Expression and clinical significance of the G1-S modulators in carcinoma of the extrahepatic bile duct.

Expression of cell cycle modulators at the G1-S boundary, the retinoblastoma gene product (pRb), p21, p16, p27, p53, cyclin D1 as well as Ki-67 was investigated with 39 extrahepatic bile duct carcinomas (BDC). The Ki-67 labeling index (LI) was higher in cases with poor differentiation, lymph node metastasis and stage III or IV. Cyclin D1 overexpression was seen in 14 cases (35.8%). This phenomenon could be observed more frequently in cases of hilar carcinoma and with poor differentiation, perineural invasion, lymphatic invasion and lymph node metastasis. Furthermore, Ki-67 LI was higher in cyclin D1 overexpressing cases. p27 expression showed inverse relationships with Ki-67 LI, lymph node metastasis and aberrant p53 expression. Although p16 and p21 expression significantly correlated with lymph node metastasis and cyclin D1 overexpression, respectively, they were not related to Ki-67 LI. pRb expression was observed in all cases. Although the LI was lower in carcinoma of upper and middle bile ducts, no correlation was established between pRb expression and other clinicopathological parameters including Ki-67 LI. Aberrant p53 expression was observed in 13 cases (33.3%) and Ki-67 LI was significantly higher in these cases. These findings suggest that p27 and cyclin D1 strongly correlate with BDC proliferation and reflect the biological aggressiveness of this carcinoma.

Fas-Fas ligand pathway does not actively work in extrahepatic bile duct carcinoma.

Fas and Fas ligand expression were immunohistochemically investigated in 34 extrahepatic bile duct carcinomas (BDC) and nine ampulla carcinomas (AC). Fas expression was regarded as positive in 22 BDC cases (64.7%). The absence or decreased expression of Fas was frequently observed in cases with high biological aggressiveness such as advanced stage, moderate or poor differentiation, lymph node metastasis, aberrant p53 expression and high Ki-67 labeling index. A similar tendency was observed in AC where two cases without Fas expression were poorly differentiated with a high Ki-67 labeling index. Fas ligand expression in infiltrating mononuclear cells adjacent to the carcinoma nests was observed in only four BDC (11.1%) and in none of the AC examined. Fas ligand expression in carcinoma cells was observed in only one BDC, which showed Fas ligand expression also in mononuclear cells and not in any AC case. These results suggest that, although BDC and AC very frequently express Fas, Fas-mediated apoptosis is only occasionally induced in carcinoma cells because of the lack of Fas ligand expression in infiltrating mononuclear cells in these carcinomas.

Factors affecting depression and anxiety in female Japanese patients with rheumatoid arthritis.

OBJECTIVE: In this study, we attempted to identify which background factors were the most important indicators regarding such psychological problems as depression and anxiety in female rheumatoid arthritis (RA) patients. METHODS: Among female RA patients aged 20 and older, who visited the outpatient clinic, and who met the Revised Criteria of the American Rheumatism Association for RA (ARA1987), 101 Class I and II patients were selected as subjects for the present study. Specially designed questionnaires and psychological tests were given to those subjects who had given their written consent for such testing, and a stepwise multiple logistic regression analysis was carried out on the 85 patients who returned all written forms. Those factors affecting psychological problems at a significance level of 5% or less were identified. RESULTS: It was shown that among sociodemographic factors, "a decrease in the frequency of going out socially after having RA" and "a higher education" worked as significantly associated variables for depression, while "a decrease in the number of friends after having RA" served as a significantly associated variable for trait anxiety. Significantly associated variables for state anxiety included "a decrease in the frequency of going out socially after having RA" and "a willingness to join an RA study group". CONCLUSION: The present study demonstrated that factors related to social interactions were significantly associated with depression and anxiety in Class I and II female RA outpatients.

Expression and clinical implications of bcl-2 in extrahepatic bile duct carcinoma: its relationship with biological features.

bcl-2 expression in 38 extrahepatic bile duct carcinomas was immunohistochemically investigated in order to clarify its clinical significance in this carcinoma. bcl-2 was expressed in 17 cases (44.7%), in which more than 10% of the carcinoma cells were positive for bcl-2. bcl-2 expression was more frequently absent in cases with advanced stages (p = 0.0642), lymph node metastasis (p = 0.0009), perineural invasion (p = 0.0647) and aberrant p53 expression (p = 0.0181). Furthermore, bcl-2 expression was inversely related to the incidence of apoptosis (p = 0.0002) and proliferating activity of the cells (p < 0.0001). These results suggest that bcl-2 plays a role in negatively regulating apoptosis and its loss can be significantly linked to the biological aggressiveness of this carcinoma.

Expression and possible role of cyclin D3 in human pancreatic adenocarcinoma.

BACKGROUND: Cyclin D3, one of the D type cyclins, has been known as a cell cycle modulator at the G1-S phase. In the present study, we investigated its expression in pancreatic adenocarcinoma in order to elucidate its clinical role. MATERIALS AND METHODS: We examined cyclin D3 as well as, cyclin D1 overexpression in 60 pancreatic adenocarcinoma by means of immunohistochemistry. RESULTS: Cyclin D3 overexpression was found in 33.3% (20 cases) of the cases, being more frequently seen in cases showing high Ki-67 labeling index, stage IV, moderate- or poor-differentiation, lymph node metastasis and large size and cyclin D1 overexpression. Multivariate logistic analysis independently linked cyclin D3 overexpression to tumor size (p = 0.0392) and stage (p = 0.0312). Cyclin D1 overexpression was observed in 41.7% (25 cases) and was related to Ki-67 labeling index, cyclin A overexpression, stage, carcinoma differentiation, lymph node metastasis, tumor size, lymphatic invasion, perineural invasion and retropancreatic invasion. It was independently linked to carcinoma differentiation (p = 0.0081), lymph node metastasis (p = 0.0426) and lymphatic invasion (p = 0.0269). CONCLUSION: These results suggest that cyclin D3, as well as cyclin D1, plays a role in the progression, including cell proliferating activity, of pancreatic adenocarcinoma.

Bimodal expression of heparin-binding EGF-like growth factor in colonic neoplasms.

BACKGROUND: Recent studies have demonstrated that heparin-binding epidermal growth factor-like growth factor (HB-EGF) plays a role in carcinogenesis and carcinoma progression. In this study we investigated the expression of HB-EGF in human colonic non-neoplastic and neoplastic tissues. MATERIALS AND METHODS: We performed immunohistochemistry using a polyclonal antibody against HB-EGF for normal colon hyperplastic polyps, adenomas and adenocarcinomas. RESULTS: Normal human colon and hyperplastic polyps did not express HB-EGF. In adenomas with moderate or severe dysplasia, HB-EGF was positive in 92% of the cases, whereas only 14.6% of those with mild dysplasia, expressed HB-EGF (p < 0.0001). HB-EGF expression was observed in 75% of carcinoma-in-adenoma cases. In adenocarcinomas, the incidence of HB-EGF expression significantly decreased as compared to adenomas with moderate or severe dysplasia (p < 0.0001) and CIA (p = 0.0005), with only 27.5% of the cases being classified as positive. In adenocarcinomas, HB-EGF expression was inversely linked to carcinoma differentiation (p = 0.0003) and lymph node metastasis (p = 0.0358). CONCLUSION: Our results demonstrated the bimodal expression of HB-EGF in colonic neoplasms and suggested that HB-EGF may play a role in colonic carcinogenesis and at an early phase of the progression of colonic adenocarcinoma.

A reverse transcriptase-polymerase chain reaction assay for detecting Highlands J virus.

Highlands J (HJ) virus is an arbovirus frequently recovered at high rates in mosquitoes collected in the eastern United States. HJ virus is primarily a veterinary pathogen causing disease in domestic birds including turkeys, chickens, and partridges. It has an enzootic cycle similar to eastern equine encephalitis (EEE) virus and is often used as an indicator species in EEE surveillance programs. Current immunologic techniques to identify HJ virus are often inefficient and can involve cross-reactivity of antibodies. Therefore, we developed a molecular-based assay by a reverse transcriptase (RT)-polymerase chain reaction (PCR) technique. Primers were constructed from conserved sequences of the E1 coding region from 19 strains of HJ virus. PCR amplifications from serial dilutions of HJ virus-infected Vero cell culture supernatants indicated that this assay could detect viral RNA at concentrations of 10 plaque-forming units per reaction. Extracted RNAs from western equine encephalitis, EEE, LaCrosse, and Jamestown Canyon viruses were not detected with this assay. RNA extracted directly from the brain tissue of a dead house sparrow and from a pool of Culiseta mosquitoes yielded a PCR product of the expected size. The RT-PCR technique developed was both sensitive and specific for detecting HJ virus from infected cell culture supernatants, bird brain tissues, and mosquitoes. This new assay will permit rapid and accurate diagnosis of HJ virus, both enhancing surveillance activities for EEE transmission risk and monitoring infections in domestic poultry and wild birds.

Immunohistochemical studies of age-associated amyloid deposition in the joint of senescence-accelerated mouse (SAM).

The senescence-accelerated mouse (SAM) is a murine model of accelerated senescence and consists of the senescence-accelerated prone mouse (SAM-P) and senescence-accelerated resistant mouse (SAM-R), the latter of which shows normal aging characteristics. SAM shows a high incidence of age-associated microscopic amyloid deposition in synovial joints and intervertebral discs and the lesion is histologically quite similar to that of humans. The amyloid fibril protein of these mice is well characterized as a murine systemic senile amyloid (ASSAM). Twenty SAM-P and three SAM-R mice were used for this immunohistological study. Synovial joints and intervertebral discs were stained by immunoperoxidase method (PAP) using anti-ASSAM and anti-mouse AA antibodies and compared with birefringence in a Congo-red-stained section. Positive staining was observed in annulus fibrosus of the intervertebral discs, blood vessels, synovia, and on the surfaces of the meniscus and articular cartilage, exactly at the same site where green birefringence in Congo-red staining was observed. Both ASSAM and AA existed in the articular structures of SAM and the incidence of AA was significantly correlated with systemic signs of inflammation at autopsy. Among blood vessels, synovium and articular cartilage, there was no one tissue where amyloid deposited earlier than others. It was postulated that amyloid is transported via synovial fluid as its fibrillar form or as a precursor and that it deposits on the surface of articular cartilage.

Studies on cytologic characteristics of mammary aspiration smears based on histologic types.

Cytologic findings of aspiration smears from 146 histologically confirmed mammary were analyzed in this paper. High cellularity, dispersal of cells in the smears and enlarged nuclei of more than 20 mu in major axis were considered to be useful key points to distinguish carcinoma from benign lesions. Irregular piling, variable nuclear shape, cellular fusion and cytoplasmic inclusions were other common findings in cancer specimens and were scarcely noticed in benign cases. However, in some papillotubular carcinomas the above mentioned characteristics were not so dominant. Scirrhous carcinomas sometimes presented small nuclei in the smears. In some fibroadenomas, the smears were occasionally very cellular and revealed dipersal of cells.

Nipple discharge cytology in mass screening for breast cancer.

Since 1977 mass screening for breast cancer has been conducted in Miyagi Prefecture, Japan; inspection, palpation and cytologic examination of any nipple discharge are part of the initial screening procedures. Among 149,681 subjects examined, 404 cancer cases and 63 papilloma cases were detected. The nipple discharges from 20,537 women were examined cytologically; of the 61 cancer cases, the smears were positive in 18 cases, suspicious in 7, negative with atypical findings in 12 and negative in 24. Ten of the cancer cases were detected exclusively by the cytologic examination of a nipple discharge. In eight of these ten cancer cases, there was no other initial evidence of the primary tumor. The cytologic diagnosis of discharges without blood from 28 cancer cases was positive or suspicious in 10 cases and negative in 18. Thirty-seven of the papilloma cases were initially detected only by the cytologic examination of a nipple discharge; neither physical examination nor mammography showed any abnormal findings.

Mucoepidermoid carcinoma of the salivary gland in pleural fluid. A case report.

The cellular features of mucoepidermoid carcinoma of submandibular gland origin observed in pleural fluid are presented. The pleural fluid contained predominantly atypical spheroid cell clusters accompanied by numerous mesothelial cells. The cells had round nuclei with conspicuous nucleoli, coarsely granular chromatin and abundant cytoplasm with vacuoles. The cellular features of the malignant cells in the pleural fluid were correlated with the histology of the parent lesion.

Aspiration cytology of breast cysts.

A cytologic evaluation was made of 57 samples of fluid aspirated from cystic breast masses during the past 12 years in our hospital. The cytologic diagnosis of the cyst aspirates from 17 cancer cases was positive in 10 cases, suspicious in 2 and negative in 5. No false-positive cytologic diagnosis was made in 40 cases of benign cystic disease. Blood-tinged aspirates were observed in 14 of the 17 cases of malignant cystic disease but in only 2 of 40 benign cases. Ductal cell clusters were detected in aspiration smears from 16 of the cases of malignant cystic disease and 15 of the cases of benign cystic disease. Apocrine cells were seen in the aspiration smears from 12 of the cases of benign cystic disease but in none of the aspirates from the 17 cancer cases.

The effects of dietary lactose and rye on cecal colonization of Clostridium perfringens in chicks.

Five experiments were conducted to determine the effects of dietary lactose and rye on cecal colonization of Clostridium perfringens in white leghorn chickens. Six days after oral inoculation of the organism, the numbers of C. perfringens organisms in the cecal contents were significantly lower in chickens on 2% and 10% lactose-supplemented feed than in chickens on unsupplemented feed. When C. perfringens was given in drinking water, 10% lactose supplementation was needed to significantly reduce the counts of C. perfringens 4, 6, and 8 days after feeding began. Effect of rye-ration on cecal colonization of C. perfringens was also examined. Counts of C. perfringens in cecal contents of chickens fed a diet containing 50% rye were significantly higher than control values 4, 6, and 8 days after feeding began. When chicks were fed a diet containing both 10% lactose and 50% rye, C. perfringens counts in cecal contents were lower than in chickens fed 50% rye only at 6 days after feeding began. Results led to the conclusion that dietary lactose is effective in reducing the cecal colonization of C. perfringens.
