Effect of ursodeoxycholate on the biliary excretion of cefotiam and sulbenicillin in patients with percutaneous transhepatic biliary drainage.

The effects of ursodeoxycholate administration on the biliary excretion of the antibiotics cefotiam and sulbenicillin were studied in five patients with stable hepatic function receiving percutaneous transhepatic biliary drainage for obstructive jaundice. Cefotiam (I g) and sulbenicillin (2 g) were administered intravenously before and after ursodeoxycholate administration, and the maximum concentrations of the antibiotics in the bile and total amounts excreted in the bile during the 4 h after administration were determined. After ursodeoxycholate administration, both the maximum concentration of cefotiam in the bile and the amount excreted increased significantly. Ursodeoxycholate also increased the peak concentration and total excretion of sulbenicillin. For both cefotiam and sulbenicillin, the amount of antibiotic excreted in the bile during the 4 h after administration showed a significant correlation with the amount of bile acids excreted in the bile. This strongly suggests a common mechanism for the biliary excretion of these antibiotics and bile acids. Ursodeoxycholate administration is a benign way to increase both the concentration and the total amount of antibiotic excreted in the bile. Therefore, it may be useful in the treatment of serious biliary tract infections, especially in patients receiving biliary drainage.

Defective expression of the 2H4 molecule after autologous mixed lymphocyte reaction activation in systemic lupus erythematosus patients.

Previous studies demonstrated that patients with active systemic lupus erythematosus (SLE), especially those with active renal disease, had a marked reduction in T4+2H4+ suppressor inducer cells in their peripheral blood. However, it was puzzling to find that active SLE patients without renal diseases often had normal percentages of T4+2H4+ cells. In the present study, we attempted to determine whether active SLE patients bearing normal percentages of T4+2H4+ cells had a defect in their expression of the 2H4 molecule on T4+ cells after autologous mixed lymphocyte reaction (AMLR) activation. The peripheral blood lymphocytes (PBL) from 50 SLE patients with normal percentages of T4+2H4+ cells (greater than or equal to 7% in PBL) were studied and the results were compared with those of 40 normal individuals. The density of the 2H4 molecule on T4 cells from normal controls increased during the 7-d AMLR; in contrast T4 cells from patients with SLE, especially those with active SLE, had defective expression of the 2H4 antigen after AMLR activation. Patients with inactive SLE, like normals, showed an increase in the 2H4 molecule after AMLR activation. Moreover, a strong correlation was observed between percent suppression of pokeweed mitogen (PWM)-driven IgG synthesis and the density of the 2H4 antigen on AMLR-activated T4 cells. Serial analysis of patients with SLE showed that the density of the 2H4 antigen expression and the suppressor inducer activity of AMLR-activated T4 cells were inversely correlated with disease activity. Thus, defective expression of the 2H4 antigen may be an important mechanism for the failure of active SLE patients with normal percentages of T4+2H4+ cells to generate suppression.

Neuronal differentiation of mouse neural crest cells in vitro.

The purpose of the present study is to analyze the effect of serum or chick embryo extract (CEE) on the neuronal differentiation of the mouse neural crest cells. When the crest cells were cultured in the medium containing serum at low concentration (5% calf serum), neurite outgrowth was observed. The active outgrowth was detected at 3-4 days in culture. However, in the medium supplemented with 20% calf serum, no neurite appeared, and the crest cells remained fibroblast-like. The differentiation of adrenergic neurons was observed when the crest cells were cultured in the medium containing CEE along with serum.

Molecular analysis of Lesch-Nyhan syndrome found in Japan.

Molecular analysis of four unrelated patients with Lesch-Nyhan (L-N) syndrome was performed. All four cases had typical clinical features of L-N syndrome, and the activities of hypoxanthine-guanine phosphoribosyltransferase (HPRT) were absent. No structural gene abnormalities were found by Southern blot analysis in all cases, and a decrease of HPRT mRNA was not detected by dot blot analysis in two of the four cases. However, HPRT enzyme proteins were detected by Western blot analysis in all cases. Our results showed that the production of immunologically reactive but enzymatically inactive mutant HPRT protein was pathogenic for all four patients.

A nutritional evaluation of vitamin E status in very low birth weight infants with respect to changes in plasma and red blood cell tocopherol levels.

Nutritional status of vitamin E was assessed in very low birth weight infants of less than 1,500 g, with respect to changes in plasma and red blood cell (RBC) tocopherol concentrations. The forty infants examined were divided into two groups in terms of their birth weight: group A, less than 1,000 g; and group B, 1,000-1,500 g. Immediately after birth, plasma tocopherol level was 335 +/- 101 micrograms/dl in group A and 316 +/- 103 micrograms/dl in group B, while RBC tocopherol was 187 +/- 48 micrograms/dl and 231 +/- 72 micrograms/dl packed PBCs, in groups A and B, respectively. In two infants, RBC tocopherol concentrations were less than 115 micrograms/dl, this level being reported as the lowest of the normal range. After birth, plasma and RBC tocopherol levels decreased markedly during 4 to 6 weeks of life especially in group A, while no decrease below normal range was documented in RBC levels in group B. A fine granule preparation of tocopheryl nicotinate (20 mg/kg/day) was administered in seven other infants, three in group A and four in group B. All three infants in group A showed no elevation of plasma and RBC tocopherol levels during the first 4 or 5 weeks of the administration, but a marked elevation thereafter. In three of the four infants in group B, an elevation of plasma and RBC tocopherol concentrations was documented after 2 weeks of the administration. The above findings indicate that no deficiency exists even in very low birth weight infants immediately after birth, while the deficiency may develop after birth, due to a poor ability to absorb fat.

Ultrastructural changes in peripheral nerves of the fingers of three vibration-exposed persons with Raynaud's phenomenon.

A finger biopsy was performed on three patients with vibration-induced white finger (VWF), and the specimens were examined by electron microscopy for peripheral nerve changes. A characteristic neuropathy with perineurial fibrosis was revealed which was often accompanied by a thickened perineurium with a lamellar structure resembling onion skin. This peculiar fibrosis consisted of elongated cytoplasmic projections of perineurial cells or fibroblasts and a greatly increased amount of collagen which occasionally contained fibrous long-spacing collagen. In the endoneurium, a decrease in the number of nerve fibers and a marked increase in collagen with fibroblasts were noted. Myelinated axons became smaller, and this occurrence suggested incomplete regeneration. The pathological changes were presumably the result of the long-term clinical course of VWF.
