[A case of eosinophilic cystitis with retroperitoneal neurinoma].

A 63-year-old female visited our department, complaining of miction pain. She had several episodes of urinary occult blood. Urinalysis included a small number of red cells, white cells and bacilli. Physical examination revealed a fist-sized mass of her right flank. Cystoscopic examination showed a botryoid tumor with multiple erythematous raised plaques. Biopsy was reported as massive infiltration of eosinophiles in submucosal layer of the bladder, i.e. eosinophilic cystitis. Antibiotics were effective for the improvement of urine findings and symptoms within a week. IVP and CT revealed the abdominal mass as a retroperitoneal tumor with cystic degeneration, and the tumor was resected. Pathological diagnosis was benign neurinoma. Both eosinophilic cystitis and retroperitoneal neurinoma are rare, and the concurrent occurrence of these two diseases has not been reported. Recent studies have suggested that eosinophilic cystitis may occur more frequently than suspected, and may be overlooked clinically and microscopically. This uncommon form of cystitis should be considered in the differential diagnosis especially when the patient has unexplained episodes of bladder symptoms and hematuria.

Hemodynamics of canine corpora cavernosa during erection.

Being able to induce erection by electrical stimulation of the cavernous nerves, we studied the hemodynamics of canine penile erection. Simultaneous recording of flow and pressure of the internal pudendal artery as well as pressure within the corpus cavernosum clearly demonstrated that increase of arterial flow preceded corporeal pressure increase. When saline was infused directly into the corpus cavernosum, with the aorta clamped, decreasing venous flow during erection could be demonstrated. Tumescence of the corpus cavernosum was found to be a result of active relaxation of sinusoidal spaces, active arteriolar dilatation, and active venous constriction. At full erection, there was still flow into and out of the corpus cavernosum, although it was reduced to only a fraction of a milliliter per minute.

Effect of alpha-tocopherol on lipid peroxidation and acyl chain mobility of liver microsomes from vitamin E-difficient rat.

Effects of alpha-tocopherol on lipid peroxidation and membrane fluidity were studied in liver microsomes from vitamin E (alpha-tocopherol)-deficient rats using NADPH as a substrate. Microsomes containing various contents of alpha-tocopherol were prepared by incubation with various concentrations of alpha-tocopherol in ethanol solution. NADPH-dependent lipid peroxidation decreased the content of polyunsaturated fatty acids, arachidonic acid and 4, 7, 10, 13, 16, 19-docosahexaenoic acid. The treatment with alpha-tocopherol before peroxidation reduced the production of lipid peroxides and the change in fatty acid composition even at the lowest content of alpha-tocopherol dealt with in this experiment, 0.2 molar fraction, while addition of alpha-tocopherol after peroxidation resulted in a slight inhibition of peroxide production and small alteration in fatty acid composition. By an ESR measurement using stearate spin probe, the alpha-tocopherol incorporated into microsomes did not alter the acyl chain mobility up to 0.2 molar fraction but reduced the mobility above 0.2 molar fraction. The acyl chain mobility was markedly decreased by lipid peroxidation. The decrease of membrane fluidity was repressed in microsomes treated with alpha-tocopherol before peroxidation, but was not repressed in microsomes treated after peroxidation. The experiment using artificial membranes of egg yolk phosphatidylcholine and rat liver phosphatidylcholine revealed that the effect of alpha-tocopherol on membrane fluidity depends on the fatty acid composition of phospholipid, especially the content of arachidonic acid. On the other hand, the mobility of the fatty acyl chain was not affected by spermine at concentrations which could inhibit lipid peroxidation. These results suggest that the inhibitory effect of alpha-tocopherol on lipid peroxidation is due to antioxidant activity rather than the indirect effect of membrane stabilization.

[Therapeutic effect of Robaveron tablet (KN-7) on neurogenic bladder].

Therapeutic effect of Robaveron Tablet (KN-7) was studied on 128 patients suffering from neurogenic bladder. Robaveron Tablet was administered 6 tablets daily for 3 weeks. The effect was evaluated by the residual urine, subjective symptoms, cystometric findings, urethral pressure profile and uroflowmetry. Remarkable decrease of residual urine was observed regardless of position of injury. The pressure amplitude defined as the difference between the maximum voiding pressure and the maximum resting pressure increased on any group of neurogenic bladder. The effectiveness, rated as moderately improved or better was 76.9% for brain disorders, 58.3% for upper motor neuron lesion and 75.7% for lower motor neuron lesion of spinal injuries, 58.3% for peripheral nervous injuries and 100% for others. No remarkable abnormality was observed in the results of side effects or clinical laboratory tests.

[Comparative double-blind trial of KN-7 tablet and Robaveron injection in the treatment of neurogenic bladder].

The clinical effectiveness, safety and usefulness of KN-7 tablet as a new oral application of the prostatic extract, on urinary dysfunction of neurogenic bladder were compared with those of Robaveron injection by the double-blind test method. In the study, 2 tablets t.i.d. and a shot of intramuscular injection 1 ml a day were given successively for 3 weeks. A total of 233 cases were reported from 37 facilities belonging to the KN-7 Clinical Research Group. Some of them were excluded or dropped out. The number of cases used for analysing the effectiveness, safety and usefulness were 214, 232 and 215, respectively. There was no bias between the two groups with a significant homogeneity in the background. In the overall clinical effectiveness, the effective rate including excellent, moderate and slightly effective was 76.9% with KN-7 and 77.4% with Robaveron. In the clinical usefulness, the rate of usefulness of slightly useful or above was 75.0% with KN-7 and 75.7% with Robaveron. There was no significant difference between the two groups in the clinical effective and useful rates at a significant level of 5%. Side effects were observed in 1 of the 114 (0.9%) patients given KN-7 and 8 of the 118 (6.8%) patients given Robaveron. The incidence of adverse reactions with KN-7 was significantly lower than that with Robaveron. Based on the results, it was concluded that KN-7 tablets, 2 tablets t.i.d., would be as effective and useful as a Robaveron injection 1 ml daily and safer than the latter in the treatment of neurogenic bladder.

[Surgical resection in pulmonary metastases of genitourinary cancers].

Four patients (2 with renal cell carcinoma, 1 with testicular embryonal carcinoma, 1 with testicular seminoma) underwent surgical resection for pulmonary metastases. Three patients except one with renal cell carcinoma are still alive without disease at 1, 2.3 and 2.3 years, respectively. Surgical resection of metastatic pulmonary sarcoma or carcinoma is now the accepted method of treatment. As chemo- and radio-therapy are often ineffective for metastatic genitourinary cancers, our current policy is to employ surgical treatment provided that the operability is assessed carefully.

Potential preservation of potency after radical prostatectomy.

Radical prostatectomy often is followed by erectile dysfunction. Various etiologies have been postulated--vascular, psychologic, or neurologic. Recently, we were able to isolate the cavernous nerves of the prostatic plexus, which innervate the erectile tissue of the corpora cavernosa, for electroerection in dogs. Acute and chronic experiments were then performed to examine the relationship between erectile impotence and radical prostatectomy. We conclude that erectile impotence after total prostatectomy is a result of injury to the cavernous nerves and that potency can be preserved if these nerves are identified and salvaged during surgery.

Hemodynamics of erection in the monkey.

Being able to induce controlled erection in dogs and monkeys, we investigated the hemodynamics and mechanism of penile erection. 'Chronic' monkey models, having had electrodes implanted around the cavernous nerves for electroerection, were studied to evaluate the details of the hemodynamic changes. The studies included: 1) arterial blood flow, 2) corporeal pressure, 3) blood gases, 4) venous flow and 5) radiography. Tumescence of the corpora cavernosa was found to be a result of: 1) active relaxation of the sinusoidal spaces, 2) active arteriolar dilatation and 3) active venous outflow constriction. At full erection there is adequate but reduced blood flow into and out of the corpora cavernosa for metabolic exchange.

A new fucosyl antigen expressed on colon adenocarcinoma and embryonal carcinoma cells.

Carbohydrate antigens on the surface of mammalian cells have recently gained renewed interest because some are specifically expressed at certain stages of cellular differentiation. Most of the useful antibodies detecting such carbohydrate antigens have been monoclonal antibodies produced by the hybridoma technique using whole cells as the immunogen or the antibodies in the sera of some of human patients. Here we report that an antiserum raised against an unusual carbohydrate linkage prepared by organic synthesis can preferentially react with certain tumour cells. Thus, an antiserum against the Fuc alpha l leads to 3Gal linkage reacted with human colon adenocarcinoma cells and murine teratocarcinoma cells but reacted only with severely restricted regions in normal tissues.

Behavior genetics of choice of oviposition sites in Drosophila melanogaster. I. Genetic variability and analysis of behavior.

Females of Drosophila melanogaster were given a choice of oviposition site either on the surface of the medium or on the surface of paper positioned vertically on the medium. A significant difference was seen in the proportion of eggs deposited on the paper among wild strains of different geographic origins. Bidirectional selection for oviposition on these two sites was effective. These selected lines were examined under various conditions to determine the factors involved in this site selection for oviposition. The lines that chose medium laid eggs only on substrates into which eggs could be inserted. The lines preferring paper showed no strict requirement for burying their eggs. Tarsal sensillae were involved in site discrimination.
