[Teratogenicity study on halopredone acetate in rats].

A teratogenicity study of halopredone acetate (THS-201), a synthetic corticosteroid, was carried out using Jcl: Wistar rats. Pregnant rats were treated subcutaneously in doses of 0.1, 0.5, 2.5 and 12.5 mg/kg/day, from day 7 to day 17 of gestation. Two-thirds of pregnant rats were killed on day 20 of gestation to examine the development of fetuses, and remaining rats were allowed to litter naturally in order to investigate the postnatal development of offspring. The results obtained from the present study were as follows. During the gestation and lactation periods, there occurred a decrease in the maternal body weight gain in 2.5 and 12.5 mg/kg groups. No influences of THS-201 administration were observed on gestation, delivery and lactation of dams. No external, visceral and skeletal anomalies attributable to THS-201 were observed in the fetuses. THS-201 administration did not have any influences on viability and development, various functions such as reflex response, learning and reproductive performance of F1 generation, and further on development of F2 generation. Therefore, it was concluded that the non-effect dose of THS-201 for the reproduction of dams and development of F1 generation was 12.5 mg/kg/day.

A case of primary systemic amyloidosis.

An adult patient with primary systemic amyloidosis associated with nephrotic syndrome and complete atrial-ventricular (A-V) block is described. This case was considered to be a slowly progressive primary amyloidosis because of a long time episode of poor appetite and nephrotic syndrome. Temporal pace making of the heart was not effective presumably due to heart muscle damage, pér sé. Autopsy specimens revealed typical features of primary amyloidosis in light microscopy and immunofluorescence. It is suggested that early detection of amyloid fibers in the heart muscles may improve prognosis in patients with primary amyloidosis.

[A multi-institutional study on postoperative adjuvant immunochemotherapy of gastric cancer].

An analysis of 3,630 gastric cancer cases from 412 institutions who were treated with 6 randomly-assigned protocols was carried out to study the effect of daily PSK and Futraful administration and the adjuvant effect of PSK and/or Picibanil (OK 5 KE) combined with MMC and FT. Evaluation of immunochemotherapy was made on the basis of three-year survival rates and immunoparameters. The most positive results were found in survival rates favoring the combined therapy (FT + PSK) over single administration (FT) to relatively early-stage patients (30%, difference), PSK, MMC and FT combination (10%), and PSK, OK, MMC and FT (22%), over single chemotherapy alone. Serum alpha-2 globulin levels, indicative of immunosuppression, were lower in PSK-administered cases. The overall findings indicated that PSK and/or OK with MMC and FT are effective in prolonging the lives of post-operative patients.

[The usefulness of prolonged high dose delayed contrast computed tomography for the diagnosis of intracranial angiographically occult vascular malformations].

It is difficult to diagnose angiographically occult vascular malformations. When conventional CT (plain & contrast enhancement) can not demonstrate the lesions, it was impossible to diagnose in the past. We developed a new technique which enabled us to diagnose them. Materials and methods; Seven cases of spontaneous intracerebral hemorrhage, in which no abnormality was detected by repeated magnified serial angiography with subtraction technique and prolonged injection technique, were examined. In each case, conventional CT (plain & enhancement using 100 ml of 60% meglumine iothalamate) was performed first, which was immediately followed by the administration of 220 ml of 30% meglumine iothalamate for one hour. CT is taken at the end of the infusion. It, we call, is prolonged high dose delayed contrast CT: PHDD-CT. Total dose of iodine used in this technique was 59.22 gI (1.0 gI/kg body weight: BW). Results; Contrast enhancement effect of PHDD-CT was much better than that of conventional CT in all the cases. In three cases, the lesions were more clearly delineated in PHDD-CT. In four cases, only PHDD-CT could demonstrate the lesions. No side effect was observed. Comments; Several techniques for better enhancement have been reported, however they used large volume of contrast medium such as 1.5 or 2.0 gI/kg BW. Our technique can be performed with more security. Based on our good results, we recommend to use this PHDD-CT technique for the diagnosis of angiographically occult vascular malformations.

[Giant aneurysm of the right coronary artery with complete proximal obstruction: a case report].

A 46-year-old woman was admitted to our hospital for evaluation of oppressive chest pain and ECG abnormalities. Her ECG on admission showed old inferior myocardial infarction. Chest radiography on admission indicated cardiomegaly (cardiothoracic ratio was 60%). The echocardiogram showed compression of the right atrium, right ventricle, and tricuspid annulus by a tumor which was located between the liver and the heart. The tumor's echo density was heterogenous. Cardiac computed tomography showed compression of the tricuspid annulus by the tumor which was located anterior to the right atrium and the right ventricle. The tumor registered 65.1 HU and was not enhanced by contrast medium. These findings strongly suggested a giant right coronary aneurysm filled with thrombus, and this diagnosis was confirmed at surgery and pathologically. Coronary aneurysms are rarely diagnosed during life, and usually are incidental findings during coronary arteriography performed to evaluate myocardial infarction or angina pectoris. The coronary aneurysm of the present case was clearly and easily recognized using computed tomography and appeared as a pericardial tumor. Computed tomography may be helpful for observing the growth of tumor and thrombus formation within it.

Hemodynamic effect of cilostazol on increasing peripheral blood flow in arteriosclerosis obliterans.

The novel synthetic antithrombotic drug, cilostazol (6-[4-(1-cyclohexyl-1H-tetrazol-5-yl)butoxy]-3,4-dihydro-2(1H)-qui nolinone, OPC-13013) was orally administered in a dose of 200 mg/day to 4 patients with arteriosclerosis obliterans (ASO) in the lower extremities to evaluate the hemodynamic effects of the drug. After 2 weeks of treatment, tissue blood flow through the pedal vessels was measured in 8 legs by the venous occlusion method using a strain-gauge plethysmograph. The flow measured during reactive hyperemia was shown to have significantly increased from 5.04 +/- 1.90 to 9.05 +/- 4.77 ml/min/100 g tissue (p less than 0.05, t-test), the rate being 80.68 +/- 66.61% of the initial level. The drug was, therefore, considered to have high clinical potential in the treatment of chronic arterial occlusion such as ASO and Buerger's disease (thromboangiitis obliterans). The drug was also shown to be of value in supporting the reconstruction of circulation in the lower extremities.

Primary structure of beta subunit precursor of calf muscle acetylcholine receptor deduced from cDNA sequence.

Clones harbouring cDNA sequences for the beta subunit precursor of the acetylcholine receptor from calf skeletal muscle have been isolated. Nucleotide sequence analysis of the cloned cDNA has disclosed the primary structure of this polypeptide, which consists of 505 amino acids including a hydrophobic prepeptide of 24 amino acids. The beta subunit of the calf muscle acetylcholine receptor, like the alpha and gamma subunits of the same receptor and the alpha subunit of its human counterpart, exhibits structural features common to all four subunits of the Torpedo electroplax receptor, apparently being oriented across the membrane in the same manner as proposed for the fish receptor subunits. The degree of sequence homology between the calf and Torpedo beta subunits (59%) is comparable to that between the gamma subunits (56%), but is lower than that between the alpha subunits of the two species (81%). Some regions of the beta subunit molecule, including the region corresponding to the putative acetylcholine binding area on the alpha subunit and the region encompassing the clustered putative transmembrane segments M1, M2 and M3, are relatively well conserved between the two species.

Primary structure of gamma subunit precursor of calf-muscle acetylcholine receptor deduced from the cDNA sequence.

Clones carrying cDNA sequences for the gamma subunit precursor of the acetylcholine receptor from calf skeletal muscle have been isolated. Nucleotide sequence analysis of the cloned cDNA has revealed the primary structure of this polypeptide, which consists of 519 amino acids including a hydrophobic prepeptide of 22 amino acids. The gamma subunit of the calf muscle acetylcholine receptor, like the alpha subunit of the calf as well as the human muscle receptor, shares features characteristic of all four subunits of the Torpedo electroplax receptor, such as the putative disulphide bridge corresponding to that in the alpha subunit proposed as being in close proximity to the acetylcholine binding site and the four putative, hydrophobic transmembrane segments M1-M4. Thus, the calf gamma subunit molecule apparently exhibits the same transmembrane topology as proposed for the fish receptor subunits. The degree of sequence homology between the calf and Torpedo gamma subunits (56%) is lower than that between the alpha subunits of the two species (81%). Verification of protein sequence by fast atom bombardment mass spectrometry. Amino acid sequence of protein S, a development-specific protein of Myxococcus xanthus.

A mass spectrometric method was applied to protein S, a development-specific protein of Myxococcus xanthus, in order to verify the amino acid sequence deduced from the nucleotide sequence of its gene. On examining proteolytic digests of the protein by fast atom bombardment mass spectrometry without separation of individual peptides, signals corresponding to individual peptides were observed in the mass spectra. The mass values of the observed signals were correlated to the theoretical mass values calculated from the predicted amino acid sequence, thereby identifying the peptides and proving the accuracy of the sequence. The previous finding that protein S is the product of the second gene of the two tandemly repeated genes on the M. xanthus chromosome ( Inouye , S., Franceschini , T., and Inouye , M. (1983) Proc. Natl. Acad. Sci. U.S.A. 80, 6829-6833) is now unambiguously confirmed.
