Sequential changes in high frequency notches on QRS complexes in progressive muscular dystrophy of the Duchenne type--a 3-year follow-up study.

In a 3-year follow-up study, sequential changes in the number of high frequency notches (HFN) on QRS complexes were investigated on 72 cases with progressive muscular dystrophy of the Duchenne type (PMD). The patients were classified into eight stages from the mildest, S(1), to the most severe, S(8), according to Swinyard-Deaver's criteria. The number of HFN progressively increased with advancing Swinyard-Deaver's stage. In the terminal stage, however, the number of HFN tended to decrease. The present observations show that the patient tends to have a small number of HFN in the early stages of the disease; with increasing severity of the disease he develops a larger number; in the terminal stage he again exhibits relatively few notches. It is concluded therefore that significant increases or decreases in the count of HFN on QRS complexes can be useful indicators for estimating the extent and severity of cardiac involvement in PMD.

Clinical significance of titered circulating microbial antibody in bacteremia.

Using an indirect immunofluorescent antibody technic, the authors determined the serum microbial antibody titer (MAT) in 26 patients (five with infective endocarditis, eight with septicemia without endocarditis, and three with access device-related bacteremia, three with fungemia without endocarditis, and seven with false-positive blood cultures). The MAT in the early clinical stage of infective endocarditis (IE) exceeded 350; after appropriate antibiotic therapy, it decreased to below 50 with clinical improvement. MAT in patients with septicemia was above 20 and less than 200; in patients with bacteremia, above 10 and less than 50; and in patients with false-positive blood cultures, it was less than 10. The authors suggest that MAT against isolated bacteria may be useful in diagnosing septicemia with or without IE, in determining the appropriate length of treatment, and in differentiating bacteremia from false-positive blood cultures.

Dissolution characteristics of Pu-contaminated soils and sediments in lung serum simulant solution.

Dissolution characteristics of Pu from contaminated Nevada Test Site (NTS) and Rocky Flats (RF) soils, and Mound Laboratory (ML) and Oak Ridge National Laboratory (ORNL) sediments in lung serum simulant solution at 37 degrees C were investigated. The dissolved Pu concentration had reached a maximum within a day of equilibration and the percent dissolved Pu at the maximum was 0.70 (RF), 0.43 (ML), 0.02 (ORNL), and 0.02 (NTS). The Pu concentrations of the RF, ML and ORNL samples in the successively extracted solutions decreased drastically but the concentration in the NTS soil extracts did not change significantly. The differences in Pu dissolution among the samples were caused by the differences in the total Pu concentration, particle size distribution, and chemical nature of Pu in contaminated soils and sediments. The higher solubility of the particulate Pu form in the RF soil relative to the ORNL sediment contaminated by dissolved Pu suggests that contamination source alone can not explain the observed differences. Variation of Pu solubility among the samples indicates that a single solubility class for dose assessment use may not be appropriate, particularly if one attempts to make comparative assessments among different sites.

Sequence analysis of the heat-labile enterotoxin subunit B gene originating in human enterotoxigenic Escherichia coli.

In this study, we determined the amino-terminal coding sequence, covering the signal peptide and the amino-terminus of the mature peptide, of the heat-labile enterotoxin subunit B (LT-B) gene originating in human enterotoxigenic Escherichia coli. Neither the signal sequence nor the amino-terminal sequence of the mature LT-B was identical to those sequences from porcine enterotoxigenic E. coli, but there was an extensive homology.

Dynamics of cardiac, respiratory, and metabolic function in men in response to step work load.

Stroke volume, heart rate, cardiac output, tidal volume, respiratory frequency, minute ventilation, end-tidal tensions of O2 and CO2, O2 uptake, CO2 output, and respiratory exchange ratio were measured simultaneously in healthy male volunteers before, during, and after upright bicycle exercise from 0 to 360 and 720 kpm/min. The circulatory variables were determined continuously once per 20 cardiac cycles and the respiratory variables breath by breath using separate computer-based systems in which an impedance pneumograph and an impedance cardiograph were incorporated. Stroke volume, heart rate, and cardiac output started to increase without measurable delay at the onset of exercise. Stroke volume increased by 20% from resting control value in response to the mildest exercise and essentially leveled off with a further increase in work load. Time constant for cardiac output increased with the increasing work load. Time constant for minute ventilation was much longer than that for cardiac output and independent of work intensity. A good synchronization between the ventilation and cardiac output responses at an initial period of transitions from rest to exercise and from exercise to rest seems to support the concept of cardiodynamic hyperpnea.

Perifoveal capillary network and visual prognosis in diabetic retinopathy.

Fluorescein angiography was carried out in 93 eyes with diabetic maculopathy and the state of the perifoveal capillary network within the zone of one half the disc diameter was studied. Nonperfusing capillary areas were found and the condition was classified into two grades: grade 1, the nonperfusing area less than 50% of the circumference of the network, and grade 2, more than 50% of the network. The visual prognosis was followed for 2 years. The initial and final visual acuities were significantly lower in the grade 2 than in the grade 1 group. There were more eyes showing visual deterioration during the follow-up period in the grade 2 than in the grade 1 group. It was thought that the condition of perfusion of the perifoveal capillary network is one of the determining factors of the visual prognosis in diabetic maculopathy.

Ouabain-sensitive insulin stimulation of ion and glucose transport in rat ventricular slices.

This study was designed to determine whether ouabain could inhibit insulin-stimulated ion and/or glucose transport in rat ventricular slices. In the slices which were obtained by tearing with forceps from the inside surface of the ventricle, insulin (0.1-100 ng/ml, 20-min treatment) stimulated ATP hydrolysis (nmol inorganic phosphate/mg tissue/20 min) and 86Rb uptake (nmol Rb/mg tissue/8 min) in a concentration-dependent fashion by up to about 25%, compared with control experiment. About 10-fold higher concentrations of insulin stimulated glucose-[14C] uptake by the slices (nmol glucose/mg tissue/30 min). Both the stimulant effects of insulin on ion and glucose transport were inhibited by ouabain (0.1-1 mM, 20-min treatment) in a similar concentration-response relationship. It thus never appears that two or more independent transmembrane signals would be generated simultaneously in response to the insulin-receptor interaction to operate ion and glucose transport systems in rat cardiac muscle. In addition, rat ventricular slices were shown to provide a sensitive tool for the evaluation of insulin effects on ion transport systems in the muscle.

The effect of hypofibrinogenemia with batroxobin on rat Masugi nephritis.

The effect of hypofibrinogenemia with batroxobin on proliferative glomerular lesions of Masugi nephritis was examined in the rat. Batroxobin-treated rats had a relatively low level of plasma fibrinogen and a reduction in glomerular fibrin deposition. Mean nuclear counts in the outer cortical glomeruli were less in the batroxobin-treated group than in the untreated group on the 7th day. However, the difference in the overall severity of the glomerular histologic lesions between the two groups was not significant. Continual injections of the same dosage of batroxobin failed to maintain hypofibrinogenemia, possibly due to the antibody formation. These results suggest that relatively low plasma levels of fibrinogen are obviously not effective enough to prevent the development of the proliferative lesions in rat Masugi nephritis.

Pretreatment serum albumin concentration and lactic dehydrogenase activity as prognostic factors in patients with small cell lung cancer.

Pretreatment laboratory parameters were analyzed as prognostic factors in patients with small cell lung cancer. Serum lactic dehydrogenase activity, serum albumin concentration, PPD skin reaction, and peripheral lymphocyte count were of prognostic importance. When these factors were evaluated by multivariate analysis together with performance status and disease extent, lactic dehydrogenase and albumin were the most influential factors related to survival.

Bone marrow examination for detection of metastasis in patients with bronchogenic carcinoma: an evaluation of 107 patients.

As a staging procedure before treatment, examination of bone marrow from the posterior iliac crest was performed on a total of 107 patients with bronchogenic carcinoma. Among them, 11 patients (10.3%) had metastasis in the bone marrow: five of 39 adenocarcinomas, five of 33 small cell carcinomas, one of four large cell carcinomas, and none of 31 epidermoid carcinomas. Leukoerythroblastosis was found exclusively in the patients with metastasis, although the presence of tumor cells in the bone marrow did not correlate well with peripheral blood cell counts. Survival following an intensive chemotherapy in patients with bone marrow metastasis was substantially longer for those with small cell carcinoma than for those with other histologic types of bronchogenic carcinoma.

The dynamic response of the cardiopulmonary parameters to passive head-up tilt.

Transitional changes in stroke volume, heart rate, and cardiac output resulting from head-up tilt were measured in healthy male subjects, using impedance cardiography. Respiratory frequency, tidal volume, ventilation rate, end-tidal O2 and Co2 tensions, O2 uptake, CO2 output, and exchange ratio were also determined breath-by-breath. When the subject was tilted upward from a supine to an upright position, the stroke volume was decreased while the heart rate was increased. The time required to attain the steady state was shorter in the stroke volume change than in the heart rate. In addition, since the heart rate was not increased so high as to be inversely related to the stroke volume, the cardiac output decreased by tilting upward. The ventilation rate remained almost unchanged within a tilt angle of 60 degrees or so. Thus, both the O2 and CO2 fractions in expired air showed transitory changes. As the original body position was restored, the cardiac output was increased to the initial level, the tidal volume showing a transient increase. During the recovery process, PCO2, in expired air increased and PO2 decreased, showing that the change in cardiac output exceeded that in ventilation in quantity. The change in ventilation moreover, preceded that of expired PCO2 by ca. 10 sec, suggesting that the enhancement of ventilation occurred through some agent(s) other than the chemical drive.

Post-translational glycosylation of coronavirus glycoprotein E1: inhibition by monensin.

The intracellular sites of biosynthesis of the structural proteins of murine hepatitis virus A59 have been analyzed using cell fractionation techniques. The nucleocapsid protein N is synthesized on free polysomes, whereas the envelope glycoproteins E1 and E2 are translated on the rough endoplasmic reticulum (RER). Glycoprotein E2 present in the RER contains N-glycosidically linked oligosaccharides of the mannose-rich type, supporting the concept that glycosylation of this protein is initiated at the co-translational level. In contrast, O-glycosylation of E1 occurs after transfer of the protein to smooth intracellular membranes. Monensin does not interfere with virus budding from the membranes of the endoplasmic reticulum, but it inhibits virus release and fusion of infected cells. The oligosaccharide side chains of E2 obtained under these conditions are resistant to endoglycosidase H and lack fucose suggesting that transport of this glycoprotein is inhibited between the trans Golgi cisternae and the cell surface. Glycoprotein E1 synthesized in the presence of monensin is completely carbohydrate-free. This observation suggests that the intracellular transport of this glycoprotein is also blocked by monensin.

An on-line computer system for monitoring respiratory and cardiac functions of patients.

A computer-based noninvasive and continuous patient monitoring system for breath-by-breath assessment of cardiorespiratory functions was developed. Transthoracic impedance changes caused by respiratory and cardiac activities were measured with separate impedance plethysmographs of different frequencies. A mini-computer system calculated on-line eight respiratory parameters (VT, f, VE, PETO2, PETCO2, VO2, VCO2, and R) in cooperation with expired gas analysis data from a mass-spectrometer, and three cardiac parameters (SV, HR and Q). These parameters were displayed simultaneously at the end of each breath. In order to improve the signal-to-noise ratio of the impedance cardiogram to a level allowing computer determination, an ensemble averaging technique was utilized. The computer-derived cardiorespiratory parameters obtained during voluntary hyperventilation and a passive tilt demonstrated reasonable changes.
