A 60-year old woman with metastatic malignant melanoma who was well responded to a combination chemotherapy including DTIC was reported. She was noted a lentigo in the left first toe and histological examination revealed malignant melanoma in October 1978. Amputation of the left lower leg and dissection of the left inguinal lymph nodes had been done. OK-432 was injected as postoperative immunotherapy. She was readmitted to our hospital with the symptoms of pain and numbness of the left arm. Physical examination revealed a palpable mass in the left supraclavicular region. Incisional biopsy of the supraclavicular mass revealed metastatic malignant melanoma. She received a combination of 100mg DTIC i.v. for 5 days, 100 mg ACNU i.v. for one day and 1 mg VCR i.v. for one day (DAV chemotherapy) postoperatively. Subcutaneous injection of OK-432 with the dose of 5 KE per week was continued. Major side effects of DAV chemotherapy were nausea and transient leukocytopenia. No serious side effects were observed. On completion of the first course of DAV chemotherapy, abnormal shadow of the left apex was completely disappeared and on completion of the third course of DAV chemotherapy, high density area was markedly decreased in the cervical CT. She gained symptomatic reliefs and was discharged in August 1983. The combination chemotherapy including DTIC appeared to be effective in the treatment of metastatic malignant melanoma.

The following investigations concerning scoliosis are in progress at Tokushima University: (1) a study of the incidence of equilibrial dysfunction; (2) a study of the nature of dysequilibrium, with special reference to the origin (functional or organic); (3) a field survey of scoliosis associated with equilibrial dysfunction in school children; (4) a study of experimental and clinical metabolic disturbances in scoliosis; (5) a study of postural reflex in experimental animals; (6) a study of scoliosis in animals with experimentally produced brain stem lesions; and (7) a study of scoliosis in animals with experimentally produced lesions in the posterior part of the hypothalamus. The hypothetic assumption is that dysfunction of the postural reflex regulation induced by functional or organic disorders at the brain stem center may play an important part in the development of idiopathic scoliosis during the period of rapid growth in children.

A method for enzymatic assay of imidazole acetic acid (ImAA) was developed, based on the strict substrate specificity of imidazole acetate monooxygenase from Pseudomonas sp. [Maki et al. (1969) J. Biol. Chem., 244., 2942-2950], which catalyzes concomitant conversion of NADH to NAD+. Thus, ImAA was determined by measuring decrease in absorbancy at 340 nm. Tissue extracts were partially purified and/or concentrated by column chromatography on Bio-Rad AG-1 before enzymatic assay. The lowest measurable level of ImAA by this method was 2 nmol.
The concentrations of the trace elements iron, zinc, copper, and manganese were determined in 53 regular infant formulas and 41 special infant formulas used for clinical disorders. Several infant formulas had lower concentrations of trace elements than human milk. The ranges of concentrations found in the formulas were as follows: Iron, 0.0 to 57.5 mg/L; copper, 0.01 to 2.14 mg/L; zinc, 0.10 to 13.5 mg/L; and manganese, 0.0 to 7.8 mg/L. In addition to the wide variation in absolute amounts of the trace elements in the formulas, large variations in the ratios of trace elements were found. While the average ratios of zinc/copper, zinc/iron, and iron/manganese in human milk range from 3.3 to 10, 2.5 to 10, and 25 to 100, respectively, they ranged from 0.4 to 74, 0.02 to 40, and 0.04 to 425 in the formulas. These differences in the ratios may be important, as the ratio can affect the absorption of the individual elements. These findings indicate that more attention should be given to the trace element levels in infant formulas.
In order to investigate the relation between regional myocardial function and electrical changes during myocardial ischemia in humans, left ventricular echocardiogram and ECG were recorded during supine ergometer exercise in 16 patients with coronary artery disease until angina or ischemic ECG appeared. During exercise in 13 patients a decrease of left ventricular posterior wall excursion or of interventricular septum excursion occurred earlier than the appearance of ST-T change on ECG. In two patients the excursion decreased at the same time as the ST-T change. Significant regional myocardial dysfunction occurred at 30 +/- 15 seconds and ECG changes occurred at 90 +/- 60 seconds (p less than 0.001). Percent systolic wall thickening occurred in similar fashion. Results support the hypothesis that regional contractile abnormalities may provide a more sensitive indicator of myocardial ischemia than ST-T changes in humans.
To study the function of pp60src, the transforming protein encoded for by Rous sarcoma virus, we have raised antibodies against synthetic oligopeptides corresponding to the primary structure of pp60src. All eight investigated peptides were immunogenic in rabbits, and four induced pp60src-specific antibodies. We screened tumor-bearing rabbit (TBR) sera for antibodies against the peptides; this revealed that five out of six of the peptides, chosen according to a high hydrophilicity plot, were related to epitopes of native pp60src, in contrast to two peptides of low hydrophilicity, which contained a cleavage site for protease. Antibodies against three of the peptides appeared to react with the kinase-active site of pp60src, as these antibodies were phosphorylated in their heavy chain upon immune precipitation. Antibodies against two of the peptides, in contrast to the others, did not precipitate pp60src when this molecule was complexed with two cellular proteins, pp50 and pp90. This observation allows speculation about the location of the pp60src site involved in the formation of this complex.

The preparation procedure for Spirulina ferredoxin-NADP+ reductase (ferredoxin: NADP+ oxidoreductase, EC 1.18.1.2, FNR) was improved by adding protease inhibitors, phenylmethylsulfonylfluoride (PMSF) and EDTA, through the whole process of preparation and by introducing an affinity chromatography step on Blue Sepharose CL-6B. The addition of the inhibitors largely prevented the formation of the minor component (FNR I), and the affinity gel chromatography simplified the preparation process, shortening the exposure period of FNR to proteolysis. However, complete removal of the heterogeneity of FNR found at the amino (N)-terminal region was not achieved even by applying the new method. The affinity chromatography on the Blue Sepharose gel was also effective in purifying spinach FNR. The affinity of this gel for Spirulina FNR was compared with that for the enzyme derived from spinach leaves. The spinach enzyme had a higher affinity than the Spirulina one. Both enzymes showed the highest affinities to Blue Sepharose at 20--30 mM NaCl concentration. The N-terminal sequence analysis revealed that there was 4 forms, which were probably modifications produced by exopeptidase action during the preparation, or even in the living cells. The longest component gave the N-terminal sequence Ala-Lys-Thr-Asp-Ile-Pro-Val-Asn-Ile-Tyr-. The others lacked amino acids successively one by one from the N-terminus. In contrast, the carboxyl(C)-terminal residues of all 4 FNR forms were tyrosine. The probable C-terminal sequence was predicted to be -Trp-His-Val-Gln-Thr-Tyr based on a study of a cyanogen bromide peptide.
Characteristics of specific cell-mediated immunity in infants with congenital cytomegalovirus (CMV) infection were studied by in vitro lymphocyte transformation test, using the whole blood culture technique. Subjects were 6 symptomatic congenitally infected infants, 11 asymptomatic congenitally infected infants, 3 postnatally infected infants, and 5 CMV-free infants. Among them, three symptomatics and seven asymptomatics were studied temporally, together with three postnatally infected infants and five controls. In the congenitally infected group, 18 of 47 (38%) specimens proved to be negative in terms of stimulation index values. Studies of temporal changes in CMV lymphocyte transformation tests revealed that in the postnatally infected group, the response turned positive in accordance with the appearance of CMV viruria and remained positive afterwards. In the congenitally infected group, the delay in development of positive reaction followed initial negativity, a phenomenon conspicuously observed in the symptomatic subjects. No significant difference was seen in phytohemagglutinin-induced lymphocyte transformation tests among congenital, postnatal, and control groups.
Urine samples of 17 patients with nephropathy were tested with precipitating anti-thermostable-kidney antigen sera (anti-kidney BE antigen), which had been made kidney specific by absorption with diverse tissues. Kidney BE antigen was demonstrated in the concentrated urine of a patient (case I.T.) with membranous nephropathy, in whom membranous nephropathy appeared to be associated with exposure to chromium. Kidney BE antigen was, however, not demonstrated in the concentrated urine from the other 16 patients with nephropathy (chronic glomerulonephritis, 7; acute glomerulonephritis, 3; lupus nephritis, 3; membranous nephropathy, 2; diabetic nephropathy, 1). Kidney BE antigen was localized in the tubular cells but not in the glomeruli, by indirect immunofluorescent staining. Kidney BE antigen was not demonstrated in the glomeruli in case I.T., even after acid elution of the sections. The possible role of kidney BE antigen in the pathogenesis of membranous nephropathy is discussed.
