Pancreatic arteriovenous malformation in a patient with gastrointestinal hemorrhage.

A 39-year old woman, with a pancreatic arteriovenous malformation (AVM) which ruptured into the pancreatic duct, was treated successfully with distal pancreatectomy and splenectomy. Histological examinations revealed an AVM with extravasation into the pancreatic ducts. A review of the cases reported to date emphasizes the importance of physical signs, individual histories and angiographic studies for early diagnosis and surgery.

Angiotoxic effects of dietary 7-ketocholesterol in chick aorta.

Although dietary 1-ketocholesterol treatment had little effect on plasma and liver lipid metabolism, aortas from chickens treated with 7-ketocholesterol contained numerous dead and dying smooth muscle cells. Chicks receiving dietary cholesterol, in amounts equal to or greater than the amount of 7-ketocholesterol, did not demonstrate significant smooth muscle cell degeneration. After eight weeks, a combined diet of 1% cholesterol plus 10-20 mg/day 1-ketocholesterol produced no larger amounts of aortic lipid deposition, and no greater increases in smooth muscle cell degeneration than a comparable group receiving 7-ketocholesterol alone. The abdominal aorta was always more severely damaged than the ascending arch or thoracic aorta in chicks fed 7-ketocholesterol. The results demonstrate a potent angiotoxic effect of 7-ketocholesterol.

Coronary arterial lesions in sexually mature non-layers, layers, and roosters.

The effects of hereditary hyperlipidemia on coronary artery atherosclerosis were studied in 77 White Leghorn (DeKalb strain) chickens ranging from 4 to 13 months in age. After pubescence, the plasma levels of triglyceride and cholesterol in non-laying hens ranged 2- to 3-fold and 2- to 7-fold higher compared to layers. Serial sectioning revealed that most lesions were found in the proximal portions of both the left and right coronary arteries. Ultrastructurally, lesions in the roosters contained no foam cells, whereas some foam cells and small amounts of stainable lipid were observed in the thickened intima of layers. Half of the non-layers had stenotic lesions characterized by many foam cells, necrotic foci, and heavy stromal lipid deposits. Continuous permeation of excess plasma lipids into the arterial wall appeared to be an important factor in the development of coronary lesions.

Morphological evidence of endogenous lipid production in swine ductus vasculature.

Specimens of ductus arteriosus and venosus from 2-month-old and 6-month-old swine were examined by electron microscope. The purpose of the study was to observe the effects of vessel closure and hypoxia on medial vascular smooth muscle cells. At 6 months of age, smooth muscle cells from the medial layer of ductus arteriosus showed signs of considerable cellular degeneration and contained lipid vacuoles which were often surrounded by granular endoplasmic reticulum. Specimens from the portal side of ductus venosus showed initial stages of smooth muscle cell degeneration and lipid vacuolization, while samples from the hepatic side of ductus venosus were nearly normal. This study contains the first reported morphological evidence of organelles in vascular smooth muscle cells which are responsible for endogenous lipid droplet production.

Morphometrical analysis of the aging process in human arteries and aorta.

Ultrastructural and morphometrical studies were conducted on vascular tissue from 205 human Japanese specimens ranging in age from 15 weeks' gestation to 90 years. Comparison of aorta size with overall body length by allometry revealed that the aging aorta has growth and involution stages comparable to other organs, and that progressive increase in aortic diameter was the major involutional change. The cellular and extracellular components of the medial sections of thoracic and abdominal aorta, renal artery, and superior mesenteric artery were quantitated from electron micrographs and compared at different ages. It was found that the renal artery rapidly accumulated extracellular material with increasing age; in the aortic tissue, however, the peak occurrence of extracellular material occurred at pubescence. Ultrastructural studies revealed a general tendency of aging medial smooth muscle cells to transform from a smooth, rounded morphology to become irregularly shaped.

Ultrastructure of developing human ductus arteriosus.

Histological and ultrastructural studies were made of 25 specimens of human ductus arteriosus obtained from abortion of autopsy, and ranging in age from 15 weeks of gestation to 7 years. Samples of ductus with normal muscular type structure exhibited active intimal thickening as early as 15 weeks' gestational age when the internal elastic lamina was found to be focally discontinuous. At the same time, intimal smooth muscle cells were closely arranged, and often intimately connected to cells of the tunica media. Ultrastructurally, both smooth muscle cells and endothelial cells at 15 weeks' gestational age contained abundant glycogen deposits. From 18-32 weeks' gestational age, glycogen deposits gradually disappeared, collagen fibres began to appear in the extracellular space and the first signs of smooth collagen fibres began to appear in the extracellular space and the first signs of smooth muscle cell degeneration became apparent. After birth, intimal thickening and degeneration of smooth muscle cells was much more pronounced. The ultrastructure of intimal smooth muscle cells indicated that intimal thickening was caused by smooth muscle cell migration as opposed to rapid proliferation; and both intracellular and extracellular membrane-bound lipid-filled vacuoles were commonly seen in the more advanced stages of degeneration. Of the ductus specimens examined, three were rich in elastic fibres. Two of these three specimens were from a group of 10 abortion cases, and the third was from a 2 weeks old full term infant who had been exposed to maternal rubella. The 2 weeks old infant had a widely patent ductus arteriosus; whether the two fetal specimens with ductus elastosis would have eventually developed into clinical patent ductus arteriosus was not clear.

CT in evaluation of the circle of Willis.

The method of visualizing the circle of Willis intentionally on contrast-enhanced CT scan was studied. The incidence of circle of Willis in our series was 86%. Only mild side effects were observed, in 7.5% of cases. CT of 60 proven cerebral aneurysms were reviewed and aneurysm was detected in 57%. This method has the potential to be a noninvasive screening procedure for evaluation of suspected cerebral aneurysm, as well as evaluation of marked arteriosclerosis, occlusion of major arteries, and mass effect of parasellar tumors.

[Biotransformation of diflucortolone valerate in the skin of rat, guinea pig and man (author's transl)].

The biotransformation of 6alpha,9-difluoro-11beta-hydroxy-16alpha-methyl-21-valeryloxy-1,4-pregnadiene-3,20-dione (diflucortolone valerate, DFV, Nerisona) was examined in vitro in the skin of rat and man and in vivo in the skin of guinea pigs. In the investigations with rat skin DFV in ethanolic solution (0.1 mumol) was added and the ester hydrolysis in buffer monitored in relation to the incubation time. In the experiments with guinea pigs and with human skin DFV was applied topically in the form of a 0.1% W/O emusion employing a dose of 6 mg/cm2 skin. After the substance which had not penetrated had been recovered by means of cotton wool and adhesive film at the end of the period of exposure the portions of skin were homogenized, extracted and the extracts separated by thin-layer chromatography. DFV is hydrolysed in the skin of rats and guinea pigs with a half-life of about 30-60 min to diflucortolone 6alpha,9-difluoro-11bets,21-dihydroxy-16alpha-methyl-1,4-pregnadiene-3,20-dione) and valeric acid. In excised human skin degradation of the ester proceeds much more slowly. Even 7 h p. appl. about 80-90% DFV and only 5-15% DF were identifiable in the skin extract. Despite the estremely low absorption rate the slow hydrolysis of DFV in human skin guarantees an adequate level of active ingredient in the skin over a long period.
