REM sleep without muscle atonia (stage 1-REM) and its relation to delirious behavior during sleep in patients with degenerative diseases involving the brain stem.

Nocturnal sleep was examined in 12 patients with degenerative diseases involving the brain stem and in 2 patients with late cerebellar cortical atrophy (LCCA). A peculiar sleep state, characterized by the concomitant appearance of a low-voltage mixed frequency EEG, rapid eye movements (REMs) and tonic EMG in mental muscles, repeatedly appeared during nocturnal sleep in all of the 12 patients with degenerative diseases involving the brain stem and it was called stage 1-REM after Tachibana et al. In 8 of the 12 patients, delirious or oneiric behavior appeared during, or soon after, the episodes of stage 1-REM. Inner experiences reported by one of the subjects well corresponded to his behavior during the episode of stage 1-REM. Stage 1-REM was not observed during nocturnal sleep of the patients with LCCA. These results indicate that a degenerative lesion in the brain stem induced stage 1-REM and delirious behavior during nocturnal sleep through abolishing muscle atonia of REM sleep and causing dissociation of the functional components characterizing REM sleep.

[A clinical and pathological study on varicella-zoster virus pneumonia].

A 48-year-old male with Adult T-cell leukemia (case 1) and a 57-year-old with acute lymphocytic leukemia (case 2) died of rapid progressive pneumonia and pleuritis. Histopathological findings of the lungs were multifocal hemorrhagic coagulation necrosis, and typical Cowdry type A and full type intranuclear inclusion bodies were observed in alveolar cells, bronchial cells, fibroblast, endothelial cells of small vessels, bronchial gland cells and pleural cells. Antigen against varicella-zoster virus (VZV) was positive in these cells by immunostaining, and the antigen was also demonstrated in other organs such as liver, spleen, pancreas, kidney, adrenal gland, esophagus, stomach, intestine and so on. Furthermore, cytomegalovirus was simultaneously superinfected lungs of both cases, and concomitant candida gastritis (case 1), aspergillus lung abscess and candida liver abscess (case 2) were observed. In the immunocompromised host VZV may involve the visceral organs and death may result from VZV pneumonia.

[HTLV-I associated interstitial pneumonia--a case report and bronchoalveolar lavage study].

A 70-year-old male was admitted to our hospital because of fever and dyspnea. The patient was seropositive for HTLV-I and ATL cells were seen in the peripheral blood in the percentage of 2-5. The proviral DNA was positive and the diagnosis of smoldering ATL was made. His chest X-ray film showed diffuse reticulo-nodular infiltrates in both lung fields. The lung tissue obtained by transbronchial lung biopsy showed the lymphocytic infiltrations in the alveolar septa and the submucosa of the bronchioles. Bronchoalveolar lavage (BAL) fluid showed an increased proportion of lymphocytes that consisted mainly of CD3+ DR+ cells and the CD4+/CD8+ ratio was 2.1 during exacerbation and 0.8 after steroid therapy. Anti-HTLV-I IgG and IgA antibodies were positive in both serum and BAL fluid by Western blotting method. It is suggested that T-lymphocyte alveolitis may occur in patients who are seropositive for HTLV-I and the immunological mechanism seems to be responsible.

Synthesis and antitumor activity of fused quinoline derivatives.

Some tetracyclic quinolines (9 and 14) with a [2-methoxy-4-[(methylsulfonyl)amino]phenyl]amino side chain were prepared and their deoxyribonucleic acid (DNA) intercalative properties, KB cytotoxicity, antitumor activity (P388 leukemia), and ability to induce topoisomerase II dependent DNA cleavage were investigated. The indoloquinoline derivative 9 exhibited the most potent activity (dose = 6.3 mg, T/C% = 300) in this series. The steric structural features of the chromophores of the compounds previously and newly synthesized were studied by a computer-associated molecular graphics technique. Relationships between the steric structural features of the chromophores and biological activities are also discussed.

Electrocutaneous spatial integration at threshold: the effects of electrode size.

Five experiments were carried out to investigate the effects of electrode size on threshold for current pulses that flowed between the ventral and dorsal sides of the right forearm. The main result was that the total current threshold decreases as the size of the cathode and anode increases at least up to 15 mm in diameter. To account for this finding, a neural summation model was proposed, assuming that the central nervous system sums up neural impulses discharged near the conduction paths under electrodes. It was also found that the current flowing from the dorsal to the ventral side provided lower thresholds and more stable localization than the opposite flow of current did. This finding suggested that current threshold and its perceived locus are determined by both the body site on which electrodes were placed and the relative polarity that was assigned to electrodes.

[Clinical study for prediction of postoperative visual acuity in cataract patients].

In order to predict more quantitatively postoperative visual acuity in cataract patients, we employed the potential acuity meter (PAM). The PAM projects a Snellen visual acuity chart into the eye by a minute aerial aperture approximately 0.1 mm in diameter. The value of determining potential visual acuity is evident because of the frequent co-existence of macular abnormalities in cataract patients. The 20 cataract patients had an average of 64.8 years (range, 48 to 84 years), and 8 were men and 12 were women. In 23 of the 25 eyes examined, postoperative visual acuity was within two lines or better than the predicted visual acuity with the PAM. Provided the operator is skillful, and if one recognizes its limitations and indications, the PAM is a useful means of predicting postoperative visual acuity in patients with mild or moderate cataracts.

[Complication of central retinal vein occlusion in iron deficiency anemia].

A case of central retinal vein occlusion as a complication of severe iron deficiency anemia is reported. A 45-year-old woman noticed severe visual impairment in her left eye. Fundus findings showed typical impending occlusion of the central retinal vein, and laboratory investigation revealed severe iron deficiency anemia. She was followed by treatment with oral iron. Her visual acuity and fundus findings improved rapidly in response to iron supplement therapy, suggesting a relationship between central retinal vein occlusion and iron deficiency anemia. It seems very important therefore to investigate serum iron levels in female patients with central retinal vein occlusion.

[A successful surgical case of ruptured thoracoabdominal aneurysm].

A successful surgical case of ruptured thoracoabdominal aortic aneurysm was reported. The patient was a 77-year-old male. He entered our hospital and was diagnosed ruptured Crawford type 3 thoracoabdominal aortic aneurysm by chest X-ray, CT scan and aortography. Operative procedure was tube graft insertion into thoracoabdominal aorta with celiac artery reconstruction. Postoperative course was excellent and discharged on 55th postoperative day.

[The penetration of ofloxacin and enoxacin into the alveolar spaces in rats].

Ofloxacin (OFLX) or Enoxacin (ENX) was intramuscularly administered at a dose of 100 mg/kg to rats (normal and pneumonia model). Bronchoalveolar lavage (BAL) was performed at 30 min after administration and concentrations of OFLX or ENX in the recovery fluid were determined. Total recovery amounts of OFLX from 10 BAL procedures were 64.7 +/- 18.7 micrograms in the normal group and 126.5 +/- 16.1 micrograms in the pneumonia group, and the amounts of ENX were 15.6 +/- 4.6 micrograms in the normal group and 32.8 +/- 6.1 micrograms in the pneumonia group. Regarding the ratio of total recovery amounts in BAL to serum concentrations of OFLX or ENX, the pneumonia group was higher than the normal group and the ratio of OFLX was higher than that of ENX. Regarding the ratio of total recovery amount in BAL to the amount in lung parenchyma, the ratio of OFLX was higher than that of ENX, but there was no significant difference between the normal group and the pneumonia group in the ratio of OFLX or ENX. With regard to the ratio of the amount of OFLX or ENX in lung parenchyma to their serum concentrations, the ratio was higher in the pneumonia group than in the normal group, but there was no significant difference between OFLX and ENX. Based on the above results, it was concluded that the permeability of OFLX into epithelium of blood capillary is the same as that of ENX, but the permeability of OFLX into alveolus epithelium is superior to that of ENX.(ABSTRACT TRUNCATED AT 250 WORDS)

[Computed tomographic findings of peripheral-type schwannoma; comparative study with histology].

Computed Tomographic (CT) findings of peripheral-type schwannomas were reviewed and compared with the histology. Ten patients were examined. Plain CT showed all tumors as hypodense mass. After injection of contrast material, they were enhanced in various fashions. The enhancement patterns were divided into four types. On enhancement CT, highly enhanced area in the tumor was consistent with hypercellular region or vascular rich area, whereas poorly stained area revealed hypocellular area, interstitial xanthomatous change or cystic degeneration. It is concluded that the inter-tumoral difference in special distribution of hyper- and hypocellular areas leads to the variety of enhancement pattern on CT images.

[Mechanisms for resistance and cross-resistance patterns of cisplatin-resistant tumor lines].

Mechanisms for cisplatin resistance were studied using mouse leukemias acquired resistance to the drug. Uptakes of cisplatin by L-1210/DDP and P 388/DDP were significantly decreased, compared with the respective sensitive lines. Glutathione contents in both the resistant lines were 1.7 times more than in the respective sensitive ones. While glutathione contents were reduced to about 10% by incubation of cells with D, L-buthionine-S, R-sulfoximine, sensitivity of the resistant cells remained unchanged. Therefore, glutathione might not relate to the mechanism for resistance in these lines. Cross-resistance patterns of L-1210/DDP and P 388/DDP as well as Colon 26/DDP to cisplatin analogs were investigated. Carrier ligands of the analogs, by which antitumor spectra would be controlled, were different from each other and leaving groups were Cl2, as a rule. As a result, L-1210/DDP showed cross-resistance only to two analogs. In contrast, P 388/DDP did so to all complexes tested. The resistance indices to four analogs were more than 50. Colon 26/DDP also showed cross-resistance to all of them, but the degrees of resistance in this line were lower than those in P 388/DDP. These facts revealed that the pattern of cross-resistance was dependent on each cell line and the completely different patterns were shown between the mouse leukemia resistant lines. It was suggested that in developing a new platinum analog, we should select a carrier ligand to which as many resistant lines would show lack of cross-resistance.

[Surgical treatment of massive pulmonary embolism--the time of the operation and its effectiveness].

The records of 6 patients undergoing pulmonary embolectomy for massive pulmonary embolism (MPE) at Kurume University Hospital during 17 years were reviewed to determine the management of surgery. The patients consisted of 2 men and 4 women. The patients' ages ranged from 29 to 68 years (mean age, 49.3 years). The records showed that one patient died of brain death after operation and the others survived. All the patients complained of chest pain, anterior chest discomfort and dyspnea. Sudden syncope was observed in 2 patients. Artificial mechanical ventilation was performed preoperatively on 3 patients. Right ventricular load was demonstrated on electrocardiograms and ultrasonograms. Pulmonary angiograms were attempted on two patients and one of them had cardiac arrest during this examination. MPE was suspected by perfusion defect of 50% to 80% of pulmonary vasculature demonstrated on lung perfusion scintigram in 4 patients. Open pulmonary embolectomy with cardiopulmonary bypass (CPB) was performed on all patients using crystalloid cardioplegia and topical cooling. Intraoperative pulmonary angiograms were performed in 4 patients to prevent residual thromboemboli. Since most thromboemboli originate below the level of the vena cava, acute double ligation of the vena cava just below the renal vein was performed to control recurrent embolism. Oral anticoagulant, warfarin, was administered for 3 months after embolectomy as prophylaxis against postoperative recurrent embolism. It is our opinion that an aggressive attitude toward pulmonary embolectomy on CPB is necessary to save lives of MPE patients. This surgical procedure is very easy and safe.

Evaluation of antitumor activity in a human breast tumor/nude mouse model with a special emphasis on treatment dose.

Eight lines of human breast tumors implanted in nude mice were treated with various antitumor agents at two different doses, maximum tolerated doses (MTD) and rational doses (RD) that were pharmacokinetically equivalent to the clinical doses; the response rates to both doses were compared. With MTD, the response rates to mitomycin C and vinblastine were 100%, and those to other agents including cyclophosphamide, nimustine (a water-soluble nitrosourea), vincristine, Adriamycin (doxorubicin; Adria Laboratories, Columbus, OH), 5-fluorouracil (5-FU), and methotrexate were 30%-50%, indicating high responsiveness to the former two agents. In contrast, when the RD were used, the response rates to the majority of these agents were 25%-40%, and those to vincristine and nimustine were 13% and 0%, respectively. These results agree with the reported clinical results compared with those with MTD, suggesting the importance of the use of clinically equivalent doses in the evaluation of antitumor efficacy in a human tumor/nude mouse system.

Possible origins of substance P-like immunoreactive axons within Onuf's nucleus of the cat.

The double immunofluorescence method revealed that 30-50% of substance P-like immunoreactive (SP-LI) axons within Onuf's nucleus of cats exhibited enkephalin (ENK)-LI, and that about 5% of SP-LI axons within the nucleus showed serotonin (5-HT)-LI. After hemicordotomy, axons with both SP- and 5-HT-LI, as well as those with only 5-HT-LI, were profoundly depleted in Onuf's nucleus ipsilateral to the lesion, but no changes were detectable in axons with SP-LI and/or ENK-LI in the nucleus.

Convergence of serotonin-, enkephalin- and substance P-like immunoreactive afferent fibers onto cat medullary dorsal horn projection neurons: a triple immunocytochemical staining technique combined with the retrograde HRP-tracing method.

Convergence of serotonin (5-HT)-, enkephalin (ENK)-, and substance P (SP)-like immunoreactive (LI) afferent fibers onto cat medullary dorsal horn projection neurons was examined by retrogradely labeling trigeminothalamic neurons and immunocytochemically labeling 5-HT, ENK and SP fibers in the same tissue section using distinct chromogens. Convergence of 5-HT-, ENK- and SP-LI fibers upon single projection neurons was often observed in laminae I and V.

Convergence of serotonin-, enkephalin- and substance P-like immunoreactive afferent fibers on single pudendal motoneurons in Onuf's nucleus of the cat: a light microscope study combining the triple immunocytochemical staining technique with the retrograde HRP-tracing method.

Convergence of serotonin (5-HT)-, enkephalin (ENK)-, and substance P (SP)-like immunoreactive (LI) afferent fibers on single pudendal motoneurons within Onuf's nucleus was demonstrated: pudendal motoneurons were retrogradely labeled with horseradish peroxidase applied to the pudendal nerve. Subsequently, 5-HT-LI fibers were stained by the immunoperoxidase method, and then ENK- and SP-LI fibers were stained by the double immunofluorescence method.
