactic dehydrogenase (LDH), alpha-fetoprotein (AFP), and albumin were studied histochemically and/or biochemically. The activity of GGT was high at the beginning of culture and then decreased rapidly. The activities of NE and LDH were high during the culture. Release into the media and localization of AFP suggested active synthesis during the early stages. AFP levels gradually decreased and could be demonstrated only in trace amounts after 3-4 weeks of culture. On the other hand, the production of albumin was weakly evident early and became more and more evident after the second week in culture. Hydrocortisone modulated AFP and albumin production. The effect of hydrocortisone was to prolong expression of AFP and to reduce expression of albumin. Electron microscopic observations showed that the cultures consisted of organelle-rich parenchymal cells associated with the pigskin basement membrane by pseudopod-like structures. These results indicate that fetal mouse parenchymal cells were cultured and modulated on a pigskin epidermal substrate.

Inhibition of phytohaemagglutinin-induced autorosette formation of human peripheral blood lymphocytes by RNA or protein synthesis inhibitor.

The induction of autorosette forming cells (ARFC) with phytohaemagglutinin (PHA) in human peripheral blood lymphocytes (PBL) was examined. After 24 h of incubation with PHA, the level of ARFC in PBL was markedly increased but subsequently decreased to about one-third of the peak level at 96 h of culture. When PBL were pre-treated with actinomycin D, or cultured with puromycin, the induction of ARFC was completely blocked. Pre-treatment of PBL with mitomycin C (MMC) had no effect on induction of ARFC, whereas DNA synthesis was completely blocked. These data indicate that the generation of autologous red blood cell receptors is a relatively early event in PHA activated PBL, and that it is independent of DNA synthesis but dependent on RNA and protein synthesis.

Effects of basement membrane matrix on the culture of fetal mouse hepatocytes.

The effects of laminin, fibronectin and type IV collagen, all of which are major matrix components of the basement membrane, upon fetal mouse hepatocyte culture were studied. Among these matrices, laminin showed the greatest effect on DNA synthesis, cell proliferation, the secretion of alpha-fetoprotein and cell attachment. The effects of fibronectin and type IV collagen were slight with regard to the promotion of growth and cell attachment. However, there were no differences in the secretion of albumin into the media among the groups. These results suggest that laminin may be necessary for the cellular growth and functional maintenance of immature liver cells during normal development of the liver in vivo.

[Immunohistological studies on the identification of T cells and T cell subsets infiltrating in human stomach cancer tissues].

T cells and T cell subsets in human gastric cancer tissues were identified using the immunoperoxidase technique with anti-T cell monoclonal antibodies (Leu-1, Leu-2, Leu-3 and Leu-4), anti-HLA-DR(Ia) monocronal antibody, and anti-Ig sera. The majority of lymphocytes infiltrating into gastric cancer tissues were Leu-1 and Leu-4 positive T cells; they consisted of a Leu-2 positive T cell subset (Tc/s) and a Leu-3 positive T cell subset (Th/i). Leu-2 positive T cells infiltrated the gastric cancer tissues of 6 patients with a moderate or marked degree of lymphocyte infiltration; Leu-3 positive T cells infiltrated these tissues of 11 patients. These results show that the degree of T cell and T cell subset infiltration may effect the host-tumor relationship.

Induction of human T-lymphocyte colonies by phorbol myristate acetate.

Colony growth of human lymphocytes by phorbol myristate acetate (PMA) was studied. PMA was able to induce lymphocyte colony growth in methylcellulose semi-solid cultures in the absence of lectin mitogens, phytohaemagglutinin or concanavalin A. PMA-induced colonies were found in cultures of mononuclear cells, monocyte-depleted mononuclear cells and the T-enriched cell fraction, whereas no colonies were obtained from the non-T cell fraction. At least one million mononuclear cells were required to form colonies by PMA. The colony cells were mainly T cells as judged from sheep red blood cell rosette formation. Surface immunoglobulin positive cells and peroxidase positive cells were not detected in colony cells. Non-specific esterase positive cells were only found to be less than 1% of colony cells. T cells formed more colonies than did mononuclear cells, presumably because of a concentration of colony forming cells and/or co-operating cells. Colony formation by PMA was induced from monocyte-depleted mononuclear cells and monocyte-depleted T cells, suggesting independence of monocytes. When mononuclear cells were precultured at 37 degrees for 5 min or 30 min with PMA and then cultured in the semi-solid medium without PMA, only a small number of colonies grew. Further studies of PMA-induced T-cell colonies will provide information the identification and characterization of immunological states in various immunological diseases.

[Relationship between T cell infiltration in breast cancer tissues and prognosis].

The relationship between the degree of T cell infiltration in cancer tissues and prognosis was studied in 26 patients with breast cancer. The degree of T cell infiltration was divided into 4 groups according to the number of infiltrating T cells. There was a good correlation between the degree of T cell infiltration and the survival period or disease-free period. It may be clinically useful to check the degree of T cell infiltration in cancer tissues for predicting the prognosis of patients with breast cancer.
