The mode of lymphatic and local spread of pancreatic carcinomas less than 4.0 cm in size.

To clarify the mode of lymphatic and local spread of small pancreatic carcinomas, we studied the histopathology of 33 patients with invasive ductal adenocarcinoma of the head of the pancreas less than 4 cm in diameter. Microscopically, lymph node metastases were found in 72.7% (24 of 33) of the patients: 17 patients had lymph node metastases at the first barrier and 7 patients had lymph node metastases at the second barrier. Capsular invasion was present in 21.2% (7 of 33) patients, retroperitoneal invasion in 84.8% (28 of 33) patients. Invasion to the common hepatic artery was identified in 9.1% (3 of 33) patients, and invasion to the portal vein system in 24.2% (8 of 33) patients. In addition, five of the seven patients with lymph node metastases at the second barrier had spread to the periaortic lymph nodes. Thus, even patients with small pancreatic cancers which were macroscopically confined to the pancreas showed microscopic extrapancreatic tumor extension, especially invasion to the retroperitoneal tissues and to the periaortic lymph nodes. This suggests that an aggressive surgical approach, including complete resection of surrounding connective tissues in the retroperitoneum and extensive lymph node dissection, is necessary to improve the surgical therapeutic results even for small pancreatic cancers.

Mesangial cell-matrix interactions: modulation of matrix expression in mesangial cells by extracellular matrices.

Aberrant extracellular matrices (ECMs) observed in mesangial areas during the progression of glomerulonephropathy could be produced by mesangial cells (MCs), interacting with altered ECM components, under the influence of various mediators. We studied the interaction between MCs and ECMs, especially focusing on matrix expression of MCs in contact with ECMs. We analyzed mRNA expression and protein synthesis of ECM components by cultured rat MCs which were plated on different types of ECMs, basement membrane-type matrix (BMM), type IV collagen and type I collagen. The level of mRNA for ECM components (alpha 1 type I collagen, alpha 1 type IV collagen, fibronectin and laminin B2) increased in MCs on the two types of collagen compared with BMM. The relative increase of alpha 1 type I collagen mRNA expression in MCs on type I collagen was greater than that of alpha 1 type IV collagen expression. At the protein level, type I collagen production by MCs on coated type I collagen showed a 2.2-fold increase relative to that on BMM. MCs on type IV collagen produced it at an intermediate level. The phenomena were not associated with differences of proliferative activity, but were accompanied by minor morphological differences. These results indicate that ECMs can modulate the phenotype of MCs in terms of matrix expression and suggest that MC-matrix interaction may in part contribute to abnormal ECM accumulation in diseased glomeruli.

Plasminogen activation system in active even in thyroid tumors; an immunohistochemical study.

The plasminogen activation system is one of the prominent factors in tumour progression since it enhances both distant metastasis and direct invasion. In this study we investigated the expression and localization of this system in clinical thyroid tumours. It was found that urokinase-type plasminogen activator (uPA), its receptor (uPAR) and plasminogen activator inhibitor type 1 (PAI-1) were expressed diffusely in most thyroid carcinomas. The lesions positive for the latter two were equal to or sometimes even a litter more extensive than those positive for uPA. The immunoreactivity of plasminogen activator inhibitor type 2 (PAI-2), however, was often partially or even entirely absent, and only 56% of the carcinomas expressed it diffusely, indicating that the two types of inhibitors in this carcinoma have quite different characteristics. No relationship between the expression of these proteins and clinicopathological parameters could be determined. THese findings suggest that the plasminogen activation system is functionally active in thyroid carcinoma but that is has hardly any effect on its general characteristics, that is, slow growth and excellent prognosis of usual types and the exceptionally sudden progression of the anaplastic type. In benign tissues, too, this system was often expressed and appeared to enhance their growth to some extent.

A study of discoloration of the gingiva by artificial crowns.

In this study, soft tissue discoloration including the marginal gingiva, gingival papilla, and attached gingiva were evaluated in subjects with artificial crowns and compared to the soft tissue of natural teeth using a spectrophotometer. Gingivae around artificial crowns had a lower value (L*) and chroma (C*ab) than the gingivae surrounding natural teeth. Hue (Hab o) shifts toward red-purple were frequently noted. There was generally a more noticeable color difference (E*ab) in the marginal gingiva and gingival papilla of teeth with crowns than was found in the attached tissue.

Intrabiliary pressure measurement by duodenal pressure loading for the evaluation of duodenal parapapillary diverticulum.

BACKGROUND/AIMS: We report a method of intrabiliary pressure measurement by duodenal pressure loading for evaluating the effects of duodenal parapapillary diverticulum. MATERIAL AND METHODS: Forty-eight patients with primary choledocholithiasis were investigated based on usual intrabiliary pressure measurement developed at our department. RESULTS: Ten of these patients with parapapillary diverticulum showed higher intrabiliary residual pressure (P) and biliary resistance (R) values than the remaining patients without diverticulum, but the difference were not significant. There were no significant differences in the frequency of patterns of the B zone angulation of the pressure curve between the two groups with and without parapapillary diverticulum. Intrabiliary pressure was measured while maintaining a manometer pressure of 250 mm H2O by infusing or aspirating physiologic saline. CONCLUSION: From the results of this measurement for 6 patients, when P and R values show increases of 50 mm H2O or more and 3.5 units or more, respectively, additional surgery for parapapillary diverticulum was considered to be indicated.

Assessment of postoperative digestive function after pancreatico-duodenectomy: a comparison of reconstruction techniques.

BACKGROUND/AIMS: Malabsorption is a frequent complication following pancreatico-duodenectomy (PD) for pancreatic carcinoma. Gastrojejunostomy followed by Billroth I type of reconstruction (PD III) has been advanced to prevent this disorder. We compared postoperative digestion and absorption determined by 131I-triolein, D-xylose and pancreatic function diagnostant (PFD) after extensive PD followed by one of two reconstructive procedures. MATERIAL AND METHODS: Postoperative digestive and absorptive functions in patients operated by Child's method (PD II) were compared with patients in whom the stomach emptied proximal to the pancreas and bile duct (PD III). RESULTS: The absorption of D-xylose was significantly higher (1.24 +/- 0.36 g vs. 0.72 +/- 0.21 g) in the PD III group. No difference was noted in pancreatic endocrine secretion between the two groups. Biliary scintigraphy revealed increased bile secretion in the PD III group. CONCLUSIONS: These results suggest that PD III is superior to PD II following extensive PD.

Nonpalpable breast cancer with nipple discharge: how should it be treated?

In this study, 26 cases of nonpalpable breast cancer with nipple discharge treated at our department were reviewed. Their discharge was either bloody or serous with a positive hematest, but all except for one were negative for cytology, while CEA value of the discharge was high in 72.7%. Mammograms were found to be unreliable for diagnosis, while abnormal findings were observed in 84.6% by ductography. However, final diagnosis was determined histopathologically from surgical specimens, showing 14 intraductal and 12 invasive ductal cancers, none with components of comedo carcinomas. Seventeen patients underwent mastectomy following duct-lobular segmentectomy and a small remnant of intraductal carcinoma was found microscopically in only one patient. All patients except for one have survived for 98 months on average with no symptoms of metastasis. These findings suggest that duct-lobular segmentectomy with an adequate surgical margin should be adopted as the final operation for selected patients with nonpalpable breast cancer involving nipple discharge.

Fas antigen expression in hepatocellular carcinoma tissues.

Fas antigen belongs to the tumor necrosis factor receptor family and is known to induce apoptosis. This protein is abundantly expressed in hepatocytes, especially in acute and chronic hepatitis. To elucidate the clinical significance of Fas in hepatocellular carcinoma (HCC), we investigated its expression in 50 HCC having various characteristics. Fas was comprehensively expressed in non-cancerous hepatocytes as well as bile ducts. It was moderately expressed in histiocytes in the stroma rather than in infiltrating lymphocytes. In HCC, Fas expression was significantly reduced when there was poor differentiation (p<0.001), portal tumor thrombus and extracapsular invasion (p<0.05) according to univariate analysis. Multivariate analysis revealed statistical significance between Fas expression and the degree of differentiation (p=0.0002). These results suggest that HCC, like non-cancerous hepatocytes, express Fas antigen, when they are well or moderately differentiated but lose this ability when they become poorly differentiated as the carcinoma advances.

Role of urokinase-type plasminogen activator in local immunotherapy.

We have previously reported that intratumoral injection of a mixture of OK-432 and fibrinogen (OK/fbg) is very effective for local immunotherapy of colorectal cancer, but has little influence on breast cancer, while addition of activated macrophages to OK/fbg (OK/fbg/mo) has a marked effect on breast cancer. In this study, we analyzed the role of urokinase-type plasminogen activator (uPA) in local immunotherapy. We found that uPA levels in breast cancer tissue were lower than in colorectal cancer tissue. Injection of OK-432 into breast cancers neither increased the uPA level nor caused tumor regression. Although OK/fbg injection caused an increase of uPA, tumor regression was restricted to the area around the injection site. With OK/fbg/mo however, uPA levels increased much more markedly and extensive tumor necrosis was observed in all cases. These findings suggest that uPA plays an important role in tumor regression during local immunotherapy.
