Increases in the maternal-fetal pH gradient that may occur during labor and delivery may increase the fetal concentration of local anesthetics. The authors evaluated effects of pH changes on the transplacental concentration equilibrium of bupivacaine. They increased the maternal-fetal pH gradient in each of six pregnant ewes from a control value of 0.15 to 0.54 by hyperventilating the lungs of the ewe and infusing lactic acid into her fetus. After infusion of bupivacaine, 0.15 mg/kg, intravenously into the mother, the drug rapidly appeared in fetal blood, with values significantly increased over control values at 1 and 5 min. The fetal/maternal (f/m) ratios were increased significantly at 5, 15, and 30 min. The f/m ratios had stabilized by 15 min in both control and experimental states, suggesting that equilibrium had been achieved. The consistently low f/m ratios are explained by the presumed similarity of the ovine maternal and fetal protein binding rates to those of man. It is concluded that the maternal and fetal pH values are major factors in the determination of the f/m ratios.

A modification of the spectrofluorometric propranolol procedure of Shand and associates and Ambler and colleagues is presented. A 3-ml volume of propranolol in serum is made basic with sodium hydroxide and extracted with 1.5% isoamyl alcohol in n-heptane. The drug is back-extracted into a mixture of 0.01 M citric acid in 50% ethylene glycol and measured spectrofluorometrically with the use of 299 nm for excitation and 352 nm for emission. Excellent linearity is observed in the 25--200 ng/ml range. The effects of sodium hydroxide, citric acid, and ethylene glycol concentration on the procedure were investigated. Ethylene glycol--citric acid in water is a better back-extracting mixture from the organic phase than hydrochloric acid. Using pentyl acetate as the extracting solvent instead of isoamyl alcohol in n-heptane did not change significantly the amount of the drug extracted. Other extracting solvents investigated did not increase sensitivity. At high citric acid concentrations a decrease in fluorescence intensity was observed at 350 nm. Interferences from other drugs using this procedure were investigated. Quinidine, methaqualone, and procainamide interfere at therapeutic levels.
A pregnant patient with a jejunoileal bypass developed hypokalemic paralysis and severe hypocalcemia and hypomagnesemia. She also had hypokalemic myopathy with elevation of muscle enzymes, which was corrected by potassium administration. This case emphasizes the severe metabolic complications that may be encountered during pregnancy following jejunoileal bypass.

A survey of faecal samples taken from 1000 suburban dogs revealed an infestation of 7-2 per cent Toxocara canis, 2-7 per cent Toxascaris leonina, 1-3 per cent Trichuris vulpis and 1-3 per cent Uncinaria stenocephala. The faecal material examined was obtained per rectum from well cared-for pet dogs resident in an area of north-west suburban London. The age of the dogs examined, both male and female, ranged from three weeks to 18 years. The clinical picture of the dogs affected by ascariasis and in particular Toxocara canis is described and some advice on treatment is included.

Ultraviolet spectrophotometry is the most commonly used technique for the determination of theophylline levels for therapeutic monitoring. Common interferences in most methods are barbiturates and xanthines. A modified method which eliminates interferences from barbiturates and most xanthines is presented. Theophylline is extracted from serum with chloroform/isopropanol at pH 7.4 back extracted into dilute hydrochloric acid and then the solution is made alkaline with sodium hydroxide. Barbiturate interferences are eliminated. Interference from caffeine, uric acid, 7-(2,3-dihydroxypropyl)theophylline, xanthine, and hypoxanthine are not observed. Theobromine, and the metabolite 3-methylxanthine interfere. Interference is not observed from quinidine, diazepam, salicylate, glutethimide, methylprylon, propranolol, methaqualone, dilatin and ethchlorvynol. Sulfanilamide, procainamide and chlordiazepoxide interfere.

Contact allergic sensitivity may follow the use of chlorocresol incorporated as a preservative into both betamethasone cream and aqueous cream, which are commonly prescribed as a mixture in South Australia. In several cases, although the clinical courses indicated sensitivity to chlorocresol, patch test reactions to this chemical were negative. In these cases positive patch test reactions to chloroxylenol, a chemical closely related to chlorocresol, confirmed the diagnoses.
Erb's palsy has been observed in 2 infants who had been treated with constant positive airways pressure for idiopathic respiratory distress syndrome. Both infants made complete neurological recovery from what was thought to be an acquired injury from the neck seal.
