Primary collision neoplasm of malignant melanoma and adenocarcinoma in the lung. A case report.

We report an extremely rare case of a primary collision neoplasm in the lung consisting of malignant melanoma and adenocarcinoma. A malignant melanoma was histologically diagnosed by a transbronchial lung biopsy of the lung nodule in a 61-year-old woman, in whom there was no demonstrable primary malignant melanoma found elsewhere. However, at autopsy the above two components of the pulmonary tumor were both histologically and immunohistochemically confirmed. There was little intermingling of the two tumors. The malignant melanoma was positive for monoclonal antibody specific for melanotic tumor (HMB45) and S-100 protein but was negative for epithelial membrane antigen (EMA), while it was the opposite in the adenocarcinoma. This case is, to our knowledge, the first reported collision tumor of the above two components.

Vimentin-positive gastric carcinomas with rhabdoid features. A clinicopathologic and immunohistochemical study.

We studied 71 patients with solid-type gastric adenocarcinoma selected from 5,437 surgically resected specimens during the period from 1975 to 1988; six had vimentin-positive adenocarcinomas, and five of these were advanced. One was at an early stage. All six tumors showed the same histologic features and had either a diffuse or alveolar arrangement, with tumor cells having either poor or no cohesiveness. Many tumor cells were round to polygonal, with eosinophilic or clear cytoplasm and large, eccentric vesicular nuclei, as seen in malignant rhabdoid tumors of the kidney. In all cases, the cytoplasm showed coexpression of vimentin and cytokeratin as revealed by double immunostaining. Four of the five cases with advanced carcinoma died of the disease 1 to 6 months after surgery. The cases with vimentin-positive tumors had significantly poorer prognoses than those with vimentin-negative tumors. We also studied adenocarcinomas of various histologic types randomly selected from our file (160 intestinal type and 69 diffuse type of Lauren) but failed to detect any vimentin positive ones. These results indicate that vimentin is expressed in some of the solid-type adenocarcinomas, which have a poor prognosis, and indicating that rhabdoid-like cells may be found in a variety of adenocarcinomas of the stomach.

Production of nerve growth factor by cultured vascular smooth muscle cells from spontaneously hypertensive and Wistar-Kyoto rats.

OBJECTIVE: Nerve growth factor (NGF) is a neurotrophic protein which acts on peripheral sympathetic nerves. Elevated NGF in vascular tissues of young spontaneously hypertensive rats (SHR) has been reported. The aim of the present study was to compare the amount of NGF secreted from cultured vascular smooth muscle cells (VSMC) and mesenteric artery and thoracic aorta segments from SHR and Wistar-Kyoto (WKY) rats. METHODS: VSMC prepared by the enzyme digestion method from the thoracic aortic media of 14-week-old SHR and age-matched WKY rats were subcultured in Dulbecco's modified Eagle's medium containing 10% fetal calf serum. Segments of mesenteric artery and thoracic aorta from 4-week-old SHR and age-matched WKY rats were similarly cultured. The NGF content in conditioned medium was measured using an enzyme immunoassay. The protein content of VSMC was measured by the Lowry method. RESULTS: Total NGF content in the cell culture medium was increased during an exponential growth phase and then gradually decreased during a quiescent phase in both rat strains. There were no significant differences in the levels of NGF secreted from mesenteric artery and thoracic aorta segments between the SHR and WKY rats. The differences in cellular protein content between SHR and WKY rats were very small. CONCLUSIONS: In contrast to the reports of increased NGF in SHR tissues, our data demonstrate that NGF secretion was lower in VSMC from SHR, and was equivalent in mesenteric artery and thoracic aorta segments from SHR and WKY rats. We have no clear explanation for these observations, but the present results indicate that upregulation of NGF in SHR tissues is not responsible for a simple enhancement of NGF synthesis in VSMC, and suggest a breakdown of the regulatory mechanism or mechanisms of NGF gene expression in SHR tissues.

Colonic submucosal tumors: a new classification based on radiologic characteristics.

OBJECTIVE: Despite advances in gastrointestinal radiologic techniques, findings from barium enema studies cannot be used reliably to predict the histologic nature of a colonic submucosal tumor. Therefore, we generated a new and comprehensive classification system for all colonic submucosal tumors based on a correlation between their radiologic characteristics and their histopathologic features. MATERIALS AND METHODS: Between 1978 and 1990, 89 patients had 95 colonic submucosal tumors diagnosed on the basis of barium enema studies and confirmed by biopsy or surgery. Of these, 39 patients (44%) were symptomatic and 50 patients (56%) were asymptomatic. Single lesions (82 cases) and multiple lesions (12 cases of lymphoma and one case of blue rubber bleb nevus) were observed. The 95 lesions included 21 carcinoids, 27 malignant lymphomas, 17 lipomas, 10 lymphangiomas, six leiomyosarcomas, five leiomyomas, and nine others. We analyzed the radiologic findings and classified these lesions according to their appearance. RESULTS: All tumors were classified on the basis of their morphologic appearance into five types: (1) wide-based sessile lesion with gradually sloping margin and smooth surface (17 cases); (2) wide-based sessile lesion, more polypoid than the first type, with a smooth surface, with or without a central depression (49 cases); (3) wide-based sessile lesion with lobulated surface (six cases); (4) pedunculated lesion with smooth or granular surface (17 cases); (5) unclassified, not any of the previously mentioned types, may be diffusely stenotic or aneurysmal (six cases). The most common finding was the wide-based sessile lesion with a smooth surface and a possible central depression; next most common was the wide-based sessile lesion with gradually sloping margin and smooth surface; next was the pedunculated lesion with a smooth or granular surface. Eighty-six percent of the lesions were single; 14% were multiple. Histologic confirmation revealed mainly lymphoma (29%) and carcinoid (22%), of which the most common finding was a wide-based sessile lesion with a smooth surface, with or without a central depression. CONCLUSION: This new classification based on radiologic characteristics covers all varieties of colonic submucosal tumors. It can serve as a guideline for predicting a specific diagnosis of a colonic submucosal tumor on the basis of radiologic findings.

Identification of rat brainstem sites with neuronal Fos protein induced by acoustic stimulation with pure tones.

Induction of Fos, a proto-oncogene c-fos protein product, was immunohistochemically examined in the rat brainstem by using pure tone stimuli (0.25 kHz, 4kHz and 8 kHz) at 60 and 80 dB sound pressure levels for 30 min. In comparison with non-stimulated control rats, the brainstem of sound-stimulated rats exhibited distinct Fos-labeled neuronal cell nuclei in i) the ascending auditory nuclei, ii) the brainstem sites which receive afferents from the auditory nuclei, i.e. the pretectum and rostroventrolateral reticular nucleus, iii) the parvocellular part of the ventral lateral geniculate nucleus, lateral reticular nucleus, ventral border region of the spinal trigeminal nucleus, and the medial vestibular and spinal vestibular nuclei, all of which lack established auditory projections, and iv) the brainstem sites where some Fos-labeled neurons were present in controls. Sound stimulation at 80 dB led to widespread Fos-labeling in all these sites. Sound stimulation at 60 dB caused a decrease in number of positive cells in the brainstem sites and yielded a tone-dependent subregional distribution pattern in the dorsal cochlear nucleus and central nucleus of the inferior colliculus.

[Extended tricuspid valve excision for active infective endocarditis associated with ventricular septal defect].

We report a patient of ventricular septal defect associated with a vegetations on the tricuspid valve and a perforation of the aortic cusp caused by infective endocarditis. Antibiotics could not relieve the high fever of this patient, and disseminated intravascular coagulation caused by sepsis and two-sided heart failure developed. She underwent extended tricuspid valve excision, aortic valve replacement and VSD closure during the active stage. Post-operative antibiotic therapy was continued intravenously for 6 weeks, and orally for 3 more weeks. Although post-operative right ventriculography indicated tricuspid regurgitation and right atrium enlargement, pressure study of the right side of the heart revealed normal hemodynamics.

[Endoscopic elecrocautery of the thoracic sympathetic chain: a minimal invasive surgical treatment].

The thoracic sympathectomy was performed with standard laparoscopic and urologic equipment. After intubation, the patient was placed in a half sitting position. A Surgineedle was introduced into the pleural cavity through a 1 cm incision at the intercostal space between the first and second rib. An apical pneumothorax was created by insufflation of two litres of CO2. A 24 ch. urological trocar was inserted through the same incision, then the electroresectoscope was introduced into the pleural cavity. The sympathetic chain could be observed through parietal pleura riding on the costovertebral junctions. These areas were electrocoagulated. The lung was expanded by limiting expiration and sucking CO2. A postoperative chest X-ray was done to check no pneumothorax. This method was a simple and safe surgery, which was available for bilateral sympathectomy.

[Immunohistochemical investigation on intraneural localization of herpes simplex virus type 1 in experimental retinitis. 1. Observation of the retina and optic nerve on the injected side].

In order to examine transfer of herpes simplex virus type 1 (HSV-1) in the retina and optic nerve in rats with experimental retinitis, an HSV-1 suspension (Miyama strain, 1 x 10(7) plaque-forming units per ml, 10 microliters) was inoculated into the vitreous body. One to seven days after HSV-1 inoculation, the animals were perfused through the heart with Zamboni's fixative. Fifty-microns-thick cryostat-cut sections through the retina and optic nerve were immunostained for HSV-1 antigen with rabbit anti-HSV-1 antiserum and avidin-biotin-peroxidase complex. Some sections were counterstained with hematoxylin. At an early stage of the postinoculation period, HSV-1 immunoreactivity was first seen in the cytoplasm of Müller cells and then in various cells of the inner nuclear layer. HSV-1 immunoreactivity was extremely dense in the nuclei of these cells, suggesting that HSV-1 replicates in the nuclei and propagates through the cytoplasm to the membrane of cell processes. At a later stage of the postinoculation period, HSV-1 immunoreactivity was seen in some retinal ganglion cells and optic nerve fibers. The labeling of the optic nerve fibers was found even in the optic tract and often accompanied by labeled glial cells adjacent to the nerve fibers.

Pathology and prognosis of gastric carcinoma. Findings in 10,000 patients who underwent primary gastrectomy.

BACKGROUND: Despite recent advances in diagnosis and treatment, gastric carcinoma remains a major cause of death in the world. METHODS: The clinicopathologic profile of 10,000 consecutive patients who underwent primary gastrectomy during 1962-1989 were reviewed and prognostic factors influencing survival in those with gastric carcinoma were analyzed in 7031 patients. RESULTS: Incidence of gastrectomy for carcinoma has increased steadily and the rate of early carcinoma exceeded that of advanced carcinoma in the recent period of 1985-1989. Five-year and 10-year survival rates were 46.1% and 35.2% in 3868 patients with advanced carcinoma, and 88.8% and 77.3% in 3163 patients with early carcinoma, respectively. In patients with advanced carcinoma, significantly poorer survival rates were noticed for patients older than 70 years of age, those who underwent total gastrectomy, tumors involving the entire stomach or greater than 10 cm in diameter, a macroscopic diffusely infiltrative pattern, adenosquamous histologic type, positive surgical resection margins, or lymph node metastasis. None of the above poor prognostic features were identified in patients with early gastric carcinoma group except for those older than 70 years of age. Although lymph node metastases were present in 10% of early gastric carcinomas, this feature did not impart a poor prognosis. Patients with advanced carcinoma grossly resembling an early carcinoma had an intermediate prognosis, suggesting the existence of a developmentally midstage lesion between early and advanced carcinoma. CONCLUSIONS: The study illustrates that the most important role for clinicians treating with gastric carcinoma should be early detection and aggressive surgery for resectable tumors, followed by detailed pathologic examination.

Mapping of the cochlear nucleus subregions in the rat with neuronal Fos protein induced by acoustic stimulation with low tones.

To explore the mode of appearance of Fos, a gene product of proto-oncogene c-fos, in the cochlear nucleus, a pure tone was given to rats and Fos was detected by immunohistochemistry. Following low-tone stimuli applied with moderate ranges of sound intensity, neurons with Fos-label occurred band-like in the ventral subregions of the dorsal cochlear nucleus. Organization of low-to-high tones in the ventral-to-dorsal orientation became visible at the caudal level. Several other subnuclei consistently exhibited Fos with no topographic pattern. Neurons interspersed in the adjoining vestibulocochlear nerve were also found to elicit Fos. Low-tone stimuli with very high sound intensity led to a rather widespread distribution of Fos, making the tone-specific distribution less visible.

A clinicopathologic and immunohistochemical study of gastrointestinal stromal tumors.

The clinicopathologic and immunohistochemical features in 120 cases of gastrointestinal stromal tumor (GIST) were reviewed. Excluding 24 cases of gastric schwannoma, 96 cases of GIST consisting of 62 benign tumors and 34 sarcoma (low grade, 17; high grade, 17), with 9 cases arising in the esophagus, 57 in the stomach, 28 in the small intestine, and 2 in the colon, were studied. All esophagus and colon tumors were benign and resembled a conventional leiomyoma histologically. However, the gastric and small intestine benign tumors mostly showed histologic features of cellular or epithelioid leiomyoma. Immunohistochemically, desmin caused a positive reaction in all esophagus and colon tumors, but only 26% of gastric and small intestine tumors. However, muscle-specific actin (HHF35) caused a positive reaction in most GIST (92%). The 10-year survival rates of the patients with gastric sarcoma and those with intestinal sarcoma were 74% and 17%, respectively. These results showed that histologic and immunohistochemical features were distinctly different, depending on the location in the gastrointestinal tract; that most GIST, excluding schwannoma, had smooth muscle differentiation; and that sarcomas had a more favorable prognosis when they occurred in the stomach rather than the intestine.

Poorly differentiated solid type adenocarcinomas in the stomach: a clinicopathologic study of 71 cases.

A review of 71 cases of solid carcinoma of the stomach selected from 2,738 cases of surgically resected poorly differentiated adenocarcinomas (PDAs) was undertaken. The average age of the patients, which included 47 men and 24 women, was 62 years. The clinicopathologic features of solid carcinomas were similar to those of the differentiated type adenocarcinomas. The solid carcinomas were divided into 42 of the solid alveolar type and 29 of the free-cell type; 32 of the former (76%) had foci of overt differentiated areas, while most of the latter type lacked such foci and had some signet-ring cells. Many tumors of both types had a prominent venous permeation and lymph node metastasis even in the early stage of invasion. The 5-year survival rates were 37% in all cases, 44% in cases with carcinoma with a limited invasion of the submucosa or the muscularis propria, and 27% in carcinoma invading down to the subserosa. These survival rates were poor in comparison with the 110 cases of ordinary PDA (diffuse type of Laurén) selected as controls. These findings suggest that tumors of the solid alveolar type are transformed from the differentiated type of adenocarcinoma, while those of the free-cell type are originally from PDAs. It is also suggested that solid carcinomas did show a high incidence of venous permeation and lymph node metastasis and that the prognosis for of venous permeation and lymph node metastasis and that the prognosis for this type of carcinoma is poor.

Distribution and cerebellar projections of cholinergic and corticotropin-releasing factor-containing neurons in the caudal vestibular nuclear complex and adjacent brainstem structures.

By using immunohistochemistry combined with lesioning and retrograde neuronal labeling techniques, cholinergic neurons and corticotropin-releasing factor-immunoreactive neurons were examined for their distribution, coincidence and cerebellar projections in feline vestibular nuclear complex and adjacent brainstem structures. Cholinergic neurons as revealed here with choline acetyltransferase immunoreactivity were found massively in the abducens and hypoglossal nuclei, dorsal motor nucleus of the vagus nerve and nucleus of Roller; less numerously in the medial vestibular, prepositus hypoglossi and solitary nuclei and the caudal two-thirds of descending vestibular nucleus; and only occasionally in the intercalated and supravestibular nuclei and cell groups f, x and z. Corticotropin-releasing factor-immunoreactive neurons were found clustered in the prepositus hypoglossi nucleus and also in cell groups f and x and the rostral two-thirds of descending vestibular nucleus, less numerously in the medial vestibular, intercalated and solitary nuclei and nucleus of Roller, and only occasionally in the caudal one-third of descending vestibular nucleus, the dorsal motor nucleus of the vagus nerve, supravestibular nucleus and cell group z. The lateral and superior vestibular nuclei did not contain either type of neuron. The two types of immunopositive neurons observed in most of the brainstem nuclei differed in cell size, distribution-pattern and rostrocaudal level of occurrence. While there were many regions which exhibited both types of immunopositive neurons, perikarya colocalizing the cholinergic and peptide markers were not detected in the brainstem. Following unilateral, partial lesioning of the vestibular nuclear complex, corticotropin-releasing factor-immunoreactive mossy fiber terminals (rosettes) disappeared from the ipsilateral flocculus. However, such lesions did not produce clear-cut changes of cholinergic terminals in the vermis. Following retrograde neuronal labeling combined with immunohistochemistry, the two types of immunopositive neurons observed in most of the brainstem sites were found to project to the vermal lobules I-III, IX and X. On comparison of these immunopositive projection neurons with non-immunoreactive, retrogradely labeled neurons, the cholinergic neurons and the peptide-immunoreactive neurons were found to constitute a major part of the total vestibulocerebellar neuronal population. The results indicate chemical heterogeneity in vestibular nuclear complex and cerebellar afferents.

Signet-ring cell carcinoma of the stomach. A quantitative analysis on its submucosal invasion using a computer image analyzer.

As has been indicated, signet-ring cell carcinoma often shows a three-layered pattern of mucus-containing cells in the mucosa, reproducing an organized differentiation in the normal mucosa. In 110 lesions of early stage signet-ring cell carcinoma of the stomach, the proportion between the area with the three-layered pattern and the non-layered area without such layered pattern was measured using a computer image analyzer. As the result, a positive correlation could be demonstrated between the extent of the non-layered area and the tumor size expressed in the largest diameter (r = 0.2777, P < 0.01). The incidence of submucosal invasion significantly increased when the non-layered area exceeded 50% of the total area of signet-ring cell carcinoma. The sites of submucosal invasion were almost always limited to within the non-layered area bearing another positive correlation between the area of submucosal invasion and that of non-layered intramucosal pattern (r = 0.4427, P < 0.01). Thus, it is indicated that the incidence of submucosal invasion in early signet-ring cell carcinoma was significantly related to a dissolution of the three-layered pattern of carcinoma within the mucosa. In addition, the presence or absence of fibrous and lymphoid stromal reactions to the carcinoma was evaluated in relation to the incidence of submucosal invasion.

Increased nerve growth factor levels in spontaneously hypertensive rats.

OBJECTIVE: Increased sympathetic innervation has been reported in spontaneously hypertensive rats (SHR); however, the precise mechanisms involved are not yet clear. Nerve growth factor (NGF), a neurotrophic peptide in peripheral sympathetic neurons, is believed to contribute to this phenomenon. METHODS: We measured the content of NGF in SHR and control Wistar-Kyoto (WKY) rats during development. Mesenteric artery, spleen, heart and sciatic nerve were isolated and homogenized. NGF content in the supernatant fractions was measured using a highly sensitive and specific two-site enzyme immunoassay. RESULTS: At 3 weeks of age, SHR had a greater NGF content in the spleen, the sciatic nerve and the mesenteric artery than WKY rats. However, these differences disappeared completely at 12 weeks of age. Cardiac NGF content was slightly lower in 3-week-old SHR and, conversely, higher in 12-week-old SHR than in age-matched WKY rats. CONCLUSIONS: These findings suggest that, except for the heart, the SHR tissues observed overproduce NGF at a young age, leading to enhancement of peripheral sympathetic nervous system activity and the production of vasoconstrictive catecholamines.

Carcinoid tumor arising in the wall of a congenital bile duct cyst.

We report an extremely rare case of carcinoid tumor arising in a congenital choledochal cyst. The dilated choledochal cyst in a 66-year-old Japanese woman was surgically resected, after which the ovoid elevated tumor, measuring 1.5 x 1.5 cm, was detected in the proximal portion of the cyst. A carcinoid tumor was histologically confirmed. To the best of our knowledge, this is the first published case of carcinoid tumor in a choledochal cyst.

Radioprotective effects of serum thymic factor in mice.

Serum thymic factor (FTS) reduced mortality of mice after total-body irradiation with 7.56 Gy X rays. The radioprotective effect was achieved by daily repeated subcutaneous injections of 3-100 micrograms FTS, while doses higher than 300 micrograms/day/mouse were neither radioprotective nor toxic. Similarly, degeneration of the spleen was moderated by 3-100 micrograms FTS but not by 500 micrograms FTS in sublethally (3.78 Gy) irradiated mice. Histological examination showed that hematopoiesis was enhanced in the spleen by daily injections of 10 micrograms FTS. Spleen cells from the FTS-treated mice incorporated more [3H]thymidine in culture with or without concanavalin A. The treatment with FTS increased the production of colony-stimulating factor in the spleen as well as in peritoneal macrophage-like cells, and caused a significant increase in the number of granulocyte-macrophage colony-forming cells both in the spleen and in the femoral bone marrow. Furthermore, FTS prevented a decrease in circulating neutrophils in the sublethally irradiated mice. Prominent overshoot recovery of myelopoiesis, which occurred occasionally in sublethally irradiated mice, did not occur in the FTS-treated mice. The decrease in blood erythrocytes was also significantly reduced. These observations imply that this thymic hormone has potential as a radioprotector.

Coincident occurrence of granular cell tumor of the stomach with an early gastric carcinoma.

In a 41-yr-old Japanese woman, a rare coincident lesion composed of granular cell tumor and early gastric adenocarcinoma was detected in the stomach. Beneath the poorly differentiated adenocarcinoma almost limiting to the mucosa, the granular cell tumor measuring 1.2 x 0.8 cm was spreading mainly in the submucosa without forming polypoid nodule. To the best of our knowledge, this is the first published report on this type of coincidence. Immunohistochemically, the cells of the granular cell tumor were strongly positive for S-100 protein, but negative for muscle markers (HHF 35, desmin), some of which connected with perineural Schwann cells surrounding small vessels in the submucosa. These features suggested that granular cell tumor in our case arose from Schwann cells surrounding small vessels in the submucosa of the stomach.
