[Responsiveness of gynecological malignancies to oral antitumor agents in subrenal capsule assay and individualization of oral adjuvant chemotherapy].

A subrenal capsule assay (SRCA) was performed to test the sensitivity of 45 gynecological malignancies, including 24 cervical and 15 ovarian carcinomas, to oral antitumor agents, UFT, cyclophosphamide (CPM) and carboquone (CQ). Additionally, using a human endometrial carcinoma (Ishikawa carcinoma), the utility of SRCA in oral adjuvant chemotherapy was also investigated. Thirty-six of 45 cases (80.0%) were found to be evaluable. Regardless of the origin or of the histological type, each gynecological tumor showed a different degree of sensitivity. The results suggested that it is necessary to choose an oral antitumor agent according to the degree of sensitivity in oral adjuvant chemotherapy. In an experimental study with an implanted tumor, 14 of 20 mice (70.0%) developed a recurrent tumor in the control group. Compared to this, the recurrence rates for tumors in mice treated with CPM, CQ and UFT were 10.0% (p less than 0.001), 25.0% (p less than 0.01) and 30.0% (p less than 0.02), respectively. All these agents were considered to be effective in preventing tumors from recurring. In SRCA, the implanted tumor was sensitive to CPM and CQ, but not sensitive to UFT. These data suggested that SRCA is useful in predicting the effect of dose-dependent agents in oral adjuvant chemotherapy, although, with UFT, a time-dependent agent, it is not clear whether SRCA is appropriate for estimating the usefulness of its agent.

[Post-transfusion hepatitis in operated uterine cancer patients].

We examined the incidence of non-A, non-B hepatitis (NANB) in post-transfusion hepatitis in uterine cancer patients treated by surgery at our hospital. The diagnosis of NANB was based on the diagnostic standards for post-transfusion hepatitis of the Japanese Society of Gastroenterology 1985. In 587 patients with blood transfusions from 1978 to 1988, 80 patients (13.6%) were diagnosed as NANB. The proportions of blood components were, preserved whole blood 44.1%, concentrated red blood cells (CRBC)+ fresh frozen plasma 31.2%, and CRBC 12.8%. The proportions of blood components changed over the years, but since 1987 those of blood components in blood transfusion have been greatly decreased. No difference in the incidence of NANB was seen in patients with up to 10 units of blood transfused, but a high incidence was observed to patients who had received over 10 units. The number of patients treated by radical hysterectomy was five hundred and sixty-two (95.7%) out of a total of five hundred and eighty seven. The average blood loss in the operation was 1,392 ml.

[Portal hemodynamic changes from partial hepatectomy--quantitative analysis of portal flow before, during and after hepatectomy, using an Doppler ultrasound system].

We investigated portal hemodynamic changes in 86 patients with hepatic tumors who underwent partial hepatectomy. Portal blood flow was measured using a sector type Doppler ultrasound system before, during and after operation. In the intraoperative studies, the portal flow in patients who underwent massive liver resection decreased significantly. On the other hand, the portal flow in patients with minor liver resection tended to increase, but not significantly. Overall, portal flow per unit of cardiac out put decreased significantly; it also decreased significantly in patients who underwent massive or major liver resection, in patients with liver cirrhosis, and in patients whose post operative clinical course was satisfactory. In the post-operative studies (10-12 days after surgery), the portal blood flow decreased significantly in patients who manifested a severe post-operative clinical course; it showed no significant change in other patients. It is thus essential to monitor the portal hemodynamics of partial-hepatectomy patients. This is most readily realized using Doppler ultrasound.

Hepatic function and portal hemodynamics in patients with liver cirrhosis.

We investigated the distribution of portal blood flow per kilogram of body weight (PBF/BW) in 112 healthy volunteers and 90 patients with liver cirrhosis using an ultrasonic Doppler duplex system. The PBF/BW in healthy volunteers showed a log-normal distribution, while the distribution was irregular and showed two peaks in patients with cirrhosis. This irregular distribution was thought to reflect their complex physiopathological state. We next analyzed the relationship between PBF/BW and the data from hepatic function tests (serum albumin, total bilirubin, indocyanine green 15-min retention rate, and indocyanine green plasma disappearance rate) in 48 patients with liver cirrhosis. The patients were divided into four groups according to their portal blood flow: group A with a hepatofugal or stagnant portal blood flow, group B with a hepatopetal PBF/BW of less than 12 ml/min/kg, group C with a hepatopetal PBF/BW of 12 or more but less than 20 ml/min/kg, and group D with a hepatopetal PBF/BW of 20 ml/min/kg or more. Among patients with cirrhosis, group A showed the worst results in hepatic function tests, group D the second worst, and group C the best. The effective hepatic blood flow was thought to have decreased because of the development of extrahepatic portosystemic shunts in the patients in groups A and B, whereas it decreased because of the development of intrahepatic shunts in patients in group D. The results of hepatic function tests deteriorated as a consequence of the decrease in the effective hepatic blood flow.

[Immunohistochemical investigation of vimentin of uterine endometrial and endocervical adenocarcinoma].

We investigated the localization and the immunoreactivity (IR) of vimentin in normal and cancerous glandular epithelia of the uterine corpus and cervix. The materials for the study were 33 cases of endometrial adenocarcinoma, 21 cases of endocervical adenocarcinoma, and 12 cases of uterine myoma which had normal endometrium and cervix. A similar study was also performed on smear materials. In normal endometrium, the vimentin was localized not only in the stromal cells but also in the epithelial cells. The IR of the vimentin in the epithelial cells was more prominent in the proliferative phase than in the secretory phase. On the other hand, in normal cervix, the vimentin was not localized in the epithelial cells. In endometrial adenocarcinoma, the IR of the vimentin was markedly or moderately positive in more than half of the cases. In endocervical adenocarcinoma, the IR of the vimentin was absent in all but 2 cases which were of the endometrioid type. The vimentin IR of the stromal cells was clearly observed in endocervical as well as endometrial adenocarcinoma. Very similar results were observed in the smear materials. In endometrial adenocarcinoma, the differentiation, the depth of myometrial invasion and the lymph node metastasis were regarded as the prognostic factors, and the vimentin IR appeared more intensive in the cases of well differentiated and superficial myometrial invasion. The above results showed that there was a remarkable difference between endometrial and endocervical epithelial cells in the vimentin IR.(ABSTRACT TRUNCATED AT 250 WORDS)

[The changes in hepatic hemodynamics and hepatic energy charge (blood ketone body ratio) induced by vasopressin infusion in chronic liver disease].

We evaluated the changes in the hepatic hemodynamics and blood ketone body ratio (KBR) induced by vasopressin in 40 patients with chronic liver diseases to clarify the effect of hepatic blood flow on hepatic energy charge expressed by KBR. Following infusion of vasopressin, the rate of decrease in portal blood flow in liver cirrhosis (LC) was significantly lower than that in chronic hepatitis (CH). Moreover, the blood flow in the hepatic artery after vasopressin infusion was greater in LC than in CH. As a result, the total hepatic blood flow, which was the sum of the flow in the above two vessels, after vasopressin infusion was greater in LC than in CH but the decrease in the blood KBR was not significant in LC. Thus, vasopressin appears to be hemodynamically safe in patients with LC, but caution is needed since it may decrease blood pressure, total hepatic blood flow and KBR in some LC patients.

[Computed tomography for the diagnosis of thrombosed aortic dissection with extravasation].

Four cases of thrombosed aortic dissection with extravasation demonstrated by CT were presented. In CT diagnosis for this type of dissection, cautions should be employed not only in an inhomogenous density area in the mediastinum and pleural cavity but also in the presence of deviation of intimal calcification and relatively high density area of crescent shape in aortic wall on plain CT.

[Evaluation of a biliary endoprosthesis using expandable metallic stents].

A biliary endoprosthesis constructed of Gianturco self-expanding metallic stents was placed in twenty-nine patients with obstructive jaundice, 27 malignant and 2 benign. All endoprostheses were placed successfully. External drainage catheters were removed in twenty-seven patients. At the follow-up, ranging from two to 59 weeks, the 30 day mortality rate was 7.4%. Twenty-eight patients had obvious clinical improvement with relief of jaundice. Seven patients experienced recurrent jaundice. In two of these patients, the obstruction was relieved by additional metallic stents. The expandable metallic biliary endoprosthesis is suggested as an effective treatment for biliary obstruction.

Clinical evaluation of tumor-associated mucin-type glycoprotein CA 54/61 in ovarian cancers: comparison with CA 125.

Studies were conducted in 343 women on the clinical utility of the newly developed tumor-associated mucin-type glycoprotein, CA 54/61, as a tumor marker in serum for ovarian cancer. The overall positivity rate of the new marker was 60%, lower than the rate of 88% obtained with CA 125 measured concurrently. Concerning tumor histology, CA 54/61 had a positivity rate of 78% in mucinous adenocarcinoma, compared with 44% with CA 125. There was no correlation between the serum levels of CA 54/61 and CA 125 (r = 0.05). CA 54/61 showed a low rate of positivity in benign disease and only exceeded the cutoff value in one patient with an endometrial cyst, whereas CA 125 had a positivity rate of 60%. The false-positive rates for CA 125 during the first trimester of pregnancy and during menstruation were 57 and 16%, respectively, whereas the rates for CA 54/61 were only 11 and 5%, respectively. Assay of both CA 54/61 and CA 125 increased the success rate in diagnosing ovarian cancer to 95% (38 of 40 patients).

[Level of inhibition of thymidylate synthase activity and 5-fluorouracil in tumor tissues after administration of UFT or tegafur].

The antitumor effects of tegafur and UFT, fluorinated pyrimidines, were investigated in endometrial carcinoma bearing mice. Following administration of tegafur and UFT, each at a clinical dosage, the levels of 5-fluorouracil (5-Fu) and the inhibition of thymidylate synthase (TS) activity in the tumor tissue were measured. As a result, the mean levels of 5-Fu in the tumor tissue were approximately six times higher in the UFT group (29.2 +/- 10.7 ng/g) than in the tegafur group (4.9 +/- 4.8 ng/g) (p less than 0.01). The UFT group showed a mean inhibition of TS activity in the tumor tissue significantly higher (61.60 +/- 2.02%) than did the tegafur group (56.08 +/- 4.87%) (p less than 0.05). Hence, not only the higher tumor tissue levels of 5-Fu but also the higher inhibition of TS activity in the UFT group as compared with the tegafur group suggested that UFT had more potent antitumor effect.

[Histopathological study regarding the protective effect of elastase on cis-platinum-induced renal toxicity].

A histopathological study was effected by using rats to evaluate whether or not elastase has a protective effect on the renal toxicity of cis-diamminedichloroplatinum (II) (CDDP). It was found that CDDP caused disorders mainly to the renal tubule--that is, atrophic and necrotic changes of the tubular epithelium. When elastase was used concomitantly with CDDP, such changes were considerably inhibited so that a relatively large part of the normal tissue architecture was preserved. The increase of the total platinum levels in the renal tissues was significantly suppressed with the addition of elastase and in the enzyme histochemical observation, the activity of the lysosomal enzymes and the alkaline phosphatase was maintained. The detailed mechanism of the protective effect of elastase against CDDP renal toxicity is unknown. However, these results suggested that elastase inhibited the platinum deposits in the renal tissue and played a role in the stabilization of the lysosomal and the plasma membranes, thus protecting the kidneys.

[An experimental and clinical study of thin-slice high-resolution CT for lung detail].

The clinical usefulness of thin-slice high-resolution CT in the diagnosis of the chest was assessed by (1) experiments using a phantom and an inflated fixed lung (Heitzman's method), (2) evaluation of the CT delineativity of the bronchi and major fissure and (3) clinical examination of patients with diffuse pulmonary diseases. 1) In a phantom experiment using catheters, 10 mm-thick slice scan showed the 6 catheters as a single faint line. By 1.5 mm-thick slice scan, the catheters were defined as 6 separate lines. The profile images suggested that 1.5 mm-thick slice scan enables delineation of more minute details of structures. 2) In an experiment using an inflated fixed lung, 1.5 mm-thick slice scan produced more informative images that resembled macroscopic or soft X-ray images. 3) By 1.5 mm-thick slice scan, the subsegmental bronchi of both the right and left lobes were identified in most cases, while identification was possible in only half the cases by 10 mm-thick slice scan. 4) By 10 mm-thick slice scan, the sub-subsegmental bronchi were not identified in most cases. However, identification was possible in approximately half the cases by 1.5 mm-thick slice scan. 5) By 1.5 mm-thick slice scan, the major fissure was delineated as a linear shadow in most cases. It was hardly recognizable by 10 mm-thick slice scan. 6) In diffuse pulmonary diseases, 1.5 mm-thick slice scan allowed more minute visualization of the patho-morphological changes compared to 10 mm-thick slice scan. 7) Thin-slice high-resolution CT is thus expected to contribute to pathological analysis and more accurate diagnosis of pulmonary diseases.

Quantitative measurement of abdominal arterial blood flow using image-directed Doppler ultrasonography: superior mesenteric, splenic, and common hepatic arterial blood flow in normal adults.

To measure volume blood flow quantitatively in human abdominal arteries, we used an ultrasonic image-directed Doppler system and electromagnetic flowmeter to first measure volume flow in canine arteries. In dogs, there was a strong linear correlation (R = 0.98) between the product of the time average of the maximum blood flow velocity and the average cross-sectional area and the volume blood flow measured by an electromagnetic flowmeter. These results enabled measurement of volume blood flow in the human superior mesenteric (SMA), splenic (SPA), and common hepatic (CHA) arteries from the abdominal wall. Comparison of pulsatility index values indicated a larger vascular resistance in the SMA than in the SPA or CHA.

Hepatocellular lipofuscin in pre-eclampsia.

Thirty liver specimens from 30 pre-eclamptic women with and without liver dysfunction were examined for the amount of lipofuscin pigment deposited. The amount of hepatic lipofuscin correlated positively with plasma urate concentrations. This finding may indicate an involvement of oxygen-free radicals and xanthine oxidase in the pathophysiology of pre-eclampsia.

Estrogen producing activity in epithelial ovarian tumors and dermoid cysts.

In 28 cases of postmenopausal women with non-functioning ovarian tumors, the estradiol (E2) level was assayed in peripheral plasma and tumor tissue extract. E2 precursors were also assayed in the tissue extract, and localization of 3beta-hydroxysteroid dehydrogenase (3beta-HSD) in the tissue was investigated. The plasma E2 levels were 153.8 +/- 205.9 pg/ml in 15 benign tumor cases, 244.3 +/- 336.5 pg/ml in 13 malignant tumor cases, and both of which were significantly higher than 12.3 +/- 3.6 pg/ml in normal postmenopausal women (p less than 0.01). The postoperative plasma E2 level (33.3 +/- 32.7 pg/ml) was significantly lower than the preoperative one (228.3 +/- 319.0 pg/ml). Twelve of 19 cases (68.2%) showed a postoperative decline in the E2 level to less than a half of the preoperative level. E2 and its precursors were found in tumor tissue. These findings demonstrate that E2 is produced by the tumor tissue itself. In addition, 3beta-HSD activity was localized in the stroma of ovarian tumors, indicating that E2 synthesis is mainly carried out in stromal cells rather than in epithelial cells.

Portal blood flow: measurement with MR imaging.

Portal blood flow was measured by means of direct bolus imaging (DBI), a method of measuring flow velocity with magnetic resonance imaging. DBI allows immediate visualization of fluid movement, thereby enabling calculation of a flow velocity from fluid displacement. In a study of 14 healthy male volunteers, portal blood flow was measured with electrocardiographic gating during the 18 seconds subjects could suspend respiration. These measurements showed a close correlation (r = .968) with those obtained by means of Doppler ultrasound (US). Increases in portal blood flow after oral administration of ethanol and glucose were measured with DBI. Glucose caused a statistically greater increase in portal blood flow volume in healthy control subjects than in patients with chronic hepatitis. Blood sugar, on the other hand, showed a significantly greater increase in these patients, possibly reflecting the greater vascular resistance of the liver. DBI is a useful noninvasive method of measuring portal blood flow without the limitations imposed on Doppler US by obesity and intestinal gas.
