Postprandial plasma cholecystokinin response in patients after gastrectomy and pancreatoduodenectomy.

Endogenous cholecystokinin release after a test meal was measured in the controls, patients with Billroth I and II anastomosis after subtotal gastrectomy, patients with a Roux-en-Y anastomosis after total gastrectomy, and patients with a modified Child's reconstruction after pancreatoduodenectomy 2 months after surgery. The postprandial plasma level in patients with Billroth I and II anastomosis was close to that in the controls. In a Roux-en-Y anastomosis, this level was slightly higher than in the controls and patients with a Billroth anastomosis. Differences in integrated cholecystokinin secretion at 120 min in different groups were insignificant. After a modified Child's reconstruction, the postprandial level was significantly lower than in the controls and in patients with a Billroth II anastomosis. One patient with a modified Child's reconstruction was examined 8 yr after surgery, and she had a normal response. We suggest that either the duodenum or jejunum used for gastrointestinal anastomosis can release cholecystokinin normally, and pancreatoduodenectomy may decrease cholecystokinin release 2 months after surgery.

[Results of several combination modalities for maxillary cancer].

Two hundred forty-six patients with the histologically proved squamous cell carcinoma of the maxillary sinus were treated at the Department of Radiology, National Sapporo Hospital, with various combined modes of surgery, irradiation and chemotherapy. Four combination modalities were used from 1966 to 1983. One hundred sixty-three (66.3%) patients were men and 83 (33.7%) were women. Five-year survival was 41.0% in T2 (16 cases), 22.3% in T3 (176 cases) and 15% in T4 (53 cases). In 72 cases treated since 1978 with tumor reduction as completely as possible, five-year survival was 43.5%. We recommand tumor reduction as complete as is possible and 40-50 Gy irradiation.

Dirofilaria infection in the breast of a woman in Japan.

Dirofilaria infection in the breast of a woman was reported from Japan. A nodule in the breast was excised and histologically examined. The sections of a female nematode were found in the center of the nodule. Based on the morphological features of this worm, it is suggested that this may represent the second human case of Dirofilaria immitis occurring in the breast.

[Development of cerebral blood flow measurement using skull impedance plethysmography].

Using skull impedance plethysmography, a developmental approach was made to the measurement of mean cerebral blood flow (mCBF) from brain volume exchanging ratio (delta V) involved in bilateral jugular venous occlusion. An experimental study thereof revealed the following results: Comparative examinations were made between total cerebral blood flow obtained by the superior sagittal sinus hydrogen clearance method and delta V X t obtained by the present approach led to the establishment of a linear correlation there between and also of the following calculating formula: mCBF = k X delta V X t (omega X sec) k: proportional coefficient t: blood flow increasing time (10 sec.) The experimental results revealed that the present approach was valid enough as an index of cerebral blood flow. Some future improvements therein seem to make also its clinical application possible. The theory and method of the present approach are described with an examination of several points raised thereby.

[Changes in renal function on response to rapid administration of cisplatinum].

Seven patients with urologic carcinomas were examined for changes in renal function as a result of administration of Cisplatinum (CDDP) at a high rate of 50 or 100 mg/m2 within 5 hours, superimposing 2 liters of fluid before and after the CDDP dose, and the results were compared with the pretreatment values. Renal function tests revealed that BUN was elevated to 20 mg/dl or more in 5 but not beyond 30 mg/dl in any of the patients, that serum creatinine was elevated to 1.4 mg/dl or more in 3 cases with a maximum of 1.5 mg/dl, and that Ccr was decreased to less than 70% of the pretreatment value in 3 patients. These manifestations of imparied renal function were all transient and reversible, being mostly restored to the pretreatment values during or within 1 month after the end of CDDP treatment, and did not necessitate the discontinuation of the treatment in any patients. The above findings proved the rapid infusion of CDDP to be as safe in terms of renal function as prolonged, continued i.v. infusion of the drug, which would these fore make CDDP treatment of outpatients possible.

Some species differences in cardiovascular responses to intravenously injected leucine-enkephalin.

Studies were conducted to determine the cardiovascular responses to leucine-enkephalin (L-enk) in three different species of animals; rabbit, dog and monkey. All animals were anesthetized with pentobarbital sodium after sedation with ketamine. Mean blood pressure (MBP) and heart rate (HR) were simultaneously monitored. The pressor and HR responses to bilateral carotid occlusion (BCO) were determined before injection of L-enk. Increased MBP and HR due to BCO in monkey were significantly greater than in the other two animal groups. Following i.v. injection of L-enk (5-30 micrograms/kg), a significant fall in MBP occurred in all groups in a dose-dependent manner; however, the time course of changes in MBP in rabbits was significantly shorter than that in the other animal groups. Significant decreases in HR after the injection of L-enk occurred in rabbits and dogs, whereas increases in HR occurred in monkeys. These results show that some cardiovascular responses to L-enk may be species dependent. These different cardiovascular responses to L-enk may be at least partly related to species differences in baroreceptor reflex sensitivity.

Suppression of rheumatoid factor synthesis by antiidiotypic antibody in rheumatoid arthritis patients with cross-reactive idiotypes.

The effect of antiidiotypic antibody on the in vitro production of rheumatoid factor was studied in rheumatoid arthritis patients with cross-reactive idiotypic determinants. Antiidiotypic antibodies (ascites IgG) were developed against monoclonal rheumatoid factor (Ka m-RF) by a cell fusion procedure. These antibodies were idiotype-specific, since: 1) they reacted only with F(ab')2 fragment of Ka m-RF; 2) they failed to react with normal IgM without rheumatoid factor activity; and 3) their F(ab')2 fragment inhibited the rheumatoid factor activity of Ka m-RF. The antiidiotypic antibody strongly suppressed the in vitro production of rheumatoid factor by lymphocytes from unrelated rheumatoid arthritis patients with cross-reactive idiotypes. The suppression was specific, since ascites IgG failed to suppress in vitro anti-keyhole limpet hemocyanin antibody production by the lymphocytes. These results indicate that antiidiotypic antibody may influence the regulation of rheumatoid factor production in patients with rheumatoid arthritis.

Pharmacokinetics and disposition of 4'-O-tetrahydropyranyladriamycin in mice by HPLC analysis.

The plasma level of 4'-O-tetrahydropyranyladriamycin (THP) declined rapidly after IV injection to mice, with a t1/2(alpha) of 0.453 min. Only 1.2 micrograms/ml THP was detected 2 min after injection of 5 mg/kg. The drug was immediately transferred to various tissues, where the drug levels were much higher than the plasma concentration. In the lung and spleen, 8.26 and 13.6 micrograms/g THP was present, respectively, 2 h after administration. Major metabolites of THP were 13-dihydro-THP, ADM, 7-deoxyadriamycinone, and 7-deoxy-13-dihydro-adriamycinone. Only 1.12% of the dose had been recovered in the urine by 48 h after injection as THP and its metabolites, according to analysis by HPLC fluorospectroscopy. The observed disposition of THP was compared with that of adriamycin (ADM). The plasma disappearance and tissue transfer of THP were faster than those of ADM. THP levels in the spleen and lung were higher and those in the heart and liver were lower than the corresponding ADM levels. Drug levels declined more quickly in most tissues in the case of THP than of ADM. Tissue distributions after single bolus and multiple injections were also compared and discussed.

Nocturnal sleep in mentally retarded infants with cerebral palsy.

Sleep EEG patterns in 23 mentally retarded children with cerebral palsy (CP) (from 4 months to 5 years of age) and 39 reference mentally retarded children of no abnormality with the exception of psychomotor retardation (from 4 months to 12 years of age) were studied throughout nocturnal sleep, and the following results were obtained. Eleven CP cases and 30 reference cases evidenced normal sleep patterns that could be classified into 6 stages. Twelve other cases in CP and 9 reference cases showed some abnormal sleep EEG patterns as follows: (i) in CP cases absence of EEG patterns characteristic of wakefulness, NREM sleep and REM sleep without spindles (n = 3); porencephaly with microcephaly (n = 1) and cytomegalovirus infection (n = 2), no characteristic EEG patterns of stages 1-4 without spindles (n = 2); cardiac arrest (n = 1) and agenesis of corpus callosum with lissencephaly (n = 1), absence of REM sleep (n = 1); kernicterus, spindles with an extremely low incidence (n = 5); perinatal anoxia (n = 2), kernicterus (n = 1), Lesch-Nyhan syndrome (n = 1) and cerebral palsy due to unknown etiologies (n = 1), and extreme spindles (n = 1); cerebral palsy due to unknown etiologies, (ii) in reference cases no spindles (n = 2), spindles with an extremely low incidence (n = 6), and extreme spindles (n = 1). Short sleep and long waking times during the night were noted in 4 out of 23 CP subjects; kernicterus (n = 2), agenesis of corpus callosum with lissencephaly (n = 1), and porencephaly with microcephaly (n = 1).(ABSTRACT TRUNCATED AT 250 WORDS)

Spontaneous production of antibodies to deoxyribonucleic acids in patients with systemic lupus erythematosus.

In vitro spontaneous anti-DNA antibody production in patients with systemic lupus erythematosus (SLE) was examined. SLE lymphocytes produced IgG and IgM anti-DNA antibody from the third culture day, and reached a plateau on the seventh culture day. This anti-DNA antibody activity in 7-day culture supernatant was abolished by pretreatment of the lymphocytes with cycloheximide, suggesting de novo immunoglobulin synthesis was required for this spontaneous anti-DNA antibody production. Lymphocytes from patients with rheumatoid arthritis (RA) and other collagen diseases including progressive systemic sclerosis, polymyositis/dermatomyositis, and polyarteritis nodosa did not produce IgG and IgM anti-DNA antibody spontaneously, but SLE lymphocytes produced substantial amounts of IgG and IgM anti-DNA antibody spontaneously. Furthermore, active SLE produced a larger amount of IgG anti-DNA antibody than inactive SLE. We observed a significant negative correlation between the number of Ia+ T cells and IgG, but not IgM, anti-DNA antibody production. Furthermore, spontaneous IgG anti-DNA antibody production was elevated after pretreatment of SLE T cells with anti-Ia and complement, suggesting that Ia+ T cells in SLE bring about suppression of autologous B cells producing IgG anti-DNA antibody.

Malakoplakia of urinary bladder following cadaveric renal transplantation.

Malakoplakia of the urinary bladder following cadaveric renal transplantation in a twenty-two-year-old woman is reported. Urinary tract infection with Escherichia coli persisted postoperatively. Three years later, gross hematuria and fever occurred. Yellow-tan mucosal plaques or nodules were observed cystoscopically, and histologic examination revealed malakoplakia of the urinary bladder with characteristic foamy histiocytes containing Michaelis-Gutmann bodies.

Aicardi syndrome. Report of 6 cases and a review of Japanese literature.

This report presents 6 cases of Aicardi syndrome (infantile spasms, agenesis of the corpus callosum, chorioretinopathy), and a review of Japanese literature. Two patients developed Lennox-Gastaut syndrome following infantile spasms. Visual evoked potential was examined in every case with binocular and monocular flash stimulations. Normal response was obtained in 5 cases, and no response in 1. Auditory brainstem response was examined in 2 cases. Central conduction time was prolonged in 1 case.

Isolation of a cDNA clone encoding pancreatic polypeptide.

We have isolated a cDNA clone encoding pancreatic polypeptide from a cDNA library constructed with RNA from an endocrine neoplasm of the human pancreas. The cDNA was inserted into plasmid pBR322 and the plasmid was cloned in Escherichia coli. Oligonucleotides (sequence in text) specific for the amino acid sequence (sequence in text) of pancreatic polypeptide were used as hybridization probes. The pancreatic polypeptide cDNA isolated was 465 base pairs long and encoded a peptide of 95 amino acids in the coding region. The 36-amino acid sequence of pancreatic polypeptide was flanked by a 29-amino acid putative leader sequence at the amino terminus and a connecting tripeptide (Gly-Lys-Arg) followed by a 27-amino acid peptide at the carboxyl terminus. The first 20 of the amino acids in the carboxyl-terminal heptacosapeptide were identical to the structure of human pancreatic icosapeptide with the single exception of an isoleucine substitution for valine in the 18th position. This alteration results from an A----G substitution in the nucleotide sequence of the cDNA and may represent a genetic variation or a point mutation in the pancreatic polypeptide cDNA.
