Timolol and postoperative intraocular pressure.

We studied the effect of timolol instillation at surgery on the acute postoperative pressure rise following extracapsular cataract extraction (ECCE) and posterior chamber lens implantation. The intraocular pressure (IOP) was measured daily during the first week following surgery in 85 eyes. Timolol was instilled immediately following surgery in 34 eyes; the remaining 51 served as controls. The mean preoperative IOP was 18.7 mm Hg in the treated and control groups. The mean IOP on the first day following surgery was 20.0 mm Hg in the timolol group and 20.7 mm Hg in the control group. Twenty-six percent of the timolol group had a pressure greater than 23.0 mm Hg (mean 35.6 mm Hg); 33% of the control group had a pressure greater than 23.0 mm Hg (mean 31.0 mm Hg). The IOP in all treated and untreated patients with ocular hypertension on the first day following surgery returned to normal within three days. Timolol, therefore, had no effect on acute postoperative pressure elevation following ECCE and posterior chamber lens implantation.

Postsurgical intraocular pressure elevation.

Eighty-six eyes undergoing intraocular surgery were evaluated for the incidence of acute postoperative intraocular pressure (IOP) rise and the relationship between initial and delayed postoperative pressure elevations. Our results show that 22.5% of penetrating keratoplasty (triple procedures and aphakic transplants) and 32% of extracapsular cataract extraction (ECCE) patients developed an initial postoperative pressure greater than 23 mm Hg. Ninety percent of penetrating keratoplasty patients and 46% of ECCE patients who had an initial pressure elevation developed a delayed pressure elevation. Forty-three percent of penetrating keratoplasty and 32% of ECCE patients with a normal initial postoperative IOP developed a delayed pressure increase. Extracapsular cataract surgery patients developed late pressure rise with approximately the same frequency as normal eyes on prolonged topical steroid therapy, regardless of the initial postoperative pressure.

Evaluation of tonsillectomy as a treatment for pustulosis palmaris et plantaris.

One hundred and twenty-four cases of pustulosis palmaris et plantaris (PPP) were studied by questionnaire survey to evaluate the effectiveness of tonsillectomy as treatment. We found that tonsillectomy as a method of treatment for PPP showed a significantly high rate of cure, against which other methods of treatment, e.g. corticosteroid therapy, PUVA therapy and so on, were compared. The examination, by means of Kaplan-Meier's method, showed a 59.2% cure rate for patients one year after tonsillectomy and 71.8% 2 years after the operation. When patients are older than 60 years, the prognosis is good, regardless of the method of treatment.

[Solubility of metal components into tissue culture medium from dental amalgams (author's transl)].

Solubility of metal components into tissue culture medium, YLH, from various dental amalgams including high copper, conventional and copper amalgam was measured with atomic absorption spectrometry. The results obtained were as follows.: 1. Mercury solubility was found much in all the dental amalgams after one day extraction. This was followed by gradual increase in solubility in the high copper amalgams until after seven days. On the otherhand, initial high solubility was maintained in conventional and copper amalgams through the whole experimental period. 2. Higher solubility of silver and copper was recorded in the high copper amalgams. 3. Zinc was only found in Dispersalloy and copper amalgam. 4. Tin was unable to be measured in all the amalgams examined. It was considered that the present results could shed a light on the mechanism for cytotoxicity yielding of the dental amalgams.

Autosomal recessive prolidase deficiency. Three patients with recalcitrant ulcers.

Three patients had prolidase deficiencies. The family pedigree of these three patients suggests that this rare disorder is inherited through an autosomal recessive gene. this genodermatosis is characterized by a number of signs and symptoms referable to the skin, CNS, teeth, ears, nose, throat, eyes, bones, and joints. Among the skin changes, recalcitrant leg ulcers are the most characteristic. At this time, there is no established method of treatment of this rare disorder, but the use of dapsone was helpful in the treatment of one of our patients.

Inhibition of amygdaloid kindling by chronic pretreatment with cocaine or methamphetamine.

Amygdaloid kindling was induced by daily electrical stimulation in the left amygdala in five groups of cats: a chronic cocaine pretreatment group; a chronic methamphetamine (MAP) pretreatment group; two treatment groups, one using pimozide and the other haloperidol during the period when kindling was being induced; and a drug-free control group. The number of left amygdaloid stimulations required for generalized convulsions was significantly greater in the two chronic pretreatment groups and significantly less in the two treatment groups, compared to the control group. All animals in the chronic cocaine and MAP pretreatment groups showed hemiconvulsions instead of symmetrical generalized convulsions and lateralized interictal discharges in the stimulated hemisphere. In rekindling to the right amygdala, seizure development was significantly suppressed compared to the control group. In the two chronic pretreatment groups, chronic administration of cocaine or MAP was followed by enhancement of behavioral responses to cocaine, MAP, and apomorphine. The association of such "sensitization" with impairment of the kindling process suggests that dopamine receptor sensitivity is an important factor in this form of epileptogenesis.
