Absolute structure-cytotoxic activity relationships of steganacin congeners and analogues.

The cytotoxic activities of optically pure and racemic steganacin congeners and analogues against KB cells in culture and the inhibitor activity of cilia regeneration in Tetrahymena were studied with regard to absolute and relative configurations. The stereochemical requirements of dibenzocyclooctadiene lignan lactones for activity were clarified.

Changes in organization and axonal transport of cytoskeletal proteins during regeneration.

Changes in solubility and transport rate of cytoskeletal proteins during regeneration were studied in the motor fibers of the rat sciatic nerve. Nerves were injured by freezing at the midthigh level either 1-2 weeks before (experiment I) or 1 week after radioactive labeling of the spinal cord with L-[35S]methionine (experiment II). Labeled proteins in 6-mm consecutive segments of the nerve 2 weeks after labeling were analyzed following fractionation into soluble and insoluble populations with 1% Triton at 4 degrees C. When axonal transport of newly synthesized cytoskeleton was examined in the regenerating nerve in experiment I, a new faster component enriched in soluble tubulin and actin was observed that was not present in the control nerve. The rate of the slower main component containing most of the insoluble tubulin and actin together with neurofilament proteins was not affected. A smaller but significant peak of radioactivity enriched in soluble tubulin and actin was also detected ahead of the main peak when the response of the preexisting cytoskeleton was examined in experiment II. It is thus concluded that during regeneration changes in the organization take place in both the newly synthesized and the preexisting axonal cytoskeleton, resulting in a selective acceleration in rate of transport of soluble tubulin and actin.

Two 68-kDa proteins in slow axonal transport belong to the 70-kDa heat shock protein family and the annexin family.

The major 68-kDa protein found selectively in the faster of the two subcomponents of slow axonal transport [group IV or slow component b (SCb)] in the rat sciatic nerve has been characterized. It was found to contain two distinct classes of proteins, S1 and S2, both of which have isoelectric points of 5.7, but differ in their solubility in the presence of calcium. The S1 protein, which contributes up to 70% of the 68-kDa component, was soluble in the presence or absence of calcium, whereas the S2 protein was bound to the cytoskeleton in a calcium-dependent manner. Further characterization of the two proteins by peptide mapping and immunological methods revealed that the S1 protein belonged to a family of proteins related to the 70-kDa heat shock protein, whereas the S2 protein was identical to 68-kDa calelectrin (annexin VI). Selective occurrence in SCb of these proteins with potential abilities to regulate protein-protein or protein-membrane interactions suggests that they may play important roles in the control of cytoskeletal organization in the axon, because SCb contains mainly cytoskeletal proteins in a more dynamic form compared with the slowest rate component, slow component a, which is enriched in the stably polymerized form of these proteins.

[A case of smoldering adult T-cell leukemia complicated by various pulmonary infections].

A 46-year-old man was admitted to Oita Medical College Hospital on October 16, 1987, because of cough and sputum. Chest X-ray and chest CT films showed diffuse reticulonodular shadow. The specimens obtained by transbronchial lung biopsy revealed cysts of pneumocystis carinii. Abnormal lymphocytes with lobulated nuclei were found 2-7% of peripheral leucocytes. The anti HTLV-I antibody was positive. According to these data, we diagnosed the patient as smoldering adult T cell leukemia with pneumocystis carinii pneumonia. The abnormal shadow on chest X-ray disappeared after SMX-TMP and pentamidine treatment. After about 1 year, he was again admitted for high fever. Chest X-ray showed infiltration with cavity in right upper lobe. Streptococcus pneumoniae was isolated from the sputum. The infiltration shadow on chest X-ray disappeared after antibiotics treatment. However, multiple nodular shadow appeared on the chest X-ray and ATL cell infiltration was found in the specimens of transbronchial lung biopsy. ATL cells in peripheral blood also increased and serum LDH and Calcium levels were markedly high. According to these data, we diagnosed the patient as having a ATL crisis. Although chemotherapy for ATL was started, the ATL, cell infiltration shadow on the chest X-ray enlarged, and bilateral diffuse patchy shadows was appeared on the chest X-ray. He died of respiratory failure on April 26, 1989. Cytomegalovirus pneumonia and ATL cell infiltration were revealed by necropsy.

[A clinical study on liver dysfunction in patients with acute measles infection].

Liver dysfunction in patients with measles infection is reported commonly in European and American literatures, but those in Japan are relatively rare. We observed the abnormal elevation of serum transaminase in 17 of 18 juvenile patients with measles from December 1989 to February 1990, however severe complications such as pneumonia or meningitis were not observed. A diagnosis of measles was made by typical clinical symptoms and the elevation of IgM antibody against measles (EIA method). Ninety four percent of the patients showed the elevation of serum transaminase, (GOT 118.6 +/- 96.2 IU/l, GPT 161.5 +/- 167.6 IU/l), and all patients recovered completely in 30.5 +/- 12.8 days after the onset. The abnormal evaluation of serum LDH was seen in 94% of the patients, (mean value was 872.2 +/- 216.2 IU/l). LDH4 mainly elevated in most cases, however, LDH5 elevated only in 4 cases. The intensity of liver dysfunction did not correlate to the severity of measles in the present cases. It is considered to be characteristic to the patients with measles that the serum level of LDH is markedly high as compared with that of transaminase.

Double-blind study of the chronopharmacotherapy of depression.

To investigate the possibility of chronotherapy with antidepressants for patients with depression, we gave single daily doses of clomipramine 150 mg/day to 30 patients with depression at three different times of day, i.e., morning, noon, or before bedtime, using a double-blind method over a 4-week period. Beneficial effects differed according to the administration time of day, with the most effective result being found for the administration at noon. Time-dependent differences in side effects were observed in tremors and dryness of mouth. An additional 10 patients were administered their medications three times a day by traditional, equally divided doses, and the efficacy was inferior than daily single doses at noon. The study results showed the significance of the administration time of day for the benefits and side effects of antidepressant therapy for depression.

Re-examination of the topographical distribution of motoneurons innervating the digastric muscle in the rabbit and guinea pig.

The topographical distribution of motoneurons innervating the digastric muscle in the rabbit and guinea pig was re-examined by the retrograde tracing method of HRP (horseradish peroxidase). Motoneurons innervating the anterior belly of the digastric muscle of the rabbit and guinea pig constituted a longitudinal cell column in the ventromedial part of the motor nucleus of the trigeminal nerve. Motoneurons innervating the posterior belly of the digastric muscle were localized in the accessory facial nucleus. No motoneurons supplying the digastric muscle were found within the main facial nucleus.

[The effect of human growth hormone on protein metabolism in the surgically stressed state].

The effects of human growth hormone (HGH) on protein metabolism were investigated. In the experimental study, thirty one male SD rats receiving TPN were divided into 2 groups (control group & HGH group). Cumulative nitrogen balance after burn in HGH group was significantly higher than in control group. (p less than 0.05) Rates of whole body protein turnover (Q), together with those of synthesis (S) and breakdown (B) were significantly higher in HGH group than in control group. (p less than 0.01) Nitrogen balance significantly correlated with urinary total catecholamine excretion in both groups. (p less than 0.01) The difference of correlations of nitrogen balance to urinary total catecholamine excretion between two groups was statistically significant (p less than 0.01) when compared by analysis of covariance. In the clinical study, 12 patients after sub-total esophagectomy receiving TPN were divided into 2 groups (control group & HGH group). Cumulative nitrogen balance after operation in HGH group was significantly higher than in control group. (p less than 0.01) Q,S, and B were higher in HGH group than in control group. Increase of S was statistically significant. (p less than 0.01). These results indicate that HGH may be greatly beneficial in improving protein metabolism in the surgically stressed state.

[Intra-graft balloon pumping--a clinical case report].

A 29-year-old female underwent tube graft replacement of distal aortic arch and descending aorta for dissecting aneurysm. After 42 minutes of aortic cross-clamping the patient was initially weaned satisfactorily from cardiopulmonary bypass (CPB). However, cardiac arrhythmia and cardiac arrest necessitated reestablishment of CPB. Electro-cardiography showed inferior myocardial infarction. To wean CPB intraaortic balloon pumping (IABP) was mandatory. But because of dissecting aortic aneurysm IABP in conventional method was contra-indicated. Intra graft balloon pumping (IGBP) was initiated while the patient was on full CPB. A low-porosity woven Dacron tube graft (22 mm) was anastomosed end-to-side to ascending aorta. A balloon was inserted into the tube graft to establish IGBP. This IGBP provided effective circulatory assist. The patient was weaned from CPB 1 hours after reestablishment of IGBP. Postoperative course was stable. The patient was returned to the operating room for removal of the balloon 3 days postoperatively. We reported the case for whom IGBP was effective. IGBP was effective circulatory support for the patient when conventional use of IABP was contra-indicated.

[Catabolism of lipoprotein-X (Lp-X) induced by infusion of 10% intralipid].

In order to clarify the metabolism of Lipoprotein X (Lp-X) induced by intravenous Intralipid 10%, in vitro experiments using purified Lp-X from the sera of the patients receiving Intralipid 10% were carried out. 1) Lp-X or high density lipoprotein (HDL) was incubated with J-774 macrophages laden with [3H] cholesterol. Marked extraction of cholesterol from macrophages by Lp-X as well as HDL was observed. 2) [3H] cholesterol labelled Lp-X or oxidized LDL (o-LDL) was incubated with J-774 macrophages. Incorporation of Lp-X into macrophages was negligible comparing to o-LDL. 3) [3H] cholesterol labelled Lp-X, low density lipoprotein (LDL), or HDL was incubated with Hep G2 cells was less than LDL, but similar to that of HDL. These results indicated that Lp-X extracted cholesterol from peripheral tissues during its formation, and it was not catabolized by the scavenger pathway, but catabolized by the LDL pathway of hepatocytes.

[Routine use of the internal mammary artery for aorto-coronary bypass surgery].

Internal mammary artery (IMA) grafts have excellent long-term patency rate and result in improved late survival. Conventional use of IMA was only for selected cases. Elder or acute myocardial infarction case had been excluded from IMA use. During past 30 months we extended the indication of the IMA use for all the ACB case. The present study was undertaken to assess the possibility of routine use of the a IMA for aorto-coronary bypass (ACB). From Nov. 1987 through May 1990, we performed consecutive 110 ACB with the protocol "routine use of the IMA for all the ACB surgery". The 78 men and 32 women (29%) had a mean age of 64 years old (range 38 to 80 years old). Thirty-five patients (32%) were 70 years of age or older. Eighty-four patients had stable angina, nineteen patients had unstable angina, seven patients had acute myocardial infarction. IMA was used in 106 patients (96%). An average of 3.6 coronary arteries per patient were bypassed, and 1.6 distal anastomosis per patient were made with IMA. Combined cardiac surgery was performed in 8 cases (ventricular septal rupture repair: 2 cases, coronary endarterectomy: 4 cases, left ventricular aneurysm resection: 1 case, mitral commissurotomy: 1 case). Post operative complication due to IMA use were infrequent and operative mortality was 6.3%. In summary, routine use of the IMA for ACB surgery was possible method as if in high risked cases (elder or poor left ventricular function). We believe this method result in improved late result. We concluded routine use of the IMA for ACB was possible and usefull method.

Representation of the tensor veli palatini muscle in the trigeminal motor nucleus of the Japanese monkey (Macaca fuscata).

Distribution of motoneurons supplying the tensor veli palatini (TVP) muscle was examined in the Japanese monkey (Macaca fuscata) by the retrograde horseradish peroxidase (HRP) method. Neurons labeled with HRP which was injected into the tensor veli palatini muscle were seen in the ventromedial aspects of the dorsolateral division of the trigeminal motor nucleus, at all rostrocaudal levels of the trigeminal motor nucleus. The vast majority of these TVP motoneurons were distributed around the margin, especially the dorsal margin, of the cluster of motoneurons which innervate the lateral pterygoid muscle.

Distribution of axons showing both enkephalin- and serotonin-like immunoreactivities in the lumbar cord segments of the Japanese monkey (Macaca fuscata).

Axons exhibiting both enkephalin- and serotonin-like immunoreactivities were observed by the double immunofluorescence method in the lumbar cord segments of the Japanese monkey. Double-labeled axons were seen almost exclusively in the dorsal horn, particularly in lamina I, lamina IIa and the lateral part of lamina V. After thoracic cordotomy, almost all of the double-labeled axons disappeared from the lumbar cord segments.

Infrahyoid and accessory motoneurons in the Japanese monkey (Macaca fuscata).

The segmental and topographical organization of motoneurons innervating the infrahyoid (IH) and the spinal accessory (AC) muscles was studied in the Japanese monkey (Macaca fuscata) with the retrograde horseradish peroxidase (HRP) method after application of HRP to the peripheral nerve branches supplying the IH and AC muscles. IH motoneurons constitute two distinct slender cell columns, a longer medial and a shorter lateral one. The medial cell column extends from the most caudal level of the hypoglossal nucleus to the lower levels of the second cervical (C2) cord segment. In the medial column, motoneurons supplying the sternohyoid and sternothyroid muscles are distributed at the medullary and C1 levels, while those innervating the omohyoid muscle are primarily distributed at the C2 level. The lateral cell column consists of motoneurons supplying the thyrohyoid muscle and extends from the most caudal level of the hypoglossal nucleus to the middle levels of the C1 cord segment. Axons of thyrohyoid motoneurons follow a dorsomedially directed bent emergent course, making a hairpin turn. AC motoneurons supplying the sternocleidomastoid (SC) and trapezius (TZ) muscles form a single slender cell column extending from the most rostral level of the pyramidal decussation to the middle levels of the C6 cord segment. SC motoneurons are distributed from the most rostral level of the pyramidal decussation to the middle levels of the C3 cord segment, while TZ motoneurons are distributed from the upper levels of the C2 cord segment to the lower levels of the C6 cord segment. At the levels of the C2 and C3 cord segments, both SC and TZ motoneurons are distributed in the AC cell column; the cluster of SC motoneurons is located dorsomedial to that of TZ motoneurons.
