Anti-idiotypic antibodies directed against anti-HBs among the patients with chronic hepatitis B.

Anti-idiotypic antibodies (anti-Id) against anti-HBs were found in the sera of patients with chronic hepatitis type B. Anti-idiotypic antibodies were detected by an enzyme-linked immunosorbent assay using horseradish peroxidase conjugated mouse monoclonal anti-HBs. Ten of 72 HBsAg positive sera contained anti-Id (13.9%). The prevalence of anti-Id did not appear to correlate with HBeAg/anti-HBe system. However, HB virus specific DNA polymerase activity was significantly higher in anti-Id positive sera. In the sera obtained from the patients treated with predonisolone before, anti-Id positive rate was higher than that in the patients without a history of predonisolone therapy. These results suggest that anti-Id may be related to the immunoregulatory mechanism of HB virus replication.

Absence of angiogenesis-inhibitory activity in aqueous humor of diabetic rabbits.

We studied inhibitory activity of angiogenesis in the eye to explore the formation of new blood vessels in diabetic microvascular diseases. When we examined the effects of extracts from various ocular tissues of nondiabetic rabbits on the proliferation of bovine aortic endothelial cells, potent inhibitory activity was found in lens and aqueous humor. Aqueous humor inhibited both angiogenesis on chorioallantoic membrane in vivo and promotion of endothelial cell growth in vitro, which were induced by retinal extracts. However, in diabetic rabbits, aqueous humor lost the ability to inhibit the growth of endothelial cells. The loss of activity became evident 1 mo after the alloxan injection and lasted while the animals were hyperglycemic. In addition, diabetic aqueous humor failed to suppress retinal extract-induced angiogenesis promotion and endothelial cell growth. The absence of the inhibitory activity of angiogenesis in diabetic aqueous humor might be involved in promotion of neovascularization in diabetic ocular disease.

[The diameter of main pancreatic duct on endoscopic retrograde pancreatography and the appearance of main pancreatic duct on computed tomography: a comparative study].

We have carried out a comparative study of the diameter of main pancreatic duct (MPD) on endoscopic retrograde pancreatography (ERP) with the frequency of detection of MPD by computed tomography (CT) in order to clarify the importance of MPD appearance on CT in the pancreatic and biliary diseases. The normal MPD on ERP was demonstrated by CT in a low frequency. MPD was most frequently observed in the pancreatic body on CT. The dilatation of MPD on ERP was found in both moderate and advanced pancreatitis group. However, the significant demonstration of MPD by CT was found in advanced group alone. We observed that CT finding of dilated duct correlated with that on ERP in advanced group alone.

The significance of nerve sugar levels for the peripheral nerve impairment of spontaneously diabetic GK (Goto-Kakizaki) rats.

This study was carried out to clarify the relationship between the slowing of motor nerve conduction velocity and nerve levels of sorbitol, fructose, glucose and myoinositol in spontaneously diabetic GK (Goto-Kakizaki) rats. The motor nerve conduction velocity in GK rats was constantly lower than in normal controls at three and nine months of age. This constant decrease in motor nerve conduction velocity in GK rats was closely related to glucose intolerance in GK rats soon after birth. Nerve levels of sorbitol, glucose and fructose in GK rats were significantly increased as compared to normal controls at nine months old, but not (except glucose) at three months old. The increase in nerve concentrations of sugars in GK rats was progressive with age. However, levels of glucose, sorbitol and fructose in normal Wistar rats remain unchanged with age. Although nerve myo-inositol levels in GK rats were lower at three and nine months than those of normal controls, a significant difference in myo-inositol levels was observed only at nine months. On the contrary, nerve myo-inositol level in normal Wistar rats did not show age-related change. These findings suggested that both enhanced polyol pathway activity and myo-inositol depletion play important roles in the reduction of motor nerve conduction velocity.

[Esophageal motility and gastric emptying in PSS patients, correlation with symptoms].

Progressive systemic sclerosis (PSS) commonly involves the esophagus. Dysphagia and heartburn are the most common esophageal symptoms. In this study we evaluated the relationship between esophageal symptoms and esophago-gastric motility. On esophageal manometry, loss of peristalsis, peristaltic contraction amplitude of distal esophagus less than 30 mmHg and decreased LES pressure were critical for esophageal symptoms. The degree of symptoms correlated to esophageal dysmotility. The gastric emptying in PSS patients was delayed, but there was no significant difference in gastric emptying between the patients with and without reflux esophagitis. Esophageal dysmotility is considered to be much responsible for the reflux esophagitis in PSS patients than gastric emptying.

[Biological evaluation of extracellular superoxide dismutase observed in a state of acute gastric mucosal lesions].

In the experimental stress ulcer models of guinea pig superoxide dismutase-like plasma substance increased in plasma. Upon chromatography on heparin-Sepharose, this substance was separated into three fractions; A, without affinity for heparin; B, with intermediate affinity for heparin; C, with relatively strong heparin affinity, and the fraction C was specifically increased by a stress and a heparin injection. Analysis by a polyacrylamide gel electrophoresis (PAGE) of the fraction C revealed SOD activity, whose molecular weight was 135 K in a native state. The fraction C seemed to be the same as extracellular superoxide dismutase (EC-SOD) reported by Marklund. Assuming to the releasing potency of heparin, the major part of EC-SOD in the vasculature was most likely located on endothelial-cell surface. A pathophysiological role of EC-SOD released into plasma seemed to be an indicator of endothelial-cell damage by free radicals of endothelial cells and polymorphonuclear neutrophils (PMNs).

[Multi-agent combination chemotherapy for adult acute lymphoblastic leukemia--individualized chemotherapy].

Eighteen adults with acute lymphoblastic leukemia (ALL) including 14 previously untreated and 4 relapsed cases were treated with individualized and intensified induction and post-remission therapy. CNS prophylaxis was performed with i. th. methotrexate alone and started early during induction. All patients achieved complete remission. In a median follow up of 65 weeks, 2 yr-disease free survival (DFS) rate was 73.5 +/- 13.0% in previously untreated patients. This treatment program has resulted in markedly improved remission rate and DFS in adults with ALL.

Biochemical examination on the increased superoxide dismutase-like plasma substance observed in a state of acute gastric mucosal lesions.

When experimental acute gastric mucosal lesions were produced in guinea pig by water-immersion and restraint stress, superoxide dismutase (SOD)-like substance in the plasma increased. On analysis by gel filtration, it was shown that the molecular weight of the increased SOD-like plasma substance was about 130,000, and even after treatment with trypsin, 84% of this substance remained. Since the molecular weight of intracellular SOD is about 40,000, it seems that this substance is similar to extracellular SOD, located on the endothelial cell-surface, as previously reported by Marklund et al. Our results suggest that in the presence of acute gastric mucosal lesions, SOD-like plasma substance is not identical to intracellular SOD, which derived from cell destruction by stress or free radical-induced microvascular damage or by hemolysis. Furthermore, this substance may itself work as a scavenger of free radicals generated under conditions, such as these described in the present experiment.

[Percutaneous recording of gastric electrical activity (electrogastrography): its technique and analysis].

In 1922, Alvarez made some electrical recordings from cutaneous electrodes, but the method has not been widely adopted. It has been caused by the technical difficulties in recording, and in extracting consistent information from the recording. Therefore in an effort to improve the ability to routinely obtain a recording of the gastric electrical activity, using cutaneous electrodes (electrogastrography, EGG), we have investigated different electrodes placement and bipolar recording techniques. We studied 10 healthy asymptomatic volunteers. Frequency and power product have been used to help interpret the potential change. We found significant frequency component at approximately 3 cycle/min. Recordings from the upper part of epigastrium were better than those from the lower part of epigastrium. This suggests that these recordings are taken from electrical activity of the stomach. We found the good placement of cutaneous electrodes and good recording techniques.

Quantitative assay of lentinan in human blood with the limulus colorimetric test.

A conventional limulus test detects not only endotoxin but also beta (1----3) glucan. Therefore, using a quantitative limulus test (the limulus colorimetric test) we studied the pharmacokinetics of lentinan, an antitumor beta (1----3) glucan, in the blood of 10 health volunteers and three patients with advanced gastric cancer. The calibration curve of lentinan in the human plasma was linear in the range of 0 to 100 ng/ml. When incubated with human plasma at 37 degrees C in vitro, lentinan had the recovery of almost 100% as compared to the initial concentration even after 60-min incubation, indicating the stability of lentinan in human plasma. When 1 mg of lentinan was intravenously administered over a 2 hr period, lentinan concentration reached the maximum levels (50-80 ng/ml) at the end of the drip infusion and decreased gradually thereafter. In the near future, the more appropriate modes of lentinan administration will be determined by further investigation of its kinetics in the human body.

Effect of eye-derived growth factors from rabbit retina on proliferation of endothelial cell from marginal vessels.

The biological activity of eye-derived growth factor(s) (EDGF) was studied in endothelial cells and other cell types. The endothelial cells of rabbit ears were aseptically collected from marginal vessels in a short time without contamination with other cell types. The cells were cultured in 10% FBS-MEM that possessed Factor VIII antigen and Weibel-Palade bodies, and showed monolayer cobble-stone formation. The retinal extracts from rabbits promoted not only endothelial cells but also fibroblasts and smooth muscle cells. Partially purified EDGF was obtained from retinal extracts by means of acetone precipitation and successive chromatographic separation with Heparin Sepharose and Sephadex G-200. The molecular weight was estimated at more than 150 kDa by gel filtration. The partially purified EDGF promoted proliferation of endothelial cells from rabbit ear marginal promoted vessels and bovine aorta, but did not affect smooth muscle cells, skin fibroblasts and Swiss albino 3T3 cells. Thus rabbit EDGF might be cell specificity in stimulating cell proliferation.

Establishment of the specific radioimmunoassay for serum thymic factor (STF).

Serum thymic factor is a humoral factor involved in the differentiation of T cells. In the present study, a radioimmunoassay system for STF was established using a specific antiserum and an iodinated synthetic STF as a tracer. Serum levels of STF-like immunoreactivities were around 30 pg/ml in human, but in contrast to previous reports serum STF levels did not show age-dependent decreases. STF-like immunoreactivities of the thymic gland were lower than those of the liver and kidney in rats. The livers of human and pig contained high STF-like immunoreactivities. The rat thymic gland extract showed three peaks of STF-like immunoreactivity by gel filtration of Sephadex G-25 which corresponded to the eluted position of authentic STF, a larger molecular size, and a smaller molecular size of STF, respectively, while the rat liver extract showed two peaks corresponding to the void volume fraction and to the position of authentic STF, respectively. When this void volume fraction was digested by trypsin, three peaks which corresponded to authentic STF, a larger molecular size, and a smaller molecular size of STF were observed. Present studies suggest that serum thymic factor is produced as a larger STF molecule in the liver and kidney.

Determination of 3 beta-hydroxylated bile acids in human serum and liver tissue.

3 beta, 7 alpha-Dihydroxy-5 beta-cholanoic acid (3 beta, 7 alpha-diOH) was detected in serum of 3 patients with intrahepatic cholestasis. 3 beta, 7 beta-dihydroxy-5 beta-cholanoic acid (3 beta, 7 beta-diOH) appeared in serum of those patients after treatment with ursodeoxycholic acid (UDC). These bile acids were also detected in only unconjugated fractions of serum of another 7 patients with chronic liver diseases, but not in liver tissue of them. The liver does not seem to from these bile acids itself because they were absent in liver tissue.
