Effect of cimetidine on the mucosal hydroxyproline content of rat gastric lesions induced by compound 48/80.

Extensive gastric mucosal lesions developed in rats after the administration of compound 48/80 at a dose of 0.75 mg/kg daily for 3 days. The hydroxyproline content of gastric mucosa was significantly increased 4 h after the last injection of compound 48/80, and subsequently decreased as the lesions healed. Treatment with cimetidine (CAS 51481-61-9) at a dose of 50 mg/kg (twice a day for 3 days) did not affect either the development or the healing of the compound 48/80-induced gastric lesions. However, the increase in mucosal hydroxyproline content was significantly reduced by treatment with cimetidine. This fact suggests that cimetidine might have an inhibitory effect on the regeneration of collagen in gastric mucosa.

Estimate of gastric acid output by evaluation of fasting gastric juice collected endoscopically.

Using an endoscope we collected fasting gastric juice for estimation of gastric acid in subjects examined by upper gastrointestinal endoscopy. Subjects were 53 in- and out-patients (46 males and 7 females) at our hospital. Seventy-five gastric secretory studies were performed in this population before and after stimulation by tetragastrin, following the guidelines of The Japanese Society of Gastroenterology and using a specially devised endoscope of narrow caliber under direct vision. Acid in fasting gastric juice was correlated with basal acid output (BAO) and with maximal acid output (MAO). There was a statistically significant correlation between them. Thus, an estimation of gastric acid secretion can be obtained from the fasting gastric juice which we usually aspirate and discard during the routine endoscopic examination.

[Evaluation of the past history of chilblain in cases of systemic lupus erythematosus (SLE) and its similar diseases].

We sent out a questionnaire to 47 patients of SLE and its similar diseases as to their past histories of chilblain. The results of the patients were compared to those of 141 cases of control. Although the percentage of cases who had revealed chilblain frequently (40.0%) and the age of chilblain onset (mean: 10.8 years old) in SLE group were not significantly different from those of control (28.4% and 12.4 years old, respectively), the chilblain which SLE patients developed had some characteristics compared to that of control, (1) higher incidence of chilblain episodes, (2) longer duration until cure, (3) more liability that chilblain leads to erosion or ulceration and (4) frequent occurrence of chilblain in the other seasons than winter. Especially SLE patients with the characteristic of (4) had higher association rates of Raynaud's phenomena, chilblain LE and livedo, suggesting disorder of peripheral circulation. It was also revealed that females are generally more liable to develop chilblain than males (females: 40.0%, males: 13.1%). Those results suggest some important relationship between chilblain and LE lesions. It is supposed that chilblain with the characteristics described above may possibly be transformed into LE lesions.

Lichen planus pemphigoides: case report and results of immunofluorescence and immunoelectron microscopic study.

A Japanese woman with lichen planus pemphigoides is reported. Immunologic characteristics of lichen planus pemphigoides antigen in the patient were investigated by indirect immunofluorescence and compared with those of bullous pemphigoid antigen or epidermolysis bullosa acquisita antigen. Ultrastructural localization of lichen planus pemphigoides antigen was studied with the use of immunoelectron microscopic techniques. Lichen planus pemphigoides antigen showed localization similar to that of bullous pemphigoid antigen but different from that of epidermolysis bullosa acquisita antigen. The antigenic stability of lichen planus pemphigoides antigen was different from that of bullous pemphigoid antigen or epidermolysis bullosa acquisita antigen. Thus this study demonstrates that lichen planus pemphigoides antigen is different from bullous pemphigoid antigen.

Photodynamic diagnosis in respiratory tract malignancy using an excimer dye laser system.

Equipment has been developed for the early-stage diagnosis and treatment of cancer using an excimer dye laser. The excimer laser beam is obtained by exciting XeCl. A 405 nm beam tuned by DPS dye is used for tumour localization and a 630 nm beam obtained with a rhodamine B dye is used for treatment. The equipment was applied clinically on the basis of extensive experimental research. Effectiveness for cancer localization was examined in 11 cases: four were early stage (three lung cancer and one vocal cord cancer), four were stage I, two were stage III and one was stage IV. All cases were squamous cell carcinoma except for one case of adenocarcinoma. Fluorescence was recognized in all lesions and the equipment was effective for localization.

Antiproliferative effect of hyperthermia and ACNU on cultured human malignant melanoma cells.

Human malignant melanoma cultured cells were treated either with ACNU (1-(4-amino-2-methyl-5-pyrimidinyl)methyl-3-(2-chlorethyl)-3-nitro sourea hydrochloride), hyperthermia, or the combination of ACNU and hyperthermia. The combination treatment inhibited the cell growth to a slightly synergistic degree compared to the respective single treatments. The present in vitro experimental results support in part the finding of our previous report that the combination treatment with ACNU and hyperthermia have a significantly synergistic antitumor effect to human melanoma transplanted to nude mice. However, the synergistic effect was much less intense in the present in vitro experiment. The difference may have resulted from the environmental differences between in vitro and in vivo experimental systems.

A case of neurofibrosarcoma associated with neurofibromatosis: light microscopic, ultrastructural, immunohistochemical and biochemical investigations.

A case of neurofibrosarcoma (NFS) with neurofibromatosis was studied by light microscopic, ultrastructural, immunohistochemical and biochemical methods. Histologically, spindle-shaped tumor cells with atypical hyperchromatic nuclei were arranged in a fascicular or sheet-like fashion. Electron microscopic examination revealed discontinuous basement membrane-like structures. Immunohistochemical study revealed S100 protein alpha chains in tumor cells. A biochemical analysis showed large amounts of gangliosides (sialic acid 13.5 micrograms/g wet tissue weight) in tumors. GM3 and GD3 were the major components; these results were compatible with the NFS ganglioside patterns found in our previous study. Results confirmed the diagnosis of NFS and indicate that NFS clearly has some neuroectodermal characteristics and that Schwann cells may possibly be related to its origin.

Effects of cimetidine and omeprazole on angiogenesis in granulation tissue of acetic acid-induced gastric ulcers in rats.

We investigated the effects of cimetidine and omeprazole on angiogenesis in granulation tissue and on the healing of gastric ulcers induced by acetic acid in rats. Either cimetidine (50 or 100 mg/kg) or omeprazole (10 or 20 mg/kg) was orally administered once daily for 9 consecutive days from the day following ulcer production. The ulcer index on the 10th and 30th days after ulcer production, and the extent of angiogenesis on the 10th day were examined. Cimetidine dose-dependently decreased the extent of angiogenesis on the 10th day, whereas the ulcer index on the 10th days was not significantly different between cimetidine-treated and control rats. The ulcer index of the groups treated with cimetidine during the initial 9-day period was increased compared with the control group on the 30th day. In contrast, oral omeprazole did not affect angiogenesis on the 10th day and decreased the ulcer index on both the 10th and 30th days. These results suggest that oral cimetidine may inhibit angiogenesis in ulcer granulation tissue possibly via the blocking of histamine H2 receptors and this may be one cause of delayed ulcer healing.

Ganglioside composition of human melanoma and response to antitumor treatment.

Ganglioside composition of human melanoma was correlated with sensitivity of melanoma to antitumor treatment with chemotherapeutic agents and radiation. The cytotoxic effect of each treatment was evaluated on 16 melanoma cell lines using the human tumor colony-forming assay. Ganglioside fractions were extracted and purified from each cell line and analyzed for the four major gangliosides in melanoma (GM3, GM2, GD3, and GD2) by thin-layer chromatography (TLC) and TLC scanner. GD2 content positively correlated with sensitivity to radiation (r = 0.753, p less than 0.001) and vincristine (r = 0.779, p less than 0.001). In contrast, GM3 content inversely correlated with sensitivity to radiation (r = -0.658, p less than 0.01) and vincristine (r = -0.692, less than 0.01). The gangliosides GD3 and GM2 were shown to have no significant correlation with any of these treatments.

[The clinical study on occurrence of TMJ dysfunction in orthodontic patients].

Occlusal disharmony is considered to be the most significant factor of temporomandibular joint (TMJ) dysfunction, but the relationship between malocclusion and TMJ dysfunction has been unknown yet. The purpose of present study was to investigate the relationship between TMJ dysfunction and occlusal disharmony (eccentric occlusal contacts and functional shift of the mandible) in orthodontic patients. The subjects were 533 orthodontic patients, 209 males and 324 females, who were received at the Department of Orthodontics, School of Dentistry, Hokkaido University from April 1985 to March 1988. Their ages ranged from 8 to 38 years old. All were prior to orthodontic treatment. The results were as follows 1. The prevalence of TMJ dysfunction was 27.6%, 24.9% in males and 29.3% in females. The difference was insignificant between males and females. 2. The patients with TMJ dysfunction increased after 13-15 years old, corresponding to dental age IV A (Hellman's classification). 3. In patients with TMJ dysfunction, the highest frequency of symptom of TMJ dysfunction was joint sounds (77.6%), the second was pain (22.4%), the third was disturbance of mandibular movement (4.8%). The complication of those symptoms were compounded in 26.3% of the total patients. 4. The prevalence of TMJ dysfunction had no relation to specific malocclusion and maxillofacial morphologic characteristic. 5. Most patients with TMJ dysfunction had occlusal disharmony (95.9%). 6. Eccentric occlusal contacts were found most frequently in patients with open bite and functional shift of the mandible was found most frequently in patients with deep bite. From these findings, the prevalence of TMJ dysfunction was frequent in orthodontic patients, however the symptoms were objective and the patients were not aware of TMJ dysfunction. TMJ dysfunction had no relation to specific malocclusion but some kinds of occlusal disharmony were easy to occur in specific malocclusion and TMJ dysfunction was induced by those etiological factors.

[Intra-arterial infusion of cisplatin and caffeine for a recurrent malignant fibrous histiocytoma].

We treated a 43-year-old man with recurrent malignant fibrous histiocytoma in right distal femur with intra-arterial infusion of cisplatin and caffeine 5 years after wide excision and chemotherapy. At the time of the first recurrence in the lateral aspect of thigh, intra-arterial infusion of CDDP (120 mg/m2) was ineffective. We treated him with radiation (7000 rad), and there was no evidence of tumor by radiological evaluation. The second local recurrence was treated with intra-arterial infusion of CDDP (120 mg/m2/1 hour) and caffeine (1.2 g/m2/24 hours x 3 days), and the tumor disappeared, radiologically and histologically. Caffeine did not increase the nephrotoxicity of CDDP, and no insomnia nor palpitation was seen. Intra-arterial infusion of CDDP and caffeine could be useful to increase the effect of CDDP for regional chemotherapy.

Systemic lupus erythematosus associated with antiribosomal P protein antibody.

Three patients with systemic lupus erythematosus had positive antiribosomal P protein antibodies. The patients are women who had mild diseases that did not involve prominent internal organs. By using an immunofluorescence method for the detection fo antinuclear antibodies that employs HEp-2 cells, these serum samples produced intense stainings on cytoplasm and nucleoli that were digested with RNase. With the use of an immunoblotting technique, these serum samples recognized three ribosomal P proteins, called PO (38 kd), P1 (19 kd), and P2 (17 kd). We suggest that antiribosomal P protein antibodies can be detected in patients with systemic lupus erythematosus and associated mild clinical features.

[Clinical features of male SLE patients--summary of our 22 cases].

We investigated clinical features of our 22 male SLE patients with main respect to 11 articles of ARA criteria for diagnosis of SLE (1982). As for skin lesions, 14 cases (64%) manifested wide spread discoid lupus erythematosus as their first symptoms, however, rather rare lesions could be seen as follows: 3 cases with nodular cutaneous lupus mucinosis, 1 with vesiculobullous LE. As to visceral manifestations, renal involvements could be seen in 11 cases (50%), among which 6 showed nephrotic syndromes. In immunological examinations, 1 case revealed C4A deficiency. The clinical tendency of male SLE cases has not been settled by now, for the disease is uncommon in men. Each of our 22 cases manifested a variety of features respectively, indicating that rather atypical cases as SLE can be more often seen in males than females. In addition, many of our cases showed remarkable changes clinically during their courses, which suggests that we should follow them carefully from now.

[Bone marrow necrosis in thymic T cell lymphoma].

A 29 year-old male patient was admitted because of exertional dyspnea, due to the pleural effusion on chest X-P on June 18 of 1988. On admission, enlargement of left supraclavicular and mediastinal lymph nodes were found. Cytological studies of pleural exudate cells showed the proliferation of atypical lymphoid cells with a few eosinophiles. Surface markers of the tumor cells were just positive for both CD5 and CD38, thus indicated that they resided as early thymocyte along T cell differentiation. Moreover, according to Southern blotting analysis, these DNA's showed rearrangement of TCR-beta genes. Based on these results, he was diagnosed to be suffered from thymic T cell lymphoma. Bone marrow biopsy on left posterior iliac bone showed tumor necrosis owing to extensive tumor which were diffusely infiltrated among collagen fibers with reticulin fibrosis. He was treated with aggressive combination chemotherapy but achieved no remission and died of cerebral vascular bleeding on Jan. 7, 1989.

Ganglioside GM2 expression on human melanoma cells correlates with sensitivity to lymphokine-activated killer cells.

Ganglioside GM2 is expressed on cell surface membranes of a variety of human malignant cells and has been demonstrated to be immunogenic in humans. We have assessed the role of the antigen GM2 on melanoma cells as a recognition structure for lymphokine-activated killer (LAK) cells. LAK cells were generated by stimulation of non-adherent peripheral blood lymphocytes (PBL) from human donors with recombinant interleukin-2 (IL-2). The selection of target cells was based on GM2 content and included 11 human melanoma cell lines and 2 human leukemia lines. Using a single-cell binding assay, LAK cell binding to target lines expressing high levels of GM2 was significantly greater than to those expressing minimum GM2. This cell-binding was specifically inhibited by addition of purified GM2 but not by other gangliosides. LAK-melanoma cell-binding was also specifically inhibited by anti-GM2 monoclonal antibody (MAb). For further analysis LAK cell lysis of melanoma target cells expressing various amounts of GM2 was assessed. A significant correlation occurred with GM2 expression and LAK cell lysis (p less than 0.025; r = 0.623). Three other gangliosides commonly expressed on human melanoma, GM3, GD3 and GD2, had no correlation with LAK cell lysis. These studies suggest that GM2 on melanoma cells is a marker for LAK cell sensitivity, as well as indicate that GM2 is a potential target recognition structure for human LAK cells.

Gangliosides of human melanoma.

Human melanoma synthesizes a large quantity of gangliosides, glycosphingolipids containing sialic acid. The authors previously have demonstrated that the ganglioside profile differs among individual melanomas and is widely heterogeneous. In the current study, a retrospective study was performed to compare the relationship between the quantity of five major gangliosides of human melanoma (GM3, GM2, GD3, GD2, and alkali-labile ganglioside) and nine clinical factors (sex, age, site, stage, tumor size, pigmentation, histopathologic type of primary tumor, chemosensitivity, and prognosis). Melanoma specimens studied were obtained from patients of our clinic and included 52 biopsy specimens and 28 cultured cell lines. Analysis of melanoma biopsy specimens have shown a differential ganglioside expression among different sites of tumor, pigmentation, and histopathologic types. Results of cultured melanoma cell lines differed from those of biopsy specimens, but ganglioside expression also differed among the site of tumor, tumor size, histopathologic types, and chemosensitivity. GM3 positively correlated with a good prognosis in both biopsy and cultured melanomas.
