[A case report of pulmonary sequestration (Pryce I) associated with incomplete lobulation of the left lung].

A 28-year-old man was treated surgically for pulmonary sequestration of Pryce type I associated with incomplete lobulation of the left lung. His chief complaint was hemoptysis. His chest X-ray film showed a mass lesion behind the cardiac shadow. Aortogram revealed a large artery arising from the descending aorta and supplying the left basal segment, and venous blood returning into the left inferior pulmonary vein. The left basal segment had no pulmonary arteries. A bronchogram showed no defects in the bronchial tree. The left basal segment was resected successfully. Histological study showed the trunk of the aberrant artery to be elastic. Differences between histological types of aberrant arteries in pulmonary sequestration of Pryce type I and the pathogenesis of intralobar sequestration were discussed.

Synthesis of KDN-lactotetraosylceramide, KDN-neolactotetraosylceramide, and KDN-Lewis X ganglioside.

Analogues of sialyl-lactotetraosylceramide, sialyl-neolactotetraosylceramide, and sialyl Lewis X ganglioside, in which the N-acetylneuraminic acid residue is replaced by a 3-deoxy-D-glycero-D-galacto-2-nonulopyranosylonic acid (KDN) unit, have been synthesized. Methyl O-(methyl 4,5,7,8,9-penta-O-acetyl-3-deoxy-D-glycero-alpha-D-galacto-2- nonulopyranosylonate)-(2-->3)-2,4,6-tri-O-benzoyl-1-thio-beta-D- galactopyranoside (4) was prepared from 2-(trimethylsilyl)ethyl O-(methyl 4,5,7,8,9-penta-O-acetyl-3-deoxy-D-glycero-alpha-D-galacto-2- nonulopyranosylonate)-(2-->3)-6-O-benzoyl-beta-D-galactopyranoside , via O-benzoylation, replacement of the 2-(trimethylsilyl)ethyl group by acetyl, and introduction of the methylthio group with trimethyl(methylthio)silane. Glycosylation of 2-(trimethylsilyl)ethyl O-(2-acetamido-4,6-O-benzylidene-2-deoxy-beta-D-glucopyranosyl)-(1-->3)- O- (2,4,6-tri-O-benzyl-beta-D-galactopyranosyl)-(1-->4)-2,3,6-tri-O-benzyl- beta- D-glucopyranoside (5) or of 2-(trimethylsilyl)ethyl O-(2-acetamido-3,6-di-O-benzyl-2-deoxy-beta-D-glucopyranosyl)-(1-->3)-O- (2,4,6-tri-O-benzyl-beta-D-galactopyranosyl)-(1-->4)-2,3,6-tri-O-benzyl- beta- D-glucopyranoside, prepared from 5 via O-benzylation and reductive opening of the benzylidene acetal ring, with 4 as a donor gave the corresponding pentasaccharides 9 and 13 in good yields. In the same way, 4 was reacted with 2-(trimethylsilyl)ethyl O-(2,3,4-tri-O-benzyl-alpha-L-fucopyranosyl)-(1-->3)-O-(2-acetamido-6- O-benzyl-2-deoxy-beta-D-glucopyranosyl)-(1-->3)-O-(2,4,6-tri-O-benzyl-be ta- D-galactopyranosyl)-(1-->4)-2,3,6-tri-O-benzyl-beta-D-glucopyranoside to yield the hexasaccharide 17. These three oligosaccharides 9, 13, and 17 were converted via reductive removal of the benzyl groups and benzylidene group, O-acetylation, removal of the 2-(trimethylsilyl)ethyl group, and subsequent reaction with trichloroacetonitrile, into the corresponding trichloroacetimidates 12, 16, and 20, respectively. Glycosylation of (2S,3R,4E)-2-azido-3-O-benzoyl-4-octadecene-1,3-diol with 12, 16, and 20 in the presence of boron trifluoride etherate afforded the expected beta-glycosides, which were transformed via selective reduction of the azido group, coupling with octadecanoic acid, O-deacylation, and de-esterification, into the target gangliosides in high yields.

Sequence and functional analyses of the rice gene homologous to the maize Vp1.

The homologous gene of the maize Vp1 gene was isolated from rice (Oryza sativa). Sequence analysis revealed that the rice Vp1 gene (Osvp1) encodes a protein of 728 amino acids and is interupted by 5 introns at positions identical to those of the maize gene. Osvp1 transcript was detected in developing embryo as early as 10 days after flowering and decreased toward maturity. Osvp1 transcript was also detected in dry as well as imbided mature embryos. The ability of Osvp1 gene product to activate a target gene was shown by transient expression experiments in rice suspension-cultured cell protoplasts using a reporter gene construct carrying the bacterial beta-glucuronidase (GUS) gene fused to the promoter of OsEm gene, the rice homologue of the wheat Em gene.

Biliary carcinosarcoma arising in nonparasitic simple cyst of the liver.

We report a carcinosarcoma arising in a nonparasitic simple cyst of the liver. Sequential imaging findings revealed that the tumour originated in a hepatic cyst and was composed of well-differentiated tubular adenocarcinoma and high-grade spindle cell sarcoma without apparent transition between the two. At the margin of the tumour, a single layer of benign cuboidal epithelium and outer fibrous bands were present, suggesting that the tumour had arisen from a nonparasitic simple cyst of the liver. Immunohistochemically, carcinoma cells were positive for cytokeratins, epithelial membrane antigen and CA 19-9 and negative for vimentin, while sarcomatous cells were positive for vimentin and negative for cytokeratins, epithelial membrane antigen and CA 19-9. Sarcomatous cells did not show any immunophenotypic features of smooth muscle cells, striated muscle cells, histiocytes, lipocytes, nerve cells, Schwann cells or endothelial cells. Ultrastructurally, no specific differentiation was seen in the sarcomatous cells.

Expression of transforming growth factor-alpha and its receptor during human liver development and maturation.

We investigated the expression of transforming growth factor-alpha (TGF-alpha) and its receptor during human liver development and maturation, using immunohistochemistry. In the fetal liver, strong immunoreactivity for TGF-alpha and its receptor was noted in intrahepatic bile duct cells of various developmental stages; moderate immunoreactivity for TGF-alpha and mild immunoreactivity for TGF-alpha receptor were found in immature hepatocytes. In the postnatal liver, reactivity for TGF-alpha in hepatocytes decreased gradually and was negative or only weakly positive in the adult liver, while reactivity for TGF-alpha receptor in hepatocytes increased gradually and was strongly positive in the adult liver. In contrast, immunoreactivity of TGF-alpha and its receptor in intrahepatic bile duct cells persisted in the postnatal liver and was positive in the adult liver. These data suggest that the system of TGF-alpha and its receptor has an important role in the proliferation and differentiation of intrahepatic biliary cells and hepatocytes in the fetal liver. The decreasing expression of TGF-alpha in hepatocytes in the postnatal liver may indicate that proliferative activity of hepatocytes gradually decreases with liver maturation. The presence of TGF-alpha and its receptor in intrahepatic bile ducts in the postnatal liver suggests that the system of TGF-alpha and TGF-alpha receptor is operative postnatally.

Stenotic kinking of the cavernous internal carotid artery with a giant intra-cavernous aneurysm: case report.

We report a giant aneurysm of the cavernous internal carotid artery with proximal internal carotid stenosis. The stenosis showed two typical features: a kink at the stenosis and location at the exit from the carotid canal. We believe that the cavernous portion of the internal carotid artery is compressed medially by the giant aneurysm and a kink occurs at the point where the artery leaves the bony carotid canal.

Identification, characterization, and developmental expression of a novel alpha 2-->8-KDN-transferase which terminates elongation of alpha 2-->8-linked oligo-polysialic acid chain synthesis in trout egg polysialoglycoproteins.

A novel glycosyltransferase which catalyses transfer of deaminated neuraminic acid, KDN (2-keto-3-deoxy-D-glycero-D-galacto-nononic acid) from CMP-KDN to the non-reducing termini of oligo-polysialyl chains of polysialoglycoprotein (PSGP), was discovered in the ovary of rainbow trout (Oncorhynchus mykiss). The KDN-transferase activity was optimal at neutral pH, and stimulated 2 to 2.5-fold by 2-5 mM Mg2+ or Mn2+. Expression of KDN-transferase was developmentally regulated in parallel with expression of the alpha 2-->8-polysialyltransferase, which catalyses synthesis of the oligo-polysialyl chains in PSGP. Incorporation of the KDN residues into the oligo-polysialyl chains prevented their further elongation, resulting in 'capping' of the oligo-polysialyl chains. This is the first example of a glycosyltransferase that catalyses termination of alpha 2-->8-polysialylation in glycoproteins.

Tissue plasminogen activator thrombolysis and transluminal angioplasty in the treatment of basilar artery thrombosis: case report.

A 61-year-old man with basilar artery thrombotic occlusion was successfully treated by a combination of local infusion of tissue plasminogen activator (t-PA) followed by percutaneous transluminal angioplasty (PTA). t-PA superselective infusion combined with the mechanical destruction of the clot by the guide wire was effective for recanalization. A Stealth dilation catheter, with a 2.5-mm balloon diameter and 10-mm length, was used for PTA. A balloon inflation time of 60 seconds with 4 atmospheres of pressure was delivered for successful dilation of the basilar artery. It was confirmed to be widely patent on follow-up angiography 40 days after the combined procedure with no further ischemic attacks apparent on clinical examination at 6 months.

Penetration by bull spermatozoa into the zona pellucida of dead bovine oocytes recovered from frozen-thawed ovaries.

The present study was carried out to determine if the zona pellucida of dead bovine oocytes obtained from ovaries stored at -196 degrees C could be used to assess penetrability of capacitated bull spermatozoa. Follicular oocytes were recovered from bovine ovaries which were frozen slowly in a box containing dry ice, plunged into liquid nitrogen, and thawed at 37 degrees C. The dead oocytes were inseminated with various concentrations of spermatozoa preincubated for 0 to 4 h. Sperm penetration rates of the dead oocytes were significantly altered by sperm concentration and preincubation time. Dead and living oocytes matured in vitro (control) gave similar patterns of penetrability based on sperm preincubation time. When sperm concentration was increased, the rate of multiple sperm penetration into the dead oocytes also increased significantly, but the rate of penetration into living oocytes did not alter significantly. All dead oocytes from ovaries stored at -196 degrees C for 1 d to 3 mo were penetrated at similar rates by spermatozoa preincubated for 1-h. Thus, we conclude that dead follicular oocytes recovered from frozen ovaries are useful for the assessment of sperm capacitation and/or the acrosome reaction in cattle.

Effects of cycloheximide on chromatin condensations and germinal vesicle breakdown (GVBD) of cumulus-enclosed and denuded oocytes in cattle.

To determine if newly synthesized protein is imperative for the resumption of meiosis in bovine follicular oocytes collected from small antral follicles, cumulus-enclosed and denuded oocytes were cultured in TCM-199 both with and without various concentrations of the protein synthesis inhibitor, cycloheximide. After 11 h of culture in inhibitor-free medium, all oocytes had undergone germinal vesicle breakdown (GVBD). However, when concentrations of more than 1.0 mug/ml cycloheximide were added to the medium, the meiotic resumption of bovine oocytes was completely blocked. This inhibitory effect of cycloheximide was fully reversible after removal of the inhibitor from maturation media. Germinal vesicle breakdown following removal of cycloheximide occurred twice as fast as in the control medium. Nevertheless, when oocytes were arrested at the germinal vesicle (GV) stage by cycloheximide, a significantly higher proportion of chromatin condensation (40 to 57%) was observed in denuded oocytes than in cumulus-enclosed oocytes (11 to 22%). Thus the cycloheximide treatment could not prevent the chromatin condensation in only denuded oocytes. We conclude that protein synthesis is a prerequisite for GVBD in bovine follicular oocytes and that cumulus cells are responsible for the complementary regulation of the chromatin condensation at the GV stage, regardless of protein synthesis in the oocytes.

Pancreatic trypsinogen and cathepsin B in human pancreatic carcinomas and associated metastatic lesions.

Expression of pancreatic trypsinogen and cathepsin B in 23 surgically resected pancreatic ductal adenocarcinomas was evaluated immunohistochemically, using a monoclonal antibody against human pancreatic trypsinogen and a polyclonal antibody against human cathepsin B. Fifteen of 20 invasive tubular adenocarcinomas (75%) expressed pancreatic trypsinogen in a coarse granular pattern located in the supranuclear cytoplasm of the carcinoma cells. In addition, metastatic lesions, including those in peripancreatic lymph nodes and neural plexuses, expressed pancreatic trypsinogen. In contrast, three intraductal (non-invasive) papillary adenocarcinomas did not express pancreatic trypsinogen. Cathepsin B expression was recognised in 14 of 20 invasive tubular adenocarcinomas (70%) in a fine granular pattern located diffusely in the cytoplasm of the carcinoma cells, while none of the three intraductal papillary adenocarcinomas had detectable cathepsin B. These findings suggest that pancreatic invasive ductal adenocarcinomas express pancreatic trypsinogen and cathepsin B immunoreactive peptides, raising the possibility that pancreatic trypsinogen and cathepsin B may act independently of each other in the process of carcinoma invasion and metastasis, like other different classes of proteases involved in the proteolytic modification of the matrix barrier.

Brain perfusion SPECT in a patient with a subtle venous angioma.

Brain SPECT of regional cerebral blood flow using I-123 IMP demonstrated a focally decreased perfusion area immediately adjacent to a venous angioma in a patient with simple partial seizures. A positive correlation was obtained among the location of the venous angioma, the decreased perfusion area on SPECT images, and the electroencephalographic focus. Anomalous venous drainage through a venous angioma may explain a perfusion disturbance in the surrounding brain of the angioma. High-resolution SPECT imaging with magnetic resonance guidance provides useful information on the pathophysiology of venous angiomas.

Carbohydrate antigen 19-9 producing giant epithelial cyst of the spleen in a young woman.

We report a giant splenic epithelial cyst which produced carbohydrate antigen 19-9 (CA 19-9) in a 21-year-old woman with left abdominal pain. Imaging techniques revealed a huge cyst of the spleen, and laboratory data revealed elevated serum CA 19-9 (830 U/ml; normal, < 27 U/ml). After splenectomy, serum CA 19-9 level returned to normal. A unilocular splenic cyst measuring 16 x 14 x 13 cm contained brown fluid (900 ml) with a high level of CA 19-9 (10,150,000 U/ml). The wall of the splenic cyst was composed of inner stratified epithelium with mixed squamous and columnar differentiation; immunohistochemical analysis showed that the epithelial cells of the cyst showed strong immunoreactivity to CA 19-9. The inner connective tissue of the cyst wall devoid of epithelial cells was also strongly positive for CA 19-9. We suggest that epithelial cells of our splenic epithelial cyst produced a large amount of CA 19-9 which diffused into capillaries of the connective tissue of the cyst wall and entered into systemic circulation, resulting in the elevated serum CA 19-9.
