Relationship between idiopathic peripheral facial paralysis and climate.

We attempted to analyse a relationship between idiopathic peripheral facial paralysis and climate. One hundred and thirty-seven patients, 76 female and 61 male, were studied. The occurrence was not correlated with the mean temperature, mean humidity or mean atmospheric pressure. It can be assumed that unfavorable weather conditions, i.e. a wide temperature range, a lower percentage of sunshine and cloudy days, may bring on idiopathic peripheral facial paralysis.

Surgical techniques of the parotid tumor.

101 cases with parotid tumors, including fourteen recurrent ones, were operated from 1976 to 1986. Surgical techniques of the recurrent cases were more complicated and difficult and often required plastic and reconstructive procedures for the skin, tissue and nerve defects. From the experiences mainly of the recurrent tumors, it was concluded that uniformly applied superficial or total parotidectomy was not always necessary but modified parotidectomy resecting sufficient and adequate tissues around the tumor should be planned individually from the prudent preoperative examinations.

[Surgery of malignant parotid tumors].

Surgical treatment of malignant parotid tumors was described from the viewpoints of radical cure and postoperative function. The function of the parotid gland is originally the secretion of saliva, but this is generally compensated for by the remaining major and minor salivary glands. Therefore, postoperative dysfunction involves possible facial palsy, deformity of the face and Frey's syndrome due to extirpated gland parenchyma and surrounding tissues. For these reasons, preservation of the facial nerve is required and resection of the gland should be as limited as the condition demands. If these procedures are impossible, plastic surgery such as nerve grafting and skin or musculocutaneous flaps is utilized. Postoperative irradiation is not applied except for unusual cases. Because malignant tumors of the parotid gland are histologically and biologically very different, preoperative examination is important for select the subsequent treatment. Superficial parotidectomy for a tumor in the superficial lobe and total parotidectomy for one in the deep lobe is the general concept adopted today. However, uniform application of this concept does not seem to simultaneously fulfil both purposes. In order to satisfy radical and functional results, delicately applied surgical techniques applied to individual cases are recommended according to the findings of appropriate preoperative and intraoperative examinations.

[Left ventricular isovolumic relaxation time estimated by the aortic root echocardiogram].

Dual echocardiogram was recorded simultaneously with phonocardiogram (PCG) to analyze the isovolumic relaxation time (IRT) of the left ventricle (LV) in 85 patients with various heart diseases and in 23 normal subjects. The measurements used were time intervals from the onset of the aortic component of the second heart sound (IIA) to the onset of the posterior deflection of the posterior aortic wall in the aortic root echocardiogram (X) (IIA-X interval), and to the onset of the most rapid opening motion of the anterior mitral leaflet (D') (IIA-D' interval) during early diastole. 1. The IIA-X interval was directly proportional to the IIA-D' interval in the entire study population. 2. The IIA-X interval was prolonged with advancing age in normal subjects. 3. The IIA-X interval was significantly increased in patients with hypertensive heart disease, old myocardial infarction, hypertrophic cardiomyopathy, and dilated cardiomyopathy, but significantly decreased in patients with mitral stenosis. Thus, the IIA-X interval, which was measured easily and noninvasively from the aortic root echocardiogram is a reliable indicator of the isovolumic relaxation time of the left ventricle, as well as of the IIA-D' interval.

Postural deviation evoked by caloric stimulation during upright standing in man.

To understand a disequilibrium of the peripheral vestibular disorders, we tried to analyze the body sway evoked by unilateral or bilateral caloric stimulation with warm and cold water systematically. As for postural deviation, the results of our study are summarized as follows. Unilateral warm stimulation evokes heterolateral and anterior postural deviation. Unilateral cold stimulation evokes homolateral and anterior postural deviation. Bilateral warm stimulation evokes posterior postural deviation. Bilateral cold stimulation evokes anterior postural deviation. Therefore we consider that there are several patterns of static disequilibrium of upright posture in the peripheral vestibular disorders.

Nasal resistance measured by anterior rhinomanometry.

The technique and principle of the active anterior rhinomanometry were justified by human examination and model experiment. The resistance values obtained from anterior method did not always agree with those from posterior one. Causes of the disagreement were considered to be distortion of the nostrils, air leakage from the apparatus and resistive component in the nasopharynx. These possibilities were demonstrated and analyzed in model experiments. The anterior rhinomanometry was concluded as a precise and convenient method to assess the conductivity of the nasal cavities.

Dynamics of mast cell degranulation in human allergic nasal epithelium after provocation with allergen.

The histamine content in the nasal epithelial layer of twenty-five patients with nasal allergy was measured before, 10 min after and 1 hr after nasal provocation with allergen. A decrease in histamine content was observed 10 min after provocation compared to the values obtained before provocation (P less than 0.05). There was a tendency for an increase in the histamine content of the nasal epithelium one hour after provocation when compared with the amounts present 10 min after provocation (P less than 0.1). Mast cells in the nasal epithelial layer of a further five patients were studied by electron microscopy 10 min and 1 hr after provocation. The rate of mast cell degranulation appeared to decrease 1 hr after provocation when compared with 10 min. Our study suggests that some mast cells commence their migration to the nasal epithelial layer over a short time period and that they may play a role in the onset of the allergic nasal reaction in patients with allergic nasal symptoms.

Deposition of sprayed particles in the nasal cavity.

Therapeutic effects of topically applied steroids is closely related to the deposition of sprayed particles. To clarify deposition in the nasal cavity, model experiments were undertaken using the gas and powder sprays. Deposits on the moistened filter paper which covered the surface of a nose model were assessed by absorbance of the extracts. The influence of the shape and the effect of continuous suction from the posterior outlet were also studied. When the nasal cavity was kept straight without any abnormal protrusions, the gas spray deposited more than 90% of particles. The amount was significantly lowered in the model of a concave septum with hypertrophic turbinates. The powder spray deposited approximately 75% of the original dose. This amount was more than 90% of actually sprayed particles because some loss was inevitable in spraying the powder. In general, the shape of the cavity more affected the deposition of the gas spray than the powder spray. This difference was probably due to the spray angle and to the size and speed of the aerosol particles. It was concluded that the powder spray was preferable to the gas spray with regard to the deposition in the nasal cavity.

Mast cells in allergic nasal epithelium and lamina propria before and after provocation. An electron microscopic study.

A study of mast cells in subjects with nasal allergy was made by electron microscopy before and after provocation in three areas; the nasal epithelium, the subepithelial layer and the deep layer of the nasal lamina propria. The ration of degranulation of the mast cells in the nasal epithelium and subepithelial layer increased after provocation. The main features of exchanged granules of the mast cells in these areas were (1) swelling of lower electron density of the area enclosed by perigranular membrane, (2) lower electron density of the total area of granular substance with fibrillar and reticular changes, and (3) the disappearance of granular substance. It was, therefore, judged that the mast cells in the nasal epithelium and in the subepithelial layer play a more important part in the onset of a nasal allergic reaction than do those in the deep layer of the nasal lamina propria.
